<content:encoded><![CDATA[<p><em>BOYERTOWN, Pa. (AP)</em> — Police say a sport utility vehicle struck first one tree and then another on an eastern Pennsylvania hillside, killing two people and leaving the third in critical condition.</p>
<p>State police in Berks County told the Reading Eagle (<a href="http://bit.ly/1646fY1" rel="nofollow">http://bit.ly/1646fY1</a> ) that the crash was reported at about 3:30 p.m. Saturday in Earl Township.</p>
<p>Police said the SUV went out of control while turning as it went down a hill, striking the trees and then landing on its side.</p>
<p>Fifty-one-year-old Sharon Richards of Exeter Township and 71-year-old Signe Wergeland of Earl Township were pronounced dead at the scene.</p>
<p>A 73-year-old man was flown to Lehigh Valley Hospital near Allentown, where he was reported in critical condition.</p>
<p>The cause of the crash remains under investigation.</p>

<content:encoded><![CDATA[<p><em>PHILADELPHIA (CBS)</em> &#8211; One teen is dead after being struck by a car while walking with friends along a highway in Montgomery County.</p>
<p>The deadly accident happened on Friday night at 11:34 p.m. Hatfield Township Police responded to an accident scene where a pedestrian had been struck at Route 309 and Sterling drive. Officer’s said the victim, Dennis Kee, age 15, of Hatfield PA, and was found near shoulder of the roadway. </p>
<p>Kee was taken to Lehigh Valley Medical Center where he was pronounced dead at 12:35 a.m. Saturday. The cause of death was multiple blunt force injuries and was ruled accidental.</p>
<p>Police officials said during the preliminary investigation it was discovered Dennis Kee was walking with four young friends on Route 309 prior to the crash.</p>
<p>Investigators located the vehicle and driver, which struck Dennis Kee, in a nearby parking lot. The striking car is a 2007 Toyota Corolla sedan which did sustain passenger side damage. The driver was identified and interviewed regarding the accident. Police have not released the driver’s name.</p>

<content:encoded><![CDATA[<p>OAKLAND (CBS SF) &#8212; A female passenger was killed and four others injured when an SUV lost control on southbound state Highway 13 in Oakland Friday afternoon, according to the California Highway Patrol.</p>
<p>The Toyota Sequoia was traveling south on Highway 13 around 3:46 p.m. just north of Redwood Road when it drifted to the center divider for an unknown reason, according CHP Officer Sam Morgan.</p>
<p>The driver over-corrected and lost control of the car, causing it to roll several times.</p>

<p>One person, a female passenger, was killed, and four others injured. Three people were transported to Highland Hospital, two in critical condition and one with minor injuries, and one was transported to Eden Medical Center with moderate injuries.</p>
<p>Investigators remain at the scene and the left lane is blocked, Morgan said. Traffic is heavy in the area and drivers are advised to find alternate routes.</p>

<p>It happened shortly before 3 p.m. Thursday on the westbound side of the freeway near Wiard Road, after Rawsonville Road, just over the border from Wayne into Washtenaw County.</p>
<p>All westbound lanes were closed following the incident, but have since reopened.</p>
<p>There was no immediate information available from police as to what might have caused the crash.</p>
<p>There were no additional injuries reported. The woman&#8217;s name has not been released.</p>

<content:encoded><![CDATA[<p><strong>SMYRNA, Tenn. (AP)</strong> &#8211; Nissan officials say an employee of a supplier died in a fatal accident at its vehicle assembly plant in Smyrna.<br />
The employee of Lake Orion, Mich.-based Complete Automation was killed Thursday in the factory&#8217;s paint plant.</p>
<p>Bob Light, a spokesman for Complete Automation, said the employee was killed when a large electrical panel fell while it was being moved.</p>
<p>Light said the company makes paint systems for Nissan and the employee, who was not identified, lived in Michigan. He said the company&#8217;s thoughts and prayers were with the employee&#8217;s wife and children.</p>
<p>Nissan said in a statement that they are cooperating with authorities as they investigate the accident.</p>

<content:encoded><![CDATA[<p><strong>HILLSBORO, Ill. (AP) &#8211;</strong> A grand jury in Illinois has indicted a Georgia truck driver in the death of an Illinois state trooper. </p>
<p>Illinois State Trooper Kyle Deatherage died Nov. 26 when a tractor-trailer struck him on Interstate 55 near Litchfield. Illinois State Police said Tuesday that a Montgomery County grand jury indicted 52-year-old Johnny Felton of Hinesville, Ga. He faces charges of felony reckless homicide and operating a commercial motor vehicle without the proper license. </p>
<p>Illinois State Police say Felton was taken into custody Tuesday in Georgia. They say he waived extradition to Illinois where he will be arraigned. Felton previously has said he had no comment on the case. </p>
<p>In December, federal regulators found that Felton wasn&#8217;t medically fit to drive commercially. </p>

