<title><![CDATA[Moderate alcohol intake reduces BPH risk in males]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><blockquote><p><strong><em><span style="color:#008000;">Cont</span></em></strong><strong><em><span style="color:#008000;">inuing study shows that while alcohol has health risks if not used in moderation, it also has benefits.  On top of data to suggest some protection against heart and vascular disease is evidence that modest alcohol consumption of 36 grams (about 1.2 ounces) daily can significantly reduce the risk of men developing prostatic enlargement.  Prostatic enlargement, or BPH, is a common fact of life for all men as they age.  After about age 40, the prostate begins to grow again (first growth is at puberty followed by stability in prostate size until middle-age).  Prostate enlargement does not necessarily cause symptoms for all men. However, a large percentage of men will be come symptomatic over time requiring either medication, surgery or alternative minimally invasive therapy (such as laser or microwave).   Interestingly alcohol consumption did not have an impact on the occurrence of LUTS (lower urinary tracts symptoms) which can consist of frequency, urgency, nocturia (frequent nighttime urination) slowed urinary stream, incomplete emptying and hesitancy (stopping and starting of urinary stream) and several studies showed that alcohol consumption increased the incidence of LUTS.  Of course the impact of some alcoholic beverages, especially beer, on urinary frequency is well known. The studies are summarized next.</span></em></strong></p>
<p style="padding-left:30px;"><strong><em><span style="color:#008000;"><span style="text-decoration:underline;">Study Summary</span></span></em></strong></p>
<p style="padding-left:30px;"><strong><em><span style="color:#008000;">In addition to age, 9 other significant determinants for LUTS suggestive of BPH were identified. Functional bladder capacity, post-void residual urine volume, treatment for cardiac diseases, education level, antidepressant use, calcium antagonist use, erectile function or dysfunction, prostate specific antigen and family history of prostate cancer were all determinants of significance.  Not all risk factors for LUTS suggestive of BPH are accounted for.  This is true as we know that 1 of 3 men without these risk factors will still be diagnosed with LUTS suggestive of BPH between ages 50 and 80 years. Some studies have indicated that alcohol consumption is associated with a decreased risk of BPH. Associations of alcohol consumption with BPH and male LUTS were evaluated which showed that alcohol consumption is associated with a decreased likelihood of BPH but not of LUTS.  The mechanism of action by which alcohol inhibits BPH is not understood and will require additional study. Compared to no alcohol intake, alcohol intake of 36 gm daily or greater was associated with a 35% decreased likelihood of BPH (p &#60;0.001). 4 studies used LUTS as the primary outcome.  Three of the four demonstrated a significantly increased likelihood of LUTS with alcohol consumption. Total followup was 4,353 person-years. </span></em></strong></p>
<p><strong><em><span style="color:#008000;">In the end we can say that moderation in alcohol consumption is most likely beneficial in many ways, including reduction in the incidence of BPH. <title><![CDATA[Benign Prostatic Hyperplasia (BPH): Definition, Symptoms, and Treatments]]></title>

<p>The statistical likelihood of men age 50 years and older being  diagnosed with benign prostatic hyperplasia (BPH) is extremely high.  In  fact, studies show that nearly 75 percent of men 65 and older will have  a diagnosis of BPH, while others may go years with no symptoms and be  totally unaware that their prostate is growing.</p>

