The competition was launched by Titanium Information Group (TIG) “looking for a new design to celebrate the properties of titanium and highlight the capabilities of advanced manufacturing”.

Our proposal was designed for the process chosen for the production of the trophy, that it’s Arcam electron beam melting (EBM) with titanium powder (Ti6Al4V) and it will be made at the Mercury Centre and coloured using anodising by Poligrat.

Auto/Sexual is another 17 Sons Records label artist that I can’t find much information about. This project has a few different releases out on the 17 Sons label, mostly Internet-download and MP3 formats of shorter style EPs. Ultra has five songs of decaying dubstep and rhythmic noise, often redacted and studded with manipulations of beats and sounds. The main focus seems to be the use of patters in the background, drum beats that have been fashioned out of different textures of sound and electronic noise.

Your enjoyment of Ultra, and quite possibly the whole Auto/Sexual project, will depend on how susceptible you are to electronica beats and rhythmic, noise-influenced rave glitch jams. There are moments of blown-out noise sound, like in the opening of the short “Disengagement is Betrayal”, and each of the songs uses electronic noises as music for beats, but for the most part Auto/Sexual is more interested in crafting jumpy, bass-heavy jams out of potentially destructive sounds rather than vice versa.

But the ideas that Ultra extols are nicely nuanced, alternating from loud extremes to pared down beats that click and clamor in the background until slowly evolving into more danceable songs. Each track often features a Venetian Snares-like breakcore drum background, but while it’s intriguing for a little while, the notion tends to wear thin once the tracks have reached their repetitive hook – for instance, “Obey or Die”, which loops through the same drum riffs over and over again.

The project is much more successful when it’s recycling more apparent electronic noise along with the drum beats, like on “Wolves or Bees” or the more minimal “Main Target”. That pairing works nicely, especially the low bass notes with the staccato click of static or glitchy samples. It’s the moments where the main focus of the track is the electro-drumming that Ultra falters, tending to mistake repetitive beats as hypnotic sound.

First of all, I want to thank you for stopping by this Blog. Here’s what has been happening to this project.

Many of you should know that a lot of things change after one starts planning a Blog. The ideas get refined, changed around, and usually brought to a whole new area that was never thought of. This certainly is true with this one.

The albums and CDs in my collection that have been marked down as Cut Outs span a whole lot of music, and usually have some tales to tell about the industry both Major and Indie. I have recently been discovering a ton of QUALITY albums, some actually great or at least exciting sounds from The 90′s when there were lots of Indie companies trying to get their foot through the doors or at least those that have tried to reissue classic sounds from the past and sadly had to discontinue either certain releases or the whole company due to a number of reasons.  Some of these have been forgotten, but some are re-issued.

I’m sure those who love Punk have noticed the first Dangerhouse collections…I also have the second one as well, also as a C-O. They have been re-issued recently, but there was a time when these compilation were distributed through BMG when an interest in classic Punk was going into the Popular Culture once again. More of that in the next post!

The quality releases also extend to albums of the 60′s and 70′s that have impressed me. Of course, there was Raw Power profiled in the last post – a very influential work that has been iconic of many music scenes throughout  the world. Thankfully, I have a number of those in actual C/O LP form, in playable, certainly not too collectible, condition (I buy them to hear them), so I should be able to display them for you very soon.

Also, there have been a number of classic collections that were released a long time back which have been given the CO in an age when people would rather get their tunes through download or don;t really have the time to seriously go through two mega discs filled with musical greatness. This is just one of two of these collections I have, and there were a couple more I don’t have (yet) seriously due to already having the songs – There is a Love Double Disc set that has been given the CO, and of course you should check it out if you have not already (or at least check out some of their music anyways). This also goes for Re-issues and other archival releases.

Also, albums that under-performed in The US that did fine in other places which remain great to my ears today.

Still, there may be some of you who are thinking “Hey! A Blog about Cut-Out albums should be about cheesy music, albums that failed, and utter garbage, right?” Well…not really.

I was actually thinking about that when I first started this up, but the more I dove into my collection, there have been numerous surprises. Add to that going through my books and magazines for some historical details as well as relying on my memories to build up on some stories, and you have an idea that this should be more than just a generic “Armchair Critic” happening that tries to play Music Hipster. I would rather be the Educator who entertains, surprises, and informs.

