<p>The <a href="http://cfd.net.au/home/topic/ultralight-plane">ultralight plane</a> took off from Renmark, in South Australia&#8217;s Riverland, on Monday but failed to arrive at Hay that afternoon.</p>
<p style="font-family:Verdana, 'Lucida Grande', 'Bitstream Vera Sans', Helvetica, Arial, sans-serif;font-size:12px;line-height:18px;margin:0 0 .8em;">The <a href="http://cfd.net.au/home/topic/disappearance">disappearance</a> was not reported until about 6:00pm (AEDT) yesterday, when a concerned friend contacted the Australian Maritime Safety Authority.</p>
<p style="font-family:Verdana, 'Lucida Grande', 'Bitstream Vera Sans', Helvetica, Arial, sans-serif;font-size:12px;line-height:18px;margin:0 0 .8em;">The authority launched a large-scale search-and-rescue operation this morning, after a plane conducted a preliminary search last night.</p>
<p style="font-family:Verdana, 'Lucida Grande', 'Bitstream Vera Sans', Helvetica, Arial, sans-serif;font-size:12px;line-height:18px;margin:0 0 .8em;">Searchers found the man&#8217;s body among the <a href="http://cfd.net.au/home/topic/wreckage">wreckage</a> of the plane about 37 kilometres west of Hay, near the Sturt Highway, shortly after 11:00am (AEDT).</p>
<p style="font-family:Verdana, 'Lucida Grande', 'Bitstream Vera Sans', Helvetica, Arial, sans-serif;font-size:12px;line-height:18px;margin:0 0 .8em;">The Australian Transport Safety Bureau and Recreational Aviation Australia have been notified and police will prepare a report for the <a href="http://cfd.net.au/home/topic/coroner">coroner</a>.</p>

<description><![CDATA[CLEVELAND [WMAT News]&#8211;The Cuyahoga County Coroner&#8217;s office is conducting the gruesome ta]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p><strong>CLEVELAND [WMAT News]</strong>&#8211;The Cuyahoga County Coroner&#8217;s office is conducting the gruesome task of trying to identify the remains of 11 people taken from a home on Imperial Avenue on Cleveland&#8217;s East side.</p>
<p>The first victim was identified Wednesday as Tonia Carmichael, a DNA test from Carmichael&#8217;s mother confirmed the identity. She has been missing for the last year.</p>
<p>Dr. Miller says there are 11 victims so far, all of whom are African-American women. Ten bodies and one partial skeleton. Six were in the house and five in the back yard. Dr. Miller says none was there for less than a few weeks, and it&#8217;s hard to determine how long some may have been there. Some could have been there since Sowell was released from prison in 2005.</p>
<p>Dr. Miller said that seven of the victims appear to have been strangled with what appeared to be a rope, tie, or an electrical cord.</p>
<p>“There is still a lot of work that needs to be done and a lot of unanswered questions that need to be addressed. Until the family of the victims get the closure they seek and ultimately the justice they deserve, this case will continue to be our focus. My thoughts, prayers and deepest condolences go out to the Carmichael family, friends and relatives,” said Mayor Frank G. Jackson.</p>
<p>Police will execute a warrant to take DNA from Sowell which will be entered into the national CODIS database. Sowell’s DNA will be compared against known suspect DNA profiles to determine whether or not he is a suspect in any other criminal matter.</p>

Forty-two other Afghan men survived and have been held in detention since.</p>
<p style="font-family:Verdana, 'Lucida Grande', 'Bitstream Vera Sans', Helvetica, Arial, sans-serif;font-size:12px;line-height:18px;margin:0 0 .8em;">The <a href="http://cfd.net.au/home/topic/coroner">coroner</a> is investigating the <a href="http://cfd.net.au/home/topic/deaths">deaths</a> but the Immigration Minister Chris Evans says that doesn&#8217;t mean the men cannot be given asylum, and he has granted it.</p>

He gets stiff just thinking about it</p>
<p>but he sniffs the autopsy stiffs<br />
from his office and is sucked out of his<br />
imagination, back to the officer&#8217;s station<br />
answering phone calls about missing persons<br />
and untagged sacks of guts with holes in them.<br />
Many of the bodies that pass through<br />
have full young heads of hair that had still<br />
grown with opportunity, in youthful spurts<br />
and he studies the features on their faces<br />
for any emotion at all that could suggest a response<br />

