<p>My house has two coconut trees (<em>Cocos nucifera</em>). After seeing its condition my uncle from Madurai consulted one his friend, who has a PhD in Botany suggested some chemicals. So my uncle Mr. Jacob Francis brought them to Chennai last month. Its called Agronol, Growel Coconut Special, Manufactired by Annai Agro Chemicals, Tamilnadu. Net wieght is 1 k.g. Maximum retail price is Rs 75.00. The expiry is three years for the packed growel. It contains <strong>ferrous iron, manganese, zinc, boron</strong> and <strong>coppe</strong>r. For each tree yo need 500 gm  of  Growel which will give them the minerals for 6 months. So yearly you need  a 1 kg per tree.</p>
<p><strong>How to use it? </strong></p>
<ul>
<li>Dig a half circle near the root are.</li>
<li>Mix the chemical with 3 or for kgs of sand.</li>
<li>Now  you cover the hole with this sand Growel mix.</li>
<li>On top of it fill the with normal  sand again. pore some water.</li>
</ul>
<p><strong>Advantages of using Growel:</strong></p>
<ol>
<li>Avoids the falling of coconuts when they are too small (young)</li>
<li>Increases the number of coconuts per season</li>
<li>Increases the size of the coconuts</li>
</ol>

<p>There&#8217;s a certain quality to Geoff Rowley&#8217;s skating, or maybe more accurately to his &#8220;being&#8221; as a pro-whatever, that is highlighted when the filmer stills his hand from marking that day&#8217;s hammer and instead follows along as Rowley pushes once, twice, and diddles about with various flatground tricks like he was getting double points for those shove-its like it was his own personal bonus round. Which it may well be, and of course we&#8217;ve only a vague idea how long any particular trick takes him to land, but if you were to pick a scrawny Brit to shoulder the weight of a baggage-heavy production like &#8220;Extremely Sorry&#8221; you could do worse than a never-say-die type who won&#8217;t stop even after he&#8217;s won that particular day&#8217;s battle. </p>
<p>So you could interpret Rowley&#8217;s lead-off position in the Flip video proper in any number of ways: that it&#8217;s the second-best part, following on some unwritten rule of video sequencing that came to prominence around the time of &#8220;Misled Youth&#8221;&#8230; that it has a frontside flip late shove-it in it, which is a trick that everyone will be feverishly gossiping about and therefore it&#8217;s pointless to put it at the end of the video because everybody is gonna just fast-forward to see it anyhow&#8230; or that Rowley looks to meet head-on the challenge of following up the two past Flip videos, some six years since the last one, half the team gone, crazy diamond Shane Cross departed, Boulala jailed, and the mountain-sized expectations of a viewing public eager to write off this new generation of longhaired tween ATVs and the custom-composed rap/rock soundtrack. </p>
<p>There&#8217;s some validity to this whole mess &#8211; you take your chance going for a trilogy when half the cast checks out after the second reel, and years of anticipatory ads and deadline back-pushing and Grand Canyon base-jumps do not dull usually a video hype cycle. Geoff Rowley the human probably cares as much about all this as he does about those who would gnash their teeth over the thought of him slaying a mountain lion, or his engaging in those aesthetically displeasing truck hang-up tricks (that would include me). &#8220;Extremely Sorry&#8221; has some of his craziest shit ever, the part will not be remembered as his best, but he is still out there doing things like that wallride 5050 and that into-the-bank ollie and the rather costly rooftop maneuver, and his brand of ultra-gnarly skateboarding has never really beaten you over the head with its ultra-gnarliness. All of which oughtta be respected, whatever comes next &#8211; b/s 360 powerslide or one of those puberty-stricken gap kickflippers. Or satanic poetry, or claymation. Or drum&#8217;n'bass music.</p>

