<p>In my Residency program, we have a 3 week rotation specifically in Musculoskeletal (MSK) things; most of this time is spent at the office of a local Orthopaedic Surgeon (&#8220;orthopod&#8221;) who does not do major surgeries but rather many physical examinations and small procedures like joint injections and aspirations. We also spend time at the cast clinic with the orthopods and plastic surgeons, practice reading plain films in radiology, and spend time with a sports medicine doc in town. This means lots of procedures!</p>

<p>Don&#8217;t let the &#8220;Internal Medicine&#8221; part of the title throw you &#8211; there aren&#8217;t a lot of General Internists who would be doing these procedures on a regular basis. More likely, Rheumatologists (a sub-specialty of Internal Medicine concerned with disease of the joints and connective tissues), ER docs, small town GPs, sports medicine specialists, and orthopedic surgeons will be the ones sticking needles into joints; and ER doctors will be the ones reducing dislocations and splinting things. However, <strong>on occasion, other types of physicians may need to aspirate a possibly septic joint on the ward, throw on a splint on something, or reduce a painful dislocation</strong> before referring a patient to another caregiver.</p>

<p>As a Family Practice resident, who may work rurally or overseas, I&#8217;m planning on being able to do some of these basic procedures on my own. That&#8217;s where the software comes in. A very simple opening screen hides a wealth of information. Behind it lies <strong>almost everything you ever wanted to know about 22 common MSK procedures, namely joint aspirations (athrocentesis), dislocation reductions, and splinting</strong> of the various upper and lower extremity bits and bobs, including:</p>

<p><strong>So, will this software do the trick?</strong> Each of the 22 procedure sections are layed out similarly, with <span style="text-decoration:underline;">Pre-Procedure</span> considerations (indications, contraindications, equipment list, and anatomy review), <span style="text-decoration:underline;">Procedure information</span> (photos for patient positioning along with a video, step-by-step instructions, and the all-important <strong>procedure video</strong> with clinical pearls), and <span style="text-decoration:underline;">Post-Procedure</span> care and complications explanation. The multimedia approach is definitely great for my &#8211; and I think most peoples&#8217; &#8211; learning styles, and it does not occur at the expense of needing an active Internet connection. That&#8217;s right &#8211; this is a totally <strong>standalone </strong>app!</p>

<p>The procedures I&#8217;ve done most recently in clinic or the ER are knee aspirations, hip dislocation reductions, and an ulnar gutter splint. Exploring these specifically, I found<strong> the software gave me a more technical and precise approach than I remember being instructed with</strong> at the bedside. However,<strong> </strong>I think <strong>the &#8220;do one&#8221; stage is where the real learning occurs</strong>, as many of these procedures depend on certain tactile feedback to know how to find the joint capsule, how much force to use on that hip prosthesis, or how to mold the splint. That said, it&#8217;s always nice to g<strong>o into a new situation armed with a bit of knowledge, or to refresh one&#8217;s memory</strong> if it&#8217;s been a long time since seeing, say, a posterior shoulder dislocation.</p>

<p>The preamble to each procedure is a quick read but covers the critical points; the integration of a little deep and surface anatomy review is a helpful feature for those in need.  It&#8217;s nice to see such attention paid to the preparation stages as well, as<strong> proper set-up can often make the success of the procedure</strong>. The videos demonstrate the important points clearly and are generally well-paced; they could easily be watched while the patient is changing for the procedure.<strong> I found the post-procedure sections might be too vague for newcomers,</strong> with statements like &#8220;monitor for infection&#8221; (are there guidelines for follow-up for this outpatient procedure?). Also, there are no reminders as to what to specify on the lab requisition or any basics about <a href="http://www.clinlabnavigator.com/Tests/SynovialFluidAnalysis.html" target="_blank">synovial fluid analysis</a> which would help guide treatment after athrocentesis.  Likewise, the follow-up for splinting is nonspecific, lacking critical information like the average duration that the splint must remain in place, or when repeat radiography should be performed.</p>

<p>This truly is a Procedural competency software; <strong>you can get the job done safely and sleekly with the clear instruction provided &#8211; you just might not know what to do afterwards!</strong></p>

Glimlachen.</strong></p>
