<content:encoded><![CDATA[<div class='snap_preview'><p>A study of heart-attack patients published in the late 1980s was wildly successful. Researchers showed they could lower the heart-attack death rate to 8% from 13% by giving patients aspirin and a drug called streptokinase. These days, though, an 8% mortality rate would be disastrous; the rate in most studies of heart attack patients is somewhere around 4%.</p>
<p>That points to a challenge for companies developing new therapies for heart disease (and other well treated maladies): The better existing therapies are, the harder it is to come up with something that&#8217;s an improvement. An essay published in this week&#8217;s JAMA calls this process &#8220;innovation to extinction.&#8221;</p>
<p>To some extent, drug companies are already shifting their focus to poorly treated forms of cancer and other diseases where it may be easier to improve on existing therapies.</p>
<p>But companies also have a few strategies for showing improvements in already well-treated populations. These strategies can be useful in developing new therapies, but can also be problematic, argue the authors of the JAMA essay (David Kent and Thomas Trikalinos, both MDs affiliated with Tufts).</p>
<p>One strategy is to shift from measuring a single endpoint &#8212; such as death &#8212; to measuring a combination of several endpoints. Sometimes, though, these composite endpoints can mask important distinctions. For example, researchers might find that a new therapy improves a composite endpoint that includes death, repeat heart attacks and repeat medical procedures, but a closer analysis might suggest that the therapy actually lowers the risk of repeat procedures but not death or repeat heart attacks.</p>
<p>Another option is to look at so-called surrogate endpoints, changes that aren&#8217;t in themselves clinically significant, but that raise the risk of future events such as heart attacks or death. This has been suggested for research in stents, the authors note. And surrogate measures are already widely used in studies of drugs used to prevent heart attacks &#8212; as the controversy over Merck&#8217;s cholesterol drugs Zetia and Vytorin shows.</p>
<p>The shift from single endpoints to composite endpoints to surrogate endpoints can be seen as a kind of evolution of a field, as outcomes get better and better, the authors suggest:</p>
<blockquote><p>The evolution of assessed outcomes from hard clinical outcomes (ie, death) to composite outcomes (including softer, less important clinical end points) to surrogate outcomes (of possible, but not definite, clinical import) may signal a maturation in technological development toward innovation to extinction.</p>