<title><![CDATA[Water Infrastructure: A Dire Need]]></title>

<p>Five years ago, when Abigail’s family moved to a subdivision in Montalban, one of the things they enjoyed the most at their new house was the splurge of strong water coming out of the faucet.</p>
<p>Developed by Sta. Lucia, the subdivision sourced water underground. It gave out water 24-7 (24 hours, seven days) and at a pressure the Metropolitan Waterworks and Sewerage System (MWSS) would have definitely approved.</p>
<p>But it wasn’t too long until the Sta. Lucia housing’s water flow came in trickles and then in drips. Three years after, water supply visited the houses just every other day.</p>
<p>Before water flow gets to dark brown from the occasionally tainted lucid water, Sta. Lucia, along with any other commercial firm, will no longer be allowed to tap water underground.</p>
<p>The National Water Resources Board (NWRB) just issued a ban on water extraction from deep wells all over Metro Manila and in all Rizal towns.</p>
<p>Of course issuance of this ban under NWRB’s amended Resolution 904 is just one thing. Enforcement of this prohibition is another.</p>
<p>And no matter how ordinary people ignore it, enforcement is obviously upon any person who drinks water or who bathes with it.</p>
<p>For groundwater exhaustion and the seeping in of untreated waste water into the aquifer and into the seas poses serious damage on human health and the environment.</p>
<p>A Department of Health (DOH) study indicated that water from deep wells in certain Metro Manila areas contain critical levels of lead, fluoride, mercury, aluminum, cyanide, arsenic, boron, manganese, and other chemicals. These harmful elements are found to harm the nervous system and disrupt other bodily functions like that of the kidney and of the reproductive system and to cause cancer. The</p>
<p>National Mapping and Resource Information Authority (NAMRIA) reported that Metro Manila, based on a Manila South Harbor survey, is sinking (also called land subsidence) as sea level has been rising at a rate of two to nine million millimeters per year.</p>
<p>Such is blamed on excessive exhaustion of groundwater. Land subsidence can cause damage to infrastructure—buildings, bridges, highways. Worse, it can cause openings or cracks on the ground.</p>
<p>Also occurring with this phenomenon is salty water’s intrusion into groundwater, deterioration of fault lines, increasing flood depths and longer time for floods to subside, and increase in high tides.</p>
<p>An alternative to the excessive use of groundwater is tapping of surface water (rivers, lakes) which is the mandate of MWSS.</p>
<p>But there is another serious water concern the Philippines has to work on—the treatment of waste water.</p>
<p>Water consumption in the Philippine is placed at a rate of one cubic meter (cu.m., 1,000 liters) per household per day, or around 15 million cubic meters per household daily or 5.48 billion cu.m. yearly.</p>
<p>With such water volume used, polluted water is pouring on Philippine rivers uncontrollably.</p>
<p>While developed countries like the United States and Japan have been incorporating infrastructures on waste water treatment into city planning, the Philippines has been taking a piecemeal approach to this.</p>
<p>It is estimated that only three to seven percent of households in Metro Manila have waste water connected to sewage treatment plant (STP).</p>
<p>As the Clean Water Act (CWA) or Republic Act 9275 mandates the drafting of the National Sewerage and Septage Management Program (NSSMP), the Local Water Utilities Administration (LWUA) is tapping a World Bank aid on this plan.</p>
<p>The CWA gave government agencies only 12 months to come up with this NSSMP. And it’s almost five years since CWA was passed in 2 004, but the actual NSSMP drafting has yet to start.</p>
<p>As the grand plan is still being awaited, an STP specialist, Engr. Andrew T. Montalbo, Eco System Technologies Inc. chief operating officer, said that integrating STPs into LGUs’ plans is a cost-effective option. This can make this common, cost-shared facility cheaper for everybody.</p>
<p>&#8221; We should go for big centralized treatment plants in cities which will have a greater regenerating impact on water flow on rivers and groundwater,&#8221; he said.</p>
<p>The CWA mandates the planning of a sewerage (drainage facilities for rainwater and household water, in general) and septage (septic tanks and sewage or human waste storage facilities) program in a national aim to treat pollution-causing water before this is thrown off to the environment.</p>
<p>Since concern for the environment is the least priority in the Philippines, pollution has already caused a tremendous strain on the environment.</p>
<p>Around 55 to 60% of waste water in the country is estimated to come from households while the rest comes from industries including agriculture.</p>
<p>On the industry side, the battle to control discharge of polluting water on rivers may already be advancing even on a slow pace, according to Engr. Jacinto S. Orcullo, Environmental Management Bureau (EMB)-National Capital Region (NCR) supervising environmental management specialist.</p>
<p>NCR only covers eight towns and cities Las Pinas, Paranaque, Valenzuela, Navotas, Malabon, San Juan, Mandaluyong, and Makati.</p>
<p>Of the 743 establishments in NCR that are potentially contributing to water pollution, only 400 have discharge permits, a permit to throw to the environment treated effluent (water discharge from known source). Of those that have discharge permits, 144 are connected to the Manila Water Magallanes STP in Makati; 42 get their waste water treated at the Rockwell STP; and nine at a Greenfield Development STP.</p>