<p>Vorige week hebben we het over lief zijn gehad. Wat hierbij ook goed helpt is glimlachen. Glimlachen naar uzelf als u voor de spiegel staat. Glimlachen en of groeten naar de mensen die u op straat tegenkomt. U zult zien dat als u vriendelijk bent u ook positieve en vriendelijke reacties terugkrijgt. Glimlach als u bijvoorbeeld kinderen leuk met elkaar ziet spelen en dit bij u een prettige herinnering oproept. Glimlachen naar onze geliefden. Als we glimlachen worden er spieren aangespannen die ervoor zorgen dat ons gezicht er ontspannen en vriendelijk uitziet, maar er worden ook stoffen in onze hersenen vrijgemaakt die ons een prettig gevoel geven! Probeert u maar eens uit: Als u merkt dat u boos of gefrustreerd wordt: glimlach! En houdt deze glimlach vast. U zult merken dat het niet mogelijk is om boos te worden! Maar ook als u voelt dat u verdrietig of depressief wordt: glimlach! Het helpt. Vaak is er geen reden om boos te worden. U kunt gewoon zeggen wat u niet bevalt en aangeven hoe u het graag wenst. U houdt het gesprek gaande en de ander zal beter bereidt zijn naar u te blijven luisteren! Boos worden levert veel stress op voor ons lichaam en veel stress is niet goed voor ons. Stress hebben we alleen nodig als er gevaar dreigt. Als u verdrietig bent omdat u een bijvoorbeeld dierbare verloren heeft, is het normaal om verdrietig te zijn. Huilen op dat moment helpt ons om ook deze soort van stress af te bouwen. Maar als u niet weet waarom u eigenlijk verdrietig bent: glimlach! Als u niet weet waarom u een beetje zwaarmoedig bent: glimlach! Het glimlachen helpt ook, door de stoffen die vrijkomen, om een keerpunt te bereiken en weer actief te worden. Glimlachen doen  we  ook als een manier van medeleven, bedankje, herkenning en erkenning. Glimlachen werkt ontwapenend en levert voordeel op. Als we gemeend glimlachen worden we bijna automatisch als een vriendelijk persoon ervaren en ontvangen. We krijgen meer voor elkaar. U heeft dit vast wel eens in de praktijk meegemaakt. Glimlachen werkt ook na. Het is niet alleen op het moment zelf als u glimlacht, dat u er voordeel van heeft. De stoffen die vrijkomen werken even daarna nog door. Het is ook een soort beloning voor ons. Immers het geeft ons een prettig gevoel. En dat willen we vaker hebben! Dus met glimlachen belonen wij onszelf ook, wat tot gevolg heeft dat we meer gaan glimlachen!</p>

<p><strong>Lief zijn.</strong></p>
<p>Lief zijn levert veel voordeel op, voor iedereen. Er heerst een prettige sfeer, iedereen is bereid tot samenwerken, men is vergevingsgezinder, men heeft meer plezier met elkaar enz. enz. Toch is lief zijn voor veel mensen niet zo vanzelfsprekend. Men vindt het moeilijk, soft, of weet niet hoe het moet: lief zijn. Lief zijn voor elkaar begint met lief zijn voor jezelf. Immers als u niet weet hoe u lief kunt zijn voor uzelf, kunt u het lief zijn ook niet uitdragen naar anderen. Hoe doet u dat? Lief zijn en wat wordt ermee bedoeld? Lief zijn betekent in dit geval: Niet zo streng zijn voor uzelf, niet zo hoge eisen aan uzelf stellen, wat milder zijn in uw oordeel over uzelf, doen wat goed voor u is, af en toe om uzelf kunnen lachen, niet altijd alles zo serieus nemen. Als u in staat bent om lief te zijn voor uzelf, bent u ook in staat om: niet zo streng te zijn voor anderen, niet zo hoge eisen aan anderen te stellen, wat milder te zijn in uw oordeel over anderen, ook iets te doen wat goed is voor een ander, ook eens kunnen lachen om die ander. Om lief te kunnen zijn voor uzelf heeft u een realistisch zelfbeeld van uzelf nodig. Lief zijn voor uzelf is niet zo moeilijk als velen denken. Ga maar eens voor de spiegel staan en zeg tegen uzelf: “Goh, meid (kerel), je ziet er helemaal niet zo slecht uit vandaag. Ik denk dat ik wel een beetje (of heel veel) van je hou en ik ga vandaag eens een leuke dag hebben.”  Ook al vindt u dat u er op dit moment niet zo goed uitziet. Bedenk: Wat zou u tegen een vriend of vriendin zeggen die vindt dat ze er niet zo goed uitziet? Als u vindt dat u iets heel stoms gedaan heeft, ga u zelf dan niet onnodig de grond in trappen. U heeft dan iets gedaan wat misschien niet zo handig was, maar dat is geen reden om over uzelf een vernietigend oordeel uit te spreken! Gebruik deze gelegenheid om erover na te denken hoe u het een volgende keer anders en meer effectief kunt aanpakken. Ook is het handiger om niet steeds te hoge eisen aan uzelf te stellen. U bent een mens, u mag fouten maken, u bent hier om te leren. Alles moet niet perfect zijn, want alles is al perfect zoals het is. Het is ons oordeel of wij vinden dat iets perfect is.  Wat voor de één perfect is, is voor een ander goed genoeg. U kunt niet aan alle eisen van uzelf en anderen voldoen. Tevreden zijn met goed, is óók goed. Probeer er een tevreden gevoel aan over te houden als u iets gepresteerd heeft!  Ook al is het nog zo klein! U heeft het goed gedaan en beloon uzelf daarvoor!</p>

<p><strong>Een stap terug.</strong></p>
<p>Het leven van alledag is tegenwoordig één grote rat-race. We vliegen van hot naar her. Alles moet gisteren klaar zijn en mensen hebben nergens tijd meer voor. Mensen worden steeds ongeduldiger. Herkenbaar? Troost u, het is niet wat alleen tegenwoordig zo voelt. Dit gevoel is van alle tijden. Onze ouders hebben hun tijd net zo ervaren zoals wij nu doen. De maatschappij is door de jaren heen ook aan het versnellen. Eerst gingen mensen te voet, toen werd de fiets uitgevonden, de auto, de trein, het vliegtuig, de raket……….En wij versnellen mee. Als u lekker in uw vel zit en u gaat mee met de flow, is dit heerlijk en kan alles niet snel genoeg gaan. Maar wat, als u die flow kwijtraakt, of niet vindt? Dan is het tijd om eens een stap terug te doen. Even uitstappen uit de trein of auto en met de fiets of te voet verder gaan. Even een stap terug doen, een versnelling langzamer schakelen. Even pauze nemen en