<p><strong>Heart Valves and it Role</strong><br />
Blood is pumped through your heart in only one direction. Heart valves play a key role in this one-way blood flow, opening and closing with each heartbeat. Pressure changes on either side of the valves cause them to open their flap-like &#8220;doors&#8221; (called cusps or leaflets) at just the right time, then close tightly to prevent a backflow of blood. Heart valve disease occurs when a valve doesn&#8217;t work right. A valve may not open all the way. Or a valve may have problems closing. If valve doesn&#8217;t open all the way then less blood moves through to the next chamber or if a valve doesn&#8217;t close tightly, blood may leak backward. These problems implies that the heart have to work harder to pump the same amount of blood. Or, blood may back up in the lungs or body because it&#8217;s not moving through the heart as it should.</p>
<p>There are 4 valves in the heart:</p>
<p>Tricuspid valve (located between the upper–right chamber (right atrium) and the lower–right chamber (right ventricle) of the heart.Pulmonary valve (located between the right ventricle and the pulmonary artery, which is the main artery that carries the oxygen–poor blood from the heart to the lungs for more oxygen).</p>
<p>Mitral valve (is located between the left atrium and the left ventricle and is the most common valve to be repaired.)</p>
<p>Aortic valve (between the left ventricle and the aorta, which is the main artery that carries the oxygen–rich blood from the heart to the rest of the body).</p>
<p>Medical datas reveals that Mitral and Aortic valves of the heart mostly requires for repair or replace. These valves are on the left side of the heart, which works harder than the right. They control the flow of oxygen-rich blood from the lungs to the rest of the body.</p>
<p><strong>What is Heart valve surgery?</strong></p>
<p>Heart valve surgery is used to repair or replace diseased heart valves. Repair means that the valve is mended, while replacement means the diseased valve is removed and a new valve is inserted in its place.There are a number of approaches physicians can take to repair defective heart valves. The best approach depends on the nature of the valve problem, and the condition of the patient suffering with valve disease.If valve damage is mild, doctors may be able to treat it with medicines. If damage to the valve is severe, surgery to repair or replace the valve may be needed. In some cases, existing heart valves can be repaired, If they cannot be repaired then they are needed to be replaced.</p>
<p><strong>What is Heart valve repair and what are the procedures?</strong></p>
<p>Valve repair can usually be done on congenital valve defects that is defects that the patients are born with and has a good success record with treating mitral valve defects.</p>
<p>Here are some procedures surgeons may use to repair a valve:</p>
<p>Percutaneous balloon valvuloplasty : This is a nonsurgical procedure to treat stenosis. In this procedure, local anesthetic is used by the cardiologist to numb the area of the body generally the groin. From where catheter( a thin flexible tube) in inserted into the blood vessel and fed all the way up to the heart. Once the damaged valve is reached, a balloon at the tip of the catheter is inflated. The inflated balloon stretches, widens and separates the narrowed valve. This procedure is suggested for patients who cannot undergo open–heart surgery because of failing health or other reasons. However in some cases there is a risk that the repaired valve may re–narrow (restenosis) or that it may no longer be able to close properly, allowing blood to leak back in the wrong direction (regurgitation).</p>
<p>Valvotomy : A valvotomy (also known as a valvulotomy) is a type of open–heart surgery in which the surgeon cuts into a valve to repair valvular damage. In this procedure narrowed valve leaflets are widened by carefully opening the fused leaflets or commissures with a scalpel.This is a procedure to strengthens the leaflets to provide more support and to let the valve close tightly. This support comes from a ring-like device that surgeons attach around the outside of the valve opening.</p>
<p>Decalcification : In this process the calcium buildup is removed from the leaflets. Once the calcium is removed, the leaflets can close properly.</p>
<p>Reshaping : Here the surgeon cuts out a section of a leaflet. Once the leaflet is sewn back together, the valve can close properly.</p>
<p>Patching : In this process the surgeon covers holes or tears in the leaflets with a tissue patch.</p>
<p>Repair of structural support : In this process the cardiologist replaces or shortens the cords that give the valves support (these cords are called the chordae tendineae and the papillary muscles). When the cords are the right length, the valve can close properly.</p>
<p><strong>What is Heart Valve replacement and what are it&#8217;s type?</strong></p>
<p>Severe valve disease means that the valve cannot be repaired, then it is need to be replaced. Valve replacement is an open-heart surgery and is mostly used to treat aortic valves and severely damaged mitral valves. It is also used to treat any valve disease that is life-threatening. . This type of procedure is typically used on heart valves that leak or on those that have thickened, hardened or stretched. Sometimes, more than one valve may be damaged in the heart, so patients may need more than one repair or replacement.</p>
<p>There are 2 kinds of artificial valves used for valve replacement:</p>
<p>Mechanical valves : These are usually made from materials such as plastic, carbon, or metal. Mechanical valves are strong, designed to last about 30 years, so they are often used if all other factors are equal. Because blood tends to stick to mechanical valves and create blood clots, patients with these valves will need to take blood-thinning medicines (called anticoagulants) for the rest of their lives.</p>
<p>Biological valves : This type of valve may be an autograft (from the patient’s own tissue), a xenograft which are made from animal tissue (a pig or cow) or an allograft or homograft (taken from the human tissue of a donated heart ). Patients with biological valves usually do not need to take blood-thinning medicines. These valves are not as strong as mechanical valves, though, and they may need to be replaced every 10 years or so. Biological valves break down even faster in children and young adults, so these valves are used most often in elderly patients.The Question that must be striking in our mind is why To Select India for such Treatments ?</p>

