<content:encoded><![CDATA[<div class='snap_preview'><p>Amazing how much can change in so short a time. Last post I was plagued by a need for a job and no hopeful prospects. Now, I have a seemingly ideal job with contract signed and certification starting in three days. I&#8217;m going to be an English teacher at Gaba. They&#8217;re an English-teaching company in Japan that specializes in one-one-one lessons (&#8220;man-to-man&#8221;, as they call it, which of course sounds awful). I applied two months ago, and then again one month ago, and then again this month, because they put out a job posting on Gaijinpot.com every month (or maybe that&#8217;s just because they weren&#8217;t getting enough teachers &#8211; but if that were the case why didn&#8217;t they contact me sooner?). They were actually where I applied first this year, but after hearing nothing but a single rejection letter in two months, I had all but given up hope. I wasn&#8217;t going to apply in November, but I just happened to remember about the website after leaving in search of a Japanese job, and whaddaya know, I get the job this time.</p>
<p>I remember thinking as I applied for November that it would probably ironically work out this time, simply because I had come so far since my first time applying, in terms of courage to call (even Japanese) employers and do interviews, etc. Those were things worth learning, so I can&#8217;t be too mad that I didn&#8217;t get the job until now.</p>
<p>However, I&#8217;m definitely glad for it now. I was getting worried.</p>
<p>I felt so out of place at the first interview (information seminar, then brief cursory interview). There were probably eight of us, and though I probably looked fairly similar to the rest of the applicants, I felt so conspicuous. They all looked exactly like the stereotype of a Caucasian English teacher in Japan, mostly due to their formal dress. Like I said, I was dressed up too so I probably looked far less out of place than I felt. I think I was the youngest.</p>
<p>Another thing worth mentioning is that having grown up in Japan, I naturally assume that any foreigner I see on the street has less experience with the culture than me. So I avoid interaction with them, seeing as they will sooner or later do something extremely stupid and embarrassing and I will be associated with them because, to a Japanese, I look just like them. Funny, isn&#8217;t it? I&#8217;m a foreigner, and yet I&#8217;ve developed the same attitude towards them as the Japanese, that is, &#8220;He is a <em>gaijin</em>, he must be stupid and culturally inept. Must distance, must distance.&#8221; I only realized this a few years ago when I worked here with guy from America and he noted, &#8220;Foreigners here don&#8217;t associate with each other, do they? They almost seem hostile.&#8221; Yeah. You are embarrassing me, go away.</p>
<p>But now I willingly thrust myself into that environment and I&#8217;m going to stay in it because now they&#8217;re my colleagues. Ha. I don&#8217;t mind terribly. They&#8217;re interesting people. I&#8217;m just being forced to embrace my foreign self more than I&#8217;m used to doing when I&#8217;m here in Japan.</p>
<p>It&#8217;s quite an ideal job though. I submit when I&#8217;m willing to work each month and the clients sign up for times on the website. I can work as much or as little as I please. I hope there are a lot of students, because I want to earn a lot. I&#8217;m also going to be teaching kids. I was relieved to hear that that&#8217;s also one-on-one, because one kid is easier to manage than a group. I just hope I can keep him/her happy.</p>
<p>Most of the lessons are &#8220;blue-booked&#8221;, meaning that the client chooses the instructor based on prior experience or maybe just the profile and ten-second video on the website. I wonder how appealing mine is. It was quite awkward taking it, I hate those situations. It was funny to imagine weird videos though; for example the stereotypical American who came to Japan for the <em>anime</em> and maids and whatever else, and now finds that he still needs money to live on in his fantasy country. So he somehow manages to get a job with Gaba, and in his profile video belts out, &#8220;<em>Konichiwa</em>! My name is Bob Smith, I&#8217;m an <em>otaku</em>! I love Japan!!&#8221; And I imagine him doing the awesome Andy &#8220;I love Japan!!&#8221; face which involves him rolling his eyes back and&#8230;you know, better than me trying to explain it would be simply showing you. <p>Like I said, he&#8217;s awesome. I don&#8217;t think he&#8217;ll mind me shooting him a little traffic, although maybe yes if I&#8217;m acquiring all sorts of creepy readers. Which I don&#8217;t think is the case, seeing as my daily view count is still in the single digits.</p>
