Received $25 GC with new prescription

Sorry for the long delay in post! I have been amazingly ill and couldn’t even look at a computer screen or tv. My doctor called in a new prescription for me at CVS.  A few weeks ago I received a CRT for a $25 CVS gift card with new or transferred script, from the price scanner. Maurice went and picked it up for me. He also grabbed me another Flu Shot coupon book

$8.75 – Prescription

Paid $8.75 and received a shiny new $25 on the spot.  So CVS gave me a prescription and I profited $16.25

Not everyone will get these $25 CRTs, me and boyfriend share 2 cards between us and I only got the CRT on one card. But ALWAYS find those cute little price checker machines in the store and swipe your card until it won’t let you anymore. It’s a little tedious because the machine is slow but still very much worth it.

yes, i am one of those crazy people who will be waiting outside in line in the g-d freezing cold at 4 am to get the best deals. which is funny, because the really bloody battles usually happen over deep discount electronics and zhu zhu hamsters, neither of which i am currently in the market for…  mostly, i’m hoarding cheap dvds and cleaning out clearance stock with extra coupons. also, i’m in it for the experience (which i will be documenting as i go). it’s totally crazy, and kind of stupid, but hands down the wildest shopping ride of the year.

target:
last year, target was my first black friday stop- this year not so much.  there’s some cute puffy vests for $7.50, and baby mama is on dvd for $3.99 (plus some mysterious “select others)…. but nothing amazing. we will be stopping in, but definitely not first.

walmart:
walmart doesn’t have much that i want, but their $2 dvds are hitting my sweet spot just right. they say “over 100 titles”, but just from the small stack in the picture, i’ll most definitely be picking up: spaceballs, young frankenstein, the mummy, and hellboy II. my dvd collection is already ridiculous- and about to get even ridiculous-er.

cvs:
totally copycatting rite aid by the way- herbal essences AND soleil razors. i’m outraged. but i’ll still shop there, because the deals are hot for the first time in months. i’m feeling lazy, so i’ll just list of the things that are free after ECBs (with or without coupons):

costuming a burlesque means needing plus size ruffle butt underpants. usually pretty pricey @ $18/ea. or $27/3, the whole store will be 40% through the 29th! kick ass.

Ok, maybe I spoke too soon when I said I was not going out on Black Friday.  After reading a few of the black Friday delas at pharmacies I may have to hit up Rite Aid and CVS.  Autumn already posted on Rite Aid but for the CVS deals that inspired me check out Saving with Shellie’s post here.  I already hae $10 ECB so I think I could get a lot of great stuff by multiple postings.

*Also be on the lookout for a coupon to double your ECBs.  I found mine by the register last week.  It sounds like just using it once will double your ECBs in November-December for use in January.

Turkeys Flying

BEDFORD—Most adherents will wait until the traditional holiday feasts, but turkey carving season has already begun for at least one commercial real estate portfolio, as UBS Investments attempts to disgorge itself from a heaping plate of troubled flex/office product by paring off the 667,000 sf to buyers hungry for price-adjusted opportunities. Thus far, it appears the prospects are eagerly biting.

At the bargain-basement rate of $29 per sf, UBS last week sold 8 Oak Park in Bedford to Boston real estate investor CRE Management Inc. and a user who will supposedly occupy half of the 65,000 sf in the two-story office building. Calls to GFI and UBS were not returned, but the $1.89 million deal is well below the $3.1 million paid just three years earlier by a joint venture partnership of Mack-Cali Real Estate, Gale International and JP Morgan. That group surrendered seven buildings to UBS this summer after determining market conditions were too harsh to hold onto the properties, having paid $53.6 million for the entire package in June 2006, or approximately $88 per sf.

Flash forward to 2009, and a debate rages whether GFI Partners overpaid at barely one-third what the space fetched mere months earlier. “Empty buildings are worth nothing right now,” claims one broker who deems the sale “a good comp” for UBS. Another says $20 per sf is more apropos, whereas others argue pricing is right where it ought to be, especially in a case such as 8 Oak Park Dr. where the occupant is getting an equity stake. “They love it,” one broker claims of the new owners of 8 Oak Park Dr. The user/partner with CRE Management Inc. is Medical Reimburesment Systems Inc., which will relocate to the building from Woburn. Its principals are Drs. Jon. L. Jenkins and Elijah Berg.

Sources also report UBS is in the process of divesting other pieces of the portfolio, which includes three buildings at Woodland Park in Andover and a pair totaling 170,000 sf at 3 and 8 Federal St. in Billerica. Turnaround specialist Joseph Zink of Atlantic Management Corp. is said to have a bead on the Billerica properties, and a user is rumored to be homing in on one of the Andover buildings.

The Billerica pricing is also estimated around $30 per sf—the two buildings traded for a collective $73 per sf in the 2006 portfolio purchase. Calls to Zink were not returned, but one source insists talks are under way. “It’s a fact,” claims the seasoned market watcher who also anticipates fresh ownership will do the properties well if acquired at a basis enabling competitive rents. “They are going to make a lot of money,” another broker says of AMC.

Pricing was not available regarding the Andover situation, but sources relay that a user is angling to acquire 600 Federal St., a two-story, 116,000-sf building that traded for $15.6 million in the portfolio deal.

Holden Pattern

Holden—It seems like just yesterday that residents of Holden were circulating a spirited petition to block development of a Walgreens on Main Street, evoking the standard fears about exacerbating traffic and compromising America’s fading small town ambiance. As is often the case, however, the protestations could not keep progress from marching onward, and the Richmond Co. ultimately prevailed in building its stand-alone, 14,700-sf store, one that today is doing daily battle for consumers with existing Holden competitors CVS and Rite-Aid.

Wilmington-based Richmond has also now come full circle in another way, repeating a recent pattern of quickly selling the property the firm develops upon completion. Three stand-alone Walgreens in Hudson, Leominster and North Reading permitted and built by Richmond have changed hands in the past year, with the previous trio scooped up by members of the Insoft family, a clan better known as an assisted living and nursing home operator. The three assets have reaped more than $25 million collectively for the sellers.