<p>The reason the prostate begins to grow is due to the lowered levels  of testosterone in the body and higher levels of <a href="http://www.vs2health.com/health/search/dihydrotestosterone%3Frankpreset%3Dfresh_high">dihydrotestosterone  (DHT)</a> and estradiol (estrogen), which causes the prostate  enlargement if left unchecked.</p>
<p>Some symptoms that those who have BPH suffer from are:</p>
<ul>
<li>blood in the urine due to straining to urinate</li>
<li>leakage of urine</li>
<li>recurrent, sudden urge to urinate</li>
<li>feeling like the bladder is never completely emptied</li>
<li>weak stream</li>
<li>nocturia – frequent urination during the sleeping hours of the night</li>
</ul>
<p>For some men, these symptoms are not that bad and can be tolerated.  For  others, the symptoms are unbearable and reduce the quality of the man&#8217;s  life.  In serious cases where the prostate has closed off the urethra  entirely, men may experience <a href="http://www.vs2health.com/health/search/urinary-retention%3Frankpreset%3Dfresh_high">acute  urinary retention</a> where they lose their ability to urinate  entirely.  If this occurs, immediate medical attention must be sought.</p>
<p>When minimally invasive and drug treatments have not helped control  the <a href="http://www.urologychannel.com/prostate/bph/treatment_surg.shtml" target="_blank">growth of the prostate</a>, then it is time to consider  any one of the many successful surgeries that are available.</p>
<p><strong>TURP</strong><br />
<a href="http://www.vs2health.com/health/search/transurethral-resection-of-the-prostate%3Frankpreset%3Dfresh_high">Transurethral  resection of the prostate (TURP)</a>, is the most commonly performed  surgery used to correct BPH issues.  A resectoscope, which houses a  light, irrigation tubes, and an electrical loop used to remove the  damaged prostate tissue, is inserted into the penis through the  urethra.  There is the possibility of damaging the muscles around the  bladder neck in this surgery.</p>
<p>The biggest side effects with TURP is blood in the urine for a few  days after surgery and the possibility of urinary incontinence and  impotence.  Nearly 30 percent of men who undergo TURP will experience  impotence.</p>

This procedure uses a high-energy laser to destroy the overgrown  tissue.  The minimally invasive procedure takes approximately 30 minutes  and there is little to no downtime.  Another bonus to this procedure is  that it does not have the occurrence of damaging surrounding tissue so  there are fewer side effects to worry about.</p>
<p><strong>Core-Therm</strong><br />
Core-therm uses microwave frequency to heat and destroy prostate  tissue.  A microwave antenna is inserted through the urethra by means of  a transurethral catheter.  Urinary retention is likely with this  procedure and a urinary catheter will be used for a few days in order  for the bladder muscles to regain control.</p>

Often thought to be better than TURP, HoLep also uses a laser that burns  the excess prostate tissue and seals the blood vessels to create less  bleeding.  This is a day procedure and urinary incontinence is possible.</p>

When the prostate is very large, there has been damage to the bladder,  there is cancer detected in the prostate, or no other procedures have  given long lasting results, a prostatectomy will be needed.  Done by  either an incision in the abdomen (suprapubic) or through the perineum  (perinial), this surgery can remove the entire prostate.</p>
<p>A urinary catheter is mandatory after this surgery.  If urinary  incontinence becomes a serious issue after the surgery and healing time  has commenced, an artificial urinary sphincter may need to be utilized  to control leaking.  There may also need to be a second surgery to  repair the bladder if it has dropped.  This surgery is called pelvic  organ prolapse and your doctor will inform you if this is necessary.</p>