Let’s hear some music for now! One of the CDs I’m holding is Clock DVA’s Voice Recognition Test, released by Contempo, and I think it’s a very good slice is EBM enough for me to get this on for you now. The release is on a label that appeared to have seriously stopped it’s main presence by 1994/5, although I think it may show up for a very sporadic release from time to time. As for the disc itself, the concept behind this is very interesting, with the first paragraph of the notes reading like this:

VRT – The crucial sound source for most robotics applications also happens to be the most intricate and the most inscrutable – the human voice. 

Mix science with occult beliefs, and then throw in a whole bunch of good beats. Clock DVA created some good sounds, and even a good video for them. Adi Newton and Robert Baker were Clock DVA for this 1993 release “Researched and developed at the Anterior Digital Research Station,” while the front cover is a voice imprint from Marshal McLuhan and Quentin Fiore’s The Medium is the Message from 1967.

Từ kết quả nghiên cứu đến thực hành lâm sàng

Trước khi dựa vào kết quả của các thử nghiệm lâm sàng có nhóm chứng ngẫu nhiên (RCT), cũng như các phân tích gộp (meta-analysis), để đưa ra quyết định điều trị trong thực hành lâm sàng, chúng ta cần đánh giá các thử nghiệm đó một cách thận trọng. Thầy thuốc phải hiểu rõ ưu, nhược điểm của nghiên cứu, nắm được thiết kế nghiên cứu và hiệu quả thực sự của nghiên cứu, bởi đó là những vấn đề nền tảng của y học thực chứng (quyết định điều trị dựa trên bằng chứng khoa học thu được từ các thử nghiệm lâm sàng).

Califf và DeMets (2002) đã trình bày rất rõ ràng và sâu sắc về tầm quan trọng của y học thực chứng, khi so sánh với y học dựa trên trực giác:

 “Kinh nghiệm cá nhân của một thầy thuốc đơn giản là không đủ để nhận biết hiệu quả của điều trị, khi xét đến khía cạnh dự phòng các biến cố trong tương lai của một bệnh mạn tính… Mặc dù thực hành lâm sàng đòi hỏi thầy thuốc phải theo dõi sát các triệu chứng cũng như tiến triển của người bệnh, đây không phải là biện pháp tốt nhất để đánh giá liệu một phác đồ có thực sự hiệu quả hay không , đặc biệt là khi điều trị lâu dài một bệnh mạn tính. Ngoài ra, trải nghiệm có được với một bệnh nhân cung cấp rất ít thông tin về bệnh nhân tiếp theo. Vì thế, để tìm ra phác đồ điều trị tối ưu, cần tiến hành các thử nghiệm lâm sàng cỡ mẫu lớn”.   

Các vấn đề về thử nghiệm lâm sàng

Như đã đề cập, việc sử dụng các thử nghiệm lâm sàng có nhóm chứng ngẫu nhiên (RCT) là cần thiết. Những thử nghiệm này có thể đánh giá, với độ tin cậy cao, về hiệu quả điều trị và dự phòng biến cố tim mạch ở những bệnh nhân THA điển hình trong một khoảng thời gian tương đối ngắn, từ 3 đến 5 năm, khi mà việc theo dõi sát bệnh nhân còn khả thi (Collins và MacMahon, 2001). Tác giả Vandenbrouke (2004) kết luận RCT là cần thiết, bởi “các nghiên cứu quan sát về hiệu quả điều trị chỉ đáng tin cậy trong một số trường hợp ngoại lệ”.

Về cơ bản, các thử nghiệm có nhóm chứng ngẫu nhiên vẫn có thể bị sai lệch, một phần do bản chất loại hình nghiên cứu này, một phần do sai lầm chủ quan của con người (Rothwell, 2005). Đặc biệt, khi các hãng dược phẩm ngày càng hỗ trợ tài chính nhiều hơn cho các thử nghiệm lâm sàng về những chế phẩm của họ, sai số càng dễ xuất hiện, có thể là sai số trong chọn nhóm so sánh, trong phương pháp nghiên cứu, hoặc báo cáo một cách có chọn lọc các kết quả nghiên cứu, hoặc đưa ra nhiều kết luận dương tính hơn so với các nghiên cứu được cấp vốn từ những nguồn phi lợi nhuận.