The Richmond Co., among the region’s leading retail developers, is not the only group who has gotten in the act of building stand-alone pharmacies and then harvesting them to buyers looking for net-leased investment opportunities. Previous issues of The Real Reporter have detailed just-completed Walgreens being sold in Bellingham and Southborough. Those assets were acquired by investors from Florida and California, whereas the Holden Walgreens was scooped up by an investor hailing from Chicago. Stratford-Cornelia LLC paid a healthy $6,86 million for the property, which is located at the corner of Main Street and Hold Road.

A few days ago, desperate to address a number of topics that would not fit in with this blog or on Kenya Imagine, I put together a couple of drafts for posts and decided to start a new blog, along with a linked Twitter account. Now, this could be ill-advised, as I’ve already got multiple Twitter accounts to deal with, plus my obligations here and elsewhere, and I’m not sure how much time I’ll be able to dedicate to each project without sacrificing either quality or my social life. Not only that, but should I ever want to cite on my CV, they could look rather haphazard, as there’s no real link between the topics they cover, and I have different levels of output and engagement in each.

Thinking back to the post I did on portfolio careers, I realised that even as I have eased more into adding writer/blogger to my list of skills, it would be difficult for a potential employer to pull my various contributions together into a coherent space. Not only that, but the diverse topics I cover mean that pinning down my “voice” could also have its own problems. On the one hand, for the projects that I consider as possible career opportunities, I want to project an image that is professional and knowledgeable; those that I am doing for myself don’t need to be quite so focused on emphasising my competencies.

If you have diverse skills and have worked in a variety of areas, you probably tailor your CV to fit each particular job you apply for. You don’t need to mention your stint as an environmental officer when applying for the event management position, but you might very well mention the stage management job you did a couple of years ago. Sending your credentials off to a family-run firm means that you won’t necessarily expound on your stint as a corporate drone in a huge conglomerate as much as you do your ability to work in small autonomous teams. This can all work well if you are given enough leeway to pick and choose the details you divulge. If you are asked for a complete work history, however, the overall impression can seem… messy.

One solution, therefore, might be to keep everything separate in practice, with one point of reference for an overview of everything you’ve accomplished. If you have worked for a couple of political thinktanks, have also volunteered for a charity and are now retraining as a international development worker, it can seem difficult to tie all of these together, particularly if you don’t wan to favour one over the other. Nevertheless, you can take a couple of steps to make your career seem more coherent.

First, recognise that each of your accomplishments and skill sets has value. Even my part-time job in a call centre is something I am grateful for, though I wasn’t at the time. Next, come up with a narrative for each of the major areas of your career, explaining how you got to where you are now. Finally, bring together the different narratives to reach an overarching conclusion. For instance, I started out as a translator, progressed to giving additional business advice, branched out into writing business plans, and as a result of blogging about tech and business, landed a gig at Kenya Imagine, where I am now picking up additional skills as a writer and editor, along with some management skillls to boot.

I do still keep my various projects apart from each other, and intend to keep doing so. Where I will pull them together to give an overview of everything I do is on a separate blog, yet to be set up, where I will explain my various projects, what I hope to achieve from them, and how they related to each other. Yes, it’s a bit more work than plonking everything down in one place, but when somebody comes to look at what I have been up to, they will be able to see everything I have done so far, without it seeming like I have flitted between various interests and areas without being able to make my mind up. Were I to pull together everything into one blog, or on to one CV, I wouldn’t make any sense.

I believe that in future it is going to become increasingly common for people to have varied CVs, as they work in a number of different sectors rather than having a specific career progression, and as the business environment changes to embrace all-rounders rather than single-skilled toilers. Nevertheless, employers will still want to be able to see how a prospective employee’s career up until the present will fulfil their job requirements, and as such it’s important that no matter how eclectic your job history is, you are able to demonstrate that you fit the part. Each part of your career may be a story in itself, but you need to marshall them to reach an overall theme.

TJ Maxx

I needed to get some new work shirts for DH, and we had received a giftcard for TJ Maxx from the wedding.  I decided to stop in & see if they had anything that would work.  I was able to get him 3 dress shirts (each with a tie, and one with cufflinks!) for $19.99 each.  The giftcard was $50, so I just had to pay $15.53 for the shirts.  Awesome!!

Overall it was a pretty good shopping day.  Its nice to get the things we need for such a low price.

Dit is het vervolg op de eerdere studie betreffende de morfologie van skeletspier-fragmenten van CVS-patiënten (‘Struktuur en funktie van skelet-spieren gewijzigd bij CVS’). In feite gaat het over dezelfde groep patiënten en experimenten die gelijktijdig werden ondernomen.

Samengevat: er werd bij CVS-patiënten veranderde gen-expressie in de spieren gevonden en deze wijzigingen bleken terug te brengen naar de volgende processen en mechanismen: energie-produktie, oxidatieve stress, spierweefsel-morfolgie (vezel-type), spier-groei/-afbraak en geleiding van zenuwprikkels naar de spieren.

Transcription profile analysis of vastus lateralis muscle from patients with Chronic Fatigue Syndrome

Chronische Vermoeidheid Syndroom (CVS) is een invaliderende aandoening gekarakteriseerd door onverklaarde chronische vermoeidheid die de normale aktiviteiten schaadt. Vele lichaam-systemen zijn aangetast en de etiologie werd nog niet geïdentificeerd. Naast immunologische en psychologische aspekten, zijn skeletale spieren symptomen prominent bij CVS-patiënten. In een poging vast te stellen welke mechanismen mogelijks betrokken zijn bij de aanvang en ontwikkeling van spier-symptomen, gebruikten we algemene transcriptoom [verzameling van alle boodschapper-RNA (mRNA) molekulen - ‘transcripten’ , tot expressie komende genen - geproduceerd in een cel of populatie van cellen] -analyse om genen te identificeren die differentieel tot expressie komen in de vastus lateralis spier [spier aan de voorzijde van het dijbeen, deel van de quadriceps] van vrouwelijke en mannelijke CVS-patiënten. We vonden dat de expressie van genen die een sleutel-rol spelen bij mitochondriale funktie en oxidatief evenwicht, inclusief superoxide-dismutase 2, was gewijzigd, net zoals genen betrokken bij energie-produktie, musculaire groei/ontwikkeling en bepaling van het vezel-fenotype. Belangrijk was dat de expressie van een gen coderend voor een component van de nicotine-cholinergische receptor-bindingsplaats verminderd was, wat een gestoorde neuromusculaire transmissie suggereert. We argumenteren dat deze belangrijke biologische processen betrokken kunnen zijn bij en/of verantwoordelijk voor de spier-symptomen van CVS.