<p><img src="http://img81.imageshack.us/img81/3282/asp.jpg" style="float:right;width:250px;height:241px;margin:0 5px 5px 0;" border="0">The CDC now reports that nearly 4,000 Americans have been killed by H1N1 swine flu. This number is supposed to sound big and scary, motivating millions of people to go out and pay good money to be injected with untested, unproven H1N1 vaccines. But let&#8217;s put the number in perspective: Did you know that more than four times as many people are killed each year by common NSAID painkillers like aspirin?</p>
<p>The July 1998 issue of The American Journal of Medicine explains it as follows:</p>
<p>&#8220;Conservative calculations estimate that approximately 107,000 patients are hospitalized annually for nonsteroidal anti-inflammatory drug (NSAID)-related gastrointestinal (GI) complications and at least 16,500 NSAID-related deaths occur each year among arthritis patients alone.&#8221; (Singh Gurkirpal, MD, “Recent Considerations in Nonsteroidal Anti-Inflammatory Drug Gastropathy”, The American Journal of Medicine, July 27, 1998, p. 31S)</p>
<p>So for every person the CDC claims was killed by H1N1 swine flu this year, common painkillers like aspirin have killed four! Yet you don&#8217;t see the CDC, FDA, WHO or mainstream media running around screaming about the extreme dangers of aspirin, do you? All those deaths apparently don&#8217;t matter. Only swine flu deaths lead to hysteria.</p>
<p><strong>Understanding risk</strong></p>
<p>According to death statistics tables available on the &#8216;net, you are ten times more likely to die in a car accident this year than be killed by swine flu.</p>
<p>Nearly 100,000 Americans die every year from adverse reactions to FDA-approved prescription drugs. That&#8217;s twenty-five times the number of people killed by H1N1 swine flu (even if you believe the CDC&#8217;s numbers). So where&#8217;s the big warning about the dangers of prescription drugs? Why isn&#8217;t the CDC warning Americans about an &#8220;epidemic of dangerous drugs&#8221; that poses a far greater threat to your health?</p>
<p>The answer, of course, is that health authorities want to push people to buy vaccines that are about to become worthless (they&#8217;re only good before swine flu fizzles out). And the only way to sell more vaccines to people who don&#8217;t need them is to hype up a bunch of scare stories by citing bold statistics that make H1N1 swine flu seem really, really dangerous.</p>
<p>But the flu is no more dangerous than aspirin. In fact, H1N1 swine flu may be safer than aspirin.</p>
<p>Here&#8217;s another quote from the New England Journal of Medicine:</p>
<p><em>&#8220;It has been estimated conservatively that 16,500 NSAID-related deaths occur among patients with rheumatoid arthritis or osteoarthritis every year in the United States. This figure is similar to the number of deaths from the acquired immunodeficiency syndrome and considerably greater than the number of deaths from multiple myeloma, asthma, cervical cancer, or Hodgkin’s disease. If deaths from gastrointestinal toxic effects from NSAIDs were tabulated separately in the National Vital Statistics reports, these effects would constitute the 15th most common cause of death in the United States. Yet these toxic effects remain mainly a “silent epidemic,” with many physicians and most patients unaware of the magnitude of the problem. Furthermore the mortality statistics do not include deaths ascribed to the use of over-the-counter NSAIDS.&#8221;</em> (Wolfe M. MD, Lichtenstein D. <p>Did you catch that? The 16,500 figure for deaths each year doesn&#8217;t even include over-the-counter painkiller drugs! If you add in those numbers, you&#8217;re probably looking at something closer to 40,000 Americans kills each year by these drugs. And that makes these drugs 1000% more deadly than swine flu (because 40,000 is ten times greater than 4,000).</font></p>

A meta-analyis published by the British Medical Journal had recently questioned the veracity of aspirin&#8217;s claim to fame. The study headed by De Berardis and his cohorts cited that aspirin&#8217;s clear benefit in the primary prevention of cardiovascular events remains unproven. That&#8217;s a big jab for the proponents of aspirin&#8217;s role in the primary prevention for the diabetic patients. Aspirin, according to their study, significantly reduces the risk of myocardial infarction only in men but not in women.</p>
<p>Everything I thought right about aspirin hangs in a balance. Should I stop prescribing this dirt cheap wonder drug to my diabetic patients ? It doesn&#8217;t say that it is harmful, it&#8217;s just questioning its benefit. It doesn&#8217;t alter the pathophysiology of the disease process as well as the drug&#8217;s mode of action.</p>
<p>Yes, I will still give it to my patients. That particular study sure got my attention but it hasn&#8217;t change by belief in aspirin. It will take me more than a meta-analysis to have a major change of heart.</p>

Tidak hanya itu mereka yang mengalami migrain diikuti dengan gangguan visual, seperti sialu, garis-garis zig-zag, dan pandangan kabur punya risiko terkena stroke 2,5 kali lebih besar serta 2,9 kali lebih besar bagi perempuan.</p>
<p>Ada beberapa cara mengobati dan mencegah migrain, termasuk berhenti menghirup asap, mengonsumsi obat tekanan darah rendah, atau mengonsumsi obat pengencer darah seperti aspirin. Bagi kaum permpuan, sebaiknya berhenti meneruskan penggunaan pil KB.</p>

<p>On another, more interesting note, I was driving home from school the other day, and heard something that, while I probably should have known, completely shocked me. Makeup and other kinds of beauty products are one of <a href="http://articles.moneycentral.msn.com/SmartSpending/blog/page.aspx?post=1391440">the most shoplifted items in 2009</a>.</p>
<p>The reason why? Because of the ailing economy, according to MSN, people feel the need to &#8220;maintain a quality of life they can no longer afford&#8221; (said by Bruce Cromley of <a href="http://www.time.com/time/world/article/0,8599,1937944,00.html">TIME magazine</a>)</p>
<p>It&#8217;s a shame, really.</p>