Ngoài những sai số kín đáo khó nhận thấy liên quan đến nhà tài trợ, còn nhiều yếu tố khác có thể khuyếch đại, hoặc ngược lại là triệt tiêu, hiệu quả thực sự của phác đồ điều trị.

Những nguyên nhân làm hiệu quả điều trị giảm đi so với thực tế

Các thử nghiệm có thể đánh giá quá thấp lợi ích thực tế của thuốc hạ áp do một số lý do sau:

Chọn đối tượng nghiên cứu không phù hợp

Bệnh nhân THA được đưa vào nghiên cứu thường dựa trên trị số huyết áp đo được ở cơ sở y tế trong vòng 1-2 tháng gần đây. Như chúng ta đã biết, đo huyết áp ở cơ sở y tế có thể tính cả những đối tượng THA thoáng qua hay THA “áo choàng trắng”, do đó làm giảm hiệu quả chung của phác đồ điều trị. Mọi thuốc hạ áp đều hiệu quả cao với bệnh nhân có huyết áp khởi điểm cao, trong khi rất ít tác dụng nếu người bệnh không có THA bền bỉ.

Thời điểm điều trị quá trễ

THA có thể gây tổn thương cơ quan đích trước khi người bệnh có số đo huyết áp đủ cao để đưa vào nghiên cứu. Vì vậy, ngay cả khi bệnh nhân được điều trị tốt, những tổn thương này vẫn không thể phục hồi, nhất là khi các yếu tố nguy cơ khác chưa được kiểm soát.

Thời gian điều trị quá ngắn

Các thử nghiệm lâm sàng thường kéo dài dưới 5 năm. Tuy nhiên, lợi ích của thuốc hạ áp thường đòi hỏi nhiều thời gian hơn để biểu hiện rõ ràng.

Điều trị không đủ liều

Phần lớn các thử nghiệm lâm sàng đều giúp trị số huyết áp giảm đi 12/6 mmHg so với thời điểm ban đầu. Mức hạ áp này không đủ để giảm thiểu các tổn thương do THA. Hiệu quả dự phòng biến chứng liên quan đến trị số huyết áp tuyệt đối đạt được trong quá trình điều trị, hơn là trị số huyết áp trước điều trị. Có tới 40% bệnh nhân tham gia nghiên cứu không đạt số đo huyết áp mục tiêu (Mancia và Grassi, 2002), do đó nếu chúng ta điều trị tích cực hơn, hiệu quả đạt được sẽ cao hơn.

Bệnh nhân không tiếp tục tham gia nghiên cứu

Trong một số nghiên cứu, khoảng 25% số bệnh nhân không hoàn thành thời gian theo dõi. Nói chung, đây thường là các bệnh nhân nặng, với nhiều yếu tố nguy cơ. Vì vậy, kết quả nghiên cứu bị ảnh hưởng nhiều (Linjer và Hansson, 1997).

Bệnh nhân chuyển sang nhóm khác

Trong mọi thử nghiệm lâm sàng, sẽ có nhiều bệnh nhân ban đầu được lựa chọn ngẫu nhiên vào nhóm dùng giả dược, nhưng sau đó sẽ chuyển sang nhóm điều trị, do trị số huyết áp tăng cao đến mức cần phải điều trị. Theo Ramsay (1996), việc điều trị cả những bệnh nhân thuộc nhóm chứng sẽ làm giảm tỉ lệ biến cố của nhóm chứng, dẫn đến hạ thấp hiệu quả thực sự của thuốc hạ áp.

Tác hại của thuốc

Trước đây, thuốc hạ áp được sử dụng chủ yếu trong nghiên cứu ở đối tượng dưới 60 tuổi là thuốc lợi tiểu liều cao, hoặc chẹn thụ thể beta giao cảm không chọn lọc. Y văn đã ghi nhận một số rối loạn chuyển hoá gây ra bởi các thuốc này, nhất là rối loạn lipid máu và nồng độ glucose-insulin. Rõ ràng, các tác dụng phụ này sẽ triệt tiêu lợi ích bảo vệ mạch vành có được từ việc giảm trị số huyết áp.