<p><a title="Androgen" href="http://en.wikipedia.org/wiki/Androgen">Androgens</a> (<a title="Testosterone" href="http://en.wikipedia.org/wiki/Testosterone">testosterone</a> and related <a title="Hormone" href="http://en.wikipedia.org/wiki/Hormone">hormones</a>) are considered to play a permissive role in BPH by most experts. This means that androgens have to be present for BPH to occur, but do not necessarily directly cause the condition. This is supported by the fact that <a title="Castration" href="http://en.wikipedia.org/wiki/Castration">castrated</a> boys do not develop BPH when they age. On the other hand, administering exogenous testosterone is not associated with a significant increase in the risk of BPH symptoms. <a title="Dihydrotestosterone" href="http://en.wikipedia.org/wiki/Dihydrotestosterone">Dihydrotestosterone</a> (DHT), a <a title="Metabolite" href="http://en.wikipedia.org/wiki/Metabolite">metabolite</a> of testosterone, is a critical mediator of prostatic growth. DHT is synthesized in the prostate from circulating testosterone by the action of the enzyme 5α-reductase, type 2. This enzyme is localized principally in the <a title="Stromal cells" href="http://en.wikipedia.org/wiki/Stromal_cells">stromal cells</a>; hence, those cells are the main site for the synthesis of DHT.</p>
<p>DHT can act in an <a title="Autocrine" href="http://en.wikipedia.org/wiki/Autocrine">autocrine</a> fashion on the stromal cells or in <a title="Paracrine" href="http://en.wikipedia.org/wiki/Paracrine">paracrine</a> fashion by diffusing into nearby <a title="Epithelium" href="http://en.wikipedia.org/wiki/Epithelium">epithelial cells</a>. In both of these cell types, DHT binds to nuclear <a title="Androgen receptor" href="http://en.wikipedia.org/wiki/Androgen_receptor">androgen receptors</a> and signals the <a title="Transcription (genetics)" href="http://en.wikipedia.org/wiki/Transcription_%28genetics%29">transcription</a> of <a title="Growth factor" href="http://en.wikipedia.org/wiki/Growth_factor">growth factors</a> that are mitogenic to the epithelial and stromal cells. DHT is 10 times more potent than testosterone because it dissociates from the androgen receptor more slowly. The importance of DHT in causing <a title="Nodule (medicine)" href="http://en.wikipedia.org/wiki/Nodule_%28medicine%29">nodular</a> hyperplasia is supported by clinical observations in which an <a title="Inhibitor" href="http://en.wikipedia.org/wiki/Inhibitor">inhibitor</a> of 5α-reductase is given to men with this condition. Therapy with 5α-reductase inhibitor markedly reduces the DHT content of the prostate and, in turn, reduces prostate volume and, in many cases, BPH symptoms.</p>
<p>There is growing evidence that <a title="Estrogen" href="http://en.wikipedia.org/wiki/Estrogen">estrogens</a> play a role in the <a title="Etiology" href="http://en.wikipedia.org/wiki/Etiology">etiology</a> of BPH<sup>[<em><a title="Wikipedia:Citation needed" href="http://en.wikipedia.org/wiki/Wikipedia:Citation_needed">citation needed</a></em>]</sup>. This is based on the fact that BPH occurs when men have, in general, elevated estrogen levels and relatively reduced free testosterone levels, and when prostate tissue becomes more sensitive to estrogens and less responsive to DHT. Cells taken from the prostates of men that have BPH have been shown to grow in response to high <a title="Estradiol" href="http://en.wikipedia.org/wiki/Estradiol">estradiol</a> levels with low androgens present. Estrogens may render cells more susceptible to the action of DHT.</p>
<p>On a microscopic level, BPH can be seen in the vast majority of men as they age, in particular over the age of 70 years, around the world. However, rates of clinically significant, symptomatic BPH vary dramatically depending on lifestyle. Men that lead a western lifestyle have a much higher incidence of symptomatic BPH than men that lead a traditional or rural lifestyle. This is confirmed by research in <a title="China" href="http://en.wikipedia.org/wiki/China">China</a> showing that men in rural areas have very low rates of clinical BPH, while men living in cities adopting a western lifestyle have a skyrocketing incidence of this condition, though it is still below rates seen in the West.</p>

<p>HoLEP was first performed in the United States in 1998; however, has been slow to gain widespread acceptance due to its steep learning curve. The procedure has been shown to have superior short and long-term outcomes to transurethral resection of the prostate (TURP) and suprapubic prostatectomy.</p>
<p>Benefits of the HoLEP procedure include complete removal of the adenoma to the level of the prostate resulting in a less than 2% retreatment rate, lack of surgical incision, and no effect on erectile function. Furthermore, HoLEP can be performed on prostate glands of any size. HoLEP is performed as an outpatient procedure or as a 23 hour observation and the urethral catheter is removed the day after surgery. All patients experience hematuria for 1 to 2 weeks post procedure, but the need for blood transfusion is 1%. Transient urinary incontinence is common, but permanent incontinence at 1 year post procedure occurs in approximately 1% of patients. We are now offering HoLEP at Mayo Clinic in  Minnesota.</p>

