On Thursday I attended a one-day class at FSI called Post Options for Employment and Training.  It was a great overview of the various resources and services available to spouses/partners to help find employment inside or outside the embassy.  The instructors did a good job in making it clear that no one is going to hand you a job. You have to network, be qualified, jump through hurdles, look out for yourself, write a good resume, and again, network.  But it is important that you know where you can get information about local law and practice, organizations operating in the country, and the various training options available.

The class was also helpful to understand the various hiring mechanisms used by embassies, as each mechanism has varying implications in terms of current and retirement benefits, non-competitive eligibility for future federal jobs, and available hiring rates.  All of this information is online at the Family Liaison Office site.  They publish a guide for employment options for family members.

The training also included a resume writing session.  At first I thought this session would be a bit basic, but the instructor was great and had a lot of experience working with FS family members, being one herself.  Her guidance on dealing with employment gaps was especially helpful as I imagine I will have several of those as we move from place to place with months in between.  As the instructors said: As a FS family member, you are guaranteed an interesting life with a variety of jobs, but you are not guaranteed anything looking like a progressive career.

A few recent statistics:

• As of June 2009, 81% of Eligible Family Members (EFMs) were female and 19% were male.
• 25% of EFMs are working inside the Mission
• 14% of EFMs are working outside the Mission
• 61% are not currently working

A recent survey indicated that over half of EFMs want to work, so based on the numbers above there is a good chunk of people that are still looking for a job.  I’ll start gearing up my networking in the new year.  Hopefully by the time we go to Nicaragua I’ll have an idea of what I’ll be doing. But it’s possible that I won’t be able to nail anything down until we actually get to post. Of course, I’ll keep you updated…

Over at FD Wholesale, we’ve been doing some trials in our R&D department bonding Annex M tails together and we’ve been able to get throughput normally associated with leased lines. We’re roughly 1.5Km from our local exchange, and when bonding 2 lines, the total sync rate was 26.7Mbps down and 4Mbps up. When we bonded 4 lines, we obtained 56Mbps down and 8Mbps up.

The applications for this are wide ranging. Consider having a client who lives 5Km+ from their local exchange. 1 DSL would offer them little throughput to sustain a number of users. Aggregate 2 or 3 together and suddenly they can start to look at IP applications that may improve business processes such as SIP or Video conferencing. Another example may be where a client can’t gain wayleave agreement to obtain a fibre run. In this instance, they can have a bonded Annex M service offeirng up to 80Mbps down and 10Mbps up. Obviously these are headline speeds and are dependant on quality of copper and line length, but in all but the worst circumstances, a bonded Annex M service can start to become a compelling alternative to EFM or FTTC. Using the BE network, this is also available immediately, nationwide. No waiting for 2012 to have a coverage of c.300 exchanges.

Currently this is something that all our channel partners are utilising, as it gives them a cost effective alternative to a leased line. Based on the Cisco proprietory protocol, traditionally the stumbling point has been the high initial price point associated with the routers. However, we’ve been conducting some trials with a manufacturer called Virtual Access using their GW7000 boxes, and they’ve been very successful in terms of throughput and stability. However, even more compelling is the fact that they lower the initial price point of the solution to sub £500.

Personally I feel that bonding Annex M tails, at the core is a lot more resilinet solution than trying to aggregate them at the client end, using an external aggregator, as it means that there is little overhead, lower packet loss and less latency. In my opinion, the main thing to take away from this is that even though fibre will still have it’s uses, the applications for DSL are ever increasing. Whereby traditionally a leased line was the only method available to provide large amounts of throughput, the landscape is ever changing to incorporate DSL.

A mon sens, deux objectifs doivent guider vos réflexions au cour de cette phase stratégique :

1/ L’enquête doit répondre à des problématiques préalablement identifiées.

2/ L’enquête doit être à l’image du public ciblé.
 
Ca parait évident et pourtant il n’est pas rare de constater que les données d’enquêtes sont inexploitables ou incomplètes aux vues de l’objectif de l’étude.
Il est donc important de se poser des questions préliminaires comme :
- Quel est la cible ? Comment échantillonner mon panel de répondant pour qu’il soit représentatif ? le taux de retour et la qualité des réponses dépendent grandement de cette sélection. Bien qu’il soit possible de redresser un échantillon à postériori, il est recommandé de le faire avec modération pour que l’étude reste statistiquement viable.
- Quel sera le support du questionnaire ? La construction d’enquête papier n’est pas la même que celle d’une enquête web, téléphonique, ou d’une enquête terrain réalisée sur Pocket PC.
- Quelle sera la régularité de l’enquête ? à la différence des enquêtes ponctuelles, les baromètres périodiques exigent des questions identiques d’une vague à l’autre pour effectuer des comparaisons dans le temps et mettre en évidence des évolutions, des tendances.
- Quel doit être le type des questions (numérique, simple, multiple, ouverte, échelle, classement …) ? le type diffère en fonction du résultat attendu : une moyenne et des quartiles seront obtenus à partir d’une question numérique, un mapping de satisfaction à partir d’une question échelle allant de « Pas du tout satisfait » à « Très satisfait », une analyse textuelle à partir d’une question ouverte…
- La géolocalisation des données est-elle pertinente ? Dans le cas d’une étude d’implantation, de satisfaction par points de ventes ou d’analyse de bassins d’emplois, il est nécessaire que le questionnaire comporte une question relative au code postal. Cette question pourra être géocodée en code INSEE pour afficher les résultats sur des fonds de cartes monde, Europe, régions, villes…

Courage, passé cette étape l’accouchement est proche !