<p>So now I have a job and will soon have a (presumably) stable source of income, but I&#8217;m still curious about making money with a blog. I figure it&#8217;s only a matter of time before word gets out about this, and then I&#8217;ll hardly know what to do with all the traffic. I hope you know me well enough by now to understand just how seriously I believe that sentence.</p>
<p>I was looking into putting ads on blogs, and I haven&#8217;t looked very far, but it seems to be very simple. I&#8217;ve also heard that you need at least a hundred views daily to start making money. Any thoughts on that? Yes, I know that I could just find a blog on that very topic, but I&#8217;m giving you a chance to speak up. Actually that&#8217;s wrong, you&#8217;ve had that chance ever since you stumbled onto this little waste pile of cognition. Now I&#8217;m giving you the <em>invitation</em>.</p>
<p>And everything is of course so much brighter now that I have a job coming. I can enjoy life. I can open my shutters and breathe in the sunlight. I can also get back to studying Japanese, because that test is closer than ever and I have so much left to study.</p>
<p>I have a lot of things I want to write to you about but it will have to wait because I&#8217;m still busy. I&#8217;ve worn a suit every day for the past three days &#8211; but I need to get used to it, because with Gaba&#8217;s dress code I&#8217;ll likely be wearing a suit six days a week from now on. I want to buy a suit rather than borrow my dad&#8217;s forever. I wonder if they have my size here, if it won&#8217;t be astronomically expensive, and if it will look cool.</p>

<p>restless leg syndrome and/or an overactive bladder, urethral hypersensitivity; involuntary genital arousal with unprovoked orgasms, onset often during early menopause, as well as the 5 diagnostic criteria of persistent genital arousal disorder (PGAD) -  :</p>
<ul>
<li>Persists for an extended period of time (hours, days, and/or 	months)</li>
<li>Does not go away after 1 or more orgasms</li>
<li>Is unrelated to subjective feelings of sexual desire</li>
<li>Feels intrusive and unwanted; and</li>
<li>Causes distress.                                                                                                                                                  They find it is  is &#8220;highly associated with pelvic varices (in all on pelvic MRI scan) and with mechanical sensory neuropathy of 	the dorsal clitoral and pudendal nerves, whose symptoms are 	suggestive for small fiber neuropathy (SFN).                                    Although all the women reported varying degrees of social 	withdrawal, desperate feelings, dysthymia, agitation, or depressed 	mood directly caused by persistent unwanted genital sensations, <em>none 	were known to have previous psychiatric disorders</em>.&#8221;</li>

<p><a href="http://www.ncbi.nlm.nih.gov/pubmed/19040625?ordinalpos=8&#38;itool=EntrezSystem2.PEntrez.Pubmed.Pubmed_ResultsPanel.Pubmed_DefaultReportPanel.Pubmed_RVDocSum" target="_blank">Leiblum</a> discriminates such disorder from Female Sexual Arousal Disorder on the basis that &#8220;FSAD women displayed the greatest problems in desire, arousal, lubrication, orgasm, and pain while women with PGAD reported somewhat more desire than the control group but did not meet the cutoff score for sexual dysfunction.</p>
<p>It is strange that no other gyne or sexual health clinics in the world have  so far reported clusters of such patients as have these two clinics in New Jersey and Den Hage .</p>
<p>Leiblum ea could elicit only perhaps 1 such case (ie 1%) at a sexual health clinic in<span style="font-family:Times New Roman,serif;"><span style="font-size:small;"> <a href="../ijsa.rsmjournals.com/cgi/content/abstract/20/8/519">London  UK.</a></span></span> From an Internet survey she <a href="../www.ncbi.nlm.nih.gov/pubmed/17680784" target="_blank">reported in 2007</a> that  in the 50% of cases who had all 5  diagnostic criteria, “<em> they were significantly more likely to be depressed  (55% vs. 38% who did not have all 5) and to report panic attacks (31.6% vs.  14.6%). They were more anxious and more likely to monitor their physical  sensations. Both groups reported high rates of childhood and adult sexual abuse,  although the PGA women reported a higher prevalence of sexual victimization.  They were significantly more likely to endorse negative feelings about their  genital sensations and also more likely to complain of chronic fatigue syndrome  than women without the condition (10% vs. 0%). There were no significant  relationships with pharmacologic agents and symptoms.  It is hypothesized that  for a subset of women, psychological factors, namely anxiety, reinforce  exacerbate and maintain PGAD.” </em></p>