Het is niet verrassend dat skelet-spieren het meest onderzochte orgaan zijn bij studies die zich richten op de lage tolerantie voor inspanning. Oxidatieve stress, door anderen en ons eigen laboratorium voorgesteld als een mogelijke bijdrage tot de pathofysiologie en klinische symptomen van CVS, is het brandpunt voor research geworden. [zie artikels van het lab van S. Fulle] Een studie vond dat reaktieve zuurstof soorten [ROS] niet enkel verantwoordelijk zijn voor molekulaire schade maar ook fungeren als belangrijke signalisering-molekulen die de samentrekbaarheid moduleren in niet-vermoeide én vermoeide skelet-spieren. Meerdere biochemische en metabole manifestaties in spiervezels bij CVS werden onderzocht maar de besluiten waren controversieel. Er werd getoond dat CVS-patiënten verminderde serum-concentraties aan acetylcarnitine hadden (Kuratsune et al. 1994) [zie ook: ‘Acetylcarnitine - verminderde opname in de hersenen’], wat mogelijks duidt op een verminderde mitochondriale energie-produktie in de spier. Matige inspanning veroorzaakt sustantiële musculaire verzuring en de aktiviteiten van adenylaat- en creatine-kinase kinase [enzymen die een rol spelen bij de energie-voorziening van spieren] in de spier zijn gebrekkig bij CVS-patiënten. Bovendien werden spier-pijn in afwezigheid van perifere weefsel-schade en gedaalde zuurstof-verzadiging na inspanning gerapporteerd. In het licht van het beschikbare maar controversiële bewijsmateriaal betreffende de betrokkenheid van spiervezels bij CVS en de karakterisatie van enkelvoudige spiervezels, met inbegrip van de bepaling van contractie-eigenschappen in vivo [zie: ‘Struktuur en funktie van skelet-spieren gewijzigd bij CVS’], beslisten we een benadering te gebruiken die niet nog niet werd toegepast om skelet-spieren van CVS-patiënten te bestuderen: de identificatie van veranderingen qua gen-expressie m.b.v. micro-arrays. Ons doel was te bepalen of consistente gen-expressie wijzigingen konden worden geïdentificeerd bij CVS-patiënten. […]

Materialen en Methoden

CVS-patiënten […] diagnose volgens de CDC-criteria. […] vijf vrouwen (gemiddelde leeftijd 44,8±3,4 jaar en 5,4±0,7 jaar ziek) en vijf mannen (gemiddelde leeftijd 37,0±3,2 jaar en 7,8±1,9 jaar ziek). […] controles hadden geen huidige of vroegere geschiedenis van diffuse musculoskeletale pijn/vermoeidheid die langer dan 10 dagen duurde. […]

Om de klinische diagnose van CVS te bevestigen en, in het bijzonder, patiënten met fibromyalgie uit te sluiten, gebruikten we ook een directe test. In een eerdere studie toonden we aan dat oxidatieve schade bij CVS de fluïditeit [omschrijvig van de viscositeit van de lipiden-lagen van een cel-membraan] en de vetzuur-samenstelling veranderde van membranen van skelet-spieren, resulterend in een specifiek en eigenaardig patroon dat volledig verschillend was van dat van fibromyalgie-patiënten. Deze laatsten, die over het algemeen hogere gelijkaardige symptomen hebben als CVS-ers, vertonen hogere membraan-fluïditeit dan CVS-patiënten en controles. [Fulle S, Mecocci P, Fanò G et al.. Med. 29 (2000): 1252-1259] Om deze reden werd voorgesteld dat deze specifieke schade aan de membranen van skelet-spieren in staat is de excitatie-contractie [de elektrische aktivatie van de spiercel zet de samentrekking in werking] mechanismen te wijzigen [zie ‘Specifieke correlaties tussen oxidatieve stress in de spieren en CVS’] en het zou dus nuttig kunnen zijn een onderscheid te maken tussen de twee aandoeningen. Micro-array

De totale spier biopten hier gebruikt als RNA-bron zullen waarschijnlijk gecontamineerd zijn met andere cel-types (gladde spier, endotheliale cellen, ‘end-plates’ [uiteinden van een motor-neuron], fibroblasten) die kunnen bijdragen tot verschillen qua gen-expressie. Transcriptie-analyse werd gebruikt om de expressie-waarden van duizenden genen gelijktijdig te monitoren, met de specifieke bedoeling transcripten en/of mechanismen te identificeren die betrokken zijn bij CVS spier-symptomen. Ons experimenteel ontwerp vergeleek het RNA van elke CVS-patient met een ‘pool’ van controle-RNA verkregen van alle controle-individuen van het zelfde geslacht. Dit controle-RNA werd aangewend om de effekten van inter-indiviu heterogeniteit onder de controles te minimaliseren en een meer homogene ‘baseline’ te creëeren, om zo een grotere betrouwbaarheid te verkrijgen dat up- of downregulering van transcripten geïdentificeerd bij individuele CVS-patiënten gelinkt zijn met spier-symptomen van het syndroom.

Genen die differentieel tot expressie komen bij vrouwelijke én mannelijke CVS-patiënten

De ‘Significantie Analyse van Micro-Array’ (SAM) identificeerde 218 genen die differentieel tot expressie kwamen (96 ge-upreguleerde en 122 ge-downreguleerde) bij de twee vrouwelijke patiënten, en 453 genen (19 ge-upreguleerde en 434 ge-downreguleerde) bij de twee mannelijke patiënten.

Terwijl de expressie van 218 genen was gewijzigd bij de vrouwelijke patiënten en de expressie van 453 genen bij de mannelijke patiënten, was de expressie van slechts 47 genen significant veranderd in de bioptie-stalen van alle patiënten: twee genen waren die ge-upreguleerd en 38 waren ge-downreguleerd in biopten van van én mannelijke én vrouwelijke patiënten; zeven genen waren ge-upreguleerd bij vrouwelijke maar ge-downreguleerd bij mannelijke patiënten. Deze genen met differentiële expressie werden gebruikt als basis van onze poging om wijzigingen in sigalisering-mechanismen in spieren te identificeren bij dit syndroom.

