<p><strong>Are you convinced ?</strong></p>
<p>This women had normal LV systolic and diastolic function with no evidence of constriction.</p>
<p>The explanation for the asymptomatic pericardial thickening is due to a healed  chronic pericarditis .This sort of localised thickening in the posterior aspect is all the more likely following a loculated pericardial effusion.Tuberculosis is a very likely etiology.But this women do not have any markers for tuberculosis.Since she is symptomatic no treatment was offered.She is being followed up.</p>
<p><strong>Discussion</strong> .</p>
<p>Q waves are not &#8221; sacred waves&#8221; to diagnose myocardial infarction.It simply indicates the  direction of current flow is away from the  recording lead of the ECG .Any thing  electrically inert , that come in the interface between the heart and the recording electrode   can record a q waveWhat are the pathological entities that can produce q waves other than infarct ?</p>
<ul>
<li>Fibrotic myocardium(DCM-Cardiomyopathy)</li>
<li>Myocardial Scars</li>
<li>Myocyte dis array(LVH, HCM)</li>
<li>Air,fluid in pericardium /pleural space</li>
<li>Pericardial thickening (As in this patient)</li>
<li>Electrical shortcircuits (WPW syndrome)</li>
<li>Rarely pure ischemia without necrosis can produce q waves (Electrically stuned myocardium)</li>
</ul>
<p><strong>Final message</strong></p>
<p><strong> </strong></p>
<p>Localised pericardial thickening is  a rare  (?unrecognised) cause for pathological q waves , that may mimic a MI.</p>

<strong><br />
Given the current state of corporate governance, what should boards of public companies be doing now to demonstrate that they are leading corporate reform, rather than being led by it?</strong></p>
<p style="text-align:justify;"><strong>David Leach:</strong> The most significant task before boards of public companies in today’s environment is restoring shareholder trust. This populist movement afoot in the marketplace can sometimes resemble a lynch mob – and as a result, many companies just want to keep a low profile and bury their heads in the sand. Much like a taxpayer that is reticent of saying or doing anything to displease the <a title="IRS" href="www.irs.gov/ " target="_blank">IRS</a> for fear that the action might trigger an audit, they are afraid to step forward.</p>
<p>Right now, there are many public companies that are behaving like private companies – and that sends precisely the wrong message. Today, good corporate governance means transparency. The boards that effectively articulate the underlying reasons for a particular course of action are the ones adapting best to the new paradigm. <p>On Friday, I participated on the biggest troop thus far: <a href="http://today.msnbc.msn.com/">NBC&#8217;s Today Show</a>. This was an official LFL troop, one that was announced several weeks ago, along with signups. I submitted my pictures of my Clone Trooper, and was selected for the group. It wasn&#8217;t until a week ago that we were told what we would be trooping, and weren&#8217;t allowed to tell anyone. So, I worked on getting my Clone up to snuff, made a new blaster and on Thursday night after work, set off for NYC.</p>

I arrived down there at 10:30, and met up with several troopers from my garrison and the Connecticut Garrison in the hotel that we were all staying at. We stayed up pretty late talking, and turned in for the remaining couple hours of sleep before we had to get up. At 3:30, our alarms went off, and we got up, dressed and carried our gear to NBC studios, where we checked in, met more troopers who were arriving at the same time. At 5, we did a walk through of what we were going to be doing for the morning, getting lined up and recieved our instructions. We then went back to the studio and suited up.</p>

<p>By 7 or so, we were finished suiting up, so we lined up, put our helmets on and went down to the street, where we crossed and waited for cameras. There was a little rehersal and changing of plans, and I ended up being on the first shift, rather than staying the whole morning. At 8 am, the show started, and cameras rolled. I was standing behind Al Roker, for his introduction, with a group of clones, and was on TV for just a couple seconds. After that introduction, my time was done, although 10-15 clones and stormtroopers were retained for upcoming shots. I, along with three other clones went back to the changing space, got out of armor and went back down to watch for a little while, then headed up to Boston for the next troop of the night.</p>