<p>Damn those Germans! Every time I think I&#8217;m done with Germany, they come up with something to pull me back.  </p>
<p>I can&#8217;t help it. <a href="http://wp.me/pyafL-3A">It&#8217;s the geek in me.</a> </p>
<p>First, it was the <a href="http://wp.me/pyafL-jS">Magdeburg water bridge.</a>  Now, I have to work in the northern Rhine city called <a href="http://en.wikipedia.org/wiki/Wuppertal">Wuppertal.</a></p>
<p>At this rate, I&#8217;ll <em>never</em> see the rest of Europe!</p>
<p>Never heard of Wuppertal? Neither had I until very recently.   Population about 350,000 &#8212; slightly larger than New Orleans, slightly smaller than the city of Oakland, built on either side of a river.  Apparently, it&#8217;s very &#8220;green,&#8221; with two-thirds of it being woods, gardens, parks, fields and so on.  </p>
<p>It&#8217;s also got some history to it, being the birthplace of<a href="http://en.wikipedia.org/wiki/Wuppertal"> Friedrich Engels,</a> who co-wrote <em><a href="http://en.wikipedia.org/wiki/The_Communist_Manifesto">The Communist Manifesto</a></em> along with <a href="http://en.wikipedia.org/wiki/Karl_Marx">Karl Marx.</a>  </p>
<p>And it&#8217;s the birthplace of <a href="http://www.bayer.com/en/Friedrich-Bayer.aspx">Friedrich Bayer.</a> Yes, <em>that</em> Bayer, the aspirin people.  Which makes Wuppertal, among other things, the birthplace of  <a href="http://en.wikipedia.org/wiki/Heroin">heroin.</a> </p>
<p>But neither those historical tidbits nor all that greenery is what&#8217;s pulling me to this place.   It&#8217;s the <em><a href="http://en.wikipedia.org/wiki/Wuppertal_Schwebebahn">Schwebebahn</a></em> &#8212; their monorail.</p>
<p>Okay, maybe you rode monorails as a kid at Disneyland or Disneyworld, but this is no amusement ride.   This is how Wuppertal gets around, as this <a href="http://www.youtube.com/watch?v=Ulg8A1tGbuE">YouTube video</a> shows.</p>
<p>Most monorail trains ride on top of their single rail.  In Wuppertal, they <em>hang</em> from theirs.  </p>
<p>Passengers ride in cars suspended in space, 24 feet over the streets and 40 feet over the river, at speeds around 40 miles per hour.   That &#8220;hang&#8221; allows them to tilt on curves, so the trains never have to slow down for them. </p>
<p>This would be some pretty slick technology even today.  The Germans came up with it in 1901, two years before the Wright Brothers got off the ground.  A century and change later, the system is still moving people  &#8212; 75,000 a day, 25 <em>million</em> a year.  </p>
<p>It even managed to survive the World War 2 bombing that <em>wasted</em> nearly half the town.  </p>
<p>Now you <em>know</em> I have to ride <em>this</em> thing!</p>

<p>Back in 1963, the Swedish firm <a href="http://en.wikipedia.org/wiki/ALWEG">Alweg,</a> which built the monorail in Seattle, offered Los Angeles a 40-mile monorail line.  They&#8217;d keep the fares until the construction was paid for, then hand it to the city. Cost to the taxpayers: zero.</p>
<p>The City Council&#8217;s response: We like our gridlock. We think smog is sexy. Go away. </p>
<p>If you drive around L.A. today, no need to tell you the rest of the story.  You&#8217;re living it.</em></p></blockquote>