Bệnh nhân không tuân thủ điều trị

Người bệnh có thể không điều trị đầy đủ theo phác đồ, dẫn đến hiệu quả của thuốc bị giảm sút. Mặc dù nghiên cứu viên có thể cố gắng kiểm soát số viên thuốc người bệnh đã uống, không có gì đảm bảo chắc chắn bệnh nhân tuân thủ tuyệt đối phác đồ điều trị.

Những nguyên nhân làm hiệu quả điều trị tăng lên so với thực tế

Ngược lại, các thuốc hạ áp có thể không có được hiệu quả như một số kết quả công bố, do trong thực tế lâm sàng không thể áp dụng đúng phác đồ như trong nghiên cứu (Rothwell, 2005). Ngoài ra, số liệu từ các thử nghiệm lâm sàng có thể đánh giá quá cao lợi ích của thuốc hạ áp, vì một số nguyên nhân trình bày dưới đây.

Tiêu chí đánh giá không phù hợp

Để tối ưu tác động của thuốc, người ta sẽ đánh giá rất nhiều tiêu chí đích, một số tiêu chí mang tính chủ quan và không thực sự có ý nghĩa như số lần nhập viện. Theo Lauer và Topol (2003), tiêu chí chính duy nhất quan trọng là tử vong do mọi nguyên nhân. Đây là một tiêu chính khách quan, không bị sai số, và có tầm quan trọng trong thực hành lâm sàng. Các tác giả này nêu rõ “Mọi tiêu chí đích đòi hỏi sự đo đạc tính toán đều mang yếu tố chủ quan của con người và có nguy cơ sai số”.

Loại bỏ các bệnh nhân nguy cơ cao khỏi nghiên cứu

Các thử nghiệm lâm sàng trước đây thường loại bỏ những đối tượng có triệu chứng tim mạch, có tổn thương cơ quan đích, hoặc có nhiều yếu tố nguy cơ. Vì thế quần thể nghiên cứu tương đối khoẻ mạnh và có tiên lượng tốt hơn.

Sự tuân thủ điều trị chặt chẽ hơn thực tế

Bệnh nhân tham gia nghiên cứu sẽ được khám bệnh và phát thuốc miễn phí, cũng như được theo dõi lâm sàng chặt chẽ hơn trong thực tế. Do đó họ có xu hướng tuân thủ điều trị hơn, và hiệu quả lâm sàng của thuốc hạ áp sẽ rõ rệt hơn.

Số liệu tương đối so với số liệu tuyệt đối

Trong phần lớn báo cáo về thử nghiệm lâm sàng, mức độ giảm tỉ lệ biến cố tim mạch (bệnh mạch vành, đột quỵ) được biểu diễn bằng độ giảm nguy cơ tương đối, là chênh lệch về tỉ lệ mắc bệnh giữa nhóm điều trị và nhóm chứng. Tuy nhiên, một chênh lệch tương đối lớn có thể chỉ là biểu hiện của một chênh lệch tuyệt đối nhỏ. Lấy ví dụ, tỷ lệ đột quỵ của nhóm điều trị là 0,9%, trong khi nhóm chứng là 0,15%. Như thế, việc điều trị đã giảm được 40% số ca đột quỵ (0,15-0,9/0,15). Tuy nhiên mức giảm nguy cơ tuyệt đối chỉ có 0,6% (0,15-0,9). Việc báo cáo kết quả theo tỉ lệ tương đối dẫn đến suy nghĩ có nhiều người bệnh hưởng lợi từ điều trị hơn lợi ích thực tế của thuốc.

Ngày nay, người ta sử dụng giá trị “số bệnh nhân cần điều trị để một người được hưởng lợi” (Number Needed to Treat, NNT) Giá trị này được tính bằng nghịch đảo của chênh lệch nguy cơ tuyệt đối. Theo đó, một thuốc giảm được 0,6% bệnh nhân đột quỵ (trong số toàn bộ quần thể nghiên cứu) đồng nghĩa với việc cần điều trị 167 bệnh nhân để phòng ngừa một ca đột quỵ. NNT “truyền đạt được cả ý nghĩa thống kê và ý nghĩa lâm sàng tới người thầy thuốc” và “có thể dùng để áp dụng các kết quả nghiên cứu vào một bệnh nhân ở một mức nguy cơ nền nhất định” (Cook và Sackett, 1995).