overdenken wat u aan het doen bent en wat de reden is dat het niet gaat zoals u het graag wilt. Wat heeft u niet effectief genoeg gedaan? Wat kunt u anders kunnen doen? Heeft u invloed op de omstandigheden? Wat is uw bijdrage? Niet de oorzaken bij anderen zoeken, maar bij uzelf. Die anderen rijden wel door. U bent diegene die even niet meekan. Dat is niet erg, het is juist goed om af en toe een stap terug te doen. Het dwingt u om eens goed na te denken met wat u allemaal aan het doen bent en, wat nog belangrijker is, of u het wel allemaal op deze manier wilt! Komt u tot de conclusie dat er wat verandert dient te worden en heeft u hiervoor ook de oplossing dan kunt u weer in de auto of trein en rijdt u weer met volle vaart verder. Weet u nog niet zo goed wat u wilt of weet u nog geen oplossing? Dan heeft u altijd nog de mogelijkheid om hulp van een ander in te roepen. Vraag eens aan een ander hoe die het probleem opgelost heeft of op zou lossen! Er zijn vaak meer oplossingen beschikbaar, alleen zien we ze vaak niet omdat we er simpelweg niet aan denken! We zijn gefocust op die ene manier, zoals we het altijd al gedaan hebben, en dan helpt het soms als we de weg aan een ander vragen. Dit kan een vriend of vriendin zijn, een kennis of familielid. Weten ook deze mensen geen oplossing dan kunt u altijd nog de hulp van een professional inroepen. En heeft u uiteindelijk de oplossing gevonden, kunt u weer in de auto of trein stappen om met volle vaart weer verder te gaan met uw leven!</p>

This pioneering seed latent within you may have been dormant, hidden and undiscovered or worst, it may have been covered by layers of neglect yet it fights for life and for the light of a new day to dawn.</p>
<p>Some may perceive or label you as overly-ambitious and others may be intimidated, even jealous over your drive but what they cannot perceive or possibly know is that you must act upon your entrepreneurial instinct to press on toward new heights. For, to the entrepreneur, to do anything contrary is tantamount to being stripped of his or her potential.</p>
<p>Such is the angst of an entrepreneur; they must continuously strive to satisfy that hunger for success until something gives and the situation turns to their advantage.</p>
<p>2) You May Just Be An Entrepreneur If you are never completely satisfied or contented working for someone else: To the entrepreneur, working for someone else is a temporal, transitional obstacle at best; and an utter, unresolved disappointment at worst. Entrepreneurs make both the best and worst employees. They are some of the best employees because they have a natural proclivity toward business and their modus operandi is fueled by principles of success that tend to place them in a category unto themselves.</p>
<p>However; they make the worst employees because they know what opportunities await them and what unfulfilled dreams still vie for their undivided attention. In this respect, they are never truly passionate about another company’s product, service or vision since invariably, it is not their own. If left unresolved, those with an entrepreneurial nature become indifferent and intolerable as employees, to the point they are difficult to deal with&#8230;</p>
<p>Such is the struggle of the entrepreneur; they are like square pegs trying to be fit into society’s round mold and are never quite able to adapt. They are forced by reason of job status to make the transition to mere employee but deep down know they must heed their heart’s calling to chart their life’s course and chip out their niche.</p>
<p>3) You May Just Be An Entrepreneur If you are a perfectionist or have a low tolerance level for ineptitude, incompetence and inefficiency: Entrepreneurs are by nature, achievers. The entrepreneur’s desire to achieve goals and see their dreams come into fruition is unparalleled. When dealing with individuals who don’t seem to possess the same drive or determination, entrepreneurs become annoyed and impatient to the point of being unbearable. Since seeking perfection in all they do is the gage by which they measure and thereby, scrutinize a matter or an individual, ineptitude and incompetence are indicative of failure and they will thereby, avoid or keep at bay those exhibiting such characteristics.</p>
<p>They will spend countless hours researching opportunities, reviewing start-up business ventures and interviewing other successful people in the hopes that somehow, amidst it all, to find that single product or idea that best embodies their ambitions. They are seldom satisfied though with what they discover and their search is never quite complete until they create their own opportunity or modify an existing one to their own liking.</p>
<p>Such is the lot of entrepreneurs; they are always seeking out that perfect situation be it in an opportunity or a relationship but are never quite able to find what they’re looking for until they create it for themselves.</p>
<p>4) You May Just Be An Entrepreneur If your thoughts are constantly at work “brainstorming” and “master-minding” on ideas for helping improve existing processes or for the development of new products: In other words, the entrepreneur epitomizes the Japanese ideal of Kaizen or “continuous improvement”. When an entrepreneur-at-heart is present at a typical, company-wide or departmental brainstorming session, others sitting near will sometimes cringe with uncertainty and trepidation over what the entrepreneur among them will say or do next since, entrepreneurs are by nature, a bit unpredictable.</p>