Lucha Libre - Le Catch Mexicain

La Empresa Mexicana de Lucha Libre (Conocida hoy día como CMLL), llegó a un acuerdo con EFM (Entente France Mexique), empresa francesa de lucha libre, para presentar la primera edición del festival de lucha libre en tierras francesas, al parecer la giras previas del CMLL rindieron frutos

Uno de los que han tenido mucha participación en la preparación de este evento es Sangre Azteca, que junto con Orlando Jiménez, otro de los organizadores, preparan el evento en donde se llevarán a cabo tres funciones completas de lucha libre, además que habrá talleres y clases, otro de los objetivos es lograr intercambios de luchadores y seguir con este tipo de eventos internacionales

These days, I’ve been focusing on writing a book I’ve been wishing to complete for some time now.  Actually, it replaces what I thought would be my first book titled “Hit or Stand.”  I’m going to try and get this book done primarily for you great friends out there asking “So, why did you go to Korea?” and my boys who I hope some day will understand what their father was thinking all these years.

For now, I’m focusing on ER: The Urgency behind Korea’s Real Education Emergency or something like that.  I have a few opinions on the future of South Korea and thought my 10 chapter book on it may help the future generations of Korea stay competitive despite Korea’s success thus far.

Back to the “day in the life of…” me…

These days, I focus a bit of time on Inside Out Busan at eFM 90.5.  I should actually be writing for my radio show, in my opinion, but decided to take a couple hours for a break today since I feel like I’m non-stop with the radio show, my classes at the University and my privates which are nowadays on Wednesday, Saturday and a couple hours on Sunday.  I’ve also been asked to do a couple more, but not sure if I want to spend more hours on Sunday and literally fail to have even a single weekend day off.  The sad irony is that I’m still making less than I was before I decided to go solo and that I’m working every day of the week.

I do have Tuesdays through Thursday’s during the days off, but have to go into the station to do the show in the evenings so they’re just 1/2 days off with my only other day off being a 75% day off on Sunday.  So, I guess I sort of have 2.25 days off split over 4 days.  I use one of the half days though to catch up on sleep from the lack of it on my Mondays that are spent early in Korea Maritime University.

Oh before talking about my weekly schedule, I am very grateful in seeing my folks who visited recently.  Also, on the near term horizon, I have Edwark Park, a Principal of a Chinese language school in or near San Diego, coming to visit.  Looking forward to seeing Ed.  What else?   I think I need to find a cheap car for the winter as well.  Any ideas anyone?  

Mondays: Korea Maritime University early in the morning (between 7 & 9a), a brief respite in the afternoon, but soon thereafter shooting over to the studio until 9′ish daily.

Tuesdays: I catch up with sleep in the mornings with the studio waiting for me in the early evenings and evenings.

Wednesdays: I finally have time for ER and other business development, but it feels like I haven’t had until now to really get to ER more.

Thursdays: repeat of Wednesday with the additional fact that I can do chores or other tasks.  I also find that I need to do a bit of cleaning & help with the recycling even though I feel like I’ve been not as good about that lately and do a bit of shopping as well.  Hello Costco!  I’ve been doing a bit of cooking too which can be a bit time consuming.

Fridays: a repeat of Monday

Saturdays: privates from early morning through 8p these days.

Sundays: 2 hour private from 3p to 5p.

There’s a little more detail here and there, but that’s the general “A day in the life of”…me!

Un neo, abbastanza tangibile però,  è che bisogna collegarsi alla rete OPM di telecom affinchè la soluzione sia implementata e quindi, se non ho capito male, essere in cascata con un altro dslam ethernet.

Se qualcuno ha ulteriori notizie, smentite o conferme, sono ben accette.

We have been thinking about joining the Foreign Service for a while now, so in general we have a good idea of what we are getting into. But the more I learn about the process, the more I realize how many details I didn’t even think about.  State’s Family Liaison Office has produced a Partner and Spouse Guide for those considering the Foreign Service. They’ve included a mini self-test to see how much you know, or more importantly, how realistic your expectations are.  Of course I didn’t see this until after my husband had accepted the offer, but thankfully nothing on here was too surprising. Take the quiz below and check out your answers on page 5 of the guide.

Fact or Fiction (answers here):

 1) The Embassy/Consulate will find you a job which matches your qualifications.

 2) Your housing will be paid for overseas.

 3) The U.S. Government gives you an allowance to choose your own house overseas.

Well, this is disconcerting.  I wonder how many unneccessary cesareans these machines caused.  How many more will they cause if hospitals don’t get these models replaced or fixed?

September 7, 2009 — Fetal monitors made by Philips Healthcare, Avalon Models FM20, FM30, FM40, and FM50 and ultrasound transducer, may produce inaccurate readings, the US Food and Drug Administration (FDA) warned on Friday.

The manufacturer states that the inaccuracies most often occur during the second stage of labor and include the following:

• Halving or doubling of the fetal heart rate (FHR)
• Switching between the fetal and maternal heart rates
• Differing audible FHR output compared with the printed trace
• Delaying of FHR display upon repositioning of transducers

“Inaccurate output readings, if not properly addressed, may lead to unnecessary interventions, failure to identify the need for interventions, and failure to identify fetal distress,” according to an alert from MedWatch, the FDA’s safety information and adverse event reporting program.

The device manufacturer recommends that users can reduce risks by confirming fetal life before using the monitor and also by using an obstetric stethoscope, Doppler, and ultrasound imaging, as per the instructions for use that accompany the device.

Clinicians should also “continue to confirm that the fetus is the signal source for the fetal heart rate particularly if abrupt changes in fetal heart rate are noted.” In addition, users should consult the instructions for use and consider monitoring the maternal heart rate using a fetal scalp electrode if a discrepancy is suspected, according to the manufacturer.

I am pretty sure that this wouldn’t be happening on a regular basis.