<p><strong>Dr. Sigurdsson</strong> earned his doctorate in biochemistry on November 23rd, after defending his thesis, <em>Chemical composition and biological activity of some Icelandic medicinal herbs</em>. Dr. Sigurdsson&#8217;s doctorate is the result of extensive scientific research that he conducted since 1995 with guidance from Dr. Sigmundur Gudbjarnason, professor emeritus of biochemistry and former president of the University of Iceland.</p>
<h2>Important plant research </h2>
<p>Dr. Sigurdsson&#8217;s research has revealed that various phytochemical compounds contained in Icelandic medicinal herbs exhibit bioactivity. The compounds can thus be used for preventive measures and other health purposes. These research findings are particularly interesting from a historical point of view, as they shed light on the use of Icelandic medicinal herbs throughout the ages and suggest that past generations knew of the medical benefits of herbs in the Icelandic flora.</p>

Out in one of the fields amongst the cows and pecan trees I found quite a number of Opuntia spp. or Prickly Pear as they are commonly known.  Other common names include Devils Tongue, nopales and Indian Fig.  Despite their harsh and abravisive appearance I find something oddly beautiful about them.  I also love the fact that the Prickly Pear can be used both as food and as medicine.  It is so cool to me to just walk out on my property and gather wild food or medicine for my family.</p>
<p><strong>Description</strong>:  Cactus with jointed pad and sharp spines.  Fruits are normally a puplish-red when the are ripe and the flowers a bright yellow.</p>
<p><strong>Location</strong>:  Found in dry sandy soils from Mass. to Florida and Texas to Minn.</p>
<p><strong>Properties</strong>:  Analgesic, anti-inflammatory, hypoglycemic, galactogogue and anti-viral</p>
<p><strong>Historical Medicinal Uses</strong>:  Native americans would remove the spines from the pads and split them open and use the pulp as a poultice for wounds, abrasions, burns and fractures.  The peeled pads have been applied to the breast to encourage milk flow or applied to other areas for rheumatic pain.  The juice from the plant applied topically has been used historically to remove warts or taken internally for kidney stones.  Baked pads have been used for gout and  Native Americans once used a tea made from the pads for lung ailments.  Recently research has been conducted which showed that Opuntia may be beneficial in hypoglycemia, benign prostatic hyperplasia and a number of conditions affecting the urinary system.</p>
<p>Food Uses:  The plant is quiet nutritious and a good source of potassium, calcium, vitamin A and fiber.  The fruits can be used to make a tea which can be turned into jelly, a syrup or candy.  The pad can be cut up eaten raw in salads.  The seed from the plant can be ground up and used to make flour.  Sliced pads with the skin removed can be cut up like green beans and either steamed or sauteed.</p>
<p><span style="text-decoration:underline;"><strong>Recipes</strong></span>:</p>
<p><strong>Nopales on The Grill</strong></p>
<p>Prepare the cactus pads as described in the preparation section above. Once you have removed the needles, nodules and thoroughly washed the pads, they are ready for the grill. Cook each pad for approximately 10 to 12 minutes on each side. While grilling, brush each side of the cactus pad with olive oil or a flavored oil of your choice. Pepper or garlic-flavored oil are often used on grilled Nopales.</p>