NNT dựa trên việc giảm nguy cơ tuyệt đối, chính xác hơn so với giảm nguy cơ tương đối. NNT có liên quan đến thời gian tiến hành thử nghiệm lâm sàng. Việc tính toán NNT có giá trị nhất khi kết hợp với sự khác biệt về biến cố (hazard difference), thường được thể hiện ở dạng tỉ lệ tử vong trong một đơn vị bệnh nhân-thời gian (Lubsen, 2000).

Đa số nghiên cứu gần đây công bố kết quả ở dạng đường cong sống còn, vẽ theo công thức của Kaplan – Meier, dựa trên ước tính các biến cố tim mạch kể từ thời điểm bắt đầu nghiên cứu. Nếu được xây dựng đúng, đường cong Kaplan – Meier thể hiện rất tốt kết quả của những thử nghiệm lâm sàng có nhóm chứng ngẫu nhiên.

Phối hợp thuốc điều trị

Để đạt được huyết áp mục tiêu, ví dụ dưới 140/90 mm Hg, các nghiên cứu so sánh (thuốc hạ áp với giả dược hoặc so sánh đối đầu hai thuốc) thường phối hợp thêm thuốc điều trị. Ở nhiều nghiên cứu, có trên 80% bệnh nhân được dùng hai đến ba thuốc hạ áp. Lợi ích được quy cho thuốc trong nghiên cứu lại có thể do những thuốc phối hợp khác tạo ra (McAlister, 2003).

Giải pháp cho vấn đề với các thử nghiệm lâm sàng

Hiển nhiên là các nghiên cứu viên cần tuân thủ các hướng dẫn về thử nghiệm lâm sàng đã được công bố, như CONSORT (Rennie, 2001) hoặc khuyến cáo của Rothwell (2005). Tuy nhiên, thầy thuốc lâm sàng cần phải biết cách đánh giá và kiểm định các số liệu nghiên cứu. Nói như tác giả Montori (2004): “Khoa học thường không khách quan”. Sự tham gia của cảm xúc vào các ý tưởng nghiên cứu, cũng như những lợi ích cá nhân liên quan đến thành tựu học thuật, có thể dẫn nghiên cứu viên đến việc thổi phồng tầm quan trọng và chất lượng của kết quả công trình nghiên cứu. Xung đột về quyền lợi càng rõ rệt khi các tổ chức lợi nhuận (gồm cả những công ty dược phẩm) cấp vốn cho quá trình nghiên cứu, đồng thời tham gia phân tích số liệu và trình bày báo cáo.

Montori và cộng sự (2004) đã đề ra các biện pháp giúp thầy thuốc không bị chi phối bởi những thử nghiệm lâm sàng có sai số:

-       Đọc phần Phương pháp nghiên cứu và Kết quả. Trong phần Bàn luận, các tác giả thường đưa ra những suy luận dưới góc nhìn thiếu trung lập.

-       Đọc phần tóm tắt và nhận xét từ những nguồn khách quan như ACP Journal Club, Evidence-based Medicine, Up-To-Date,…

-       Nhận biết những phương pháp so sánh không chặt chẽ. Người ta thường lựa chọn những phương pháp so sánh yếu trong các nghiên cứu đối đầu, rõ ràng nhất là thử nghiệm về thuốc chẹn beta atenolol (Carlberg và cs., 2004)

-       Hiểu rõ các tiêu chí của nghiên cứu. Tỉ lệ tử vong do mọi nguyên nhân là tiêu chí quan trọng và ít bị sai số.

-       Nhận thức được các hiệu quả điều trị ít có ý nghĩa, nhất là khi số liệu được công bố dưới dạng chênh lệch nguy cơ tương đối. Lưu ý trường hợp khoảng tin cậy 95% chứa cả giá trị ngưỡng.

-       Lưu ý các phân tích dưới nhóm. Các phân tích cần thỏa mãn một số điều kiện nhằm đảm bảo sự khác biệt giữa các đáp ứng dưới nhóm là thực, cụ thể là nghiên cứu chỉ kiểm tra một số nhỏ các giả thuyết đã được xác định từ trước khi có kết quả nghiên cứu (Freemantle, 2001).