Or more likely they would put in an internal monitor and then mom would have that unneccessary intervention because the machine malfunctioned.

I am going to print this out and put in my doula bag so I can check at the births I attend to be sure these models aren’t being used, or if they are to warn the parents, nurses and OBs about the defect.

This is one of the risks when we let machines be our decision makers.

First on the agenda is that the Contrarian and I are celebrating ten years of wedded bliss today. That means dinner out which is about all we do. We could say we celebrate every day, but seriously we don’t lack from spending time together so it’s not that big a deal to either of us. We got a lovely card from CEC. It was lovely to be remembered.

Last night I had my first EFM class, and I came home with HOMEWORK!. Not that I didn’t expect it. I’m eagerly looking forward to this wonderful opportunity to explore the bible in even greater depth than I have so far.

I’d describe myself as a novice but competent knitter. I have made two sweaters, both of which I still wear. I have yarn and a beginning for another, as well as a couple of other projects, and I’m getting that knitting urge once again, so I expect to start soon.

Vicki, over at knittingdragonflies, has turned me on to a totally new concept to me, the top down method. Most sweaters, as I’ve seen are started from the bottom and knitted in sections that you then piece together. This method obviously starts from the top, and I think is done in one piece. I’m interested and intrigued. If you are slip over and get your instructions. Happy crafting!

As many of you already know, my point in these Short Takes is to make you the darling of the cocktail and dinner party, and the go to person at the water cooler. My goal is to give you that eclectic education that makes you look smart and so with it among your peers, and indeed makes you stand above to be admired and fawned over as the person who just knows everything.

So you probably need to know that the Kepler, as in Johannes Mission is a creation of NASA designed to find habitable worlds around distant stars. Word is that they are finding it might be possible to locate exomoons around some of those planets. So we have a double chance of finding habitable orbs in the sky. The technology hopes to allow the scientific ones the ability to detect the imperceptible wobble that occurs when a planet is pulled by both its sun and its moon.

All this is of course important because the need to transport to another world becomes increasingly attractive as the GOP continues to ramp up the craziness that has attacked them for the past twenty years or so. Sane humans may have no choice but to leave to preserve said sanity as it were. The only small problems left to address will be things like faster than light travel, and incidentals like how to create food replicators ala Star Trek. We are clear that we can’t live any kind of civilized life without them!

More wingbat shittery from the Rethugs and their spokesmorons. It seems that President Obama has done a piece that school districts can optionally play to their kids when they return to school. It’s all about working hard and being good students and learning some stuff. Pretty much what you would want you kids to know huh?

Not so, the right wing nut cases. No, this is “indoctrination” and right out of Maoist dogma. All the usual players are weighing in and that means you know its bunk. But no doubt they will convince their duped minions to raise the usual hue and cry.

Increasingly mainstream America looks with tired disbelief at the robotic lock-step ranting of the mostly evangelical ultra right.

But it does point up something. With only the promise and the verbal ranting on abortion and gay rights, you can convince the religious right to ignore its own self interest and vote for those who economically and politically destroy them. And better yet, the actual politicians can continue their own personal lascivious living. Getting caught means only having to say you are sorry and the devil made you do it.

This pretty much sums up my knowledge of how cars work. From toasters to nuclear power plants, if it needs electricity or runs on a motor, I prefer to think of it as magic. I work under the theory that there is only so much room in my noggin, and I don’t clutter up the space with stuff I don’t need to know. Plug it in, turn the key, work.

It is undoubtedly true that the way to potty train a dog is to keep it kenneled and take it out at regular intervals. The animal will not “foul” its sleeping quarters.

One might assume that oil companies, specifically Chevron,  operate on the same principle. If Ecuador is any evidence, they have fouled that country mightily with their obscene practices.

After doing their best to cast dirt on the litigants and the judge, they are prepared to accept that they are going to be held liable to the tune of 27 billion.  The people there are suffering from increases in cancer, miscarriages, birth defects and other problems.

This is of course the ugly American syndrome for which we are so famous. We screw other people in other countries for our personal benefit. Chevron, I suspect would not even have qualms about doing the same in the good ole US of A. Good reason to keep them out of the drilling business in Alaska and off shore.

Okay, don’t get me wrong, I’m no fan of Levi Johnston. He strikes me as a fairly vacuous human being, who hasn’t given much thought to much beyond his own nose yet in his short life.

But, given that he was the almost son-in-law of our dear Sarah (The Quitter) Palin, he remains in the news.

With competing desires to sell books and make some money, they undoubtedly will both view this feud as helpful to their respective endeavors. If they should join forces, they would only need another 3/4 of a brain to be an intact human being.

Still, if you have the desire to read about Mr. Johnston’s take on Mrs. Palin, no better place can be found than Vanity Fair, the best of the scrumptious interview mags. (I still have my copy of the one they did on Johnny Depp a month or so ago, and I’m not letting go of it anytime soon.)

No doubt half of what Levi has to say is pure bunk, but then that means half is scintillating gossipy goodies. You can pick and choose which you want to believe of course.

Which all goes to show ya that the human species is a hardy bunch. As the evidence builds that a huge portion of the US public is stone cold crazy as loonies, these two prove that life does indeed go on in the human zoo.

