I conquered my first weekend on plastic surgery call. Despite my anxiety and horrible expectation it wasn&#8217;t that bad. 4th of July night was spent in the OR operating on a patient who fell from her boat ladder and suffered a propeller injury to both legs. She had large complex lacerations involving the lateral leg and knee joint space. Unfortunately she suffered a foot drop on one leg necessitating a nerve graft with Avance (acellular nerve conduit). We restored her peroneal nerve function, sutured her lacerations, and skin-grafted some degloved portions. Overall, the patient is doing very well and should make a full recovery.</p>
<p>Today marked my introduction into flap reconstruction. We performed a free radial forearm flap to cover a lateral heel wound after a melanoma excision. An island of skin was removed from the arm including the palmaris longus tendon, radial artery, and cephalic vein. This skin island was transported to the lateral aspect of the heel and plugged into the posterior tibial artery, saphenous vein.  The procedure duration was about 5hrs w/ excellent result. Close flap monitoring tonight will ensue. My second OR case involved a free latissimus dorsi flap to cover a lower leg degloving/tibial fracture after someone wrecked their ATV. I caught that tail end of the case, but was able to assist w/ skin graft coverage of the flap. This may sound gross, but the lat flap has some resemblance to a flank steak.</p>
<p>Bilateral DIEP flaps for breast reconstruction are planned for tomorrow&#8230;.I&#8217;ll be the low-man on the totem pole, and won&#8217;t be able to do too much, but it will be my first DIEP&#8230;.</p>