<p>sometimes the worlds eclipse, and the places where we met&#8211;to eat, to walk together, to talk about the listening, to laugh&#8211;make your presence so real. the corner with a bus stop bench where i saw you after work one day last june, and it was my turn to pay for lunch. i walk across the street from that corner five days a week now, because i still work there. it&#8217;s october of the same year and you&#8217;ve been dead for three and a half weeks.</p>
<p>when the pieces of the heart fall away, the underpinnings become visible. what kind of structure has built itself in that tender, enclosed space? what underlies the exterior, the living red that is designed to shatter and reinvent itself?</p>
<p>the doctor told me to expect either diarrhea or unstoppable tears the weekend after treatment. she said i was reliving the moment of impact, the instant i found out about your death, the experience of those seconds, the first hours in which it was confirmed, your name checked against the report made by the town&#8217;s coroner. </p>
<p>my lover and i gathered your eyeglasses, sneakers, phone and drove them to the coroner&#8217;s office to be joined with the rest of you there, in a fridge somewhere behind the generators at the back of the building. M., my lover, told me to try not to think about that, but the coroner  said that your body was &#8220;unviewable&#8221;&#8211;it was easy, then, to know that you had been destroyed. by the impact. shattered to pieces by the force of a freeway car at six a.m., long before traffic slows to stop-and-go. </p>
<p>it was still dark when you left. by the time my alarm crowed at six, you were gone, your car out of sight where it had been parked outside. the front doors of the house were unlocked, the television on loud. you and your purse had disappeared. i thought you had driven home, not onto the freeway onramp a mile away, not pulled over and left your new car on the shoulder, not stood between the lanes without your shoes and glasses and waited for a fast-moving car, not stepped in front of it. </p>
<p>not died at twenty-three. </p>
<p>it is the places i remember being with you, the reality and gravity of your flesh, my spirit leaning against the vibrations of your voice, where i cross into the moments you were here. in the days after your death, i see myself where you were, waiting for death to claim you by force. using the car as a bullet, an actual gun became unnecessary. which parts of you remained after the first shattering? did the cells know to expect that sudden dispersion? did you feel the ones that followed? my cells imagine they are yours, over and over again, stretching themselves to recreate the moment you were taken away.</p>
<p>you told me you wore green on purpose that day, the day you had asked to meet again. our meeting turned to evening and you stayed the night, silent in the next room. </p>
<p>. . .</p>
<p>before retiring after midnight, i slice cucumbers for my swollen eyes, the bags of water that have collected beneath each one. unsightly patches of red mark the tears that scratched my face today, when remembrance of you became inescapable. any tasks i could complete, even if i could accomplish much in this halted state, cannot re-member those ruptured cells and limbs, cannot gather the blood that flowed away into a stain on a gasoline-streaked highway.</p>
<p>the clouds move, the light shifts as sunset nears dusk. gold shafts of sunlight break, and are broken by, the blanket of gray heading north.  twilight paints colors that mirror dawn, marking the time of Maghreb prayer, and a new moon rises.  </p>

<p>The case under review is rather straightforward. An elderly gentleman was evicted from his bachelor apartment because he was behind in his rent.</p>
<p>The Toronto Housing Authority is the agency charged with the responsibility of looking after the poor and managing subsidized housing on behalf of the City.</p>
<p>Al Gosling was 82 years of age. He lived in subsidized housing for years. He didn’t have any family to help him out, so he fell somewhat behind in his paperwork.<br />
You know how much bureaucrats love paperwork. Well, Al was not sending them any. So what did they do?</p>
<p>Very simple, they evicted him. He came home one day in June, and the locks were changed. He had nowhere to go, so he slept in the stairway. His food was in his apartment, so he went without, and grew tired and weary. He was taken to a homeless shelter, but he became gravely ill. He was 97 pounds.</p>
<p>Finally, Al was taken to the hospital in October. He was there for a few days, then he died.</p>
<p>What was the great piece of paper that led to Al’s death? It was a form that said he didn’t have any other sources of income. Therefore, he qualified for the subsidy.</p>
<p>Without that piece of paper the Toronto Community Housing Centre decided to fight. They wanted that piece of paper and he didn’t provide it. So, let’s take him to Court and evict him.</p>
<p>Fortunately, the Coroner believes that something went wrong here. Right now the Coroner is undertaking an investigation. Hopefully, this will lead to a decision to hold an inquest.</p>
<p>Yes, I do have a word of advice:</p>
<p>• Don’t get old<br />
• Don’t get sick<br />
• Don’t be poor</p>
<p>And, the number one thing to remember: fill out your paperwork. They don’t take any excuses!</p>