<p style="padding-left:30px;"><em><strong>Limitations: </strong>New evidence on aspirin for the primary prevention of CVD is limited. The dose of aspirin used in the RCTs varied, which prevented the estimation of the most appropriate dose for primary prevention. Several of the RCTs were conducted within populations of health professionals, which potentially limits generalizability.</em></p>

<p><strong>What does this article say?</strong> The excerpts above are from the abstract from a 2009 article from the Annals of Internal Medicine.  It describes the research that has been done to determine the health outcomes of ingesting ‘baby,’ or low-dose aspirin on a regular basis.  From the article’s conclusion: <em>“</em><em>Aspirin does not seem to affect CVD (cardiovascular)  mortality or all-cause mortality in either men or women.”</em><em> </em><em> </em></p>

<p><strong>So what does this mean? </strong>There is a big difference in examining whether a treatment decreases a disease rate, changes overall lifespan, or increases your health.  Aspirin has been shown to decrease the incidence of certain cardiovascular complications in certain groups.  But as it says above, it did not actually lead to any increase in lifespan in the groups that were studied.  The official recommendations thus suggest discussing whether trading one type of complication, like cardiovascular disease, for another, such as gastrointestinal problems, is worth it.</p>
<p><strong>What are the experts saying about this?</strong> Here is an excerpt of an article from cnn.com’s health section: “<span style="text-decoration:underline;"><strong>Risks of daily aspirin may outweigh the benefits</strong></span>,” By Denise Mann, Health.com.  November 4, 2009.</p>
<p style="padding-left:30px;">&#8220;<em>This article synthesizes what many people in the field are beginning to feel: The risks of daily aspirin therapy exceeds the benefits in people who have not had a heart attack,&#8221; says Dr. Steven E. Nissen, the chairman of cardiovascular medicine at the Cleveland Clinic, in Ohio.</em></p>
<p style="padding-left:30px;"><em>“Currently, the American Diabetes Association (ADA) recommends low-dose aspirin for primary prevention in people with diabetes who are at risk for cardiovascular disease&#8211;but this will be changing.</em></p>
<p style="padding-left:30px;"><em>&#8220;Because of some recent studies suggesting that the benefit is not very large, and because aspirin can also have risks (intestinal bleeding or hemorrhagic stroke), the January 2010 recommendations will recommend it mostly for higher-risk people than was the case in the past, when it was recommended for people with more moderate levels of risk and above,&#8221; says Dr. M. Sue Kirkman, the vice president of clinical affairs for the ADA.</em></p>
<p style="padding-left:30px;"><em>“The authors of the new analysis say there&#8217;s not enough evidence to justify the routine use of low-dose aspirin to prevent cardiovascular disease in apparently healthy people, including those with elevated blood pressure or diabetes.”</em></p>
<p><strong>What does this mean?</strong> The research showed that there is no certain benefit for increasing lifespan.  In other words, you need to minimize your chances of cardiovascular disease by minimizing your so-called risk factors:  High blood pressure, excess weight, diabetes, high cholesterol.  To do this, we recommend you start implementing proper eating, moving and thinking wellness lifestyle strategies today.</p>
<p><strong>Who do I pass this on to? </strong>Pass this on to a friend, family member or co-worker that wants to learn how to protect their health.  If they have concerns about their and health and wish to prevent serious disease, they need to start pro-actively working to get as healthy as possible today.  As this new research shows, relying solely on testing and treatment is not a reliable strategy.  Refer them to our office or another wellness chiropractor to help them get started today.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>I have a very bad knee that has given me lots of trouble.  It started years ago when I was still actively downhill skiing and hit one too many black diamond runs that I had absolutely no right being on&#8230; came home in struts one too many times &#8211; you get the picture.</p>
<p>Over the years, I have smashed it pretty good slipping in the winter on polished marble floors coming in to work on snowy days.</p>
<p>The pain finally got so bad I had to admit something was really wrong, and I visited an orthopedic doctor.  Fully expecting a diagnosis of something easily fixed with a little surgery, like a bone chip, I was dumbstruck when the X-rays revealed osteoarthritis in the knee with very diminished cartilage.  His advice?  Knee replacement. &#8220;When the pain gets so bad you can&#8217;t stand it, come see me and we will give you a new knee.&#8221; </p>
<p>I was  &#8211; first &#8211; stunned and &#8211; second &#8211; determined to do everything I could to make the knee last because I was not even 50 at the time of the diagnosis!  However, not being able to walk without pain was also severely cramping my lifestyle, so I had to balance one with the other.</p>
<p>I was already using other therapeutic grade essential oils for aromatherapy, relaxation and to help me sleep, so I wanted to consider essential oil therapy for natural pain relief as much as possible, as long as it worked!  This amount of pain was serious, as anyone can attest to who walks on knees with arthritis!</p>
<p>I began using Young Living PanAway oil regularly on my knee, twice a day. I kept that up for nearly a year until PanAway became unavailable due to a shortage of one of the key oils, helichrysum.  I am not sure of the specifics (I believe it was a weather issue), but I DO know that Young Living is exact and particular about the quality of plant material they will accept to use for their oils, and in 2008 helichrysum was not available. Nothing was of the proper quality and quantity Young Living needed, as it was explained to me.  So, I searched for a substitute because I was having wonderful results with PanAway and I wanted to continue my pain management with essential oils.</p>
<p>I researched Wintergreen essential oil, one of the ingredients in the PanAway blend, and decided to try it. The essential oil of Wintergreen <em>(Gaultheria procumbens) </em>contains primarily methyl salicylate, which is extremely analgesic, antispasmodic, and anti-inflammatory. Methyl salicylate is a precursor to common aspirin (acetylsalicylic acid).</p>
<p>The smell alone was amazing! The aromatherapy properties alone were helpful to me, but &#8211; better than that - I had excellent positive results with the Wintergreen alone!</p>
<p>On the plus side, Wintergreen is less than 1/3 the cost of PanAway, so I was very excited to get good results with a much less expensive oil.</p>
<p>I am very happy to report I have continued to use Wintergreen essential oil before bed and upon rising, and have &#8211; for the most part (weather can still affect the knee to some extent) &#8211; pain free days.  A key to effective pain management with essential oils is continued use.  My experience has been that the more regularly I use the oils, the more effective they seem to be. The PanAway has been back in stock for quite some time, but I have stuck with the Wintergreen because it is effective and costs less.</p>
<p>One caution &#8211; some people can be sensitive to Wintergreen applied neat (undiluted). The easiest thing to do is dilute it with a good quality oil.  You can use Young Living V-6 Oil blend, olive oil, or whatever natural (not rancid) oil you may have in your kitchen. The dilution helps proptect the skin and it also slows the absorbtion of the oils.  The relief won&#8217;t be as immediate but will be fully effective.  There is no diminishing of efficacy when the oil is diluted with a proper oil.</p>
<p>Conclusion?  I personally <em>HIGHLY</em> recommend Young Living therapuetic grade essential oil Wintergreen based on several years of very successful use and effective pain management with serious arthritis in my knee.  It has been like a natural liquid aspirin in a bottle!</p>