          ถูกตั้งข้อสังเกตว่าวีเจสาว จ๋า-ณัฐฐาวีรนุช ทองมี เป็นบุคคลต้องสงสัยในการปล่อยข่าวคู่กรณี นางเอกสาว หยาด-หยาดทิพย์ ราชปาล อัพยาและไปขลุกอยู่บ้าน “ไผ่ วันพ้อยท์” ให้กับรายการ “แฉแต่เช้า” ทางคลื่นวิทยุ 94.0 EFM ออกมาแฉจนเป็นเรื่องเป็นราวใหญ่โต โดยนางเอกสาวโกรธจัด! ขู่จะฟ้องดำเนินคดีหากทางรายการไม่แก้ข่าวให้ถูกต้องโดยเร็วที่สุด!! ล่าสุด “วีเจจ๋า” ออกตัวไม่รู้ไม่เห็นเรื่องอะไรทั้งสิ้น ก่อนเปรยไม่อยากพูดถึงและไม่ขอเกี่ยวด้วยทุกเรื่อง โต้!เปล่าให้ข่าวไม่อยากร่วมงานด้วย พร้อมปัดปฏิเสธงานคู่ “หยาด”!!!

          ผู้สื่อข่าวถามถึงกระแสเม้าท์หนาหูว่าเป็นคนปล่อยข่าวเรื่องคู่กรณี “หยาดทิพย์” อัพยาและค้างบ้าน “ไผ่ วันพ้อยท์” ให้กับรายการ “แฉแต่เช้า”  จนสาวหยาดขู่จะฟ้องดำเนินคดี วีเจสาวรีบโบ้ยพร้อมเปรยต่อทันทีว่า

          “อันนี้ไม่รู้เรื่องเลยค่ะ (รุมกันแก้แค้น?) ไม่ค่ะ จ๋าไม่รู้เรื่องเลยค่ะ ไม่รู้เรื่องแน่นอนค่ะ จ๋ายังไม่รู้เรื่องเลยใครฟ้องใคร ไม่มีค่ะไม่เกี่ยวเลยไม่รู้เรื่องด้วย จ๋าก็ไม่ได้ยินว่าเขาพูดนะคะ เอาเป็นว่าจ๋าไม่มีเรื่องดีกว่า อยู่สบายๆ จ๋าไม่มีปัญหาเหมือนกับว่ามีคนพยายามพูดอะไรอย่างนี้ แต่จ๋าไม่มีอะไรอยู่แล้วเงียบเท่าที่จะเงียบค่ะ เพราะจ๋ารู้สึกว่าจริงๆ แล้วมันคงไม่มีอะไรเลยก็ได้”

         “ส่วนเรื่องที่ไม่ขอร่วมงานกับหยาด ขอไม่ตอบดีกว่า จ๋าไม่ได้พูด จ๋าไม่ได้พูดอะไรเลยมานานมากแล้ว (ทุกวันนี้เจอกันพูดคุยกันได้มั้ย?) ไม่ได้เจอเลยค่ะ แต่จ๋าไม่มีปัญหาอะไรอยู่แล้ว ก็ไม่ขอตอบเรื่องนี้ (สามารถร่วมงานกันได้มั้ย?) ก็สบายๆ เอายังไงก็ได้ (มีข่าวว่า “จ๋า” ปฏิเสธงานคู่ “หยาด”?) จ๋าว่าจริงๆ แล้วจ๋าไม่อยากมีเรื่องมากกว่าค่ะ อยากให้มันหยุดตรงนี้แล้วจบตรงนี้ แล้วใช้ชีวิตปกติดีกว่า (จริงอย่างที่มีข่าวใช่มั้ย?) ไม่ใช่ค่ะ ยังไม่มีอะไรเลยค่ะ”

         พอถามต่อถึงกระแสข่าวอดีตหวานใจหนุ่ม “หลุยส์” เฮิร์ทหนัก ถึงขั้นออกปากขาด “จ๋า” ไม่ได้ วีเจสาวเปรยว่า

          “ไม่หรอก…เขาอยู่ได้ (เขาบ่นกับเพื่อนๆ? ) จริงเหรอ ไม่หรอกจ๋าคิดว่าไม่ เขาคงอยู่ได้ เราก็ต่างคนต่างให้กำลังใจอาจจะอยู่ในระยะห่างที่มันมีระยะห่าง (เขามาขอคืนดีมั้ย?) ไม่มีการขอคืนดีค่ะ เขาโทรมาคุยก็เคยมีค่ะ พูดคุยกันธรรมดานานๆ ทีค่ะ (ตื้อมั้ย?) ไม่ค่ะ ไม่ตื้อ เขาไม่เคยตื๊อเลยค่ะ เขาก็เป็นคนที่อยากจะมีอนาคตของเขาอยากจะใช้ชีวิตอยากจะทำงานอะไรอย่างนี้ จ๋าคิดว่ามันก็เป็นจุดที่ต่างคนต่างใช้ชีวิตแล้ว แต่ก็ยังเป็นคนที่รู้จักกันให้กำลังใจกัน ทุกวันนี้อยู่อย่างนี้ก็ดีค่ะ ก็คุยกับเขาได้เพียงแต่ว่าด้วยหลายๆ อย่างอาจจะไม่ได้มาสนิทกัน”

นางเอกสาว หยาด-หยาดทิพย์ ราชปาล ยั๊วะสุดขีด!! หลังถูกรายการ “แฉแต่เช้า” ของคลื่นวิทยุ 94 EFM  มั่วข่าวอัพยาและค้างอ้างแรมบ้านไฮโซหนุ่ม “ไผ่ วันพ้อยท์” ไม่ยอมกลับบ้านจน “คุณแม่” ต้องออกตามล่าหาตัวให้ควัก! นางเอกสาวโว!! ถ้าทางคลื่นวิทยุไม่ออกมา “ขอโทษ” ขู่จะดำเนินคดีการฟ้องร้องให้ถึงที่สุด!  เผยมีผู้ใหญ่คอนหนุนหลัง แต่ปัดไม่ขอระบุว่าเป็นใคร ฟุ้ง! อยากรู้เรื่องอะไรถาม เอส-วรฤทธิ์ ได้ ยืนยันไม่ขอเคลียร์ใดๆ ทั้งสิ้น แม้คู่กรณีจะให้หวานใจหนุ่ม “เอส” มาเกลี่ยกล่อมก็ตาม!!!