<p>Jantung orang dewasa kira-kira sebesar kepalan tangan orangnya dengan berat sekitar 340 gram. Semakin besar tubuh kita, maka semakin besar pula jantung kita. (Paus biru yang panjangnya 30 m dengan berat 100 ton, memiliki jantung seberat 500 kg).</p>
<p><!--more-->Setiap menit jantung kita berdenyut sebanyak 72 kali. Jika berdenyut kurang dari 72 kali selama 1 menit, maka kondisi tubuh kita sedang fit. Tetapi jika berdenyut lebih dari 72 kali maka kondisi tubuh kita sedang tidak fit atau usia kita masih sangat muda. Jika dihitung-hitung, jantung kita berdenyut sebanyak 38 juta kali selama setahun.</p>
<p>Jantung kita sangatlah menakjubkan. Coba bandingkan dengan otot bisep lengan atasmu. Cobalah kalian mengangkat beban seberat 2 kg selama 1 menit dengan cara menekuk naik turun sebanyak 72 kali. Lama-kelamaan bisep kalian akan semakin lelah bahkan bisa menimbulkan nyeri. Kemudian bagian lengan tersebut akan gemetar dan tidak mau mengangkat beban lagi untuk sementara waktu.</p>
<p>Berbeda dengan otot jantung kita, meskipun berkontraksi dan mengendur sebanyak 72 kali dalam semenit, namun jantung tidak berhenti untuk beristirahat dan tidak terlalu lelah untuk bekerja. Subhanallah.</p>
<p>Jantung bertugas untuk memompa darah ke seluruh tubuh. Dengan setiap kerutan otot jantung, darah beserta oksigen dan nutrisinya akan didorong sampai ke ujung kaki dan lengan yang jauh dan ke organ vital seperti otak dan hati. Dalam waktu 1 jam (dengan aktivitas duduk sambil membaca atau menonton TV) saja, jantung kita sudah mengedarkan sekitar 285 liter darah melalui bilik-bilik dan pembuluh darahnya.</p>
<p>Untuk lebih jelasnya, berikut penjelasan cara kerja jantung :</p>
<p>Jantung merupakan otot berongga yang memiliki empat ruang (bilik). Jalan masuk darah dari satu bilik ke bilik berikutnya dikendalikan oleh empat pintu yang disebut katup jantung.</p>
<p>Bilik atas sebelah kanan (atrium kanan) mengumpulkan darah yang sudah terpakai dan sudah kehabisan oksigen yang mengalir masuk ke seluruh tubuh. Kemudian darah turun menuju bilik yang ada dibawahnya yaitu ventrikel kanan. Ventrikel kanan berfungsi memompa darah ke paru-paru. Disini darah mengambil pasokan oksigen segar dari udara yang kita hirup.</p>
<p>Paru-paru mengirimkan darah yang sudah disegarkan itu kembali ke jantungmenuju bilik kiri atas (atrium kiri). Kemudian darah diturunkan ke bilik kiri bawah (ventrikel kiri) yang nantinya akan mengirim / memompanya ke seluruh jaringan dan organ tubuh yang sudah menunggu, termasuk dinding otot jantung.</p>
<p>Jantung bisa menjaga ritme aktivitasnya dikarenakan adanya sebidang kecil sel-sel khusus di otot jantung yang bertugas sebagai alat pacu yang membangkitkan impuls listrik untuk menjaga jantung tetap berdenyut dengan irama yang teratur.</p>