<p>But they have  anything but nymphomania (origin 1775: Oxford English dictionary), although they may be so mislabeled ie pseudo-nymhomania (Fenichel 1933).  Kinsey&#8217;s 1948 book on Female Sexual Responsiveness did not even mention, index nymphomania.  Kuperman 1961  in his chapter on <a href="www.amazon.com/...Sexual-Behaviour-Albert.../087668083X" target="_blank">Sex Hormones</a> unknowingly  implies the  difference between nymphomania and pseudonymphomania: &#8220;nymphomania may occasionally be treated successfully and paradoxically with androgens.. these patients who respond to androgens by a decrease in desire for frequent stimulation are probably those who have been unable to achieve satisfying orgasm, which androgen provides..   in other such patients, progesterone suppositories as an antiandrogen agent may diminish unwanted desire and erotic tendencies. &#8220;</p>
<p><a href="http://www.ncbi.nlm.nih.gov/pubmed/18004995">Stuckey ea</a> describe a single case who was cured by coil embolization of pelvic varicose veins- a more realistic therapy than embolization of the clitoris to infarct it, or amputation as was practiced by eminent UK physicians in Victorian times.. .</p>
<p>MANAGEMENT:</p>
<p>Women with  RGS/PGAS do not have either a central  arousal disorder or  craving for love/attention,  but vascular- neuropathic clitoral engorgement; which topical progesterone or anaesthetic eg lignocaine cream  may relieve by treating the endpoint, not the cause.</p>
<p>If varices are the strong associate, perhaps it is worth considering the pathophysiology of varices, which are apparently often associated with sensory neuropathy, presumably through swelling pressure on nerves &#8211; local varicose oedema. <a href="http://www.ncbi.nlm.nih.gov/pubmed/19347704?" target="_blank">Vercellini ea</a> note that pelvic varices are one common cause of pelvic pain in women.</p>
<p><!-- 		@page { margin: 2cm } 		P { margin-bottom: 0.21cm } 		A:link { so-language: zxx } -->Increased pressure and thrombosis  aside, varicose veins are strongly associated with female gender, ie with testosterone:estrogen level about 1/200th of that in middle-aged men, and  loss of collagen  (ie ascorbic acid) <a href="http://ang.sagepub.com/cgi/content/abstract/57/5/546" target="_blank">in smooth muscle and extracellular matrix</a>.</p>
<p>Higher female estrogen  is associated with stronger bones, and oedema, stress incontinence and vascular <em>relaxation; </em>but it notoriously contributes nothing to muscle growth and strength except in the unique uterus itself &#8211; only estrogen  grows the  uterus.  Only androgen grows body muscle mass and strength. From early menopause, testosterone falls gradually;  but especially with fattening, estradiol falls gradually but fat-derived estrone increases, reversing the premenopausal estradiol&#62;estrone dominance. Hence across midlife women mostly shrink their skeletons and lean mass but expand their fat mass steadily- ie couch potatoes develop increasing fatness frailty.</p>
<p>Hence (compression stockings for varicose legs aside), especially in women, apart from raising the legs, the foot of the bed at night, we commonly see varicose vein discomfort and distension in the legs and anus (piles) relieved by a few grams of bioflavinoid &#8211; ascorbic acid blend a day. A topical cream may augment this.</p>

Mai 2009 – Erneut hat die Deutsche Gesellschaft für Zahnerhaltung (DGZ) auf ihrer Jahrestagung den GABA Praktikerpreis der DGZ für herausragende praxisorientierte Arbeiten auf dem Gebiet der Zahnerhaltung verliehen. Die Auszeichnung ging diesmal an PD Dr. Stefan Rupf aus Homburg/Saar für seinen Vortrag „Diagnose und Behandler bezogene Erfolgsrate von Wurzelkanalbehandlungen“, präsentiert auf der Jahrestagung der DGZ im vergangenen Jahr. Der Preis ist mit 2.500 Euro dotiert.</p>