Bên cạnh các cạm bẫy nêu trên, chúng ta cũng cần nhiều thử nghiệm được thiết kế dựa trên thực tiễn, nhằm tập trung trả lời các câu hỏi của bác sỹ lâm sàng, giúp họ đưa ra quyết định điều trị. Đa số nghiên cứu ngày nay được tài trợ bởi hãng dược phẩm, với mục tiêu hàng đầu là khẳng định ưu thế của sản phẩm của họ. Các viện nghiên cứu hay những tổ chức phi lợi nhuận lại thường hướng đến khoa học cơ bản, hoặc các vấn đề rất chuyên sâu, hơn là đầu tư cho những thử nghiệm lâm sàng tốn kém đem lại lợi ích thiết thực cho thầy thuốc điều trị.

Trong hoàn cảnh đó, các nhà lâm sàng cũng như sinh viên y khoa cần tận dụng tốt hơn những nguồn thông tin y học thực chứng sẵn có. Thư viện Cochrane là hệ thống cung cấp phong phú nhất, tuy nhiên ngày càng có nhiều nguồn khác nữa, đa số là miễn phí.

If you been paying attention to SynthWave recently you’ll have noticed that there is more and more music being made that feels a lot like old School EBM. French producer Perturbator takes that retro harshness and mixes it up with a healthy dose of Jarre on his new EP ‘Night Driving Avenger’.

The title track growls like and angry robotic beast as those machine arpeggiated basslines hammer along at breakneck speed while Perturbator shows off his true skill, melody and solos. On every track on the EP his ability to craft emotional musical beats that really get across the mood of the tune or just impress the hell out of you with his skills. As a big fan of Jarre and Vangelis is awesome to hear this kind of epic synth work set against a pounding beat. ‘Grim Heart’ carries on in this tradition, the EBM aggression playing against the shining synth work. It’s not all high-octane powerhouses though, ‘Electric Dreams’ is a beautiful, haunting peice that would easily be suited to the quieter moments in BladeRunner, take that Vangelis! The EP contains one more track, ‘Miami Sunsets’, but I’ll make you head on over to Perturbator Sound Cloud to hear it so you can check out his vast back catalogue of synth epics.

According to Evidence Based Medicine (EBM), healthcare professionals should make conscientious, explicit and judicious use of current best evidence in their daily practice.¹

How does one come to do this?  

Clinical practice guidelines are systematically developed statements to assist practitioners with decisions about appropriate health care for specific patients’ circumstances.²

In  medicine scientific evidence is classified as follows:

• Level of evidence A: recommendation based on evidence from multiple randomized trials or meta-analyses
• Level of evidence B: recommendation based on evidence from a single randomized trial or nonrandomized studies
• Level of evidence C: recommendation based on expert opinion, case studies, or standards of care.³ 

Once the level of evidence has been reviewed, recommendations can be made. These recommendations are also classified:

Class I: conditions for which there is evidence and/or general agreement that a given procedure or treatment is useful and effective

Class II: conditions for which there is conflicting evidence and/or a divergence of opinion about the usefulness/efficacy of a procedure or treatment

Class IIa: weight of evidence/opinion is in favor of usefulness/efficacy

Class IIb: usefulness/efficacy is less well established by evidence/opinion

Class III: conditions for which there is evidence and/or general agreement that the procedure/treatment is not useful/effective and in some cases may be harmful.

Thus, level of evidence C and class II indicate, respectively, recommendations lacking supporting evidence and those subject to uncertainties about the appropriate medical decision.³

Using techniques from science, engineering and statistics, such as the systematic review of medical literature, meta-analysis, risk benifit analysis, and randomized control trials. Ex cathedra statements by the “medical expert” are considered to be least valid form of evidence. All “experts” are now expected to reference their pronouncements to scientific studies.³ 

Based on this set of guidelines, can we classify religious writings like the Bible, the Bhagavad Gita or for that matter any religious book?

Let us give it a whack…

Is any of these books based on multiple randomized trials or meta-analyses?

No.

One randomized trail or nonrandomized studies?

No.

Is it based on expert opinion, case studies or standar of care?

Yes  (even this Yes is up for debate)

So, according to this review, the Bible and other religious books are  a Level of evidence C.

And now, on to set the class recommendation.

