Degnan</b> (Washington, DC)<br />At the outset, this book shows every promise of being a great book for a psychiatry, psychopathology, or behavioral sciences course.  It has a great layout, plenty of USMLE-style review questions (most of which are actually good), and covers a broad spectrum of topics.  Yet, here the expectations end unfulfilled.</p>
<p>The text is too long to be a review book and yet, too perfunctory and simple to be a textbook.  It lies in a middling area of inadequacy.  The authors would have done better to include more detail so as to be comprehensive and/or restructured the text to eliminate a lot of unnecessary sentences dedicated to explaining simple concepts that belittle the, no doubt, educated reader.  There are much better texts in terms of learning behavioral sciences for the Step 1 such as the BRS or High-Yield books by Barbara Fadem, these books are much more succinct and stick to the relevant material.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>Congratulations to the America people.  You have successfully elected the one person that can, and will, completely destroy the Constitution.  Thanks to electing Obama, we are losing all our freedoms one by one.  The government now controls our energy usage thanks to Cap and Trade.  The government now controls our thoughts and the expression of those thoughts thanks to the Hate Crimes legislation.  The government will soon control our choice of doctors, medical procedures, and general lifestyles thanks to the health reform bill.</p>
<p> Is this the change that you wanted America?  For all the faults that George Bush had, he wasn’t intentionally trying to destroy the foundation of this nation.  Obama is doing this on purpose.  Having grown up surrounded by communists he was doomed to at least lean in that direction.  So it is no surprise that with every situation that comes up, he looks to his communistic philosophies for the answer.  In every case, he is wrong.  Communism does not work.  Just look at the fall of the Soviet Union and the horrible conditions in Cuba as examples of how bad communism can turn out.  Even China has started to implement more capitalistic solutions to problems because the old communistic approach doesn’t work.</p>
<p> I read an article this past week about a “Constitutional Congress” meeting in Illinois.  This meeting had a lot of people with a large range of differing philosophies, but one major goal.  They want to preserve the original intent of our founding fathers.  They want to resurrect the Constitution.  They drafted a document that restricts the federal government from stealing our freedoms.  Especially our freedom of speech, religion, and gun ownership.  The document also calls the Patriot Act unconstitutional along with all these energy initiatives and the health care reform.  I agree with most of what they came up with but I don’t understand how it can make a difference.  Unless the American people start demanding that their representatives actually represent them in Washington, congress will just keep doing what they have been doing this past year.  How can we actually implement and ratify the document that was produced?</p>
<p> The only solutions to our issues are voting out the current representatives or impeaching them.  I think we need to start impeachment proceedings.  Maybe if one or two get impeached, the rest will finally start to pay attention.  I would prefer to vote them out of office, but that would mean putting up with a lot of them for another couple of years. </p>
<p> The only other possibility is the Supreme Court.  If our current justices would actually do their job, none of these pieces of legislation would make it into law.  Since every single one of these bills undermines something in the constitution, the Supreme Court needs to strike them down and declare them unconstitutional.  But they have made it abundantly clear that they do not want to hear anymore cases related to our first amendment rights.  They recently chose not to hear a case concerning a valedictorian’s right to mention God in their commencement speech.  By refusing this case, they allowed a blatant violation of an individual’s right to free speech.  This is inexcusable.</p>
<p> Our country is in a world of hurt right now and unless we act soon to take our country back from these communists, we will lose our country for good.  Come on people wake up and fight for this country and the constitution.</p>