Specifieke metabole veranderingen

De genen met differentiële expressie wijzen naar de mogelijke betrokkenheid van meerdere sigalisering-mechanismen en cellulaire processen in skelet-spieren bij de ontwikkeling van CVS. Ze omvatten (a) controle van de oxidatieve toestand, (b) DNA-herstel, (c) energie-balans, (d, e) trofische/inflammatoire processen, (f, g) groei-/apoptose-mechanismen, (h) neuromusculaire transmissie en (i) vezel-fenotype.

a) Oxidatieve stress. Ten minste drie mitochondriale genen die coderen voor proteïnen die direct of indirect gecorreleerd zijn met een onevenwicht in de oxidatieve status in de spier, waren ge-downreguleerd bij zowel vrouwelijke als mannelijke patiënten: (i) superoxide-dismutase 2 (SOD2), dat betrokken is bij het metabolisme van superoxide-anionen en een ‘opruimer’ van ROS gegenereerd in de mitochondrieën; (ii) ferrodoxine 1 (FDX1), dat een ijzer-zwavel proteïne is, in staat om elektronen te transfereren van NADH naar cytochroom-p450 en ook betrokken bij ROS-generatie; en (iii) NADPH-dehydrogenase-quinone 1 (NQO1), dat codeert voor een enzyme in het cytosol [intra-cellulaire vloeistof] dat in staat is te beschermen tegen toxische agentia.

Ook ge-downreguleerd was het gen coderend voor […] (IHPK3), belangrijk in de context van verhoogde oxidatieve schade. […]

b) DNA-herstel en gen-expressie. Eén van de twee genen die ge-upreguleerd waren bij CVS-patiënten was het gen coderend voor […] (POLB), een enzyme dat DNA dubbele helixen herstelt […]. Belangrijk was ook dat we vonden dat het gen […] (CITED2), een proteïne dat het start-complex vormt voor […] acetylering, ge-downreguleerd was. Verminderde acetylering […] aktiviteit van POLB verhoogt. Van het gen […] (SFPQ), dat ook ge-downreguleerd was, werd aangetoond dat het interageert met RNA-polymerase II om de transcriptie te reguleren. Naast een mogelijke rol bij oxidatieve stress, zou downregulering van IHPK3 kunnen resulteren in […], wat het DNA-herstel vermogen wijzigt of mRNA-export verstoort. Ook ge-downreguleerd was het gen coderend voor […] (SRRM2), een component voor een proteïne van de cel-kern matrix dat funktioneert als een co-acktivator voor pre-mRNA ‘splicing’ [verandering van genetische informatie na transcriptie van DNA naar mRNA: tijdens het RNA-processing worden ‘introns’ (overbodige, niet-coderende stukken) uit het pre-mRNA geknipt en de exons van het pre-mRNA aan elkaar geplakt].

Downregulering van het gen coderend voor […] (NM23A/NME1) zou een poging tot compensatie-respons kunnen zijn, bedoeld om DNA-schade te verlichten.

Het is moeilijker de funktionele betekenis te interpreteren van genen die bleken ge-upreguleerd te zijn bij vrouwen maar ge-downreguleerd bij mannen. Deze genen omvatten […] (DYRK2), dat p53 [tumor-proteïne 53, een transcriptie-factor, reguleert de cel-cyclus en funktioneert als een tumor-suppressor] reguleert om apoptose [geprogrammeerde cel-dood] te induceren in respons op DNA-schade; […] (TBL1XR1), die betrokken is gebleken bij histoon-binding [eiwitten die aan DNA binden; zie ‘Samenspel tussen de Glucocorticoid Receptor en Nuclear Factor-κB’]; en […] (PDE4B), dat waarschijnlijk geen rol speelt in CVS-spieren.

c) Energie-balans. Eén kenmerk van het CVS-spier gen-profiel was de onderdrukte transcriptie van meerdere genen betrokken bij het energie-meatbolisme van skeletale spiervezels. We vonden dat twee […enzymen, […] (PFKFB3) en […] (PFKFB1) ge-downreguleerd waren; wat suggereert dat glycolyse [afbraak van glucose, waarbij energie vrijkomt, tot pyrodruivenzuur, dat verder kan verbrand worden of anäeroob omgezet tot lactaat] en/of gluconeogenese [vorming van glucose uit niet-koolhydraat bronnen zoals bv. aminozuren, maar vooral uit pyrodruivenzuur] verstoord waren. […] Ook ge-downreguleerd was het gen coderend voor […] (PDK4), dat pyruvaat-dehydrogenase fosforyleert, een enzyme dat belangrijk is voor de oxidatieve [omzetting] van pyruvaat dat uiteindelijk wordt gebruikt voor oxidatieve fosforylatie [ox-fos; mitochondriaal proces waarbij energie-rijke elektronen hun energie afgeven in een serie redox-reakties; zie ook: ‘M.E.(cvs) als Mitochondriale Ziekte’]. Bovendien was […] (GOT1) ge-downreguleerd, wat suggestief is voor een toename van de gluconeogenese zonder verbruik van aminozuren., consistent met een poging van de CVS-spieren om glucose te besparen. Genen betrokken bij de fosforylatie van nucleoside-difosfaten waren ook ge-downreguleerd: inclusief […] (AMPD3), een belangrijk enzyme in purine-afbraak [purine is één van de stikstof-basen waaruit nucleïnezuren zijn opgebouwd] in spieren. Het […] (ABCA5) bleek ook ge-downreguleerd, wat suggereert dat de efflux [verwijdering van opgestapeld cholesterol uit de vaatwanden] van cholesterol uit de vezel geïnhibeerd is.