        ผู้สื่อข่าวถามถึงกระแสข่าวว่า “คุณแม่” จะฟ้องรายการ “แฉแต่เช้า” หลังมั่วข่าวแอบไปอัพยาที่บ้าน “ไผ่วันพ้อยท์” นางเอกสาวเปิดใจว่า

        “เอ่อ ไม่อยากให้เรียกว่านักข่าวค่ะ เพราะว่าจริงๆ คือ…เหมือนที่มีเรื่องใช่ไหมค่ะ คือสถานีวิทยุไม่ใช่นักข่าว หยาดไม่รู้ว่า…เขาอาจจะเคยเป็นนักข่าวมาก่อน แต่ว่าหยาดคิดว่าเขาเป็นสื่อค่ะ คือยังไม่ได้บอกว่าฟ้องนะค่ะ เพียงแต่ว่าเหมือนข่าวที่ออกมาหยาดเดือดร้อนมาก ตอนแรกหยาดก็ไม่ได้ซีเรียส แต่ว่าพอตอนหลังมันกระทบหมดทุกคน แล้วหยาดเดือดร้อน คือผู้ใหญ่บอกว่าชื่อเสียงหยาดเสียหาย แล้วคุณพ่อคุณแม่ไม่พอใจมาก อย่างคุณพ่อไม่ได้อยู่ในวงการ เขาก็ไม่เข้าใจว่าทำไมถึงมีสิทธิ์พูดอะไรที่มัน…จะพูดอะไรก็ได้เหรอ”

        พอถามต่อว่าคิดจะเคลียร์กับเจ้าตัวที่ปล่อยข่าวนี้หรือเปล่า สาวหยาดทำหน้าเม้ง ก่อนเปรยต่อว่า

       “ถ้าถามหยาด หยาดคงไม่เคลียร์กับเขานะค่ะ แต่ว่าหยาดแค่อยากรู้ว่าเขาอ่านมาจากไหน เท่าที่หยาดสอบถามจากบุคคลรอบตัว เขาบอกเขาอ่านจากแหล่งที่เชื่อถือไม่ได้ ไม่ได้อ่านจากหนังสือพิมพ์ที่พวกพี่ๆ เขียน หยาดเลยอยากถามว่า ทำไมบางทีแหล่งข้อมูลที่มันไม่ได้เป็นตัวหนังสือ มันไม่ได้ออกมาเป็น…พิสูจน์ไม่ได้ว่าจริงหรือไม่จริง ทำไมถึงเอาออกมาพูดใช่หรือเปล่าค่ะ แล้วหยาดเดือดร้อนมากเลยตอนนี้ มีปัญหา แบบหยาดก็รู้สึกแย่ว่าไปไหนคนก็มองว่าเราเป็นเด็กติดยาเหรอ…ใช่มั้ยคะ อย่างข่าวเรื่องผู้ชายเนี่ยค่ะ หยาดบอกตรงๆ เลยว่า หยาดไปสนิทกับอีกคนแล้วอ่ะ หยาดก็มีข่าวกับผู้ชายคนอื่นไปแล้ว ทำไมต้องไปโยงคนเก่าด้วยคะ ถ้ามีอะไรก็ไปถามพี่เอสดูก็ได้ค่ะ อย่างหยาดนี่รู้จักกันมาหลายปีดูเหมือนคนใช้ยา หรืออะไรหรือเปล่า”

        “หยาดไม่เคลียร์ค่ะ ถามว่าไม่พอใจมั้ย…ก็ไม่พอใจค่ะ!! คือรายการนี้พูดถึงหยาดมา 3-4 ปีแล้วค่ะ แต่ว่าหนนี้แรงที่สุดแล้ว (คือเขาเอ่ยชื่อหยาดเลย?) ไม่เอ่ยค่ะ แต่เขาใบ้ชนิดที่พวกพี่ฟังดูก็รู้ว่าเป็นหยาดป่าวค่ะ เพราะเขาพูด 8.30 ถึง 11 โมง นักข่าวสัมภาษณ์หยาดไม่รู้กี่สิบคนหน่ะ เพราะฉะนั้นมันก็แบบเป็นหยาดอยู่แล้วที่เขาพูด หยาดไม่อยากมีเรื่องแล้วค่ะ เรื่องเยอะมาก หยาดก็ขอให้เขารับผิดชอบค่ะ รับผิดชอบแล้วก็…คืออะไรที่คุณทำโดยที่คุณคิดว่าเป็นใครเหรอ จะพูดอะไรก็ได้เหรอ รับผิดชอบให้หยาดด้วย แล้วไม่ต้องถามว่าต้องรับผิดชอบยังไง เพราะเป็นคนทำต้องรู้ว่ารับผิดชอบยังไง”