<p>9 Kurzvorträge hatte die Jury, bestehend aus Hochschullehrern und Praktikern, zu bewerten. „Dr. Rupf beschäftigte sich in seiner sehr klaren und sachlichen Präsentation mit der in der Praxis vom Behandler und Patienten immer wieder gestellten Frage nach der Erfolgsprognose einer Wurzelkanalbehandlung (WKB)“, heißt es in der Begründung der Jury. Rupf war zu dem Fazit gelangt, dass der Erfolg einer WKB unabhängig von der Tatsache sei, ob die Behandlung von einem Studenten oder einem approbierten Zahnarzt durchgeführt wurde. Er wies nach, dass 62% der Behandlungen nach 3 Jahren erfolgreich abgeschlossen werden konnten und nur 18% als Misserfolg bewertet werden mussten. Selbst bei Zähnen mit nachgewiesener apikaler Läsion konnte laut Rupf fast die Hälfte (47%) erfolgreich therapiert werden.</p>
<p>Der Praktikerpreis wird jährlich von der DGZ in Zusammenarbeit mit GABA, Spezialist für orale Prävention, vergeben. Die Auszeichnung der besten Falldemonstration eines approbierten Zahnarztes oder Studierenden der Zahnheilkunde im Praktikerforum der DGZ erfolgt immer im Rahmen der DGZ-Jahrestagung im Folgejahr. Wichtige Kriterien für die Vergabe sind nicht nur die Qualität der Dokumentation, die Komplexität der Problemdarstellung und der Vortragsstil, sondern auch die Praxisrelevanz. „Mit dem Praktikerpreis wollen wir die klinische Tätigkeit auf dem gesamten Gebiet der Zahnerhaltung unterstützen und den Austausch zwischen Praxis und Wissenschaft fördern“, erläutert Dr. Andrea Engl, Direktorin Medizinische Wissenschaften bei GABA.</p>

<p>Und jetzt ein paar Gedanken von Jemandem, der sich seit einer Reihe von Jahren mit der Endodontie beschäftigt.<br />
Herausragend und praxisorientiert, das sind die ersten Schlagworte, die mir ins Auge fallen.</p>
<p>Hohe Messlatte.<br />
Erfolgsprognose von Wurzelkanalbehandlungen, ich stimme zu, dass interessiert jeden Patienten.<br />
Die Frage kommt eigentlich immer: &#8220;Wie gut sind meine Chancen, den Zahn zu erhalten, Herr Doktor ?&#8221;</p>
<p>Soweit bin ich noch  d´ accord.<br />
Aber jetzt kommt das erste KO &#8211; Argument.<br />
Der Erfolg der WK ist unabhängig davon, ob die Behandlung von einem Studenten oder Praktiker durchgeführt wurde.<br />
Hallo ? Zwickt mich, wenn ich träumen sollte&#8230;</p>
<p>Was sind das für Studenten ?<br />
Die fantastischen Vier ?<br />
Die Liga der aussergewöhnlichen Gentlemen ?<br />
Die Unglaublichen ?<br />
Alvin und die Chipmonks ?</p>
<p>Es muss sich sehr viel verändert haben an den deutschen Unis in den letzten 19 Jahren.<br />
Denn zu meiner Zeit hätte sich kein Student auf die Fahnen geschrieben, erfahrenen Zahnärzten im Bereich der Endodontie auch nur annähernd das Wasser reichen zu können.<br />
Hut ab also vor dem, was heute den Studenten geboten wird.</p>
<p>Komisch nur, dass man, wenn man mit frischgebackenen Zahnärzten, direkt von der Uni, spricht, von dieser Einschätzung nicht viel zu spüren ist.<br />
Und vielmehr der Eindruck entsteht, dass nach wievor im Bereich der Endodontie universitärer Nachholbedarf besteht.</p>
<p>Gut, die andere Schlussfolgerung wäre, dass die Praktiker, die als Vergleich herangezogen wurden, nicht viel dazugelernt haben im Laufe ihres Berufslebens, also auf dem Niveau der Studenten stehengeblieben sind.</p>
<p>Auch keine besonders prickelnder Gedanke.</p>
<p>Wie groß war denn nun die Erfolgsquote ?<br />
62 % Erfolg nach 3 Jahren ?<br />
Hab ich richtig gelesen ?<br />
Nicht einmal 2 von 3 Zähnen erfolgreich behandelt ?<br />
Nach 3 Jahren ?</p>
<p>So dass davon auszugehen ist, dass diese schon jetzt schlechte Quote, im Laufe der nächsten Jahre sich noch weiter verschlechtern wird.</p>
<p>Au Backe. Im wortwörtlichen Sinne.<br />
Und im Falle von von nachgewiesenen apikalen Läsionen nur 47 Prozent Erfolgsquote.</p>
<p>Da kann ich ja gleich Roulette spielen und auf Rot oder Schwarz setzen.<br />
Das ist unterirdisch, um Herrn Bohlen zu zitieren.</p>
<p>Und eine Steilvorlage für jeden Implantologen.</p>
<p>Take them out.<br />
Raus damit. Und rein mit dem Titandübel.<br />
Der hält wenigstens.<br />
So schlecht kann man so gut wie  kein Implantat setzen, dass es nicht mindestens in 62  % der Fälle 3 Jahre lang durchhält.</p>
<p>Und ich kann es keinem Patienten verübeln, der zu solchen Schlüssen kommt.<br />
Offensichtlich kann man sein Geld verbrennen oder in wurzelzubehandelnde Zähne stecken.</p>
<p>Beides kommt vermutlich aufs selbe raus.</p>
<p>Fakt ist.<br />
Es geht besser als die anvisierten 62 %.<br />
Deutlich besser.</p>
<p>Ich sage sogar, dass bei Zähnen mit &#8220;nachgewiesener apikaler Läsion&#8221; nach 3 Jahren eine deutlich höhere Erfolgsquote erzielt werden kann als jene 62 %, die hier als &#8220;herausragender&#8221; Maßstab insgesamt vorgestellt wurden.</p>
<p>Kann ich den Beweis erbringen ?<br />
Noch nicht.</p>
<p>Aber als Erstes werde ich versuchen, an die Studie im Original heranzukommen, um zu schauen, ob das Studiendesign für eine weitere Untersuchung im eigenen Klientel durchführbar ist.</p>