<p>Yet, their ideas are over-the-top and revolutionary. Therefore, entrepreneurs typically visualize what others overlook or have not completely thought-through. Their imagination is constantly at work; analyzing and theorizing possible conclusions or solutions until a resolution occurs or a resignation follows. Though they are often criticized in public for their “wild notions”, it is the entrepreneur who is typically called on in private for his or her conclusions in such matters from an almost, consultative stand-point.</p>
<p>Such is the case with entrepreneurs; their minds and thought processes typically out-pace that of their peers in the discovery of new ideas or paradigms and as such, must slow down till others catch up or worst, separate themselves altogether until they do it themselves.</p>
<p>5) You May Just Be An Entrepreneur If you are constantly in “create mode”; conceptualizing new product ideas, dabbling in the arts, playing musical instruments and/or tinkering with gadgets in an effort to figure out how things work: It is no coincidence that the movers and shakers of society past such as the DaVinci’s and the Benjamin Franklin’s of their time were inventors, artists and / or proficient musicians. There is a direct correlation between the creative faculties of the mind and higher levels of intelligence. This creative stroke of genius is what beckons the entrepreneur forward in his or her quest for success and sustains them when all has not occurred according to plan.</p>
<p>Entrepreneurs by interpretation are creators; they create innovative products, they create companies from ground-floor on up to top-tier and they create opportunities for others to benefit from in terms of their effort and labor. Innumerable are society’s advancements and conveniences that could safely be attributed to the work of an entrepreneur. Their noted mark of influence is shared by all and is routinely experienced on an ongoing basis.</p>
<p>Such is the plight of the entrepreneur; their efforts often go unnoticed and worse, are accredited to others. Their crowning achievements are typically so frequent and routine that others capitalize off of their works and take credit for the entrepreneur’s efforts.</p>
<p>6) You May Just Be An Entrepreneur If you don’t accept paradigms as routine; but rather, you break them and create new ones: Paradigms are established patterns of thought that become the standard norm for how individuals function and carry out their daily duties. These paradigms are “safe-zones” for many since they represent a form of tradition that’s been accepted and adapted as standard rule. Yet, to the entrepreneur; their inquisitive mind questions the establishment, bucks up against tradition and compels them to map out a new set of rules; their own!</p>
<p>Since, the entrepreneur by instinct is a trend-setter, a rule-breaker, and in most cases, an over-achiever; he or she cannot be boxed in or made to fit into society’s mold. Rather than making the pivotal mistake of attempting to tame the entrepreneur to become society’s civilized pet, society rather needs to release the entrepreneur out into the wild where he or she can roam, rule and run their course, uninhibited.</p>
<p>Such, however is the role of the entrepreneur; to leave his or her indelible mark on society by setting certain changes in motion that usher in a new paradigm shift in forward thinking and yet, be forced in many ways to watch from a distance as well as to savor the satisfaction of what they’ve accomplished in silence.</p>
<p>7) You May Just Be An Entrepreneur If you live your life as if endowed with purpose and empowered by a higher calling from somewhere beyond… To some, the entrepreneur may seem indifferent, aloof or even a bit out-of-touch. This isn’t necessarily the case but their sense of quiet contemplation and reserve can be unsettling. Typically, people will mark them as different and place them in a category unto themselves. If the truth be told though, entrepreneurs ARE different in every sense of the word. Entrepreneurs are dreamers whose thoughts of grandeur and success keep them up long into the night and rising up early into the morning as if to seize the day. It is this desire for better things, this drive to break free from the status-quo that makes the entrepreneur stand out and easy to spot in a crowd.</p>
<p> Entrepreneurs may initially cause unease and unrest to those closest to them but it is the impact the entrepreneur leaves behind that often drastically improves circumstances for those same individuals, though entrepreneurs are seldom credited for doing so. In this regard, it could indeed be said that entrepreneurs do live their lives as if endowed with purpose and empowered by a higher calling because, by their mere actions- they break society free from the routines that have kept her at a stand-still and in doing so, usher in positive changes that improve the lives of those they’ve touched.</p>
<p>Such is the legacy of the entrepreneur; he or she may positively impact the lives of many and improve society on a routine basis but will most likely, not see the full scale of their influence nor realize the part they have played in the grand scheme of positive change they have introduced to their world.</p>