        “แต่ถ้าไม่รับผิดชอบ จากการปรึกษาผู้ใหญ่หลายๆ คน เขาบอกให้ฟ้องค่ะ (แนวโน้ม?) ยังไม่ได้เตรียมฟ้องค่ะ ต้องดูว่าเขาจะรับผิดชอบมั้ยค่ะ เพราะหยาดก็ยืนยัน ณ ตรงนี้คะว่า หยาดไปไหนกับคุณแม่ทุกวัน วันไหนที่หยาดไม่กลับบ้าน คือหยาดไปค้างกับเพื่อนหยาด ซึ่งคุณแม่ไปรับไปส่งตลอด ทุกวันนี้หยาดไปเรียนมหาวิทยาลัย กินข้าวกลางวัน หยาดยังลงมานั่งกินข้าวกับคุณแม่ที่โรงอาหารเลย เพราะเขาต้องรอเราที่จะขับรถพาเรามาทำงานต่อ แล้วรถคันที่หยาดใช้ หยาดไม่ได้ขับมานานแล้ว เพราะมันชน แล้วมันก็ซ่อมอยู่ด้วย แล้วหลายๆ อย่างค่ะ ซึ่งแบบมันเป็นไปไม่ได้ค่ะ แล้วชื่อนี้ค่ะ (ไผ่ วันพ้อยท์) ขอร้องเลย คือเขาอาจจะไปฟังมาจากคนที่เขารู้จักคนๆ นี้ หรืออะไร หยาดไม่ทราบ ณ ตอนนี้ คือถ้ามีการรังแกกันอีก โดยความที่ไม่ถูกต้องแบบนี้ ถ้าหยาดโดนรังแกจากทางนั้นอีกคงฟ้องค่ะ”

        พอถามต่อว่าได้พูดคุยกับทางสถานี “94 EFM” บ้างหรือยัง นางเอกสาวโบ้ย พร้อมเปรยต่อว่า

        “อันนี้ไม่บอกละกันว่ามีการคุยอะไรกันไหม แต่อย่างที่หยาดบอก ณ ตอนนี้ เขาไม่ได้รับผิดชอบอะไรค่ะ ก็อยากให้เขาแสดงแบบความแมนๆ หน่อย หรือถ้าคิดว่าหยาดติดจริงๆ ก็ออกมาพูดเลย หยาดจะได้ฟ้องเลยว่าหมิ่นประมาท (ปรึกษา “เอส” มั้ย?) ไม่ได้ปรึกษาค่ะ คิดว่าเขาคงไม่ทราบเรื่องอะไร คือหยาดไม่อยากฟ้องหรอกค่ะพี่ จริงๆ แล้วหยาดแค่ต้องการให้เขารับผิดชอบค่ะ ถ้าเขารับผิดชอบหยาดก็ไม่ฟ้องแน่นอน เพราะทุกวันนี้หยาดทำงานก็เหนื่อยแล้ว หยาดจะไปขึ้นศาลขึ้นอะไรให้เหนื่อยทำไม แล้วหยาดไม่ได้เป็นคนก่อเรื่อง แบบอยู่ดีๆ ตื่นมาก็มีคนมานั่งแฉอีกแล้วอ่ะ (เพราะ “เอส” ทำอยู่ที่นั่นด้วยเลยไม่ฟ้องหรือเปล่า?) ไม่เกี่ยวค่ะ ถ้าใครทำผิดหยาดก็ไม่สนว่าเขาจะสนิทกับใคร”

         ผู้สื่อข่าวถามต่อถึงผลกระทบจากข่าวดังกล่าวส่งผลอะไรกับเราบ้าง นางเอกสาวตอบด้วยน้ำเสียงสุดเซ็งว่า

         “โอ้โห!! ข่าวลงเยอะมากค่ะ เพื่อนหยาดก็งงบอกว่าแม่หยาดอยู่กับหยาดตลอดเวลาอยู่แล้ว ถ้าถามว่าหยาดเสียหายมั้ย? หยาดว่าหยาดเสียค่ะ กับการที่บอกว่าหยาดไปนอนค้างบ้านผู้ชาย ไม่ยอมกลับบ้าน หยาดเสีย ส่วนกับงาน ณ ตอนนี้ยังไม่กระทบ แต่คาดว่าทุกคนก็ต้องคิดนะค่ะ เพราะว่าหยาดว่าเรื่อง “ยาเสพติด” เป็นเรื่องที่ร้ายแรงที่สุดแล้วนะค่ะ (กระทบจิตใจ?) กระทบค่ะ (ทางสถานีติดต่อมามั้ย?) ไม่มีค่ะ…เงียบ แล้วหยาดไม่ชอบว่า…โอเคอีกสิ่งหนึ่งที่อยากจะบอกตอนนี้เลยนะคะว่า คุณอาจจะพูดอะไรก็ได้ แต่หยาดขอร้องนะคะ วันหลังเวลาที่พูดถึงหยาดอย่าได้หัวเราะคึกคักตลอดเบรก ไม่ใช่เรื่องสนุก บอกได้เลยว่า ถ้าเอ่ยชื่อแม่หยาดมาเนี่ย ไม่มีสิทธิ์หัวเรานะคะ ไม่ใช่แบบ…เป็นเรื่องสนุกเหรอ หัวเราะ หัวเราะไม่มีมารยาทด้วย บอกได้เลยว่าที่หยาดโกรธตอนนี้ คือหยาดรู้สึกว่าเขาเห็นเรื่องหยาดเป็นเรื่องที่น่าหัวเราะเยาะเหรอคะ หัวเราะตลอดช่วงที่พูดเนี่ย คุณจะเป็นคนหัวเราะเยอะหรือเปล่าเราไม่รู้ แต่เรารู้ว่าคุณไม่มีมารยาทกับเรา”

When a woman goes to the hospital to have her baby, she is first attached to an electronic fetal monitor machine. It is made of two straps that go around the abdomen and hold two electrodes which are connected by wires to a machine by her bedside. For at least 20 minutes, the woman’s contractions are timed and measured, and her baby’s heart rate is recorded, both printed onto a sheet of paper by the computer attached to the wires.
Some women find them reassuring, others find them a nuisance, but the important question is whether or not they are useful. The following articles will address those points. Most hospitals will say they require you to have a “20 minute strip” when you walk in the door, and again every 1 for 20 minutes. If a complication develops, or you are high risk to begin with, you are told you must have “continuous” fetal monitoring. In the end, you as the patient have the right to choose whether to use the monitor or not. The alternative, known as intermittent auscultation with a hand held Doppler, is also discussed in this newsletter.
As always, do your own research before you make a choice so that you are well-informed.