<p>Khasiat aspirin untuk mampu melindngi seorang wanita dari resiko stroke ternyata tidak dapat digunakan pada lawan jenis mereka, pria. Padahal Aspirin banyak direkomendasikan oleh dokter untuk digunakan pada pria dan wanita penderita penyakit jantung.</p>
<p>Dokter dari Harvard Medical School sekaligus peneliti dari `Brigham and Women&#8217;s Hospital` di Boston ini mengingatkan bahwa sangat berbahaya melakukan generalisasi pada dampak penggunaan aspirin pada wanita dan pria. &#8220;Kami sudah mengetahui jawaban dari isu itu.&#8221;</p>
<p>Studi yang dilakukan oleh `The Women&#8217;s Health Study` sebelumnya melakukan pengujian pada penggunaan aspirin dan vitamin E pada wanita. Hasilnya konsumsi vitamin E tidak memberikan sebuah solusi yang baik bagi wania karena suplemen itu tidak cukup membantu namun malah merusak.</p>
<p>Hampir 40.000 wanita yang berusia 45 tahun telah mendapatkan aspirin sebesar 100 milligrams yang diberikan secara acak. Setelah 10 tahun pengguna aspirin menunjukan bahwa mereka memiliki peluang 17% lebih rendah terkena stroke karena kemungkinan besar aspirin mampu mencegah terjadinya pembekuan darah.</p>
<p>Stroke ditengarai terjadi karena aliran darah ke otak tidak lancar karena adanya pembekuan darah. Sementara wanita berusia diatas 65 tahun memiliki keuntungan yang lebih tinggi dengan mengkonsumsi aspirin dimana mereka mampu terhindar terkena resiko stroke sampai 34%. Catatan penting lainya adalah proteksi aspirin bisa secara efektif terjadi pada wanita non perokok dan mantan perokok.</p>
<p>Namun manfaat ini tidak berlaku pada wanita yang menggunakan hormon setelah terjadinya masa menopause. Hasil penelitian Dr Paul ini memang sangat baik karena para wanita memiliki peluang yang besar terkena stroke ketimbang serangan jantung, sebuah hal yang berbeda terjadi pada pria yang memiliki resiko tinggi terkena serangan jantung ketimbang stroke.</p>