<p>In conclusion, entrepreneurs have left their indelible mark on society through the roles they have played in the advancement of technology, scientific breakthroughs and in pivotal, paradigm shifts that have helped procure new phases in our development as a civilized people. No doubt, without the impact of the entrepreneur, society may very well still be stuck in the agricultural age or worst. In the final assessment of things, it will be said by future generations that the entrepreneurs; those daring enough to do differently than what society’s expectations had confined them to- were the true heroes and heroines of their day; not the politicians, the celebrities or the CEO’s of large corporations. Though they are few and far between, society can still breathe a sigh of relief knowing that there always has and always will be the entrepreneur among us working quietly behind the scenes; contemplating development, creating improvements and causing advancement in the world to which they’ve been assigned…</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>Have you ever had those clients who ignore your wardrobe consultation.  When they show up for the family portrait session, one is in navy blue, another in green slacks with a white shirt, another in a bright print dress, and one of the kids in a shirt that says, &#8220;HOLLISTER&#8221; across the chest.</p>
<p>Here is an easy solution&#8230;</p>
<p>Since you can end up with either a high-key or low-key image, make sure you are ready to switch lighting and backgrounds appropriately.  As to the clothing here is what I&#8217;ve done.  I&#8217;ve hit the thrift stores looking for various sizes of what blouses and white men&#8217;s shirts for those high-key situation.  For the low key, I have bought a bunch of  black crew-neck shirts in all sizes.  I watched for places like Wal-Mart to have them on sale.  I have also purchased many black tops from thrift stores.  Thus, I can put my clients in black or white tops depending on whether we want high- or low-key.</p>
<p>What do I say to the client?</p>
<p>I educate them.  I show them a photo where someone sticks out like a sore thumb and say, &#8220;This is why I would like to ask you to put this shirt on. &#8221; </p>
<p>Because many people are visual, they will say, &#8220;Oh, I see!&#8221; and sometimes they will even comment on what a good photographer you are for noticing stuff like that.</p>
<p>If you have the budget, and the space, you can expand by adding slate blue button up shirts, or other colors of shirts.  When you figure you can get a crewneck for a few bucks on sale or a button up shirt for a couple of bucks at a thrift store, you can get quite a collection of sizes for under $50.</p>
<p>I even bought a whole range of children&#8217;s sizes of bib-overalls for $50.  So, if I have a family  of grade-school aged kids, I can put them all in bib-overalls and not worry so much about the shirts.</p>
<p>Remember, have a &#8220;Plan B&#8221; for the times when they do not listen.</p>

<p>vi chiedo scusa se non riesco a scrivere con regolarità continua e soprattutto giornaliera. ciò non dipende né da mancanza di argomenti da trattare né tantomeno della voglia. l&#8217;unico problema è il tempo, di ciò ve ne chiedo scusa promettendo che cercherò di essere il più regolare possibile.</p>
<p>detto questo si può iniziare.</p>
<p>il lodo Alfano è stato bocciato dalla Consulta.</p>
<p>la legge è ritornata ad essere uguale per tutti.</p>
<p>ciò che trovo insopportabile e inammissibile è una maggioranza e un Presidente del Consiglio che non ammette le regole della democrazia, che di fronte a qualsiasi impedimento alla sua strada predeterminata e preconfezionata non riesce ad essere quanto meno istituzionale.</p>
<p>mi tornano in mente le parole dell&#8217;on Schultz: &#8220;quello che mi è chiaro e che è difficile accettare è che un Presidente del Consiglio europeo nell&#8217;esercizio delle sue funzioni quando deve affrontare il minimo contraddittorio possa perdere il contegno in questo modo&#8221;</p>
<p>già, perchè da mezzogiorno in poi e appena uscita la sentenza i suoi colonnelli si sono lasciati andare: &#8220;porteremo il popolo in piazza&#8221;, &#8220;faremo la guerra&#8221;, &#8220;è una sentenza politica&#8221;. ecco che poi interviene il diretto interessato che riesce ad attaccare chiunque, la Corte Costituzionale, il Presidente della Repubblica, la stampa, la televisione, la magistratura e anche l&#8217;on. Bindi la sera a Porta a Porta, insultandola sul piano personale definendola &#8220;più bella che intelligente&#8221; e dando il via a castelli per il suo &#8220;zittella e petulante&#8221;.</p>
<p>trova poi li stesso la soluzione a tutti i problemi in Italia: e annuncia festante &#8220;viva Berlusconi&#8221; e si denomina &#8220;san Silvio da Arcore&#8221;.</p>
<p>ciò che mi lascia perplesso è tutto questo.</p>
<p>quando una Istituzione non fa atro che invocare la maggioranza e il consenso popolare, intesi ben diversamente dai concetti di Rousseau e Locke, come unico mandante si rischia di assumere sempre più tratti dittatoriali.</p>
<p>e badate bene che non ho mai scritto o detto parole simili, ma sempre più mi tremano le gambe.</p>