Electronic Fetal Monitoring: Everything You Need to Know

Electronic Fetal Monitoring is a form of listening to your unborn baby’s heartbeat and to measure the strength of your contractions. Two straps are wrapped around your belly that have doppler-like instruments attached. These instruments send measurements to a machine, which then records the information received on strips of paper, which are kept with your chart after the birth. The use of EFM became widely popular among hospitals in the 1970s, without proper testing and research on its use. EFM quickly replaced intermittent monitoring by nurses with hand-held dopplers or fetoscopes.
There are two very big reasons why EFM became so popular, although I’m sure there are many more. One is that a “strip” from the machine is hard proof of the progress of your labor; with it, physicians can defend themselves in court should a patient decide that the caregiver made a mistake while caring for her. Judges will more likely rely on this information rather than a handwritten chart provided by the nursing staff. Two, a hospital that routinely uses EFM requires less staff, since one nurse can monitor many patients at the nurse’s station. Less staff requires less cost to the organization, which leads to a more stable income for the head employees of the hospital.
Generally, when a woman questions the purpose of the monitor, she will be told that it is to protect her and her baby, and that they are less likely to suffer injury or death with “proper monitoring”. The hospital managers do not tell them about the physician’s fear of malpractice suits or the concern over budget. Women and their families are also given a lack of information concerning research and statistics like the ones provided
below. Here are what a few medical professionals have to say about EFM:

“In most hospitals, doctors use fetal monitors routinely, although one study of 70,000 pregnancies found no difference in outcome between monitored and unmonitored patients, and other studies have shown that monitoring results in an increase in infant mortality among patients monitored.” (How to Raise a Healthy Child … In Spite of Your Doctor, Robert Mendelsohn, MD. 1984)

“Williams Obstetrics (the most widely used textbook in the U.S.) says, “an equally satisfactory outcome for the fetus can be achieved without continuous electronic fetal monitoring of the fetal heart rate, continuous intrauterine pressure recording and fetal scale measurement, if the mother and fetus are closely attended by appropriately trained labor room personnel.” (A Wise Birth, Penny Armstrong, CNM and Sheryl Feldman, 1990)

Using an EFM system also leaves room for disagreement over the treatment of a particular case. Sheila Kitzinger describes a study where four obstetricians were asked to examine the same fetal traces and to determine if it was necessary to deliver immediately. Surprisingly enough, they only agreed in one out of five cases1.

“EFM has increased the cesarean rate with no increase in fetal wellbeing.” (ACOG, 1996)

The CBE Reporter indicated that broad studies had been done, “showing no difference in fetal outcomes using either IA [Intermittent Auscultation] or EFM”. (CBE Reporter, 2000)

“This ubiquitous machine has not been associated with any noticeable lowering of infant mortality. Research done since then shows that intermittent listening with a fetascope [or doppler] is every bit as good at detecting a baby who is in trouble from lack of oxygen. From the mother’s standpoint, intermittent listening is far better – less painful and less likely to lead to ineffective labor and cesarean section.” (Ina May’s Guide to Childbirth, Ina May Gaskin, 2003)

Another issue related to EFM that is often downplayed or ignored altogether, is its effect on the type and amount of support that women receive while in labor. I have witnessed myself how easy it is for the doctor or midwife, nurse and even the husband and labor coach to be drawn the EFM screen. Instead of asking the mother how she is feeling or if the contraction has ended, they stare at the screen and don’t bother talking to the mother. Not only does EFM leave the mother feeling alone and unnecessary in her endeavor, but it also prevents her from adopting optimal postions for moving the baby into and through her pelvis.
Therefore, in most instances, it would be wise for a woman to request a fetascope or handheld doppler during her labor. She will be more comfortable out of bed, and she will inevitably receive more humane care, rather than from a machine.
Every woman has the right to use EFM, either continuously or intermittently. On the same hand, every woman has the right to information concerning the benefits and risks to each. If you have concerns about its use, use the information provided here to discuss with your caregiver its necessity for your particular pregnancy, labor and birth.
And of course, I highly recommend you to do your own research on procedures that are likely to be used on you or your baby. This will reasure you that you are getting the correct information, and that which is necessary to provide true informed consent to those procedures.

Is Computerized Labor Monitoring the Future of Labor Assessment?

All women who have given birth with a birth attendant present are familiar with the standard method for determining cervical dilation and position of the baby’s head during labor: a manual vaginal exam every 1-4 hours until the child is born. However, there is a new piece of equipment on the market which may change hospital labor monitoring as we know it.
In 2001, an Israeli obstetrician decided that with all the technology available for childbirth, there had to be a way to create a machine that would accurately determine the same factors as a vaginal exam1. In 2007, he and his team finally received FDA approval for the “Barnev Computerized Labor Monitor” (CLM for short) in the active stage of labor, and began marketing it to hospitals across the USA2.
The CLM equipment consists of a doppler placed on the abdomen, two censors attached to the cervix, and one sensor attached to the baby’s scalp. The information is then displayed on easy to read screens next the patient’s bed and in the nurse’s station.
According to the Barnev company, over 300 women participated in a trial of the CLM, resulting in the following alleged benefits: a faster, simpler, and less painful way to determine cervical dilation and fetal position, freedom to use Pitocin or an Epidural, freedom to have a ‘natural’ childbirth, quicker treatment of fetal distress, CPD, failure to progress, and other labor complications3. Other reported benefits include cost effective management of labor, a legitimate defense for care providers experiencing lawsuits4, less unnecessary cesarean sections, forceps and vacuum extractions5.
What the press releases have not stated is that there are moderate risks involved with the CLM, as classified by the FDA. These risks include: patient injury, electrical hazards, ultrasound tissue damage, electromagnetic interference and electrostatic discharge hazards, mismanagement of patient, adverse tissue reaction, and infection6.
Other considerations that have not been discussed include the risk associated with rupturing the membranes, which is necessary to insert the electrode into the baby’s scalp, the potential for increased discomfort, and intervention if the CLM restricts the patient to her hospital bed. It is necessary for further studies in these areas to be conducted before the CLM can be freely used on all women.
In fact, some of the midwives originally involved in the CLM clinical study were removed from the program because of their strong hesitancy in such use of technology. One of the midwives told Midwifery Today that she believes “this product takes advantage of and potentially harms women and their babies in labor, all for the purpose of economically profiting a biotech company”7.
Although computerized labor monitors offer many benefits, use of manual vaginal examination still gives all necessary information to the care provider. Together with the lack of studies necessary for appropriate use of CLM, it may be wise for women to temporarily question its use when the equipment comes to their hospital.