<content:encoded><![CDATA[<p><strong>NEWARK, N.J. (CBSNewYork)</strong> &#8212; A bus driver has been charged with striking and killing a pedestrian in a hit-and-run at Newark Liberty International Airport last fall.</p>
<p>Fidel Ortez, 47, of West New York, N.J., was charged Monday with second-degree leaving the scene of an accident resulting in death, and third-degree endangering an injured victim, Essex County, N.J., prosecutors announced.</p>
<p>Ortez allegedly struck Jeffrey Ziesler, 61, of Howell, N.J., around 4 a.m. Nov. 12. He also allegedly failed to stop and help Ziesler afterward.</p>
<p>The indictment charging Ortez was returned Friday, and he was arrested at his place of work Friday evening, prosecutors said.</p>
<p>Ortez was released on $50,000 bail.</p>
<p>Prosecutors did not specify the bus company for which Ortez worked.</p>

<content:encoded><![CDATA[<p><strong>MALIBU (CBSLA.com) —</strong> The Coroner Sunday announced the name of the woman fatally struck by a limousine on the Pacific Coast Highway in Malibu.</p>
<p>Sarah Salam, a 21-year-old resident of Germany, was struck in the 20300 block of PCH Saturday about 8:45 p.m. near the Moonshadows Restaurant, according to Lt. Todd Weber of the Sheriff&#8217;s Malibu/Lost Hills Station.</p>
<p>&#8220;The driver stopped, was cooperative and there was no DUI,&#8221; Sgt. Shawn Rude of the Malibu/Lost Hills Station said. &#8220;It may be an unfortunate accident.&#8221;</p>
<p>The limo driver was on his way to pick up a client, Ruda said.</p>
<p>During the investigation into the accident, PCH was closed from 8:45 p.m. Saturday until 2:15 Sunday morning.</p>
<p>The driver was not cited.</p>

<content:encoded><![CDATA[<p>SANTA ROSA (CBS SF) - The Sonoma County coroner’s office has identified the worker who was killed by falling sheets of glass Friday morning as 21-year-old Hugo Tapia of Windsor.</p>
<p>Tapia was employed by Volt Workforce Solutions, California Division of Occupational Safety and Health spokesman Peter Melton said Friday afternoon. The company has offices in Santa Rosa and San Francisco.</p>
<p>Melton said preliminary reports indicate Tapia and two other workers were moving an A-frame rack on rollers that contained seven 100-pound sheets of glass at Thermalsun Glass Products in the 3000 block of Brickway Boulevard in Santa Rosa.</p>
<p>“The initial report was the glass sheets fell on him. I’m guessing he died from the weight of it,” Melton said.</p>
<p>There was no initial indication from Cal/OSHA or the Sonoma County Sheriff’s Office that Tapia was cut by the glass.</p>
<p>Sheriff’s Lt. Tim Duke said during the movement of the glass sheets, the A-frame tipped “causing the glass sheets to dislodge and topple” the worker.</p>
<p>An autopsy is scheduled for Monday, a coroner’s office spokeswoman said.</p>

<content:encoded><![CDATA[<p>VACAVILLE (CBS SF) &#8211; A Vacaville man who stopped to help a crash victim on eastbound Interstate Highway 80 in Dixon Thursday morning died after he was struck by a car, a California Highway Patrol sergeant said.</p>
<p>The Sacramento County coroner’s office identified him as 36-year-old Spencer West Myers.</p>
<p>The incident began at about 4:55 a.m. when a Fairfield woman lost control of her 2004 Toyota Camry while driving east in the No. 2 lane west of Dixon Avenue, CHP Sgt. Phil Grant said.</p>
<p>The Toyota overturned and landed on its roof in the fast lane, Grant said.</p>
<p>Myers pulled over near the center divider east of the crash scene to help, Grant said.</p>
<p>A short time later, both Myers and the overturned Camry were struck by a 2002 Dodge Stratus driven by a Fairfield man, Grant said. The woman in the Camry was ejected from her car.</p>
<p>She and Myers were both taken to the University of California at Davis Medical Center, where Myers died at 6:11 a.m., according to the coroner’s office. The woman is expected to survive, Grant said.</p>
<p>The driver of the Dodge was not injured, and was not cited for any violations, Grant said.</p>
<p>Grant said the CHP does not want to discourage people from stopping to help at accident scenes, but wants to remind them to be aware of their surroundings.</p>
<p>The deaths of good Samaritans “happen all too often,” Grant said.</p>
<p>The crash is still under investigation.</p>
<p>The accident blocked lanes, and the CHP issued a Sig-alert at 5:05 a.m. All of the eastbound lanes were briefly shut down at Midway Road at about 5:20 a.m. to allow a medical helicopter to land on the highway.</p>
<p>All lanes were cleared shortly before 7 a.m.</p>