<p>Dear Mankind,</p>
<p>I beg pardon on the tragedy last September 26, 2009, Saturday. I console you amidst the pain. I know you’re badly hurt and sad&#8230;my sympathy be with you!</p>
<p>As of now, perhaps you still irk but allow me to express my grief so you’ll understand why. On the night of Friday (Sept. 25), I was so depressed, so sad, desperate and felt worthless and uncared&#8230;and so I burst in tears. I was so hurt; it was because of you&#8230;I blame you. Yes, you curse me for sending the storm and flood&#8230;but I’m sorry, I can’t hold the temper anymore. I blame you for the pain I feel. I’ve been desperately appealing for exigent help and care but you denied. I demand you for your failures and arrogance&#8230;you’re getting to the nerve! You’re defiling my beau!</p>
<p>My hair, the forest, grows scantly and now looks awkward for a woman like me. I told you to plant another tree whenever you cut some but you didn’t. Instead, you even razed the rest&#8230;the whole wildlife and their niche. The land which is my delicate skin is now infested and filthy. You littered your dirt everywhere and bathed me with your toxic chemical wastes. You even deformed my skin and body by digging holes for quarry and mining, thus destructing the natural landscapes turning them prone to landslides. My blood (lakes, rivers, seas, oceans, et al) is now battling with sepsis due to pollutants&#8230; Whenever my hypertension strikes, it means my veins are clogged with garbage and debris. When my heart pumps rapidly due to a tickle which is the earthquake, it forms big waves and even tsunamis enough to claim lives. I have given the enough temperature according the places you belong but still unsatisfied. You keep heating your environment by burning fossil fuels and other burnable materials. My eyes are now partially blind because of the fog and smokes carrying acids. I’m partially deaf because of the uncontrolled noise pollution. I have a cough; it’s due to the inhaled domestic and industrial smokes as well gaseous chemicals. My bones, the rocks, are now flaking because of the acid rain that pours in me whenever I bathe. And when I drink water, I can taste the repugnant juice of your garbage.</p>
<p>Man, you caused my ordeals&#8230;and yours. I condemn what you did but not you because I love you. There are ways to meet the proper change and I want you to act on that. Let’s do our part&#8230;mutually. I’m so old by now. Though weak and tired, I still slowly revolve around the sun and though dizzy, I still patiently rotate so I can give you the nights and days. Sometimes, I often stumble because of my weakening body&#8230;you may feel me shook-up, remember me when there’s an earthquake&#8230;forgive me if it may bring damages to life and property.</p>
<p>Man, I have my flaw too, just like you. I can’t provide unlimited resources which you derive all your needs. Please protect your nature: the wildlife, flora and fauna, your marine resources, aerial and land resources, too.</p>
<p>I’m sorry when I can’t easily control my clouds; it results to uneven rain distributions causing La Niña and El Niño. And whenever goes beyond control, too, results to the formation of storms which are the typhoons. I mistakably pour hailstorm on the tropical areas as well cast tornadoes. My old cap, the ozone layer has its holes unfilled; I’m worried because of the UV rays. My aging bones, the rocks, are now brittle&#8230;slowly weathering. My unhealed wounds, the volcanoes, may pop their sap&#8230;be prepared.</p>
<p>I’m appealing, take good care of me&#8230;I need your help. Learn to recycle to eliminate voluminous wastes&#8230;segregate them properly and trash them at the right place. Plant more trees. Eliminate burning excessive fossil fuels and anything. Shield me from ruin. Be environmentally aware&#8230;pro-nature. I’m afraid and I don’t know if I can still live long&#8230;I still can’t imagine the time comes when I’ll be drifted away and disrupts&#8230;help me, save me! Please&#8230;</p>