<p>Other companies that do not have discharge permits may have pending cases with the Pollution Adjudication Board (PAB) which issues cease and desist order (CDO) on companies discharging non-compliant water. In some cases, companies issued with CDO that do not want to invest in treatment plants already shut down permanently.</p>
<p>But a serious concern now is on other non-compliant establishments that discharge not more than 30 cubic meters (cu.m.) per day like fast foods which have proliferated in Metro Manila.</p>
<p>Department of Environment and Natural Resources (DENR) is now resolving questions on whether to exempt these establishments from the water effluent standard since previous regulations exempt them from it. Because of this previous rule, DENR may still issue closure orders on these small firms. But as soon as they’re able to put up treatment facilities, they can re-operate without having to pay penalties.</p>
<p>&#8220;It will be useless to regulate the big ones if you don’t regulate those that discharge less than 30 cubic meters per day,&#8221; said a DENR official. DENR is presently amending department administrative orders 34 and 35 which include resolution on this.</p>
<p>The government specifically monitors five major water quality aspects of effluents (water discharge from known sources).</p>
<p>The first is the biochemical oxygen demand (BOD), the amount of oxygen required for the decomposition of organic matter from a pollution source which should be not more than 50 milligrams (MG) per liter. The next is total suspended solids (TSS), measure of undissolved solid particles in water such as silt, decaying plant and animal matter, and domestic and industrial wastes. TSS should not be more than 70 MG per liter. The rest are acidity which should be at 6.5 ph; color which should be at 150 units; and oil and grease, not more than five MG per liter.</p>
<p>Amid efforts on regulating water discharge in the Philippines, one glaring problem comes out prominently&#8211; the fragmentation of agencies regulating waste water in the Philippines.</p>
<p>While EMB-NCR monitors eight towns, waste water from majority of the establishments in Metro Manila and nearby regions is being monitored by the Laguna Lake Development Authority (LLDA).</p>
<p>In the provinces, local government units (LGUs) are tasked to monitor waste water from households while regional offices of the Department of Environment and Natural Resources, particularly EMB, have jurisdiction over industries.</p>
<p>Under section 8 of the CWA, LGUs and related agencies are mandated within five years to &#8220;connect sewage lines to subdivisions, condominiums, commercial centers, hotels, sports and recreational facilities, hospitals, market places, public buildings, industrial complex… and households to available sewerage systems&#8221; so that polluting water can be treated.</p>
<p>&#8220;Obviously, LGUs have not yet been taking the law seriously,&#8221; said a DENR official.</p>
<p>The government also apparently has to work out on a consolidated national report on compliance with effluent standards of industries, even those from the provinces.</p>
<p>An EMB official said that such national report may be available with the private sector&#8211; the Pollution Control Association of the Philippines or the Philippine Chamber of Commerce and Industry (PCCI).</p>
<p>The absence of such monitoring report obviously makes regulation of effluent discharge difficult. The enforcement of effluent standards in the Philippines is faced with other difficult problems.</p>
<p>While certain condominiums may be willing to put up treatment plants, some of these condominiums in Metro Manila are located in cramped areas within narrow roads where there are barely space to make way for treatment plants.</p>
<p>And with the numerous squatter communities crowding Metro Manila, one can only tell how much water pollution all of these are bringing.</p>
<p>&#8220;For squatters situated near rivers, we know where their human waste is going. But what about squatters in landlocked areas, where do you think do their feces go?&#8221; said a government official.</p>
<p>With a weak enforcement of water quality laws, it is not surprising why coliform bacteria is contaminating Metro Manila’s drinking water wells. And the Department of Health reports that 12 Filipinos, mostly children, die each day from diarrhea.</p>
<p>Moreover, the Philippines , courtesy of the Meycauayan-Marilao-Obando rivers, also gets the notoriety of being on the Blacksmith Institute’s &#8220;World’s Most Polluted Places.&#8221;</p>
<p>As of 2005, the Meycauayan River had a BOD of 119.9 MG per liter, for above what is acceptable.</p>
<p>This happened as industrial waste from tanneries, gold, and precious metals refineries, waste from lead recycling facilities and other industries, and the rest of household waste are dumped in the rivers without regard for existing laws.</p>
<p>It is unthinkable that these river systems are a source of drinking water for 250,000 people in the area. Its pollution is also a reason why nearby residents also complain of many health problems such as respiratory illnesses and headaches.</p>
<p>The lack of compliance to the country’s water standards has already left Pasig River incapable of growing aquatic resources for food while Laguna Lake’s fishes like tilapia and dalag contain heavy metals like mercury, arsenic, chromium, cadmium, and lead.</p>