Following the evidence this type of writings must be a Class III ( “Class III: conditions for which there is evidence and/or general agreement that the procedure/treatment is not useful/effective and in some cases may be harmful”.)

So if the Bible or any religious writing were subject to evaluation of scientific evidence it would ultimately be seen as Level C and Class III, not acceptable as grounds for any kind of treatment, in fact you would most likely be setting yourself up for a nice malpractice lawsuit if you based your treatment on this level of evidence.

The biography “Man Leben – One Life” is composed by Carla Drift based upon the bundled posts from September – December 2011 about the quest to “Who are you”.

Man Leben, Narrator and Carla Drift are the three main characters in this part of the Odyssey.

The Odyssey to “Who are you – survey into our existence” is an quest with many stages. The search for “Who are you” is about you and me and all that is in connection with us. Nothing is on beforehand excluded. Are you and I connected or are we separated? What makes you to the person who you are? Who are you before your birth and who will you be after your death? The answer to these questions is currently unknown, but nevertheless we raise these questions.

Dear Readers,

I’m happy to share another clean eats with you! While research is divided over the roll of food sensitivities on AD (eczema), many people find benefit from eating a hypoallergenic diet. This is a diet devoid of foods found to be troublesome to the individual. Those foods may cause digestive upset, skin irritation, fatigue, and more.

This recipe for Mai Tasty Lentil Soup is devoid of all the major allergens: gluten, dairy, corn, nut, soy and eggs. Just keep in mind I cook from the heart, rarely using measurements. Let your taste buds be your guide!!

Tasty Lentil Soup by Mai

3-4 c lentils (red/brown or green)

–soak these overnight and drain the rinse water

2-3 onions, diced

2 large carrots, diced

3 large celery stalks, diced

6 medium potatoes, chunked

2 cups peas, frozen or fresh

~1/3 c tomato paste

~1/4 c red wine

~1 tbsp EVOO

~12 cups of water (or enuff to just cover the veggies)

1/2 tbsp turmeric

1/4 tbsp cumin

basil, rosemary, thyme, oregano give it a nice flavor (or add your favorite commercial Italian seasoning blend)

salt, blk pepper, to taste

red pepper flakes &/ cayenne powder, to taste

Combine water, lentils, potatoes, carrots, onions, and celery in your crock pot (or large cooking pot). Cook until warm. Add tomato paste, salt, pepper, turmeric, cumin, red pepper flakes/cayenne, and Italian blend. Cook until potatoes and carrots are only slightly firm. Add peas, wine, and EVOO. Simmer until peas are thoroughly warm. Decrease temperature, allow soup to marry (settle/sit) covered for 15-20 mins.

Eat and Enjoy! This should feed ~8 adults.

This is a great clean out your fridge recipe leftover mushrooms, ham, chicken, would all be a good addition

Anything with the name Fredrik Croona attached and it is worth more than just a cursory glance, as chances are you will be delving into something that in the very least will rile up the senses and inflame the darkest corners of the soul. With his new solo project Cynical Existence he does that and much more. A Familiar Kind of Pain is the debut EP from the venture and a release that invades and clings with the greatest of satisfaction whilst twisting and manipulating for the deepest pleasure. Across eight tracks the release rampages and stomps incessantly, and from start to finish provokes and taunts to inflame the ear and far beyond every second of the way.

From Göteborg, Sweden, Croona has constantly kept the ears of industrial and ebm fans challenged and fulfilled as formerly part of Menschdefekt and more distinctly since with his band Project Rotten, as well as collaborations with varied artists most recently with the likes of Washington duo Scream Machine. He began Cynical Existence in 2011 with the intent of bringing old school harsh ebm and industrial into a more emotionally driven sphere. Croona himself states that the project “does not intend to create something new and unique, just something personal.” Released April 1^st on dark electro label Engraved Ritual the release leaps at the ear with an intention and passion that can only be organically inspired from within, its infectious conjurations forging a deep and unrelenting connection to its listeners.

The EP opens with Always and Forever’a pulsating and bewitching track which paces itself from the off. Straining at but never let off the lease the song taunts and prowls around the ear, its dark electro veins twitching around crystalline flashes of sound and resourceful melodic infections which finger the senses with acidic energy. The song is a strong opener if not one to overload the senses, that pleasure is left for the following tracks to bring.