<description><![CDATA[It&#8217;s the last week of school now. Somehow, i dun really miss SP but i miss being a student. So]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p>It&#8217;s the last week of school now. Somehow, i dun really miss SP but i miss being a student. Sometimes i really wonder what will happen if i am a JC student instead. Perhaps my results will be better and i won&#8217;t be too confused about where to go next then.</p>
<p>Last sat was an eventful day. After I had my piano lesson in the morning, I met Kai and we went cityhall shopping. It&#8217;s really nice to shop with him because we will always ended up chatting and walking along the cityhall link without going into any shops. Chatting with him make me feel like a grown up and it makes me feel stressless. After that, we walked to Marina and I ate my late lunch at the foodcourt there. Didn&#8217;t really eat much since Kai and I would be going somewhere special to have our afternoon tea. To know where the special place is, you just got to read more to find out*wink*</p>
<p>Later, i received a missed call and I decided to call back to see who was looking for me. A lady picked up the phone and declared that it is the Pan Pacific Hotel calling. As usual, i introduced myself and asked if there&#8217;s anyone who called me just now. The lady interrupted my sentence before I could even finished and said something very fast that I couldn&#8217;t catch what is she saying. I repeated my question and this time round, she interrupted again and said &#8220;Sorry, we could not track the caller.&#8221; in a very irritated tone. I was kind of pissed off for her attitude but I juz said &#8220;Is okay, thanks&#8221; and I put down the phone. I was pretty upset with her service. I understand she couldn&#8217;t track the caller but must she kept interrupting my sentence and used such an irritated tone to answer me? She even raised her volume. I always thought Pan Pacific hotel is a high class hotel in singapore with excellent customer service. But is this all they could give? I went to other countries and their services are much better even though they are not as high class as Pan Pacific. They couldn&#8217;t even handle a phone call, what has it got to say about the hotel services?</p>
<p>Anw, despite the anger, I still went to Pan Pacific for the afternoon tea because I had a one for one voucher and I had already made an appointment. I suppose the hotel just called to confirm the appointment. Once i stood at the entrance, i somewhat guessed who have answered my call just now. However, I tried not to be biased cos my guess might be wrong. We waited at the entrance for a few minutes and expected someone to come and serve us. However, all the waitress just stood at the counter and stared at us as if we are out of position. (And for god&#8217;s sake, kai n I dressed quite properly. I wore a dress and high heels whereas Kai wore and collar long sleeved top with jeans and black shoes) I got irritated with their stares and I just walked in, holding kai&#8217;s hand. Then finally, a lady, i supposed is the manager, came forward to serve us with a smile and directed us to a two-seater. I would say that&#8217;s a start of good services.</p>

See the stick with a lump of white crystals above the cup of tea? That&#8217;s sugar and i seriously think is super cool. i&#8217;ve never seen it before and the quantity are just right enough. Kai and I ordered fruit tea since those are more special.</p>

<p>A tray of pastries! 2 person share!!! it consists of chicken pie, beef pie, opera cake, blueberry custard puff, chocolate cake, scones, smoked salmon sandwiches, whole meal bread with tuna sandwiches, and many many more=)</p>

<p>My favourite cake of all time. it tastes like kinder bruneo and is not very sweet, unlike the opera cake.</p>
<p>Blueberry custard puff is another favourite of mine. Although the pic doesn&#8217;t looks nice bt it&#8217;s really very very nice.</p>

<p>Together, i had 2 cups of tea and a cup of coffee(free flow) and dun see that these pastries are small, they can actually make youvery full. And with the 1 for 1 voucher, it costs us less than $30 for the both of us!</p>
<p>After the food, we rushed off to CCK&#8217;s cc to see Kai&#8217;s mum perform. His mum actually went for a singing course and that night, the students had a so called &#8220;review concert&#8221; and kai&#8217;s mum did very well! I really got to applause for her cos it was her first time singing on stage yet she presented herself well and sang with confidence. =) Here are some pics i took.</p>