Slechts één met een mogelijk belang voor het energie-evenwicht bleek ge-upreguleerd bij vrouwen én mannen: […] (VLDLR). Dit gen codeert voor de receptor die verantwoordelijk is voor de opname van VLDL [‘very low density’ lipoproteïnen; deeltjes die vetten en cholesterol uit de lever via het bloed naar weefsels transporteren] in de spiervezel en betrokken is bij het primair mechanisme voor vetzuur-transport in skelet-spieren.

d) Atrofische processen. Een ge-downreguleerd tanscript betrokken bij de atrofische processen was […] (FOXO3A). Proteïnen van deze familie zijn transcriptie-factoren die de expressie reguleren van meerdere genen [FOXO transcriptie-factoren controleren fundamentele cellulaire processen zoals metabolisme, cel-differentiatie, cel-cyclus, DNA-herstel en andere reakties op cellulaire stress] die ge-downreguleerd bleken bij onze screening (inclusief PDK4, SOD2 en GADD45, die van belang zijn bij cellulaire responsen zoals glucose-metabolisme, regulering van de cel-cyclus en apoptose). Het is het vermelden waard dat een ander gen dat betrokken is bij het FOXO-mechanisme, […] (H1FX), coderend voor een nucleosoom-proteïne dat DNA linkt met het nucleosoom [complex van DNA en histoon-eiwitten dat de gen-expressie regelt], ge-downreguleerd was.

Belangrijk in de context van atrofische processen: het ubiquitine-afhankelijk katabolisme [merken van proteïnen, door koppeling met ubiquitine, waardoor ze kunnen worden afgebroken] was waarschijnlijk ook onderdrukt; wat blijkt uit de downregulering van ubiquinatie-factor E4A (UBE4A), coderend voor de bijkomende conjugatie-factor E4. Deze factor, die sterk tot expressie komt in skelet-spieren, vervangt het E3-enzyme en is in staat ubiquitine te transfereren naar zijn doel. De onderdrukking van ubiquitine-afhankelijke processen in CVS-spieren wordt verder bevestigd door de downregulering bij vrouwelijke patiënten van […] (PSMD3), coderend voor een non-ATPase subunit [betrokken bij de afbraak van ge-ubiquitineerde proteïnen] van het proteasoom [proteasoom = cytoplasmatische of nucleaire eiwit-complexen die afwijkende - overbodige of beschadigde - proteïnen afbreken via proteasen]. Bij mannelijke patiënten werd de downregulering van […] (PSMB2), coderend voor een beta subunit van het 20S proteasoom [onderdeel van het proteasoom dat ge-ubiquitineerde molekulen afbreekt], vastgesteld.

e) Inflammatoir proces. De volgende genen waren ge-downregeuleerd bij alle patiënten, wat een vermindering van de inflammatoire respons in CVS-spieren suggereert: (i) […] (SLPI), een gen coderend voor een serine-protease [proteasen die peptide-verbindingen, waarin één van de aminozuren serine is, doorknippen] met meerdere funkties bij aangeboren gastheer-verdediging, inflammatie en infektie; (ii) GRO2 oncogen [gen dat ervoor zorgt dat een cel zich als een kanker-cel gaat gedragen], […], een krachtig chemotactisch agens voor polymorfonucleaire leukocyten [stof die meerkernige witte bloedecellen aantrekt]; (iii) […] (GBP2), coderend voor een proteïne dat de proliferatie en de angiogene respons [angiogenese = vorming van nieuwe bloedvaten] van endotheliale cellen [bedekkende cellen van bloed-/lymfe-vaten en lichaamsholten] op inflammatie tegenwerkt, en ook weerstand biedt tegen de proliferatie van vesiculair stomatitis virus en encefalomyocarditis vrius; (iv) […] (TSC22D3/GILZ), een gen coderend voor de mediator van glucocorticoïd-geïnduceerde immunosuppressie, dat interfereert met AP-1 [zie ‘Samenspel tussen de Glucocorticoid Receptor en Nuclear Factor-κB’] […] om de binding van aktief AP-1 met zijn doelwit-DNA te inhiberen.

f, g) Groei/apoptose en cytoskeletale regulering. Meerdere ge-downreguleerde genen gemeenschappelijk bij mannelijke en vrouwelijke CVS-patiënten zijn betrokken bij cellulaire groei en apoptose-mechanismen, suggestief voor een relatieve uitdoving van deze inter-connecterende mechanismen. We vonden dat […] (FOS) en […] (MYC) ge-downreguleerd waren. FOS en MYC zijn transcriptie-factoren betrokken bij de proliferatie en cel-cyclus regulering […] maar ze induceren ook apoptose in gevoelige cellen. FOS downregulering is consistent met de downregulering van het met FOS interagerend proteïne TCS22/GILZ, waarmee het een gemeenschappelijke signaal-transductie mechanisme deelt. Ook ge-downreguleerd waren […] (AATF), een transcriptie-factor die EF2-transcriptie [EF = elongatie-factor; proteïne dat het verlengen van peptide-ketens bij proteïne-synthese vergemakkelijkt] stimuleert en cel-cyclus progressie bevordert, en […] (CEBPD), die werd gelinkt aan cel-groei en apoptose in spiercellen.

Vele genen betrokken bij controle van focale adhesie [cel-matrix adhesies of FAs; specifieke types grote macro-molekulaire samenstellingen waarmee mechanische kracht en regulerende signalen worden doorgegeven] en cytoskeletale en/of regulering van de extracellulaire matrix bleken ook ge-downreguleerd bij CVS-patiënten. Onder andere: (i) […] (PRELP), coderend voor een eiwit dat type-I collageen linkt met heparaan-sulfaat basale membranen [heparaan-sulfaat speelt een rol bij angiogenese, coagulatie en tumor-metastase]; (ii) collageen type-V alfa 3 (COL5A3); (iii) […] (LIMK1), coderend voor en proteïne dat eiwitten betrokken bij de organisatie van het actine-cytoskeleton linkt; en (iv) […] (SAT1), coderend voor een eiwit betrokken bij de regulering van apoptose, cellulaire proliferatie en cel-cyclus progressie.