<P style="text-align:justify;margin:0 0 6pt;" class="MsoNormal">The two-day workshops for the MOH and provincial health workers in Hue (November 24-25), Ho Chi Minh City (November 27-28), and Hanoi (November 30-December 1), will introduce and discuss the circular, recently approved by the Minister of Health, titled &#8220;Guidelines on the Implementation of Infection Control in Health Care Institutions,&#8221; which will take effect on December 1, 2009. Following deliberations on the new circular, health officials will develop provincial implementation plans. </P><br />
<P style="text-align:justify;margin:0 0 6pt;" class="MsoNormal">For more than 10 years, the MOH has actively worked to develop a national infection control program, beginning with its first infection control regulation set out in 1997. To ensure that the regulation remains relevant and consistent with evidence-based research, this regulation has now been carefully updated with the technical assistance of the WHO. </P><br />
<P style="text-align:justify;margin:0 0 6pt;" class="MsoNormal">This circular is a landmark achievement for the Ministry of Health in their efforts to improve the national capacity for infection control. &#8220;We recognise that hospital-acquired infections are one of the leading causes for the steep increase in disease burden and hospital costs due to prolonged hospital stay and excessive antibiotic use for patients,&#8221; said Mr Pham Duc Muc, Vice Director of the Administration of Medical Services. &#8220;This circular is an important level document which consolidates the basic for organisation, implementation and application of the general regulations on infection control in the healthcare facilities.&#8221; </P><br />
<P style="text-align:justify;margin:0 0 6pt;" class="MsoNormal">This circular will be the Ministry of Health&#8217;s driving force for ensuring health-related activities are safer and care for all patients, staff and visitors. It designates responsibilities to all healthcare departments, healthcare workers and healthcare providers for reducing of hospital-acquired infections in public and private health care facilities in Viet Nam. </P><br />

<p>It’s the most abhorrent first time in the country’s history of calamity, a tropical storm brought a devastating and enormous flood dipping many places like Metro Manila, Pampanga, Cainta, et al. The nation succumbed on the flash flood happened last September 26, 2009, Saturaday. Just 9 hours of nonstop downpour was enough to equate a month-long rain (410 mm rain gauge range, which was even higher than the normal rain gauge range). Due to the incident, it must be cursed by many for the ruin it brought, tremendously.</p>
<p>Undeniably, tropical storm Ondoy and the flash flood it brought seem as a sample of the perfect “Destroyers and Assassins”. They are envoys of destruction and death – destroying voluminous properties, taking 300 lives, and leaving numerous individuals homeless. What must be the message of these calamities?</p>
<p>Although we grieve from such tragedy, it’s not an excuse for us to ignore the lessons behind it. Really, experiencing such is detestable and once again, proved that experience is a harsh teacher&#8230;it gives the test first before the lesson. I’m sure, we learned a lot from Ondoy.</p>
<p>Why the killer flood happened is an indication of mankind’s negligence. An end justifies the means an action employed; therefore, anything we do has its connection to the outcome. It’s safe to say that we are the cause of the killer flood. Man made the massive pollution causing the world’s threatening “global warming” which contributes to the climate change and the deterioration of the ozone layer.</p>
<p>Mother Earth perhaps is calling for us. Let’s get hand in hand saving the Earth from destruction. Let’s go back to the basic – plant more trees, dispose garbage properly and let’s eliminate burning too much fossil fuel that contributes to the pollution of our land, water/marine and air biodiversity.</p>
<p>We can’t resist from the changing climate as well the uprising calamities, but we can resist wasting lives and properties by ensuring preparedness and safety. Let’s adopt the Boy Scout’s motto: “Be Prepared”. Preparedness won’t make us futile and cry over a spilled milk. The positive side of the tragedy is that it brought us lessons that we must not be too reluctant and numb of what our surrounding needs…it’s an eye-opener for us to perform our duties towards the nature with dedication. And due to the rage, it surfaced many heroic Filipinos who helped willingly their fellows who’re in dire need to save their lives. Some heroic fellows even paid life just so save the rest…flagging the sacred sacrifice, giving their life in exchange other’s safety. The spirit of sharing, concern, help, love and care flourished… awakening everyone the virtue of “Bayanihan”.</p>
<p>A calamity unexpected brings a grotesque disaster but a calamity being prepared brings less ruin and grief. So, when nature’s rage strikes, behold to its possible lessons it may bring which will serve as the mirror to check out what’s lacking in our part. Wait not for another disaster, life is too short to waste on insignificant things…your belongings; your loved ones and your life can be lost in a blink of an eye when you’re hit by nature’s rage.</p>