These confirm the heart-healing benefit of human testosterone replacement, as  we have seen for years in practice. </span></span></p>
<p><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">These findings are in striking contrast to solo estrogen supplement, which greatly improves vasodilation and thus perfusion and hypertension, but has no demonstrable benefit on general muscle or myocardial strength and mass, or arrhythmia; but increases fat mass including in muscle. <p><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Microbiologist Dr <a href="http://en.wikipedia.org/wiki/Paul_de_Kruif" target="_blank">Paul de Kruif </a>(1890-1971) was perhaps the first to write the history of the developmental studies on testosterone <a href="http://books.simonandschuster.com/Super-T/Greg-Ptacek/9780684824529/excerpt/1" target="_blank">The Male Hormone </a>(1945) describing exciting clinical experience with it already before WW2 in the hands of medical scientists like Drs  Hammond, Werner  and Lalouche. </span></span></p>

<p><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">More obliquely, <span style="color:#3366ff;"><a href="http://www.ncbi.nlm.nih.gov/pubmed/19144729" target="_blank">Chapman, Horowitz</a> </span>ea from Adelaide Australia  have just published a small study showing  dramatic benefit of <strong> (oral)</strong> </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Testosterone and a nutritional supplement (475kcals, 9gms protein a day)  on reducing hospital admissions in undernourished older mean 77yrs  men mean BMI 19kg lean mass 43.4kg fat mass 12.8kg, and women mean BMI 18kg/sqm, lean mass 28kg, fat mass 15.8kg; thus the females especially were cachectic; </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"><em>combined supplements <strong>abolished</strong> hospital admissions over a year,  compared to 70% admission rate (only one patient had elective admissions, for separate hip replacement) without any supplements</em>.It is common cause that such oral testosterone supplement gives very shortlived spikes in serum testosterone, thus does not raise the <em>mean</em> level.<br />

<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"><strong>DURATION CONFUSION IN THE ROME TRIAL</strong>?: the </span></span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"><span style="text-decoration:underline;">Ro</span></span></span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"><a href="http://content.onlinejacc.org/cgi/content/abstract/54/10/919" target="_blank">me paper </a>by Caminiti, Rosano ea   perhaps  has a sub-editing typo:  It (the Tables)  clearly says they measured bloodlevels and cardiovascular response  at baseline and </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">at 3 months</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">after commencing <a href="www.nebido.com/" target="_blank">testosterone undecanoate injection Nebido </a>1gm  at baseline and then the  6week shot.</span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">but Nebido  1gm is generally given every 3 months (after the first 3 gms   each 6 weeks apart). This is because  it lasts for 3 months when given on a regular 3monthly basis.The definitive study by <a href="http://jcem.endojournals.org/cgi/content/full/89/11/5429" target="_blank">Schubert ea in Germany </a></span></span><span style="font-family:Times New Roman,serif;">confirmed that the </span><em><span style="font-family:Times New Roman,serif;">trough TT</span></em><span style="font-family:Times New Roman,serif;"> level over about 30months on 1gm Nebido 3monthly is about 16.3nmol/L – but a peak initially over the first 3weeks post injection to around 30nmol/L- ie rarely outside the physiological range of young men.</span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Hence the 4th shot would have been due 6months after the 1st shot.  As the Rome  abstract says, it is a 3monthly  administration long term. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">So we await clarification that Caminiti &#38; Rosano’s team  actually did the final measurements 3 months </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">after the 3rd shot</span></span></em><strong><span style="font-family:Times New Roman,serif;"> </span></strong><strong><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">ie at 6 months ( 24 or 26weeks)?</span></span></strong><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">when trough  testosterone TT level  as they report was  predictably  up from ~8 to ~18 nmol/L. Otherwise their ‘after’ results would be after only 2gm Nebido over the first 3 months. More plausibly, their rsults are in fact as they say after 6months, after exposure to testosterone  for 6months.</span></span></p>
<p style="margin-bottom:0;"><strong><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">WHAT IS “</span></span></strong><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"><strong>LONG TERM” IN A DRUG TRIAL</strong>? </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">It is puzzling how the authors could call </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">3 months</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">of replacement </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">long term</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">in their trial</span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">?</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Short term  in the context of human lifespan </span></span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">is surely </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">up to a year</span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"> </span></span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;"> (even at a mean of 70yrs age as in this cohort – average life expectancy is about 15years in a first world cohort)</span></span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">; medium term would be up to perhaps 5years; and long term surely much beyond that. The only truly longterm RCT  ever done was the 20 year UKPDS. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">But the FDA now routinely colludes with the marketing hype of drug companies to claim that even 3month trials are enough to register new designer drugs for chronic use- relying on experience, major adverse effects once the drug is in routine use to indicate if too many gullible patients are being poisoned or killed by the generally non-essential  wannabe new drug before they cancel it. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">But it is common cause that serious injuries and degeneration eg of bone, muscle, brain etc can take 2 years or more to heal, before one can no longer expect much more recovery towards optimal function.  Even 6months is very early to expect optimal improvement, heart remodeling  on conservative dose anabolic  HRT equivalent to about 8mg TT a day imi or sc, since 1gm every quarter  has been determined to be an </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">average </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">optimal dose for Nebido, maintaining the blood TT at a minimum of about 16.3nmol/L. . But it depends on individual metabolism, habits, diet, smoking, alcohol and other estrogenic intake, antiandrogens and androgen receptors.</span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">It is common cause that the magic formula for long cardiovascular life for men and women includes not just optimal diet and lifestyle but also  maintenance of <strong> optimal </strong>bloodpressure,  and optimal levels by  supplementing  abundant vitamins B,C,D,K, some vitamins A and E,  the essential minerals especially calmag, zinc, iodine, iron, chromium, selenium, manganese, boron; and  the ”co-vitamin” human biologicals that deplete from early adulthood  in particular  <a href="http://en.wikipedia.org/wiki/Omega-3_fatty_acid" target="_blank">EPA+DHA,</a> CoQ10, carnitine, ribose,  arginine, carnosine, proline, and chondroglucosamine, and all our other hormones especially  gonadal, adrenal, thyroid, serotonin, GABA; and the plant prohormone metformin. in case of resistant overweight let alone obesity or diabetes.. A new study <a href="http://www.ncbi.nlm.nih.gov/pubmed/19414638?ordinalpos=10&#38;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_DefaultReportPanel.Pubmed_RVDocSum" target="_blank">from Japan </a>also shows that metformin protects from experimental heart failure.       Even modest <a href="http://www.fasebj.org/cgi/reprint/1/5/394" target="_blank">boron supplement</a> has potent benefit in reducing excretion of and doubling serum testosterone and estradiol levels in magnesium-repleted postmenopausal women.<br />
</span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">But as endocrinologists, specialist physicians or family practitioners experience when interviewing an older patient , the likely answer the GP or specialist receives when enquiring about sex hormones – most patients will rebuff the query with “my gyne / urologist looks after that”. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">This despite the fact that most gynecologists and urologists were trained primarily as </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">surgeons</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">and in </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">reproductive organ</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">problems and symptom relief ie short term – not in the pathophysiology of all the other internal diseases and prevention and care of the aging diseases, in which internists, geriatricians, general practitioners are trained and experienced after leaving  undergraduate medical school. </span></span></p>