Ook meldenswaardig is de downregulering van twee bijkomende proteïnen: (i) […] (PTK2/FAF), coderend voor een cytoplasmatisch proteïne gelokaliseerd op focale adhesies tussen groeiende cellen, dat belangrijk is voor transductie van externe signalen; en (ii) prion-proteïne (PRNP), ook bekend van bv. Creutzfel-Jakob ziekte […], coderend voor een glycoproteïne gehecht aan het cel-membraan […]; kenmerken die consistent zouden kunnen zijn met rollen qua cel-adhesie en trans-mebraan signalisering. Een ander gewijzigd gen in CVS-spieren, een anti-angiogene factor met een rol in de focale adhesie, is thrombospondine (FLJ14440, THBS1, THBS2), dat ge-upreguleerd blijkt bij vrouwelijke maar ge-downreguleerd bij mannelijke patiënten.

h) Neuromusculaire transmissie. Het gen CHRNA1, coderend voor een component van de acetylcholine binding-plaats van de nicotine-receptor […] bleek ge-downreguleerd. De afwezigheid van dit gen-produkt zou een verstoord vermogen om te antwoorden op motor-neuron ‘vuren’, belangrijk voor trage-vezel-type specificatie, kunnen impliceren.

h) Spiervezel-fenotype. De transcriptie-factor […] (MYF6/MRF4/herculine) bleek ge-downreguleerd bij CVS-patiënten. Bij muizen accumuleert MRF4 in trage vezels en er is bewijs dat dit gen de verschuiving naar het trage vezel-type reguleert. Dit suggereert dat her-modelering van spieren gericht is naar het snelle fenotype bij CVS-patiënten, een interpretatie die overeenstemt met de resultaten van vezel-typering op eiwit-niveau [zie ‘Struktuur en funktie van skelet-spieren gewijzigd bij CVS’].

Downregulering van de volgende genen geïdentificeerd in de huidige transcriptoom-analyse ondersteunt ook deze visie: (i) calmoduline 1 (CALM1/fosforylase-kinase delta), coderend voor een belangrijk calmoduline-afhankelijk kinase dat betrokken is bij het decoderen van intracellulaire calcium-oscillaties tijdens myogenese en de differentiatie van het trage-vezel fenotype [calmoduline, CaM, ‘CALcium MODULated proteIN’, een calcium-bindend proteïne dat verschillende doelwit-proteïnen kan binden en reguleren, en zo verscheidene cellulaire funkties beïnvloeden]; en (ii) […] (Egralfa3), coderend voor een transcriptie-factor die sterk tot expressie komt bij ontwikkelende spier-spoelen [sensorische receptoren in de ‘buik’ van een spier die veranderingen detekteert in de lengte van deze spier]. Aangezien spier-spoelen meer aanwezig zijn in de delen van de spier waar type-I vezels geconcentreerd zijn, zou downregulering van dit gen consistent kunnen zijn met een spier-modelering in de richting van het snelle-vezel type. Andere observaties consistent met de visie dat gen-expressie veranderingen bij CVS een verschuiving van het vezel-type van traag naar snel weerspiegelen omvatten: (i) upregulering (beperkt tot vrouwelijke patiënten) van myosine lichte-keten kinase 2 (MYLK2), coderend voor een calmoduline-afhankelijk kinase dat sterk tot expressie komt bij regenererende en volwassen snelle vezels; en (ii) verandering (downregulering bij mannen, upregulering bij vrouwen) […] (CASQ2), coderend voor een proteïne dat kenmerkend is voor trage spiervezels, en belangrijk voor de opslag van calcium in het sarcoplasmatisch reticulum en regulering van de ryanodine-receptor [zie ‘Molekulair mechanisme voor verminderde inspanningscapaciteit’].

Bespreking

In deze studie vergeleken we de gen-expressie profielen van de vastus lateralis skelet-spier bij CVS-patiënten en gezonde controles. In deze context was het doel van onze analyse te bepalen of er consistente gen-expressie wijzigingen zijn bij CVS-patiënten die mogelijks de betrokkenheid kunnen tonen van skelet-spieren bij de manifestaties van deze ziekte en aanwijzingen zouden kunnen geven over welke cellulaire processen betrokken zijn. Eerdere studies door onze groep hebben specifieke oxidatieve veranderingen in DNA en lipiden in spier-stalen van CVS-patiënten aangetoond. Het is het vermelden waard dat Mn-SOD [superoxide-dismutase] deficiëntie geassocieerd is met ernstige lipiden-peroxidatie en andere ge-downreguleerde genen, zoals FDX1, NQO1 en IHPK3, kunnen mogelijks bijdragen tot de verstoring van anti-oxidante mechanismen. Daarenboven argumenteerden we dat oxidatieve schade zou kunnen voortkomen uit mitochondriale dysfunktie, gezien de gedocumenteerde strukturele veranderingen van mitochondriale cristae van de spier-mitochondrieën bij CVS, zoals eerder geobserveerd. [zie ‘Afwijkende Pijndrempels en Morfologie in de Spieren bij CVS’] Eén van de twee genen die ge-upreguleerd waren in spieren van mannelijke én vrouwelijke CVS-patiënten is het gespecialiseerd DNA-polymerase POLB (zeer belangrijk omwille van zijn ‘base excision’ [cellulair mechanisme dat beschadigd DNA herstelt door het verwijderen van kleine base-paar gebreken] herstel-aktiviteit), dat mogelijks betrokken is bij de respons op verhoogde oxidatieve schade. POLB-aktiviteit kan post-transcriptioneel verhoogd zijn door een vermindering van POLB-acetylering, die een verwacht gevolg zou zijn van CITED2 downregulering. We zouden ook kunnen speculeren dat het cyclisch voorkomen en de variabiliteit van spier-symptomen bij dit syndroom een weerspiegeling zou kunnen zijn van het weefsel-specifiek vermogen van POLB om ROS-geïnduceerde schade te herstellen. In een eerdere studie werd verhoogde anti-oxidante aktiviteit in CVS-spieren aangetoond, te wijten aan verhoogde aktiviteit van peroxidase, transferase en katalase. Deze uitkomst lijkt een poging van de CVS-spieren te vertegenwoordigen om zichzelf te beschermen tegen oxidatieve stress, gebruikmakend van anti-oxidante enzymen die tegen ROS werken in het cytoplasma, en POLB dat in de cel-kern DNA-mutaties helpt herstelllen. Deze positieve feedback is in overeenstemming met de hypothese van verhoogd peroxinitriet (Lane er al. 1998) die reeds werd voorgesteld. In feite zou stikstof-oxide kunnen reageren met ROS en het krachtige oxidant peroxinitriet genereren dat op zijn beurt het niveau van Mn-SOD zou kunnen doen dalen.