Any type of floor requires care, whether it be bare hardwood floors or flooring covered with carpeting or linoleum or tile. Hardwood floors can be scratched by dirt, dented by high-heel shoes, or ruined by water, so care must be taken.</p>
<p>The elegance of hardwood floors improves the appearance of practically any home, with the lustrous, vacuum cleaner,  brown or golden tones adding a touch of life to the rest of the decor.</p>
<p>When you lay your hardwood floor, you&#8217;ll find that there are many styles and colors of wood to choose from, from as to birch to cherry to hickory to maple to oak. The style of the floor also varies, from parquet, to plank, to strip. And finally, the colors vary from blonds to reds to honey to natural. So matter, vacuum cleaner,  what decor you have, you will find a type of hardwood floor to complement it.</p>
<p>Hardwood floors are durable, but they do need to be taken care of. But, vacuum cleaner,  then &#8211; so do carpets! Unlike, vacuum cleaner,  a carpet, hardwood,, vacuum cleaner,  vacuum cleaner,  floors &#8211; properly treated &#8211;  are not stained irreparably if you spill a glass of cranberry juice or something similar on them &#8211; so long as you clean it up immediately. Carpets may hide the bits of grit or sand that may come off your shoes as you enter the home, but they do dirty your carpet. However, it is slightly more serious on hardwood floors, as the finish can be scratched by ground in dirt. So its best to take, vacuum cleaner,  care to ensure that, vacuum cleaner,  this doesn&#8217;t happen. Prevention is always better than cure.</p>
<p>Mats and Rugs<br />
Mats with bristles placed outside, vacuum cleaner,  your doors will allow visitors to wipe their feet. Once they enter the house, shoes should be discarded and replaced with indoor slippers to wear instead. (This is a good idea even if you do have carpeting, as it&#8217;ll save, vacuum cleaner,  you having to vacuum more often than necessary.) </p>
<p>Furniture<br />
It&#8217;s always a good idea to place felt pads under all furniture legs, so that they may be moved about easily when you&#8217;re cleaning. Move furniture carefully so as not to gouge or scratch the floors. </p>
<p>Water<br />
Excessive water can damage hardwood floors just as it will damage, vacuum cleaner,  carpet. Hardwood, vacuum cleaner,  floors are usually treated to repel water &#8211; nevertheless any spills should be cleaned up immediately. If you have a hardwood floor in your kitchen, area rugs in front of the refrigerator (if it has an icemaker), the dishwasher, and the kitchen sink are essential.</p>
<p>Daily Care For Hardwood Floors<br />
There are no special tools needed to, vacuum cleaner,  clean your hardwood floors, vacuum cleaner,  on a daily basis &#8211; a broom works just fine. </p>
<p>Weekly Care<br />
Hardwood floors are usually waxed, or have a polyurethane coating over them. Just as with cleaning linoleum, or even furniture made of wood, you&#8217;ll want to make sure you use the proper cleaner so that the finish doesn&#8217;t yellow. Consult with the manufacturer of your flooring to learn which cleaner they recommend. If you&#8217;re going to mop, vacuum cleaner,  your floor, make sure you use a very dry damp mop &#8211; excess water can seep into seams and ruin a wood floor.</p>
<p>However, with the proper care, your hardwood floors will grace your home for a long time to come.</p>

<p><strong>That said, I have decided to divulge my &#8220;secrets&#8221; for beautiful skin.  With a background in cosmetology, yoga, herbalism and aromatherapy&#8230;not to mention a mother who had a penchant for vitamin supplementation, I now give you the full monty on how I do it.</strong></p>

<p><strong>10 pointers from a tantric yogini that will keep your face dewy, fresh and lovely all winter:</strong></p>

<div>WEDNESDAY, Nov. 25 (HealthDay News) &#8212; Thanksgiving is a time of eating turkey, gathering with family &#8212; and sometimes taking your elderly loved one to the emergency room.</div>

<div>According to a New York City physician, here&#8217;s why: When family members who have not seen each other in months gather during the holiday season, they are often alarmed by the health of their older relatives and rush them to the hospital, sometimes unnecessarily, said Dr. Tamara Kuittinen, director of medical education in the Department of Emergency Medicine, at Lenox Hill Hospital.</div>