<content:encoded><![CDATA[<p>BEN AVON (KDKA) &#8211; Police are investigating after an early Wednesday morning accident that left two people dead and two others injured.</p>
<p>According to police, the accident happened on Ohio River Boulevard in Ben Avon around 2:30 a.m. near Brighton Road.</p>
<p>Three people were in a car traveling north on Route 65, when it crossed the center line and was t-boned by another vehicle.</p>

<p>A man was the only occupant of the second vehicle and he was taken to the hospital in serious condition.</p>
<p>The driver and front seat passenger of the first vehicle were killed on impact. The victims have been identified as 19-year-old David Mooney and 22-year-old Christopher Lantzy, both of New Brighton were killed in the accident.</p>
<p>A female in the back seat was taken to the hospital in serious condition.</p>
<p>“Actually, before we even got the call, one of my officers heard the impact. It was just from what I can see, a direct impact. We’re not seeing any skidding. It looks like the one crossed over and there was direct impact,” Ohio Township Police Chief Norbert Micklos said.</p>

<p>While Ohio Township Police could not say for certain, they&#8217;re looking at speed playing a potential role.  Nearby, Bellevue police may have seen the vehicle minutes before the crash.</p>
<p>&#8220;He believes he saw that same vehicle traveling down 65 and their town was unable to catch up with them.  Came through 65 at a high rate of speed.  He believes it was the same vehicle but was never able to catch up to it,” Ohio Township Police Corporal Don Manz said.</p>
<p>Meanwhile, Mooney’s death ends an eight-month investigation by New Brighton Police.</p>
<p>They say De’Andre Moon, a star running back at Rochester High School, was shot and killed last year on a house boat owned by Mooney.</p>
<p>He claimed to police that the shooting was in self-defense.</p>

<content:encoded><![CDATA[<p>BAY POINT (CBS SF) &#8212; An Antioch man died after he was crushed by an industrial mixer at a manufacturing plant in Bay Point on Monday, authorities said Tuesday.</p>
<p>Emergency responders were dispatched to the Henkel Corporation building at 2850 Willow Pass Road around 11:20 a.m. on reports of a person injured by machinery, Contra Costa Fire Protection District spokesman Steve Aubert said.</p>
<p>Crews transported the man to John Muir Medical Center in Walnut Creek, where he died from his injuries. A coroner’s deputy identified him Tuesday as 26-year-old David Eleidjan.</p>
<p>The California Division of Occupational Safety and Health, or Cal/OSHA, is conducting an investigation into the incident.</p>
<p>So far investigators have learned that Eleidjan became caught in an industrial mixer machine that crushed and amputated his legs, Cal/OSHA spokeswoman Erika Monterroza said.</p>
<p>The 26-year-old had been hired by Tennessee-based staffing company HRComp to work temporarily for Henkel, she said.</p>
<p>Cal/OSHA, which was notified of the fatal incident around 11:40 a.m. Monday, has launched a dual investigation into both employers.</p>
<p>Monterroza said the investigation could take up to six months to complete and will involve interviewing witnesses and examining company safety practices.</p>

<p>The accident happened around 8:20 p.m. Monday at Gratiot and Sanford avenues. Investigators say a driver behind the wheel of a dark-colored vehicle hit and killed a man before fleeing the scene.</p>
<p>Police did not immediately release the victim&#8217;s identity.</p>
<p>Anyone with information about the accident is asked to call Detroit police at 313-596-2260 or Crime Stoppers at 1-800-SPEAK-UP.</p>

<p>The victim was hit by Train 544 traveling from San Jose to Sacramento, Magliari said.</p>