<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Oral HT eg premarin, progestin is rightly condemned  much beyond 10 years or age 60yrs </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">precisely because it is too risky</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">(as confirmed by the  monumentally misguided but useful $billion  Women’s Health Initiative)</span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">-</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">or for </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">sexual problems- </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">which few health professionals are both trained and comfortable to address, and about which older people seldom complain precisely because they are already so impaired by the </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">preventable </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">major aging diseases – cardiovascular, metabolic, mood, mental, muskuloskeletal, malignant etc. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Studies showing the major benefits of testosterone on heart failure and ischemic heart disease of course do not discuss the pros and cons of enterohepatic versus the alternative (parenteral) route;  and to what extent the CVS benefits of TT are mediated by the  muscle-anabolic TT itself as opposed to by its daughter metabolite estrogen – with which no muscle anabolism, strengthening has been described except obviously in the uterus itself, and in promoting vascularization and endothelium. It is sad fact that estrogen alone promotes increasing adiposity while shrinking muscle mass, and dissolving collagen in eg the perineum. This causes the paradox that while estrogen strengthens brain, perfusion, skin, hair and  mucosa, it doubles the incidence and severity of stress urinary incontinence SUI, which SERMS in turn double again; with progestin perhaps halving the degree of SUI. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Only for women, at the behest of the drug industry and their non-human sex hormones, do doctors ignore the physiology, that androgens are the parents of estrogens, with the corollary that having dominance of the daughter- estrogen (whether by being overweight, or taking estrogen or progestin, or in the perimenopause)  s<em>uppresses</em> levels and necessary anabolic benefits of androgen. They willfully ignore what is long proven, that estrogen dominance  has no direct benefit antidepressant benefit- if anything, while relieving menopause symptoms, it causes inner hostility- whereas maintaining testosterone balance relieves  depression in 2/3 of postmenopausal depression.<br />