<p><strong>HEART TALK: Always keep a check on the symptoms of heart disease</strong><strong></strong></p>
<p><strong> </strong>The common stress test doesn’t tell the full story about your heart condition. Know what more needs to be done</p>
<p>This is a story worth listening to, more so if you have undergone a stress test on the treadmill to check your heart condition and have got a negative result. Senior cardiologist Dr. Balbir Singh of the Apollo Hospitals in New Delhi narrates, “This is about my professor at All India Institute of Medical Sciences. Some years ago, on feeling somewhat uncomfortable, the professor, a cardiologist, went for a stress test. Much to his relief, it turned out to be negative. But just two weeks after the test, he suffered a heart attack.”</p>
<p>Dr. Singh’s rationale is, “It ran in his family. So even if a stress test shows negative, you need to look at the symptoms too, such as your cholesterol level, your lifestyle, etc. and also whether there is a heart condition in the family.”</p>
<p>But five years after the American Heart Association established that stress tests may fail to identify irregularities of the heart in patients, and cardiologists worldwide are convinced the technique has serious limitations, few people here are aware of the limits of the test. Almost always, a negative stress test frees a patient of any fear of cardiac problems, and a positive result invariably triggers panic.</p>
<p>But Dr. Singh highlights, “A stress test can have false positive or false negative results.” He elaborates on when to worry: “A patient need not worry much about a negative stress test result only if he/she is asymptomatic — that is, if he/she shows no possible symptoms of a heart condition. But if they do, or have already had a heart attack, a negative stress test doesn’t mean they are safe. They still need to do an angiogram.”</p>
<p>This is particularly important in the case of women. “Many women, even after the ECG shows irregularities of the heart or otherwise, show negative during a stress test for non-specific causes. It is better to go for a stress echo.” During stress echo, a small prop is put on the patient while he/she runs on the treadmill to reap accurate results.</p>
<p>If an asymptomatic person shows the stress test as positive, he/she should ideally go for a stress echo or a stress thallin test before undergoing an angiogram straightaway to establish his/her heart condition. A stress thallin is a more expensive proposition but is far more accurate than a common stress test, as it uses radio-nuclear material.</p>
<p>For the layman, Dr. Singh explains how a stress test works. “A stress test puts the heart to stress as you start running on the treadmill. During the test, both blood pressure and heart rate go up, thus facilitating blood flow to the heart. But if there is an arterial blockage, then the blood flow will be less. But mind you, it picks up a blockage only if it is 75 per cent blocked.” However, an angiogram shows up even 55 per cent blockage of the artery.</p>
<p>If the stress test is not always accurate, why do doctors then ask patients to undergo one? He responds, “Like an ECG, it has its importance, particularly in asymptomatic persons. Also, since Indians are more prone to heart disease, we don’t recommend bypass surgery if the blockage is below 75 per cent.”</p>
<p>While a stress echo takes about an hour and costs Rs.4,000, a stress thallin test takes about five to six hours and is priced at Rs.8,000.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>So last night I had an interesting experience, admittedly a bit bushed I climbed into bed around half ten, tired but feeling ok&#8230;and seconds later proceeded to have what I instantly thought was a heart attack/stroke.</p>
<p>&#8230;my chest felt heavy, my heart started beating faster and suddenly my entire right side went numb, then my whole body cramped and contracted in on itself involuntarily (extreme continuous <em>Myoclonus</em> &#8211; quite possibly the single most interesting and terrifying feeling I have ever experienced.</p>
<p>If I could have laughed I would have, as the first thing I thought of upon viewing the amazingly taut position of my fingers was an episode of <em>House</em> (<em>Half-Wit</em>) where a piano player experiences <em>Myoclonic</em> contractions in his hands.</p>
<p>Unable to even reach for a phone or call for help properly, I spent the next two hours (and boy it felt like a long and lonely time, but not a full two hours &#8211; I&#8217;m still surprised at that) fighting with my breathing to pull my body back into shape until finally I managed to calm myself down, un-cramp my right hand and call an ambulance.</p>
<p>Being that I live on the third floor of my house it was certainly a hilarious situation that I had to come down three flights of stairs and open the door for my rescuers to assist me (keeping in mind that I was paralyzed in bed and had to tenderly shimmy my leg muscles into action) &#8211; and even more so that they were acting annoyingly bored, and in no rush to help.</p>
<p>So a trip to the <em>Royal Victoria</em> hospital occurred after I insisted that at twenty three years old, this was a serious matter worthy of at the very least, an <em>ECG</em> (<em>Echocardiogram</em>) &#8211; and after a few blood tests, and other assorted screenings including a chest <em>x-ray</em> I was astonished to be told that I had a viral infection which (in my weakened state from having not had too much sleep and basically being run down) had taken the opportunity to play havoc with my immune system&#8230;which in turn decided to attack me. Coupled with the hyperventilation, a panic attack and the sheer shock that ensued it was added that I probably made the matter worse unintentionally.</p>
<p>Awesome&#8230;</p>
<p>Apparently the infection had been lying dormant within my system for a week or so, perhaps more, and that now I was &#8220;ok&#8221; &#8211; &#8220;take these antibiotics for seven days, get some sleep and you&#8217;ll be fine.&#8221;</p>
<p>To say I was shocked (had a lot of shocks during the night looking back on it) at the result is quite the understatement, as a mere six hours earlier I thought that was it &#8211; adios.</p>
<p>Thinking that seven days worth of antibiotics will sort out what I thought nearly killed me is strange, flippant even.</p>
<p>Well, at least it wasn&#8217;t <em>Swine Flu</em>.</p>

<li>His underlying rhythm has clearly changed; it&#8217;s now much broader and faster. This means it&#8217;s either VT or SVT with block. For reasons that will be the subject of another post take it on good authority that this is VT</li>

<p style="text-align:justify;"><span style="color:#ff99cc;">Then after checking,I have to wait for awhile for the results&#8230;the doctor said..Im perfectly ok and super healthy..everything&#8217;s normal..I felt so weird that everything is ok but my chests were still hurt..I asked the doctor over and over again and finally he said..its probably because you have digestive problem..lol I was like what??hahhaha I actually was unable to burp and that made me hard breathing since it pressuring my heart..LOL&#8230;I was like worried to death and its only because I was unable to burp..Hana is feeling much better today..although my chest do still hurt a little bit..oh yaa I didnt go to school,been given day off today..hehe..In this seven days..I only went to school for one day..lol..</span></p>