<content:encoded><![CDATA[<p><strong>HIGHLAND PARK (WWJ) &#8211; </strong>At least two people are dead following a Friday morning accident that shut down Woodward Avenue for more than five hours.</p>
<p>Talking to WWJ Newsradio 950, police said two people were killed and two more were critically injured.  Later in the day, some reports said an additional victim had died, but police said they would not confirm the additional death until family members were notified.</p>
<p>The accident happened around 3:30 a.m. Friday on Woodward and Courtland Street, in front of the old McGregor Branch of the Detroit Public Library, south of Davison Freeway.</p>
<p>Highland Park police say just prior to the crash, an officer was attempting to stop a southbound traveling van on Woodward. When that van wouldn&#8217;t pull over however, the officer decided to back off.</p>
<p>&#8220;Due to the weather conditions, they did not feel it was safe to continue on. The van crossed left of center, striking a northbound van, resulting in two fatalities in the northbound van and two critical in the van that was traveling southbound,&#8221; Ofc. Christopher Zuellig said.</p>
<p>Reporting live from the scene, WWJ&#8217;s Mike Campbell said it appeared one of the vans caught fire after the collision, the other was pushed onto the front lawn of the library. Both vehicles were completely wrecked, with glass and other debris littering the roadway.</p>
<p>An investigation is ongoing.</p>

<content:encoded><![CDATA[<p>SAN LORENZO (CBS SF) &#8212; A 15-year-old boy was fatally struck by an Amtrak train in San Lorenzo on Thursday evening, officials said.</p>
<p>The train hit the teenager on the tracks north of the Hayward Amtrak station around 6:40 p.m. and the Alameda County coroner’s bureau was called to the scene, Amtrak spokesman Marc Magliari said.</p>
<p>The victim was identified by the Alameda County coroner&#8217;s bureau as 15-year-old Austin Price of Hayward.</p>
<p>Price was hit by Train 544 traveling from San Jose to Sacramento, Magliari said.</p>

<p>Authorities said the Price was among three teens playing a game of chicken on the railroad tracks across from San Lorenzo High School at the time of the crash. The other teens were not hurt in the incident.</p>
<p>&#8220;Those juveniles were apparently engaged in a game of chicken with one another. One of the juveniles, a male, was struck by the northbound Amtrak train,&#8221; said Deputy April Luckett of the Alameda County Sheriff’s Office.</p>

Before he could send it, police say the 22-year-old University of Northern Colorado student drifted into oncoming traffic, jerked the steering wheel and went off the road, rolling his car.</p>
<p><a href="http://hosted.ap.org/dynamic/stories/U/US_TEXT_DEATH?SITE=AP&#38;SECTION=HOME&#38;TEMPLATE=DEFAULT" target="_blank">According to the Associated Press</a>, Heit died shortly after the April 3 crash in Greeley, but his parents and police are hoping the photo of the mundane text on his iPhone will serve as a stark reminder to drivers.</p>
<p>The photo, published Wednesday in The Greeley Tribune, shows Heit was responding to a friend by typing “Sounds good my man, seeya soon” followed by a few letters before crashing.</p>
<p>In a statement released through police, Heit’s mother says texting while driving can “tear a hole in the heart of everyone who loves you.”</p>

<p>Deputy Fire Chief Gary Wilkinson said they received a call about the incident, along westbound I-696 at Mound, at around 5:30 a.m. Thursday.</p>
<p>Wilkinson said a semi truck ran over a 1997 Saturn coupe &#8212; cutting it in half.</p>
<p>The driver, a 50-year-old Dearborn man, was killed. He was the only person in the crushed vehicle.  The truck driver was uninjured.</p>
<p><a href="http://detroit.cbslocal.com/2013/04/11/heavy-rain-causes-crashes-gawker-delays-on-metro-detroit-freeways/">Reporting live from the scene</a>, WWJ Newsradio 950&#8242;s Vickie Thomas described it as one of the worst accidents she&#8217;d ever seen.</p>
<p>Wilkinson described the rescue effort.</p>
<p>&#8220;They were able to peel the door off of the car, but they couldn&#8217;t get the guy out. So our heavy rescue showed up and it took them about four, five minutes using the jaws [of life] to get him out,&#8221; Wilkinson said.</p>
<p>The man was rushed by EMS to a local hospital where he was pronounced dead. State police will release his name once his family is notified.</p>
<p>A preliminary State Police investigation revealed that the Saturn driver lost control and struck the concrete retaining wall.</p>
<p>The roadway was wet from overnight rain at the time of the crash. Police say alcohol was not a factor.</p>
<p>As the investigation continues, police are asking anyone who may have witnessed the accident to contact State Trooper Mathis at (248) 595-1725.</p>