<p>I told Kai&#8217;s mum that she was very beautiful that night and according to Kai, her mum was very happy and kept smiling the whole night. lol. Well, but i am not bootlicking, she&#8217;s really pretty.haha.</p>
<p>And that marks the end of my eventful day. i went home at around 11plus cos I was waiting for kai&#8217;s dad to drive me home.lol. It was really a nice day.and i hope to go back to the restaurant again!!!</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>Shortly after my previous <a href="http://paullau.wordpress.com/2009/11/13/5th-floor-design-release/">post</a> regarding the ambiguous and very internal decision-making process on the 5th Floor Atrium project, the school took a rather drastic move. Whilst not implying any correlation, here&#8217;s what I wrote on Friday last week :</p>
<blockquote><p>Now, the school finally has had the guts to show everyone the design, of what has been publicized as an atrium, in the most nonpublic way possible – Posting it on a notice board in the foyer.</p></blockquote>
<p>Shortly there after, in the middle of last week, the school choose to move the notice board from the side of the foyer to the center, blocking up two of the four entrances along the way.</p>
<p><!--more--></p>
<p>Whilst a major disruption to the heavy flow of traffic that ought to have been considered, it&#8217;s certainly a step forward in that at least more people will know about the existence of the abstract photos.</p>
<p>If the school truly intends to get our &#8216;true&#8217; opinion on this issue, perhaps it would act on these suggestions : </p>
<blockquote><p>There doesn’t seem to be a mechanism to comment or give suggestions on the design, nor did it appear that any student opinions were even involved in the creation of the design.</p></blockquote>
<p>As of now however, it&#8217;s a one way communication. No details, no consultation, no questions, just some rather obscure, abstract and vague artist impressions created solely from the thoughts of the school leadership team.</p>

<p>I love the pops of white against the mahogany paneling. Houses like this make me seriously consider putting <a title="Terrazzo at Wikipedia" href="http://en.wikipedia.org/wiki/Terrazzo" target="_blank">terrazzo</a> floors in our house, despite my association of terrazzo with elementary school buildings.</p>

<p>The white terrazzo floors, white painted wood beam ceilings, and dark mahogany paneled walls create the perfect contrast of materials and colors. The photo on the right shows the view down a hallway to a typical Eichler atrium. We have a similar (though narrower) vista from our living room to the master bedroom, also interrupted by a sculptural chair. (Our chair, however, is IKEA <a title="IKEA Vibbyn at Polyvore" href="http://www.polyvore.com/cgi/thing?id=974040" target="_blank">Vibbyn</a>, no longer available.) The location of the chair has always disturbed the sight lines in our house for me; maybe if the chair were of a smaller scale and more interesting provenance I could actually get behind it.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>It took a carefully worded email of mine to kick start a process whereby the school leadership began to consult the student council regarding the project to renovate the 5th floor, the minimum that could have been expected of them. Right through to the end of the last school year, I attempted to prod the school leadership into publicizing the project given that they had all but ignored the negative opinion on the project within the school community. With the project due to go forward despite strong opposition, I requested endlessly for more openness with regards to the project, suggestions that fell on deaf ears. During focus week, large cranes proceeded to tear down the canopy as many students have since realized.</p>
<p>Now, the school finally has had the guts to show everyone the design, of what has been publicized as an atrium, in the most nonpublic way possible &#8211; Posting it on a notice board in the foyer. There doesn&#8217;t seem to be a mechanism to comment or give suggestions on the design, nor did it appear that any student opinions were even involved in the creation of the design.</p>

<p>The design appears reasonable bar the carpet like flooring that was rejected by Student Council well ago, the lack of interior design and complete absence of chairs and tables. The refectory is built into the side, however no means of queuing appear to have been considered.</p>
<p>It&#8217;s time to at least see what the school is using our millions on. So stop by the notice board on your way in to school and see the design for yourself.</p>