<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">THE CRIMINAL RISKS OF </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">ANY </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">INTRAMUSCULAR INJECTION WHEN ALL THAT IS REQUIRED IS <a href="../2009/04/10/the-crucial-importance-of-route-lower-dose-of-sex-hormone-replacement-shrt/" target="_blank">SHORT NEEDLE SUBCUTANEOUS ie SC.</a></span></span><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Finally, it is a risky and potentially dangerous myth that </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">any </span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">routine injections must be  given </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">intramuscularly imi</span></span></em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">. Unless there is a rare special indication for intramuscular (or intravenous) injection,  insulin   and Nebido and all “imi” injections should simply be given into a pinched fold </span></span><em><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">subcutaneously sc</span></span></em><span style="font-family:Times New Roman,serif;"> </span><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">ie under loose skin, ideally the upper outer buttock,  even preferable to the loose abdominal wall skin around which diabetics are advised to rotate insulin injections. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">This  works as well sc  as imi -  but why advocate  the grave multiple – potentially paralysing or even fatal- risks of unwarranted  imi injection with a needle longer than about 10mm (the skin being  at most 2 – 3mm thick)  for which health practitioners regularly end up before a court martial when the monotonously predictable mishaps occur with the routine ~25mm long “imi” needle – intramuscular hematoma/abscess if not severe (sometimes permanent)  neuralgic pain or distal paralysis or gangrene from hitting a major nerve or artery, to pulmonary embolism from intravenous injection? </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">For this reason, a 25g needle usually suffices for most purposes, except  Nebido or eg Primogyn depot injection when a 23g needle is less tedious and slow since these are  suspended in a thicker oil vehicle ( than water-soluble insulin which easily goes through a 27 and even 29g needle). For usual eg depotestosterone cypionate or enanthate, or Mixogen/ Primodian Depot combinations, a 25g needle is ideal. </span></span></p>
<p style="margin-bottom:0;"><span style="font-family:Times New Roman,serif;"><span style="font-size:small;">Imagine accidentally injecting 4ml of Nebido &#8211; oil- intravenously into an older male? This cannot happen with short 23g needle subcutaneous injection into the upper outer buttock &#8211; which is painless and causes at worst a transient bump.<br />

How about deficiencies in magnesium, calcium, and boron? Together, boron, calcium, magnesium, and vitamin D help to prevent osteoporosis.(<a href="http://wholehealth4life.wordpress.com/wp-admin/#4">4</a>) </span></p>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">Consider the following fact. Sixty-eight percent of all Americans are not getting enough calcium. (<a href="http://wholehealth4life.wordpress.com/wp-admin/#2">2</a>)</span> <span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">With back and joint problems affecting greater segments of the population, it would be helpful to know some basic facts. According to the United States Food and Drug Administration, calcium is essential to strong bones. </span></p>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">Approximately 75 percent of the United States population is deficient in magnesium. Recent reports show magnesium to be beneficial in migraine headaches, depression, and chronic fatigue syndrome. (<a href="http://wholehealth4life.wordpress.com/wp-admin/#2">2</a>)</span> <span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">Properly produced magnesium chelate has reduced PMS [premenstrual syndrome] symptoms.(<a href="http://wholehealth4life.wordpress.com/wp-admin/#1">1</a>)</span> <span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">In general, magnesium helps in comfort and relaxation. </span></p>