De downregulering van fosfofructokinase-transcripten bij al onze geteste patiënten suggereert de hypothese dat CVS, in bepaald opzichten, vergelijkbaar zou kunnen zijn met type-VII glycogenose of Tarui-ziekte, een zeldzame erfelijke ziekte veroorzaakt door fosfofructokinase-deficiëntie in spieren. Een tekort aan dit enzyme resulteert in glycogeen [polymeer van glucose, als energie-reserve opgesalgen in de lever en spieren] opstapeling in weefsels, wat symptomen veroorzaakt die inspanning-intolerantie of voortijdige vermoeidheid, zwakte en stijfheid bij inspanning, en pijnlijke spier-krampen omvatten. In veel gevallen heeft dit defekt systemische consequenties maar in andere gevallen beperken compenserende mechanismen de effekten tot bepaalde weefsels. In enkele patiënten met Tarui-ziekte werd de AMP-deaminase aktiviteit van erythrocyten verhoogd door calmoduline-afhankelijke intracellulaire calcium-verhoging. Bij CVS-spieren vonden we dat AMP-deaminase én calmoduline-fosforylase-kinase ge-downreguleerd waren. Dit is direct bewijs dat CVS en Tarui-ziekte verschillend zijn, gezien beide pathologieën klinisch significante spier-zwakte veroorzaken maar enkel bij CVS zijn de patiënten niet in staat hun verzwakte toestand te verhelpen met rust. De geobserveerde downregulering bij mannelijke én vrouwelijk patiënten van het gen coderend voor AMP-deaminase verdient bijkomende commentaar. AMP-deaminase is een enzyme dat van uiterst belang is bij het behouden van de beschikbaarheid van ATP bij inspanning. Eerdere studies hebben bewijs geleverd van gebrekkige adenylaat-aktiviteit in spieren van CVS-patiënten na matige inspanning [Fulle S, Mecocci P, Fanò G et al.. Specific oxidative alterations in vastus lateralis muscle of patients with the diagnosis of Chronic Fatigue Syndrome. Free Radical Biol. Med. 29 (2000): 1252-1259]. Deze verandering zou kunnen gelinkt zijn met de hoge vermoeibaarheid van CVS-patiënten tijdens uitputtende arbeid. De verstoorde afbraak van purine-nucleotiden in de spier werd geobserveerd bij andere ziekten zoals glycolytisch defekt ziekte en ook bij mitochondriale ziekten waarbij de lipiden- en koolhydraten-oxidatie zijn aangetast.

Het intense gebruik van anaërobe glycolyse voor de nood aan ATP tijdens matige inspanning en het verhoogd aantal glycolytische vezels impiceert lactaat-accumulatie. We kunnen suggereren dat in CVS-spieren er een samenloop is van metabool falen die een verstoring van aërobe energie-produktie, onderdrukking van de purine-afbraak en verhoogd lipiden-verbruik omvat.

Alle spier-stalen van CVS-patiënten vertoonden downregulering van FOXO3A, coderend voor […] één van de vooraanstaande regulators van ubiquitine-ligasen. Als we de mogelijke rol van fosforylatie bij FOXO-inaktivatie ter zijde laten, wat we niet bestudeerden, vonden we dat meerdere aan ubiquitine-proteïne-ligase verwante genen, die molekulaire doelwitten zijn voor FOXO-aktiviteit, waren ge-downreguleerd. Te samen genomen, wijzen deze gegevens naar een onderdrukking van atrofie-signalen in CVS-spieren.

Immuun-dysfunktie werd bij sommige CVS-patiënten gedocumenteerd […]. In onze analyse vonden we geen aanwijzing voor veranderde immuniteit noch enige indicatie voor inflammatoire respons. In tegenstelling daarmee vonden we dat enkele inflammatoire respons genen ge-downreguleerd waren.

Eén van de meest opvallende wijzigingen bij de huidige transcriptoom-analyse bij CVS-patiënten, is de uitgesproken downregulering van CHRNA1, coderend voor […] een component van de acetylcholine-bindingsplaats. De vermoeidheid ervaren door CVS-patiënten zou een centrale óf perifere oorzaak kunnen hebben. Het eerste hangt af van de ‘vuur-frequentie’ en het tweede van neuromusculaire transmissie en/of excitatie/contractie-koppeling. Eerdere rapporten suggereren dat de symptomen van vermoeidheid bij CVS kunnen worden afgeleid van een falen qua ‘vuur-frequentie’ van motor-neuronen en niet van een perifeer defekt in de spier-contractie. Onze resultaten leveren sterk bewijs voor deze opinie, suggererend dat daling van de transcripten leidend tot insufficiëntie wat betreft deze belangrijke cholinergische receptor component zou kunnen bijdragen tot verstoorde neuromusculaire transmissie.

[Dit zou ietwat contradictorisch kunnen lijken… Vroegere studies wijzen NIET op een perifeer defekt en de resultaten hier ondersteunen dit. TOCH wijst de bevinding van gestoorde neuromusculaire transmissie WEL op perifere betrokkenheid… Dr Pietrangelo verklaart (persoonlijke communicatie): “We hebben geen bewijs voor de etiopathogenese van CVS, dus weten we niet of de oorzaak centraal of perifeer is, maar toonden dat skelet-spieren bij CVS-patiënten (vezels en geaktiveerde genen) veranderd zijn. Dit wijst er op dat ook de periferie betrokken is door bv. gestoorde neurotransmissie, mitochondriale defekten, oxidatie enz. Meldenswaardig is dat bewijs is gevonden dat de toestand van de spieren het lot van motor-neuronen beïnvloedt (Musaro A et al., Cell Metabolism (2008) 8, 425-436, ‘Skeletal Muscle Is a Primary Target of SOD^1G93A-Mediated Toxicity’) dus denken we dat de interaktie spier-zenuw, en vice versa, heel belangrijk is.”. Een aantal hier gemelde wijzigingen qua gen-expressie wijzen consistent naar een verschuiving van trage- naar snelle-vezel fenotype zolas reeds aangetoond. Down-regulering van MRF4 en calmoduline-kinase suggereren dat regulerende mechanismen die het trage-vezel fenotype ondersteunen onderdrukt zijn. De downregulering van een gen zoals CASQ2, typisch voor het trage-vezel fenotype, en upregulering van MYLK2, hoewel beperkt tot een subgroep patiënten, is volledig consistent met deze interpretatie.