<p>People that have not experienced a heart attack or stroke in the past should not take Aspirin on a daily basis. Those that have experienced these cardiac conditions should consult their physician before assuming they should take Aspirin that often.</p>

<p>Healthy people, who use aspirin as a precaution against heart attack, might do more harm than good, warns a review study.</p>
<p>“Taking the drug daily in the belief that it is a way to protect against heart attack or stroke can lead to serious abdominal problems, including internal bleedings,” said the study published in medical journal <em>Drugs and Therapeutics Bulletin</em> (DTB).</p>
<p>Asserting that stomach bleeding could be fatal, Ike Ikeanacho, editor of DTB said, “The risks should not be dismissed. In people who have never had a heart attack or stroke, the evidence does not support them being on this drug.”</p>
<p>Pointing out that many healthy people take aspirin every day as a precaution, Ikeanacho said, though they don’t have any precise figures but it is very common for people to start themselves on aspirin.</p>
<p>“While some people would continue to take the drug no matter what the risks, most were unaware of its potentially dangerous side-effects,” he added.</p>
<p>Asking doctors to review use of aspirin with all patients they think are taking the drug, including diabetics and otherwise healthy people, the study said, those persons who have already had a heart attack can continue with the drug if prescribed by their doctor.</p>
<p>The drug is currently prescribed to those who have already had a heart attack and diabetics over 50 years of age and having blood pressure, The Telegraph reported.</p>

<description><![CDATA[I wish I knew what to write right now. For those of you who read and enjoyed the first part of Backw]]></description>

<p>For those of you who read and enjoyed the first part of <em>Backwards Marathon</em>, you&#8217;ll be excited to know that the second part will [most likely] be ready by the end of the week, if nothing distracts me.  Seeing as my Xbox 360 is dead and <em>WoW</em> is gone, I figure there won&#8217;t be any distractions at all.  This next part should be a little bit more exciting, with more dialogue.  I purposely left out a lot of dialogue in the first part because I thought that it didn&#8217;t need it, seeing as she was trapped alone in her house, and I didn&#8217;t want to portray K-One as a crazy person.</p>
<p>In other news, I&#8217;ve been sick for the past couple of days, but it seems to be getting a little better.  I fixed a cocktail of Amoxicilin, aspirin, and multivitamins yesterday, and ever since then, I&#8217;ve felt a literal boost in my immune system (along with everything else).  I figure I&#8217;ll stick with this regiment for about 9 more days, until I get everything cleared up.  I&#8217;m tired of being sick, and I honestly blame that surgery on my tailbone that I had at the beginning of the year; my immune system hasn&#8217;t been quite right since.</p>