<p>Car horns blared. Some impatient drivers began driving on the shoulder to get past the accident. Other cars followed, unaware or uncaring of the dead man in the road. I watched the tires pass us. The dead man stared out, unseeing. His eyes waxed and waned&#8230;waxed and waned&#8230;waxed and waned as the lights of the passing cars slid over his face. The road glittered like a pool of hundreds of constellations. It was beautiful. I remember wishing that he could see it.</p>
<p>The police arrived. Thankfully, they didn&#8217;t see me in my hiding place on the side of the road. I knew that if they had found me, they&#8217;d make a big deal about reuniting me with my parents. The last thing I wanted was to sit in an uncomfortable car for hours listening to old Korean disco while the police did their job.</p>
<p>The policemen placed flares everywhere and told everyone to stand back. As they waited for the coroner to arrive, a bright, blue tarp was laid over the hitchhiker&#8217;s body. It matched his bag. The crowd on the other side of the police tape stared at the tarp, as if expecting the dead man to rise up from beneath it. I turned over onto my back, no longer interested.</p>

<p>If you&#8217;ve been following this blog, you know that we&#8217;ve been trying to locate the full transcript of the coroner&#8217;s jury inquest into the death of <strong>Kathleen Savio</strong> from May 1, 2004.</p>
<p>To date we&#8217;ve been able to share with you the pages containing <a href="http://petersonstory.wordpress.com/2009/06/12/transcript-testimony-of-officer-from-kathleen-savios-death-inquest-he-was-never-at-the-scene/" target="_blank">the testimony of Herbert Hardy</a> (pages 7-15). We&#8217;ve now located the first six pages of the transcript, in which <strong>Susan Doman</strong> (then Savio) testifies, and we&#8217;re pleased to be able to share them with you.</p>
<p>EXCERPT:</p>
<blockquote><p><strong>Q.</strong> Is there anything else you&#8217;d like to add, ma&#8217;am?</p>
<p><strong>A.</strong> Yes. I just &#8212; it&#8217;s very difficult for my family because of my sister telling us all the time, and I can figure, everyone, everything &#8212; one that she has seen that if she would die, it may look like and accident, but it wasn&#8217;t. She just told me last week, and she was terrified of him. He always threatened her. He had her in the basement one time. He did, many, many things to her. He only wished for her to go away.<br />
                                                            And it&#8217;s just very hard for me to accept that, what happened. His reactions after this were a laughing matter. Cleaning everything out, ready to get rid of the house. It&#8217;s very hard.</p>

<p>Sue Doman&#8217;s testimony is especially interesting at this time, as this Friday is the date of Drew Peterson&#8217;s next hearing in which the defense will be arguing against the constitutionality of the &#8220;hearsay law&#8221;.</p>

<p>Usually within a day or two after a death friends gather and the life is celebrated in the home. Then the burial or cremation occurs</p>

<p>This story APPEARS  to be Accurate. The whole Scenario  fits totally with the events that followed the Home Funeral of Patrick Swayze.</p>

<p>I checked twitter this morning and discovered that Patrick Swayze had finally succumbed to his pancreatic cancer. The poor bloke. His slow and painful death will doubtlessly be overshadowed by Michael Jackson&#8217;s, and perhaps that&#8217;s ok. What isn&#8217;t OK, however, is that when I typed Patrick Swayze into Google, the first thing that came up was a site called <a href="http://www.sikipedia.org" target="_blank">sikipedia</a>. This site took me straight to a whole bunch of disgusting and tasteless jokes about Patrick Swayze. He&#8217;s barely cold for God&#8217;s sake!</p>
<p>I just want to say to anybody out there making jokes about Patrick Swayze &#8211; I think it&#8217;s outragous the way people are making fun of him when he died so recently. You wouldn&#8217;t be joking if it was someone in your family who&#8217;d been in so many shit movies, so have some respect people.</p>
<p>The debate on whether or not Swayze sped up his death by continuing to smoke is now underway. Personally I think it took guts to admit that quitting smoking would do nothing to save him. Fair play to the guy. Although perhaps <a href="http://jackpetrubi.wordpress.com/2009/09/08/smoking-does-make-you-look-cool/" target="_blank">smoking doesn&#8217;t make you cool</a> in this case.</p>
<p>They may not put baby in the corner, but looks like they&#8217;ve sent you to the coroner Pat. RIP mate.</p>