<p>un Presidente del Consiglio non può permettersi di definire il Presidente della Repubblica, non può permettersi di infangare la Corte Costituzionale, non può permettersi di insultare la stampa, le televisioni, la satira, gli onorevoli.</p>
<p>un Presidente del Consiglio dovrebbe istituzionalmente accettare la sentenza con rispetto ed avere un minimodi rispetto delle leggi e della Costituzione, ciò che limita il potere del sovrano, primo scudo dei cittadini per la difesa delle libertà.</p>
<p>questo veniva scritto nel 1700 da Montesquieu.</p>
<p>io non chiedo né dimissioni né Governi tecnici. e non per disciplina di partito, ma perchè la Politica non deve perdere di vista i cittadini e il bene comune. E se ci fosse un Governo tecnico o delle elezioni anticipate i cittadini non riuscirebbero a capire il perché.</p>
<p>Ora c’è bisogno di tutt’altro</p>
<p>e quindi Berlusconi inizi una buona volta a pensar alla <em>res pubblica</em>,la smetta di fare della sua persona un Messia, non vada in televisione, sui giornali e in radio a fare comizi disdicevoli e vergognosi come quello fatto a porta a porta ieri sera, la smetta di scopare la sera e inizi ad andare in Parlamento, tentare di aiutare chi non arriva a fine mese, e farci uscire da questa crisi.</p>
<p>perchè, ma forse non lo ha sentito, impegnato nell&#8217;attaccare chiunque, il Presidente della Banca d&#8217;Italia oggi a Venezia certo non ha descritto un&#8217;Italia florida e sana. &#8220;la ripresa è lunga e fragile&#8221; così ha detto.</p>
<p>che Berlusconi lavori per questo. per tutti, e non solo per se stesso. </p>
<p>che, infine, presti i suoi avvocati a Dell&#8217;Utri, accusato da Giuffrè, un pentito, di essere il tramite tra Forza Italia e la mafia.</p>

<p>Waking up to sheep staring back at you, this is Shetland, souls are searching and seals are wailing. I&#8217;ve never seen a setting quite like this, feeling cold, yet the warmth of the people I&#8217;ve met so far &#8211; out weighs that, talk about a culture shock, although its all oddly intriguing.</p>
<p>Trying to set a good routine in all aspects; praying, deepening, consulting, reflecting, teaching, exploring and swimming? I&#8217;m finding that this learning is a gradual change, blogging from a converted church-library, bizarre, but the Internet&#8217;s still the same.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>I&#8217;ve invested a number of hours attending to technical and communication needs for my new volunteer team.  I LOVED setting up the Google site and I loved setting up the polldaddy.com poll to identify our contracting questions.  I wonder if all these Web 2.0 efforts are tiresome or overkill for things that could happen as easily with thoughtful communications.  I mean, it *is* a group of eight people.  Can it be that hard for us to all just communicate with an email, and work it out from there?  My resistance to managing by email is high.</p>
<p>I am finally taking some time to read Flawless Consulting.  I actually have a day away from my Intrepid work (well, actually its been a week), and I&#8217;m hopping on the opportunity to keep myself soaking up whatever I can from the wealth of resources that CCP provides.  This book is a goldmine.</p>
<p>In reading just ten pages, Block has emphasized the importance of feelings, trust, shared responsibility and establishing my own needs along the way.  He says:  &#8220;The consultant needs to constantly keep in mind how much the client is owning feelings versus talking as if just and observer of the organization&#8221;(p. 15).  First, the absence of this kind of awareness of feelings in my work is something that I have recently discovered, and stems from my recent discoveries of this lack in my personal relationships.  What?  I don&#8217;t recognize and acknowledge feelings clearly and openly?  To do this with a client feels a little foreign, but the opportunities by doing it seem palpable for the first time.  It does mean not counting so much on my intuition and vocalizing what I&#8217;m hearing, seeing and feeling.</p>
<p>I&#8217;m also wondering what I might do with Block&#8217;s assertion that I can begin by solidifying my assumptions about effective organizations and how &#8220;our own consulting behavior should be consistent with the style of management we advocate to our clients&#8221;(p.17).  What are my assumptions about what good management ought to be and how do my expectations of client impact my beahavior?  Really?</p>
<p>Transparency of strategic and tactical vision, Servant Leadership, Honesty and Integrity, Affirmative Coaching, Supporting collaboration</p>
<p>And at a more technical level?  I have assumptions about how teams develop software or websites.  I have have assumptions about how small non-profits function well together (but those are old and I could apply some time into thinking through those).  I also have assumptions around how consulting teams work together, and ID teams work together.   And IT teams, and client services teams.</p>