Ondanks de aanwezigheid van individuele en geslacht-specifieke variaties, zijn er significante en consistente wijzigingen in de transcriptie-profielen van spieren bij vrouwelijke en mannelijke CVS-patiënten; in het bijzonder zijn energie-produktie, management van oxidatieve schade, neuromusculaire transmissie en bepaling van het vezel-fenotype de belangrijkste biologische processen die betrokken zijn. Het is het melden waard dat vele processen gesuggereerd bij transcriptie-profiel analyse onafhankelijk ondersteuning vinden in eerdere rapporten, zoals in het geval van veranderde oxidatieve status en vezel-fenotype wijziging. Deze studie ondersteunt sterk de visie dat spiervezels direct betrokken zijn bij de funktionele en strukturele veranderingen die aan de basis liggen van de pathogene mechanismen van de ziekte en we kunnen besluiten dat spieren op molekulair en cellulair niveau betrokken zijn bij CVS-patiënten.

De twee studies (over veranderingen in vezel-fenotype en dit hier) plus het feit dat dit waarschijnlijk niet het resultaat is van deconditionering is opwindend. We vroegen hoe deze veranderingen ev. zouden kunnen worden omgekeerd Oefen-programmas lijken geen mogelijkheid, gezien vele patiënten een flinke terugval kennen na inspanning. Is er een andere (farmacotherapeutische) optie? Dr Pietrangelo: “ De enige suggestie die we hebben is uithouding-oefeningen. Dit zou het vezel-fenotype kunen doen verschuiven naar het trage type. Dit is echter slechts een opinie, we weten niet of dit effektief zou kunnen werken, en of het mogelijk is dit te doen bij deze patiënten en welk soort protocol zou moeten worden voorgesteld.”.

The University of Southern Nevada would like to extend a very special thank you to everyone who participated in and supported the university’s 1oth Annual Scholarship Golf Tournament. The event, held on Monday, Nov. 16 at the ultra-exclusive Cascata Golf Club in Boulder City, was by far the institution’s most successful.

According to Barbara Wood, USN’s director of university relations and tournament organizer, 88 golfers played in the tournament, which was the golf course’s largest in 2009. The tournament, along with silent and live auctions, raffles and other fundraising activities, raised more $91,000 with net proceeds of more than $50,000 benefiting student scholarships and services.

“The weather was absolutely perfect,” said Barbara Wood. “The golfers had a great time playing the spectacular mountain course. The food was incredibly delicious and the auction, led by national auctioneer Christian Kolberg, was both entertaining and effective. KLAS-TV’s morning news anchor, Dave McCann, presented an outstanding awards program.”

This year’s tournament included some high-profile players, including Bob Miller, former Nevada Governor; Brian Greenspun, chairman and CEO of the Greenspun Corporation and editor of the Las Vegas Sun;  Michael Montandon, former North Las Vegas Mayor and current gubernatorial candidate and numerous members of USN’s Board of Trustees.

Among the 23 teams, the winners were:

1st Place – The Buckeye Boys, led by Dr. Justin Kullgren, assistant professor of pharmacy practice at USN’s College of Pharmacy.
2nd Place - Featuring players from River Park Ten, Layton Construction Company and MHTN Architects.
3rd Place – A team led by USN’s President, Dr. Harry Rosenberg.

Newly public Dollar General has beefed up its board with Warren Bryant and Bill Rhodes, two veterans of the retail sector. The appointments bring the Goodlettsville-based retailer’s board to six.

Bryant, 63, was president and CEO and later chairman of Longs Drug Stores, the West Coast chain acquired by CVS last year. (That buyout also provided Dollar General with Todd Vasos, its current merchandising chief.) Prior to joining Longs, Bryant was senior vice president at Kroger from 1999 to 2002.

Rhodes, 42, was elected chairman of AutoZone in June 2007, adding that title to the president, CEO and director positions he’s held since 2005. He joined AutoZone in 1994 from Ernst & Young.

Rick Dreiling, Dollar General’s chairman and CEO, said Bryant and Rhodes have “worked across multiple facets of a retail organization, developing expertise in store operations, supply chain management and finance. We are confident that both will bring valuable knowledge to our newly-expanded board and look forward to their contributions as we drive the Dollar General business forward.”

The New York City Council is voting on a resolution this morning supporting the idea that Internet service providers cannot discriminate against web traffic on their networks. As gestures go, this resolution supporting net neutrality is more symbolic than it is useful, and to that end has already yielded a couple of photos of FCC Chairman Julius Genachowski and New York Mayor Mike Bloomberg shaking hands and looking concerned about the future of NYC broadband.

However, New York City is a nice case study for competitive broadband, so I hope Genachowski took notes while he was there. Verizon, (s vz)  Time Warner Cable (s twc) and  Cablevision (s cvs) all serve the city (although they don’t all compete head to head in every block), so some residents there have access to both fiber to the home and cable’s wideband service using DOCSIS 3.0. Our own Ryan Lawler, who lives in Harlem, is paying $33 a month for Internet service from Time Warner Cable, which currently has the monopoly in his building. However, he says he just got a note informing him that Verizon FiOS will also soon be available, so we’ll see what happens to his cable speeds (which are currently 9 Mbps down and 500 kilobits per second up) and service in the next few months.

The state’s CIOwants to collect real data on broadband penetration and speeds so that it can get a sense of customers’ level of satisfaction with their service as opposed to just knowing which areas have broadband and which don’t. A Congressman from the state is also attempting to pass legislation that would give the FCC the power to regulate certain aspects of net neutrality and broadband access. So given the competition, better data collection and consumer-friendly legislative efforts, I hope Genachowski is doing some learning, not just politicking for a symbolic victory on net neutrality.

Target and other national chains (such as Best Buy and Kohl’s) have opportunities to open small-format stores, not only in urban markets where a smaller footprint is required, but also in smaller markets that can’t sustain the volume of a full-size prototype store. The challenge to Target in particular is to consider how to edit its businesses and assortments to suit the urban customer. Does it focus on consumables, HBA and other convenience-based categories? (If so, it runs the risk of being perceived as a large-format Walgreens or CVS.) It’s easier to determine which businesses ought to be eliminated completely (I would start with categories like CDs and automotive) than to decide which categories to focus on. But if Target plays to its strengths in trend apparel and home goods, its brand positioning becomes a lot more consistent, despite the company’s more recent emphasis on food.