<p><strong>Scrambled Nopales</strong></p>
<p>1 or 2 cactus pads</p>
<p>8 Eggs</p>
<p>1/4 lb. of cheese (your choice)</p>
<p>salt &#38; pepper to taste</p>
<p>Prepare the cactus pads as described in the preparation section above. Once you have removed the needles, nodules and thoroughly washed the pads, slice into bite-size pieces. Sauté the sliced pads in a small amount of butter for 5 minutes. Remove from the heat and set aside. Beat the eggs in a mixing bowl; add shredded cheese and the sautéed cactus pieces. Pour the egg mixture into a skillet and scramble. Serve warm with salt and pepper to taste.</p>
<p><strong>Nopales Rellenos (Stuffed Cactus Pads)</strong></p>
<p>12 tender cactus pads</p>
<p>3 cups of water</p>
<p>6 slices of Machego or Panela cheese</p>
<p>1/4 onion, thinly sliced lengthwise</p>
<p>1 clove of garlic</p>
<p>Salt to taste</p>
<p>1/2 cup of flour</p>
<p>4 eggs, separated</p>
<p>1 1/2 cups vegetable or olive oil</p>
<p>1 can of tomato sauce (12 ounces)</p>
<p>Prepare the cactus pads as described in the preparation section above. Once you have removed the needles, nodules and thoroughly washed the ads, boil in 3 cups of water with the garlic, onion, and salt. Drain.</p>
<p>On each of 6 cactus pads place a slice of cheese and 3 to 4 pieces of onion. Top with another cactus pad, secure with wooden toothpicks and coat with flour.</p>
<p>Beat the egg whites until stiff peaks form, then add the yolks and beat for 1 to 2 minutes more to create a batter.</p>
<p>Heat the oil in a frying pan, dip the stuffed cactus pads into the egg batter and fry until golden on both sides. Drain on paper towels.</p>
<p>Serve drenched with cooked tomato sauce.</p>
<p><strong>Nopales Salsa</strong></p>
<p>1 lb. cleaned cactus pads</p>
<p>1/2 lb. tomatillos</p>
<p>1 small white onion</p>
<p>2 garlic cloves</p>
<p>2 poblano peppers</p>
<p>1/2 tsp. of salt</p>
<p>2 tsp.of fresh lime juice</p>
<p>1/2 tsp.of cumin</p>
<p>2 Tbs. cilantro</p>
<p>Prepare the cactus pads as described in the preparation section above. Once you have removed the needles, nodules and thoroughly washed the pads, grill for about 7 minutes on each side. Slice the grilled pads into strips. Place tomatillos, cubed onions and garlic in a baking dish, then cook in a 450-degree oven for 20-25 minutes. Roast poblanos on grill or under the broiler, then peel them and remove the seeds. Place all ingredients in a blender and mix until well chopped. A little water may be needed to moisten the salsa. Serve chilled with chips or use to season tacos, burritos or other Mexican dishes.</p>
<p><strong>Nopales Salad</strong></p>
<p>2.2 lbs. Nopales (cactus pads)</p>
<p>1 onion, halved</p>
<p>4 cups water</p>
<p>2 Tbs. salt</p>
<p>2 large tomatoes, chopped</p>
<p>1 large onion, chopped</p>
<p>4 green chiles &#8211; serrano or jalapeno &#8211; chopped</p>
<p>Prepare the cactus pads as described in the preparation section above. Once you have removed the needles, nodules and thoroughly washed the pads, chop into bite-size pieces. Place the chopped Nopales into a pan with the 4 cups of water, halved onion and salt. Bring to a boil, reduce heat and simmer 30-45 minutes or until tender. Drain Nopales and combine with remaining ingredients. Taste and adjust seasoning, adding more salt if necessary. This dish gets better if you let is sit a few hours in the refrigerator before serving. Serves 4 or more.</p>