What is Birth a Miracle Services?
Birth a Miracle Services is the name of the birth doula and childbirth education service that I started in 2002.
A birth doula is a person who offers informational, physical, emotional, spousal, and advocacy support to women through pregnancy, birth, and the early postpartum period. I also offer traditional childbirth preparation classes, birth art classes, and childbirth counseling.
All of this is available to women within an hour of my home in West Paris, Maine for a sliding scale fee. Single, teen, and low-income moms can receive my birth doula support for free.

Inspired Birth

Yesterday was the first day of class for Education for Ministry (EFM).  It’s  a four-year program designed to help Christian lay people prepare to be active participants in the whole ministry of the church.  The curriculum comes from the Sewanee School of Theology of The University of the South, but is taught in small, local groups.

Year one, which covers Old Testament, requires the most study time.  Yesterday, when I got home with my books, I told DH “This could seriously cut in to my knitting time!”  Knitting will continue for my sanity, but finished objects will be displayed less frequently.  I considered shutting down the blog — for about 3 seconds.  It was clear I would miss y’all too much.

Time to share some pictures!  Last weekend, around noon, I looked out the keeping room windows and saw the tall ships in port. The Canadian geese were in procession down our lake.  A tradesman that comes to our house told us he believes they live on a lake a few miles from ours.  I counted, and again saw 13 geese, the same number I saw in the Spring, so I believe he is correct. Evidently, it was just a “road trip” for the geese; I haven’t seen them since their Saturday visit.

Here’s to a lovely weekend for  you, filled with lots of knitting and no frogs.

This is probably best read if you have something calming to do at the same time, say, while  Nursing Johnny Depp.  The oxytocin released from breastfeeding may calm you more than you’ll be fired up in our comparison of ‘medical birth for all’ issues and out of hospital birth debates today to the Vietnam era.

Can we ever be on the same team?

“Domino theory” is the phrase coined during Eisenhower’s presidency in the 1950s to justify the hastening entry of the U.S. into foreign nations in order to stop the spread of communism.  Swap out the players of Eisenhower’s era with the ‘natural childbirth’ era you’ll have an idea of how ACOG fights the legal battle to erode the protection of normal birth ~ if not home birth ~ as a right for all women living in the USA.  If just one state votes to protect home birth then neighboring states will and so on and so on.  Home birth will spread like wildfire and healthy birth outcomes for healthy women will be the norm.  Incredible.

Basically, with a normal birth experienced care provider you can expect that:

• You will receive quality evidence based information not cheesy rationales.
• You will not be exposed to pitocin or dangerous medical inductions.
• You will not be exposed to internal fetal monitoring requiring the artificial rupturing of the bag of waters and penetrating your baby’s fetal scalp.
• You will not be falsely lead to believe all women need pain medications because labor is difficult.
• You will have a lower incidence of infections because there will be infrequent vaginal exams if any at all.
• You will not have limited mobility because you are not connected to monitors.
• You will not have limited mobility to deliver your baby because your practitioner isn’t planning to control your baby’s emerging from your with an episiotomy, through “super crowning” or otherwise ensuring you are on a bed in certain positions (usually some form of the “C” or semi-reclined position).

Women and babies laboring normally don’t typically fall like a line of dominos towards a cesarean, episiotomy, forceps or vacuum delivery.  It’s the interventions that push them over.  Remember the pit to distress order?  Start your birth un-naturally or make it un-natural at some point with pitocin and/or an epidural, you’ll arrive at a greater risk ratio for mechanical or surgical delivery.   The domino theory espouses there is no time to wait, each intervention must be applied now because of the one applied previously, until eventually the penultimate goal, birth, must occur now.

Natural childbirth is currently your best insurance against un-necessary interventions and insurance for a normal and healthy birth.  If, laboring at home or in an independent birth center you are transferred it is not likely to be an emergency scenario but a scenario where the need for medical observation is warranted.

Certified Nurse Midwives are on the rise as a result of increasing numbers of women seeking midwifery care.  Hospitals and OB practices that have midwives in their group “look better” to consumers.  In order to employ midwives without risk to their own profit however they must show midwifery care as the practice of medicine, which midwifery is not.  To these practitioners only such a person with medical training, in these instances nursing, is recognized as a ‘midwife’ then.  Is it a coincidence then that midwives find themselves engaged in an internal battle themselves?

The domino theory today is alive and well, hobbling maternal and newborn outcomes. Dominos don’t always fall, but ‘medical birth for all’ advocates will always try new set-ups. Stand up for birth.  Choose the integrity of midwifery care.  Deliver with both feet on the ground!