<p>Kuittinen&#8217;s observations are not exactly scientific, but over the years she and her colleagues have seen definite increases in the numbers of elderly patients seeking care in emergency departments during this time of year. Her colleague, Dr. Carl Ramsay, associate chairman of Lenox Hill&#8217;s department of emergency medicine, said he sees a 10 percent to 20 percent increase in the number of elderly emergency department admissions during the holidays.</p>
<p>The cause is probably twofold: relatives often don&#8217;t keep up with their loved one&#8217;s health over the course of the year. So, when they get together, they are often shocked by the condition of the elderly family member. Also, Kuittinen added, children of aging parents often are not aware of how the normal aging process takes its toll on people, and they rush their loved one to a health-care provider.</p>

<p>&#8220;We need to better understand elder self-neglect – who is at risk and why – so that we can find solutions and establish appropriate policies,&#8221; said Dr. Xinqi Dong, the study’s lead author.  &#8220;This is particularly important because reports of self-neglect to social service agencies are rising.</p>
<p>&#8220;Moreover, as our aging population is rapidly increasing in size, elder self-neglect will likely become an even more pervasive public health issue.&#8221;</p>
<p>The study was based on records drawn from the Chicago Health and Aging Project, a study of 9,056 individuals 65 years of age and older who live in Chicago’s South Side. Over the 12-year course of the study, 20 percent of these seniors were reported to the Chicago Department of Aging because of concern about suspected self-neglect.</p>
<p>Elder self-neglect generally manifests itself as &#8220;a refusal or failure by the person to provide himself/herself with adequate food, water, clothing, shelter, personal hygiene, medication and safety precautions.&#8221;</p>
<p>Twice as many women as men and more than seven times the number of African Americans as whites were reported for self-neglect. Those reported, compared with those not reported, tended to be over the age of 80 and have a lower socioeconomic status. The individuals also tended to have nutritional deficiencies, medical conditions and cognitive, physical and psychological deficits.</p>
<p>However, independent of all these factors, lower levels of social well-being – specifically, limited social networks and little social engagement – were major risk factors for self-neglect.</p>
<p>The study has important implications for health and social service professionals, Dong said.</p>
<p>&#8220;Professionals who work with the elderly need to be mindful not just of their patients’ health profile, but also of their social well-being, a factor that may put them at risk of self-neglect,&#8221; Dong said.  &#8220;With social services being cut, and community and city resources lacking to help seniors, the problems of isolation can only grow worse.&#8221;</p>