<p><strong>PRICKLY PEAR PICKLES</strong></p>
<p>Prickly pear</p>
<p>3 c. vinegar</p>
<p>3 c. water</p>
<p>6 tbsp. salt</p>
<p>Dill</p>
<p>Garlic cloves (1 for each jar)</p>
<p>Jalapeno peppers (1 for each jar)</p>
<p>Cut needles off prickly pear, slice and pack into sterilized jars. Put one clove garlic, 1 jalapeno pepper and sprigs of dill into jar. Boil water, vinegar and salt until salt is dissolved. Fill jars with boiling brine to within 1/2&#8243; of top. Put on cap and screw band firmly. Process in boiling water bath 10 minutes. Tastes best if let cure for at least 6 months.</p>
<p><strong>CACTUS PRICKLY PEAR JELLY</strong></p>
<p>Pick prickly pears with leather gloves on your hands. Take off spines. Rinse the fruit and place in kettle, adding enough water to cover. Boil until quite tender, squeeze through jelly bag or jelly press. To every 2 1/2 cups of juice add 1 (1 3/4 oz.) package powdered pectin and boil for a couple minutes. Then add 3 tablespoons lemon juice and 3 1/2 cups sugar. Stir often and boil hard for 5 minutes. Pour in jelly glass and seal with paraffin.</p>

<p><strong>Timing for Angiography:</strong>  A <a title="NEJM Absract page" href="http://content.nejm.org/cgi/content/abstract/360/21/2165" target="_blank">NEJM paper</a> from a group <a title="Mehta's faculty page" href="http://fhs.mcmaster.ca/ceb/faculty_member_mehta.htm" target="_blank">out of McMaster University led by Dr. Shamir Mehta</a> performed a prospective randomized trial to assess the timing of coronary angiography following presentation with acute coronary syndromes.  They compared intervention at or under 24 hours to intervention at or after 36 hours.  Although the primary outcomes did not show a significant difference, there was a significant difference in the composite secondary outcome of death, myocardial infarction, or refractory ischemia that &#8220;showed early intervention to be superior to delayed intervention in high-risk patients.&#8221;</p>
<p><strong>Timing for Insomnia Drugs:</strong> A <a title="NCBI Abstract" href="http://www.ncbi.nlm.nih.gov/pubmed/19454639?ordinalpos=3&#38;itool=Email.EmailReport.Pubmed_ReportSelector.Pubmed_RVDocSum" target="_blank">paper in JAMA</a> from <a title="Morin website" href="http://www.psy.ulaval.ca/ecole/personnel_enseignant/menu_enseignants.php?choix=Morin" target="_blank">Charles Morin&#8217;s group at Université Laval</a> showed that for</p>
<blockquote><p>&#8220;patients with persistent insomnia, the addition of medication to [Cognitive Behaviour Therapy ('CBT')] produced added benefits during acute therapy, but long-term outcome was optimized when medication is discontinued during maintenance CBT.&#8221;</p></blockquote>
<p><strong>Timing for Cataract Surgery:</strong>  Another Canadian <a title="JAMA Abstract on Medline" href="http://www.ncbi.nlm.nih.gov/pubmed/19454637?ordinalpos=4&#38;itool=Email.EmailReport.Pubmed_ReportSelector.Pubmed_RVDocSum" target="_blank">JAMA paper</a> in the May 20th issue comes from <a title="Bell webpage" href="http://www.stmichaelshospital.com/research/profile.php?id=bell&#38;navMenuID=6.2.0.0.0" target="_blank">Chaim Bell <em>et al.</em> out of St. Mike&#8217;s in Toronto</a>.  It takes a retrospective look at cataract surgeries and shows a significant association between administration of the alpha-adrenergic receptor blocker tamsulosin &#8212; given for benign prostatic hyperplasia (&#8220;BPH&#8221;) to many men in the applicable age group &#8212; and adverse ophthalmic events following cataract surgery.  Interestingly, there were no significant associations with exposure to other alpha-blocker medications used to treat BPH.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>I get extremely cross when people think that <a href="http://en.wikipedia.org/wiki/Progesterone">progesterone</a> is simply a female sex hormone – it’s not just that!  So, you can imagine what I told my male friend when he came to me for advice about his anxiety and migraines, then turned to me and said “ I don&#8217;t want to turn into a woman”!  Progesterone is commonly thought of as a ‘female’ sex hormone.  This is misleading as it is vital for sustaining not just health but life itself in all mammals of both sexes.</p>
<p>As a man gets older testosterone is converted into di-hydrotestosterone (DHT), which some believe is the cause of <a href="http://en.wikipedia.org/wiki/Benign_Prostatic_Hyperplasia">benign prostatic hyperplasia (BPH)</a> and cancer.</p>
<p>Oestrogen levels also increase as a man gets older. Oestrogen is known to stimulate cell growth. Reading between the lines, because as yet, there is no definitive study done on this, it appears to be the increased oestrogen level which is the problem and not the two testosterones. As progesterone is a powerful counter-balance for estrogen, progesterone for men is essential.</p>
<p>If in fact testosterone were the culprit, then men aged ±22 would have the highest incidence of BPH and cancer, as testosterone levels are at their highest point in the early 20&#8217;s, but of course they don&#8217;t. From the early twenty&#8217;s to the late twenty&#8217;s t<a href="http://en.wikipedia.org/wiki/Testosterone">estosterone</a> makes it&#8217;s greatest drop, thereafter it continues to decline, but at a slower rate.</p>
<p>Progesterone also down regulates the action of oestrogen if used in a sufficiently high dose. The endogenous <a href="http://en.wikipedia.org/wiki/Oestrogen">oestrogen </a>made by humans is now being supplemented by synthetic oestrogens found in the environment. They are now found in food, air, water, plastics, skin care products, no one can avoid them. Some authorities speculate this is the cause of the increase in problems such as <a href="http://en.wikipedia.org/wiki/Hyperplasia">hyperplasia </a>or cancers of any hormonally sensitive tissues, such as the prostate, endometrium, cervix and breasts. So, despite often being, erroneously, thought of as a &#8216;female&#8217; hormone progesterone for men is essential to preserve masculinity!</p>