<p>Bamberg &#8211; Links ist ein Regal mit Kopfkissen, rechts steht der Schriftzug „Bettwäsche für Singles“. Es ist ein ungewöhnlicher Ort für einen Vortrag zur Geschichte des Bamberger Bahnhofs, in die Karl Martin Leicht, Geschäftsführer vom Betten-Friedich, seine zahlreichen Gäste lud. „Wir feiern das 50-jährige Jubiläum unserer Räumlichkeiten und als Mitglied der Aktiven Mitte organisiere ich hier gern öfter solche Veranstaltungen“, erzählt Leicht.</p>
<p>Dann tritt der Ehrengast und Bahnhofsexperte Professor Wilfried Krings ans Katheder: „Der Bahnhofsplatz gilt heute als wenig attraktiv – noch weniger als der Maxplatz. Wenn man das Gebäude verlässt und auf dem Vorplatz steht, sieht man nichts mehr vom Weltkulturerbe.“ Doch es gab Zeiten, in denen das anders gewesen sei. „Wo heutzutage die Autos fahren, drehte früher die Straßenbahn ihre Runden“, erklärt Krings.</p>
<p>Die wechselvolle Geschichte des Bamberger Bahnhofs begann bereits vor über 160 Jahren. Schon im Jahre 1844 erhielt die Stadt einen Eisenbahnanschluss. „Das kam daher, dass Bamberg früher ein wichtiger Verkehrsknoten zu Land und zu Wasser war“, so Krings. Das Bahnhofsgebäude unterteilte man damals noch in drei Klassen. Wo früher die Angehörigen der zweiten und dritten Klasse auf ihre Züge warteten, „da gibt es heute Fast Food und Taxis“, scherzt der ehemalige Prorektor der Universität Bamberg. Auf die Frage, warum der Bahnhof so weit von der Innenstadt entfernt liegt, hat er eine einfache Antwort: „Die Verkehrsstation wurde dort gebaut, wo keine Hochwassergefahr bestand.“ Um die Haltestelle an das Stadtzentrum anzuschließen, wurde schließlich eine „Schneise“ zum Schönleinsplatz geschlagen. So entstanden die heutige Luitpold- und Willy-Lessing-Straße.</p>
<p><a href="http://thejuergen.files.wordpress.com/2009/11/bahnhof.jpg"><img class="size-full wp-image-610 alignleft" title="bahnhof" src="http://thejuergen.files.wordpress.com/2009/11/bahnhof.jpg?w=219&#038;h=254" alt="bahnhof" width="219" height="254" /></a>Für den Bahnhof in seiner heutigen Form hat Prof. Wilfried Krings hingegen wenig übrig: „Das Konzept der Stadt, den Bahnhof als Nebenzentrum zu etablieren, ist nicht nachhaltig.“ Ein Shoppingzentrum, wie das Atrium, sei kein vernünftiges Konzept. Das sei von vornherein „verkorkst“ worden. Die Stadt solle sich stattdessen auf die Funktionen der einzelnen Stadtteile konzentrieren. „Der Bahnhof ist ein Mobilitätszentrum. In dieser Funktion sehe ich Potenzial.“, so Krings. „Investitionen die mit Verkehr zusammenhängen, wie ein Fahrradparkhaus oder ein Busstation, wären vernünftig.“ Der vom Aufsichtsrat der Stadtwerke vor kurzem getroffene Beschluss, ein Parkhaus für Fahrräder und Autos zu bauen, sei ein erster Schritt in die richtige Richtung.</p>

<p>On Saturday, October 10th, the SoCal Awards were announced.  Design Line Interiors took home two awards &#8212; one in the category of &#8216;Best Interior Merchandising of a Model Home&#8217; for our interior design at Atrium Plan 2, a Davidson Communities model home located in Eastlake Vistas. </p>

<p style="text-align:justify;">With the foundation package of the main parking deck complete, we are moving forward with foundation work for the building entrance atrium area and adjacent stairwells. This location will be the primary means of access to the parking deck and elevators, escalators and <a href="http://www.cartveyor.com/" target="_blank">Cartveyor™</a><strong>.</strong> With the anchor retail establishment located on the third level of the parking deck, there was some question in regards to how to get shopping carts to-and-from the store.  This issue was resolved with the design of a conveyor system that will transport shopping carts from any level of the parking deck, to the desired location. The Cartveyor™ will be conveniently located adjacent to the escalators so that all purchases will remain in the purchaser&#8217;s sight when moving from one level to the next.</p>
<p style="text-align:justify;">In addition to atrium foundation activities, we are also conducting inspection and testing procedures which will continue throughout construction. These procedures help to ensure that quality is maintained in each area. Currently, we are testing the compaction and thickness of the dense grade of stone that was completed last week, as well soils and concrete testing.</p>
<p style="text-align:justify;">We have Inspectors from Buchanan County, ECS and S&#38;ME who ascertain diversity in quality control. Our goal is to provide Grundy an establishment that is solid and will stand the test of time; therefore, each process that goes into the building of this structure will maintain the highest level of excellence that can be offered.</p>

<p style="text-align:justify;">The building pad operation for the Grundy Town Center is now complete.  At the close of last week, crews unloaded and spread the final loads of granular (crushed stone) base.  This process required a total of over 4,000 tons of local granular material to cover in excess of a 110,000 square foot area.  </p>
<p style="text-align:justify;">Now that weather protection of the pad is in place, we are moving ahead with our next phase of operations, footing installation for the atrium and adjacent atrium stairwells.</p>