That boy is hopeless.<br />
Hope is a dish best served with love.<br />
Hopelessness is a dark, lonely place.<br />
I need hope.<br />
I have no hope.</p>
<p>Hope is, in and of itself, such a simple concept isn&#8217;t it?  I mean it is all about looking forward.  A positive spirit.  A light.</p>
<p>Having hope implies a future.  It is like  a light at the end of a dark tunnel.  We use the word hope endlessly.  And actually I believe it is a most overused and bastardized word, much like love.</p>
<p>For some reason we, as a society, have a tendency to take words of depth and crush them into absolute meaninglessness, mostly by sheer overuse.  Love is the example because we love chocolate and we love teddy bears and we love chicken fried steak and we love money.  Teenagers are in &#8216;love&#8217;.  Couples think sex is love.  The divorce rate shows the truth in that.  Teen pregnancy rates show the truth in that.</p>
<p>Hope is a critically important word, like love.  There is depth and meaning, emotion.  Something &#8216;more&#8217;.  A spirit if you are so inclined.  And we hope for so many things these days don&#8217;t we?</p>
<p>I hope we have McDonalds for dinner.  I hope that cop doesn&#8217;t pull me over.  I hope I can finish this in time.  I hope my boss doesn&#8217;t get mad.</p>
<p>I hope that I don&#8217;t die in Afghanistan.  If I do, I hope my family will be able to survive and move on.</p>
<p>I hope that a heart becomes available.  Am I selfish to think that?  Someone has to die to save me.  I hope they don&#8217;t suffer.</p>
<p>I hope my legs work.  I hope I don&#8217;t have to be in a wheelchair and be taken care of.</p>
<p>I hope someone can spare me some change.  I haven&#8217;t eaten in two days.</p>
<p>I hope my dad doesn&#8217;t drink tonight.  I&#8217;m so tired.  I don&#8217;t want his &#8217;special&#8217; time any more.</p>
<p>Hurts doesn&#8217;t it?  Hurts to think we are so shallow and self absorbed.  Hurts to forget there are people in this world who are barely hanging on and need real hope.</p>
<p>So what is hope?</p>
<p>Hope is the concept of having a future.  Hope is knowing you have a present.  And hope implies &#8216;better&#8217;.  If you are not in a good place, hope is that things will improve.  Whether that means survival, less pain, full belly or something more.</p>
<p>So if you are in a good place, if you are happy, what does hope mean to you?  I mean, you could hope that it continues.  But really, isn&#8217;t that just another selfish statement.  So what does the happy person do with hope?  This is the crux.  This is the great, killer part of hope.</p>
<p>You need to share it.  You have to take your joy and send it out into the world.  You need to make your hope someone else&#8217;s.  You need to put your joy on the market for those who need it most.  Because you will stagnate without making use of that hope.</p>
<p>Hope is a spirit, no matter what your belief system may be.  Hope is the manifestation of joy.  So the two groups need each other.  The hopeless need the hopeful.  There is no hope alone.  Hope requires help.  Hope demands you make use of it.  And the hopeful need the hopeless.  Because there is an economy in hope.  In order to maintain your stash of hope, you have to invest it.  In others.  And you will receive it back in spades.</p>
<p>In this society that preaches me above all.  In this neighbourhood that requires we look away when the old man next door waves.  In this corner of town where we look down on the homeless.  In this mall where the snicker at the poor schmuck in the wheelchair.  In this school where we tease the girl with the strange bruises.  In this park where the teenage boy is drunk and we want him arrested for his anger issues.</p>
<p>Where is their hope?</p>
<p>Here&#8217;s a mirror.  Do you see it?  It&#8217;s you.  You are their hope.  You are their moment of kindness.  You are their salvation.  You are their light.  You are their love.</p>
<p>Yes, I said love.  You don&#8217;t need a bible to figure out that love is the basis of every gift.  Love makes you want to help.  If there is no love in your heart, there is no hope.  And those hopeless souls who need help, need a hand, need someone will remain.</p>
<p>Hope is the basis of society as is love.  It is the &#8216;we&#8217;.  It is the &#8216;us&#8217;.  We are one.  Unfortunately that basic truth is a lie.  Right now we are far from one.  But it sits upon each of our souls to take back that darkness.  To rip open the shroud and show the light.</p>
<p>I am you and you are me and we are all together.</p>
<p>Aren&#8217;t we?  I live in hope&#8230;</p>

Seeing that I was barely able to get to the bathroom on my own, she told the company I was not ready to enter rehab and insisted that I needed at least another day in the hospital.</p>
<p>She was right, and I was grateful for the intervention. I was in no shape to argue with insurance bureaucrats whose goal is to save money and who had no interest in, nor any way to assess, my well-being.</p>
<p>As health issues go, mine was a relatively minor concern. I now realize that in the complex world of modern medicine, nearly all patients, and especially those who are critically ill, need an advocate, someone to negotiate with medical professionals, insurers and others to ensure that they are receiving optimal care.</p>
<p>David Wayne Smith, a disability specialist at the Arizona Arthritis Center in Tucson, became an advocate for his 58-year-old son, who had been thrown from a horse and lay near death in a hospital room 500 miles away. He had several broken ribs, bruised lungs and serious breathing problems, Smith wrote in the September-October issue of Arthritis Self-Management. For three long weeks, during which there were many close calls with death, his son lay in a drug-induced coma.</p>
<p>Steeling himself against profound feelings of helplessness and fear, Smith realized he had to become part of his son&#8217;s treatment team as a patient advocate.</p>