<ul>
<li>20% of 40-year-old men</li>
<li>60% of 60-year-old men</li>
<li>90% of 70-year-old men</li>
</ul>
<p>Consider those rates of incidence carefully. How old are you? How old is your father? What are the odds that someone you know has BHP?</p>
<p>Accordingly, when was the last time that you heard anything about BHP? If you&#8217;re anything like me before I came to medical school, you may have never heard of it. Subjectively then, it&#8217;s easy to appreciate that men&#8217;s health issues are under-discussed, and need more attention.</p>
<p>BHP is non-cancerous enlargement of the prostate gland. Compare the pictures below:</p>

<p>The left shows a normal prostate. The right is a prostate with BHP. If you recall the normal prostate anatomy that was discussed in <a href="http://jbooy.wordpress.com/2008/11/04/do-you-know-where-your-prostate-is-at/">an earlier post</a>, you&#8217;ll remember that the urethra travels directly through the prostate gland, carrying urine from the bladder to the exterior. When the prostate becomes enlarged in BHP, the urethra can become compressed and narrowed.</p>
<p>The symptoms of BHP are exactly what you might expect for a narrowed urethra: difficulty voiding, slow stream, frequency of voiding, nocturia (needing to wake up in the night to void), and urgency. There can also be an increased risk for bladder infections.</p>
<p>BHP is treated in a few different ways, depending on the severity:</p>
<ul>
<li>Lifestyle changes: reducing fluid intake (particularly before bedtime), developing a voiding schedule, and avoiding diuretics like coffee and alcohol.</li>
<li>Medication: some drugs can relax the smooth muscle of the bladder and urethra for some relief of symptoms</li>
<li>Surgery: removal of the prostate and reconstruction of the urethra will permanently relieve BHP. But it comes at a cost: the vas deferens joins with the urethra in the prostate, and so men undergoing this surgery will be rendered infertile.</li>
</ul>
<p>There&#8217;s good evidence to suggest that BHP is induced by androgens (male hormones) like testosterone. Androgens bind to receptors in the prostate gland, signalling it to grow and expand. It&#8217;s a signalling/response process that takes a long time, hence why the incidence of BHP increases with age, and why most men will get BHP if they live long enough.</p>