<title><![CDATA[At-Home Death on Hospice Care]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p>If a loved one dies at home on hospice care, the first call goes to the <a href="http://www.hospicefoundation.org/">hospice organization</a>. All hospice programs have registered nurses on call 24/7, and in most states specially certified nurses may officially pronounce the deaths of home hospice patients. Check with a local provider to make sure this is the case in your state.</p>
<p>It helps to have a funeral home already selected to call after pronouncement, even if you haven’t made funeral plans. The hospice nurse will call the funeral home of choice to take the body to their facility. Most mortuaries handle transportation of a body from the home in unmarked vans, not in a hearse.</p>
<p>While waiting for the mortuary, the nurse will offer to wash the body to the extent necessary or desired by the family, and remove catheters, IVs or medicated patches. The family can help with the washing if they wish.</p>
<p>Most state Board of Pharmacy rules prohibit hospice nurses from taking back leftover narcotics. The current procedure is for the nurse to tally up and destroy, with a family member as witness, any controlled substances the patient was taking, such as narcotics and sedatives. This is done by pouring the pills and liquids into a sealable plastic baggie, pouring cooking oil, vinegar, or bleach over them, and discarding the bag in the trash. “Flushing medications down the toilet is ‘out’ these days, to protect groundwater and river quality,” said Nancy Costea, a hospice nurse.</p>
<p>If a patient has had a recent fall with an injury that might have contributed to the death, such as a hip fracture, the nurse will have to call the Office of the Medical Investigator (OMI) or the coroner’s office. The investigator on duty will determine over the phone if they need to take the case. If so, OMI’s removal team will take the body to their facility. OMI is also called if there’s suspicion of abuse or neglect, or if circumstances point toward a drug overdose.</p>
<p>A word about Do Not Resuscitate (DNR) orders: People who go on hospice acknowledge that they are going to die. Under current Medicare regulations, hospice providers cannot require that a DNR document be signed and in place. However, most hospice programs strongly encourage taking that step. This means that no heroic measures will be taken to restart heartbeat or breathing when the patient starts to fail.</p>
<p>Family members should <em>not</em> call 911 when the end seems near. Even with a DNR order, once emergency medical services are called, a home death gets needlessly complicated. Keep the hospice phone number posted by or programmed into the phone, and make sure anyone staying with the patient is aware of the importance of calling for the hospice nurse, <em>not</em> 911.</p>

<title><![CDATA["Expected" vs. "Unexpected" Death]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p>&#8220;Expected&#8221; deaths, which happen in settings such as hospitals, nursing homes or hospice settings, still manage to seem &#8220;unexpected.&#8221; Whether a death is a complete surprise or not, here are a few things that happen on the way to a funeral.</p>
<p>The first call in an “expected” death goes to the primary doctor or hospice provider for the deceased, to facilitate pronouncement. Emergency room doctors can also record pronouncement. After the time of death is officially recorded, the body can be moved and preparations made for final disposition of the body. The second call would be to a funeral director, if previous arrangements have been made. If arrangements aren’t already in place, a boatload of decisions will need to be made under duress.</p>
<p>Chances are, you’d be in the same boat with an “unexpected” death, such as an auto accident, suicide, homicide, when a person under the age of 18 dies or a body is discovered under mysterious circumstances. A police officer, coroner or medical investigator will make the pronouncement to the best of their ability given the circumstances.</p>
<p>By health code, a body has to be processed within 24 hours – before decomposition begins – in one of four ways: refrigerate, embalm, cremate, or bury. Embalming and refrigeration give the family a few more days with the body before cremation or burial, allowing distant relatives to travel for a funeral. Both Jews and Muslims are prohibited from embalming and those religions dictate burial within 24 to 72 hours.</p>
<p>The funeral director or the Office of the Medical Investigator prepares death certificates, with information completed by a medical certifier. You might be surprised at the information you need to supply about the deceased. In addition to their legal name, you need any AKAs (also known as), Social Security number, birth date and place, marital status at death, mother’s name prior to first marriage, military service discharge information, and more.