<p>I&#8217;m reflecting a lot on my first consulting engagement with Slalom.  I was mentored heavily, but also had a great deal of autonomy and made a number of things happen.  I was empowered to do many things and the objectives were really clear.  I do feel like I would do it very differently, and see the ways in which I was functioning from a fearful place, rather than engaging with the client and developing a collaborative effort with them.  I wish that I had somehow proven some value that would have kept me involved with them.  There was a great range of needs there that I would have enjoyed working with.  A marketing team that was developing a training/on-boarding program.  Interesting problems!</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>It has been reported that the International Monetary Fund is supporting ousted Honduran President Manuel Zelaya by suspending any monetary assistance after Zelaya was removed by military coupe and told to leave the country&#8230;Zelaya who has since returned to Honduras and is residing at the Brazilian embassy; is negotiating with the United States who has decided to withdraw from sending aid to Honduras after the current interim government took power by a vote from its own congress who voted to oust Zelaya after he wanted to ratify the constitution which controls the number of terms an elected leader can serve. Mr Zelaya claims he only called for a constituency hearing to vote on the matter to see if the people would support a ratification to Hondura&#8217;s constitution. </p>
<p>But with Zelaya now claiming that Israeli mercenaries are plotting to assassinate him (with no evidence to support it) and that he was being subjected to mind altering gases and sound waves being emitted from something that looks like a satellite dish. It is enough to make one wonder if Zelaya is a credible person.  Could this whole scenario be taking place in order to destabilize Honduras so that the wealthy elite could take over the whole government? If people were setting out to kill Zelaya, wouldn&#8217;t they have done it already? Is the story about Israeli mercenaries really an indirect reference to the State Of Israel and an attempt to discredit Israel&#8217;s legitimacy and right to exist?  As if they needed more problems.</p>
<p>Mr. Zelaya has not really given any reason why we should believe that Israelis are behind anything in his country and this makes him all the more controversial and sound like a right wing conspiracy taken from the headlines of a newspaper that had existed in the former Nazi Germany of the late 1930s&#8230;What makes things more interesting is a story circulating about the coupe against Zelaya was orchestrated &#60;a href=&#34; <a href="http://www.infowars.com/honduras-military-coup-engineered-by-two-us-companies/">&#8220;&#62;between the CIA and two Private corporations, particularly fruit growers like Chiquita and Dole.</a></p>
<p>The IMF and World Bank&#8217;s decision to suspend monetary transactions could be a normal routine that would occur with any foreign government that is experiencing a shift in its policies or is not operating in a democratic manner, and could only be to reflect the <a href="http://www.google.com/url?sa=t&#38;source=web&#38;ct=res&#38;cd=3&#38;ved=0CBMQFjAC&#38;url=http%3A%2F%2Fwww.nytimes.com%2F2009%2F09%2F04%2Fworld%2Famericas%2F04honduras.html&#38;ei=587ESqmsJ8aplAevip2SAw&#38;usg=AFQjCNGdoe97P_e1l2ZaQGVnRrD2ogWAUQ&#38;sig2=DCIP7CZgBLwANTRBXU5vuA">U.S. State Departments decision to withhold aid. </a> The World Bank headquarters is located in Washington D.C.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>I&#8217;m reading the Ten Step Consulting process by Geoff Bellman and wondering which steps I am good at and where I could improve.  My initial reaction is that I could improve in the area of presenting information to the client.  In the &#8220;feedback&#8221; and &#8220;alternatives&#8221; steps.  I think I tend to overwhelm with possibilities (myself and the client).</p>
<p>I am thinking of Connie and the project charter that I provided her and how I wavered after I delivered the goals to her.  I got so deep into the charter and the precision of it, it was hard for me to stay at a 30,000 foot level to hear her reaction.</p>

<description><![CDATA[On Wednesday morning we got up very early and we were on the road by 6am.  My Father, my Mother and ]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p>On Wednesday morning we got up very early and we were on the road by 6am.  My Father, my Mother and myself.  We were finally on our long awaited journey.  We were on our way to the radiology oncologist consult at the cancer center in New Jersey.  The drive was probably about 1 1/2 hour long and our appointment was at 8:30 but I knew my Dad was nervous and I wanted him to be extra early so that he wouldn&#8217;t have more stress and could be there and just settle in.</p>
<p>As I stated in my last <a href="http://fightingoffcancer.wordpress.com/2009/09/09/dinner-with-dad/" target="_blank">post</a> he didn&#8217;t look well and hadn&#8217;t been eating right, so I am becoming even more concerned about him.  We had all our eggs in one basket with this consult, we needed this, we were depending on this, we couldn&#8217;t move forward without it.</p>