A Familiar Kind Of Pain leaves no emotion and feeling untouched or scorched. The likes of the mesmeric and shadowed Dead Eyes (See No Future), Release Me a song that lays wanton and intrusive razorblade sharp melodies through the ear before expanding into migraine inciting heavy cranial thumping like a aural version of Scanners, and the blackened recessed Licking My Skin all gather one up with a leech like grip and consumption which is glorious. Varied and unpredictable the release drains and ignites the senses at the same time, the result an irresistible experience that though one is left a lifeless shell has to dive right back into the experience immediately again.

The EP grips hard and impressively from the off and only ascends from there on in but there are two tracks which make the release an essential have alone. Insecure is a snake charmer of a song. It hypnotises and captivates with dazzling and caustic melodies that exploit and incite limbs and heart into capitulation whilst stomping with thunderous beats on a shattered body. It is a deranged puppeteer with the devil in its soul and probably the best song heard in a long time anywhere. Paradox comes very close to rivalling it though with its anthemic fist thrusting unrelenting heavy electro raging. The song sweeps one up in its surging waves of blood pumping energy whilst all the time plundering and blistering the ear with tempestuous slithers of lethal melodies and delicious sonic molestations.

A Familiar Kind Of Pain is immense, a stunning play with light and dark in sound, emotion, and heart. It avoids any formulaic moments by sheer ingenuity and the allowing of instinctive thought and emotions to drive its course. Without taking away from his fine musical exploits to date this might well be Fredrik Croonas finest moment, it is certainly one of the best things to hit the genre in a long time.

I’m happy to share with you that Clarke has been much better these past 2-3 days. She’s sleeping better, scratching less, and less red/inflamed. While her signs and symptoms of AD (eczema) are never fully absent, she is fully present. Whole and complete. The largest contributor to her current picture is a stressing one. Corticosteroid creams. Why is this troubling me? Because of all the harm that corticosteroids can do.

AD is linked to ADHD, asthma, scarring, and others. It is a common family stressor, mental-emotionally and financially. AD effects up to 20% of children*. First line conventional treatment for AD is topical corticosteroid use. They work by suppressing the body’s immune response to decrease inflammation. Long-term use of topical corticosteroids of any potency should be avoided in areas susceptible to side effects like the face and skin folds [Great, all the places that infants tend to need it the most!]. These creams are intended to be used for 5-7 days at a time, max. However, our doctor and dermatologist told use to cover Clarke in this stuff head to toe 2-3 times each day!

There’s a problem here!! Topical corticosteroids are known to cause thinning of the skin. This leads to integrity loss of the skin barrier. They also potentially suppress the hypothalamic-pituitary-adrenal (HPA) axis. Among other things the HPA axis influences the stress response, digestion, immune response, mood, emotions, sexuality, and metabolism. This risk is greatest for people just like Clarke: severe AD, under two years old, large affected surface area, much inflammation, high number of flares (check, check, check, check, check!).  All those factors and the potency of the cream contribute to the degree the HPA axis is suppressed.

Not to be all doom and gloom. I’ll change the phrase, “Look what corticosteroids can do. ” to… “Look what corticosteroids can do!”. The do decrease inflammation which can inhibit the itch-scratch cycle. This will likely decrease risk of scarring. Though we were told to slather our child up 2-3 times a day with these creams, recent research (Nov 2011) has found that twice daily use is no better than once daily use.

Ultimately, it’s up to the family to be informed, weight the risks and benefits, and to make individualized decisions. We are currently trying to find the best balance of days on and off of corticosteroid creams. Initially we tried one day on, one day off. At the time, that didn’t prove useful. Now we are trying 5-7 days on, as many days off as possible (14 would be great!). We are going to try to use the cream once daily for 5-7 days even if we feel she is only slightly inflamed. When we first decided to use these creams we thought we’d use a small amount one time and that would be that. But that’s not the case. Corticosteroid use is short term palliation of a long term problem. It doesn’t address the root- the ‘why’ Clarke is susceptible to this condition. Discovering that intricate interplay is the journey we’re on.

*Recently AD has become more prevalent in Western countries. Some speculate this is due to an increasing intake of polyunsaturated fatty acids (PUFAs), especially high omega-6 in relation to omega-3 leading to inflammation.