It now appears that a more correct version of that saying should be: &#8220;You are what you absorb&#8221;. A mineral that is not absorbed cannot get into the bones to strengthen them. The amount absorbed is more important than the quantity consumed. </span></p>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">In the case of calcium, there is widespread use of inorganic forms of calcium for supplementation, such as oyster shells. Yet by properly combining [chelating] calcium with an amino acid [a component of protein] to create an organic chelate, 57 percent more replacement calcium was delivered to the bones than with inorganic calcium.(<a href="http://wholehealth4life.wordpress.com/wp-admin/#2">2</a>) </span></p>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">Chelated minerals provide 3 to 10 times greater absorption than the non-chelated ones, and are thus well worth the small additional cost. Another example is magnesium, which is absorbed 87 percent when properly chelated, but only absorbed 16 percent when taken in an inorganic non-chelated form.(<a href="http://wholehealth4life.wordpress.com/wp-admin/#2">2</a>) </span></p>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">The mineral boron may retard bone loss.(<a href="http://wholehealth4life.wordpress.com/wp-admin/#4">4</a>)</span> <span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">Since osteoporosis is occuring in larger numbers of the population, this is important news. Bones have osteoclasts that break down old or damaged bone cells, while the osteoblasts work to replace the lost bone. Osteoporosis occurs when the osteoblasts can not replace lost bone tissue as fast as the osteoclasts break it down. Osteoclasts deplete bone at a faster rate after menopause, leaving women at a greater risk of bone degredation. Boron appears to have a moderating effect on this process. </span></p>
<h2><span style="color:#3333cc;font-family:Times New Roman, Times, serif;"><strong><em>Is There A Boron &#8211; Arthritis Connection?</em></strong></span></h2>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">Over the last decade it has become common knowledge among nutritionally oriented health care practitioners that there seems to be an interesting relationship between lower selenium levels in the soil and higher incidence of breast cancer. As one looks at a map of the USA in which shaded areas represent low levels of selenium in the soil and then compares it to a map where territories with higher reported cases of breast cancer per capita are shaded, the two maps resemble each other to a great extent. </span></p>
<p align="justify"><span style="font-size:x-small;color:#3333cc;font-family:helvetica, sans-serif, verdana, arial;">A similar picture is being presented by Rex E. Newnham, Ph.D., D.O., to demonstrate a high probability for a causative relationship between the lack of the mineral boron in the diet and the manifestation of arthritis (as observed in various populations around the world). </span></p>

<li>Thirty years ago it was observed that a patient with osteoarthritis who lived in Australia in an area where the soil was deficient in boron, responded well to 6 mg. of boron a day. Within three weeks he experienced reduction of swelling and stiffness. Once he was free of symptoms for a full year without taking boron, symptoms returned. After returning to taking boron his condition improved once again within ten days.</li>
<li>Populations that live in an area where the soil or the drinking water contain high levels of boron are free of arthritis (boron intake of 910 mg./day). Low boron levels in soil or water correlate with much higher than average incidence of arthritis (boron intake of less than 2 mg./day).</li>
<li>Unpublished information from Australia reveals that thousands of people consumed boron tablets in the form of borax (a cockroach poison containing 10% elemental boron). Surprisingly the use of this product as a nutritional supplement was only promoted by word of mouth.</li>
<li>Populations in Jamaica, Mauritius and Fiji where either the soil or the diet is low in boron show a higher than average occurrence of arthritis.</li>
<li>The amount of boron in soil is directly proportional to the amount of organic matter. Therefore, highly fertilized crops (with commercial fertilizer) provide much smaller quantities of boron.</li>
<li>South African natives who ate a high boron diet had a 3% incidence of arthritis. After they moved into town and changed their diet they fell into the sad statistics of modern city dwellers who average 20% incidence of arthritis.</li>
<li>Some hot mineral spas in New Zealand where the water boron content is very high acquired the reputation of healing arthritis.</li>
<li>Animals with arthritic symptoms showed improvement with a dose of 1 mg./25 Kg. of body weight and returned to normal activity.</li>
<li>The observations which were studied suggest that boron intake at the level of 5-6 mg./day is consistent with prevention of arthritis. A higher dose may be required for treatment.</li>

<title><![CDATA[My Annual Non-Obvious Dinner, Idea #1]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p>For the past few years, I&#8217;ve co-hosted an interesting dinner with Jeff Rollins, where I ask each guest to speak briefly on a way they think the world will change in the next 5 years &#8211; that is not obvious.</p>
<p>In November I hosted 55 leaders in a wide variety of disciplines &#8211; from horticulture to economics.  The guest list included; 2 Governors, a Senator, 5 CEO&#8217;s of companies of more than $3 billion in revenue, Chief Investment Officers of more than $38 billion, 11 venture capitalists, etc&#8230;.so you get the idea.</p>
<p>(nice thing about Wilmington is that it&#8217;s halfway between Washington DC and New York, and we get Baltimore and Philadelphia even closer)</p>
<p>The 55 ideas were then voted on by each table, and below is the first of the 6 finalists &#8211; on ways the world will change that are not obvious.  <em>The only rule was no attribution without permission &#8211; but the ideas were free</em>. Please share your ideas on other non-obvious predictions and your thoughts about this one.  My next dinner is on December 8th in Wilmington &#8211; and I&#8217;m always looking for really interesting and provocative people to invite.</p>