<p>We were of course very early so we had a good hour to sit and relax.  We received a pre-packet with a ton of forms to fill out before even entering the building, but there was more waiting for him, so that kept his mind busy for a while and helped us pass the time.  We were called into the doctors office right on time.  The doctor&#8217;s nurse asked my Dad a lot of very important health questions, and then the doctor came in.  She went over his medical history, did a full examination and then sat down to talk to us.  She said what we had known and heard already, words like&#8230;very rare, MAC, sweat glands,  not a lot of case  history.  She had told us that she believed he might have success with radiation therapy.  There is not enough evidence (case study) to substantiate it not being successful.  She also said that after he does the course of radiation therapy that it is likely a follow up surgery will  be needed to make sure the radiation therapy did indeed get the cancer.  She explained in great length and detail about the side effects and what to expect with the radiation treatments.    There was a lot of information to take in.</p>
<p>The doctor told us that she believed the cancer is not deep but wide, which in her words is a good thing because it is easier to treat. This has to do with where the cancer is (in his cheek) there is not a lot of wiggle room so to speak which traditionally makes treatment more difficult.  We were very happy to hear that.  My Dad was very happy to hear that news.  Rightly so the re-constructive surgery he has already had done on his cheek has effected him emotionally.  This news he later told me was the best news of the day for him.  That made me happy as well.  Any one word, any one sentence, that made him happy, made me happy.</p>
<p>She said that she is going to take all her information, what she thinks the course of action for my Father should be, and bring it to the doctor conference.  She explained that the doctor conference is when they all sit down together and discuss patients to bring together all their experience and knowledge to give the patient the best possible treatments available.  We have the results of that conference with the other top notch doctors at the cancer center within 7-10 days.</p>
<p>7-10 days and our journey will have purpose.  7-10 days and our journey will once again begin again.  7-10 days and we will have a definite course of action.</p>
<p>After leaving the cancer center we went and enjoyed lunch at a local diner.  We sat down quiet for a while, then began to dissect the morning consult.  The result left us with far more answers this time than questions.  My Dad was very happy.  He had a ray of hope now.  I believe his mind had been beginning to wander off a bit the past few weeks.  I believe that it has been effecting him and his diet.  In fact I know it has, I&#8217;ve witnessed it first hand.   It is very hard having your parent go through such a difficult time and not being able to just walk across the room and see how he is doing, to just go across the street and knock on the door just to see how he is doing.  It was indeed nice having both my parents up here with us if only for two days.  I believe it did them good to get away for even the little while they did.  You know, sometimes a change of scenery is good for the soul.  I also believe that having a definitive course of action laid out in front of you from top to bottom works wonders for the easing of your mind.</p>
<p>&#8230;.and so with this the journey goes forward.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>The traditional branching tree control structure within an organization is simply not designed to cope with the ever changing management requirements dictated by the life-cycle changes within a large project. The fact that various input and output measures vary over the project’s life suggests that project management must focus on universal project dimensions such as cost, time and performance (quality).</p>
<p>As an example of how interface problems vary over the life of a project, consider the two functions of R&#38;D and production over the life-cycle of a given product. Before the introduction of the product, R&#38;D must be closely matched with production. R&#38;D may be doing reliability tests which will lead to engineering changes. Production will be doing production design and process planning, which may be affected seriously by engineering changes. Thus, good communication is essential to avoid wasted resources in production.</p>
<p>On the other hand, in the growth phase R&#38;D is likely to be focusing on developing the next product, while production will be ramping up production and producing long runs to avoid production losses due to setups. Thus, there will be relatively little explicit conflict between R&#38;D and production at this phase.</p>
<p>In the decline phase, R&#38;D will be in the design phase on the new product and will withdraw all R&#38;D from the declining product. Production will be heavily involved in cost control. Again there will tend to be no apparent conflict, but good managers will make sure production is adequately consulted on the new design.</p>
<p>It is clear from the example that a full project management structure which focuses on future products as well as current products can help R&#38;D to interact in a more useful fashion.</p>

<p>This is NONSENSE for several reasons:</p>
<p>- smartphone sales grew in value in first 6 months of 2009, in some cases more than 100%<br />
- smartphones generate huge mobile data bills<br />
- subsidised  phones bring in postpaid clients, more revenue than prepaid ones<br />
- these terminals keep the users loyal to the brand<br />
- these cheap smartphones make users pay 50$ / month instead of cheaper postpaid options</p>
<p>By any means iPhone and smartphones are the ONLY good news for mobile operators all over the world.</p>