<p><strong>Best Idea from Table I: Look before You Leap</strong></p>
<p>This is a story about the emergence of the new mass hysteria, that recognizes that hybrid vehicles — are the solution to all of our moral, ethical, political and environmental problems, and will also reduce oil import dependence.</p>
<p><span style="text-decoration:underline;">What happens if you get what you wish for?</span></p>
<p>If you look at the implications of a successful program for accelerating the commercialization of these hybrid levels, particularly to the level where by 2020 one third of all light duty vehicles, and assume that the ones which are currently closest to commercialization and about to enter the market are typical of what will mature into this new market niche, what will you find?</p>
<p><span style="text-decoration:underline;">You would find a dramatic increase in the demand for <a href="http://en.wikipedia.org/wiki/Lithium-ion_battery" target="_blank">lithium ion batteries with cobalt oxide cathode</a></span><span style="text-decoration:underline;"><a href="http://en.wikipedia.org/wiki/Lithium-ion_battery" target="_blank">s</a></span><a href="http://en.wikipedia.org/wiki/Lithium-ion_battery" target="_blank">.</a> If five million vehicles per year sold in the United States by 2020 &#8211; you would increase the US imports of refined cobalt by about twenty-five thousand metric tons, or by forty percent of current world production. <img class="alignright size-thumbnail wp-image-178" title="tesla-roadster" src="http://yet2comblog.wordpress.com/files/2009/08/tesla-roadster.jpg?w=150" alt="tesla-roadster" width="150" height="112" /></p>
<p>The only places that could increase their production at this rate are the northeast Shava region of the Democratic Republic of the Congo, the Tibetan plateau, and eastern Siberia — not all of whom are dedicated to the economic and environmental success of the United States.</p>
<p><span style="text-decoration:underline;">In addition, the electric drive train of these vehicles depends upon a class of elegant engineered allo</span>ys: hard magnets, <a href="http://en.wikipedia.org/wiki/Neodymium_magnet" target="_blank">neodymiun-boron magnets</a> that are currently used both in the existing generation of Prius, Honda Civic, Ford Escape, and all of the other hybrids. These magnets for the electric motors, regenerative braking systems, and the power-assisted steering systems rely on the production of rare earth oxides&#8230;ninety percent of which are produced in two mines, one on the Tibetan plateau and one in northwest China.</p>
<p>Last year, for the first time, the Chinese government shut down exports of rare earth oxides because domestic production exceeded demand. As a result, the production of these magnets for use in hybrid vehicles was suspended. Toyota and Honda were able to impel the Japanese government to release minerals from their national strategic stockpile. Ford, GM and Chrysler do not have quite the same relationship with their government as Toyota and Honda, and faced a little greater difficulty.</p>
<p><strong>There are alternatives that give us advanced vehicles that we hope for: plug-in hybrids, all-electric vehicles, advanced conventional hybrids, but they dont use the designs that are closest to commercialization today or the cheapest.</strong></p>
<p><strong> </strong>If we rush the market subsidizing what is currently available, we may find ourselves in the pathway of an unexpected surprise; shifting dependence on fragile and unstable areas for oil imports to a new dependence on the heirs of Mobutu Sese Seko in the Congo and HU Jintao and his allies in China.</p>

<p>According to  Bahram Arjmandi, PhD., a researcher at Florida State University,  Tallahassee, dried plums not only prevented bone loss but can also reverse it.  Preliminary results showed plums are particularly beneficial for premenopausal women–they are asked to eat 8 to 10 dried prunes a day and showed promising bone benefits.  How?  Prunes contain high amounts of  super antioxidants, polyphenols, which can restore bone mass and structure, thereby beefing up bone mineral density. Another study showed that prune’s polyphenols also enhance osteoblast (bone formation) activity.</p>
<p>But that’s not all.  Plums are storehouses of beneficial minerals and vitamins–high levels of vitamin C, vitamin B1, B2, B6, potassium, vitamin K, boron and dietary fiber.  Boron is known to prevent osteoporosis.</p>
<p>So, let’s snack on prunes and include plums in our fruit selection or toss them in our food–a natural sweet way to stay strong and bone-happy.</p>
<p>Plums are plentiful in the summer and make cool summer treats.  If you like to experiment with plums in food, try this plum salsa.</p>

<p>1.  Chopped all the above ingredients and put them in a large mixing bowl.</p>
<p>2.  Add the juice of 1/2 a lemon or 1 small lime.</p>
<p>3.  Drizzle 1 tbsp of olive oil (optional)</p>
<p>4.  Add garlic salt or regular salt (if you don&#8217;t like garlic <img src='http://s.wordpress.com/wp-includes/images/smilies/icon_razz.gif' alt=':P' class='wp-smiley' /> ) to taste.</p>
<p>5.  Toss everything together and chill before serving.</p>
<p>You can serve plum salsa with grilled meat or  with some good crunchy tortilla chips.  Good with satay and kebobs too.</p>
