<description><![CDATA[As the landscape for Healthcare IT software and services continues to expand, the federal government]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p>As the landscape for Healthcare IT software and services continues to expand, the federal government has noted that security and data management will need some innovation and reinvention regarding the electronic medical record.  Approximately $60 million has been allocated to researchers for the following according to an article in <a href="http://www.healthleadersmedia.com/page-2/TEC-243827/HHS-Offers-60M-Grants-for-Strategic-Health-IT-Projects">HealthLeaders:</a></p>
<p>According to the article, funding for the four-year research period will target:</p>
<ul>
<li>Security of health IT research to develop security and risk mitigation policies and the technologies to build and preserve the public trust as health IT systems become ubiquitous.</li>
<li>Patient-centered cognitive support research to align health IT with the day-to-day practice of clinicians as they provide care.</li>
<li>Healthcare application and network platform architectures research to achieve electronic exchange and use of health IT securely, privately, and accurately.</li>
<li>Secondary use of electronic health record data research to improve the overall quality of healthcare, population health, and clinical research, while protecting patient privacy.     &#8212;-The program is called SHARP, Strategic Health IT Advanced Research Projects.  Applications are due January 25, with awards anticipated in March 2010. <content:encoded><![CDATA[<div class='snap_preview'><p>John Goodman of the conservative Dallas based think tank National Center for Policy Analysis (NCPA) issued a &#8220;William Wallace-esque&#8217; FREEDOM pitch today on his blog entitled <a title="Permanent Link to Reforming Health Care the Right Way" rel="bookmark" href="http://www.john-goodman-blog.com/reforming-health-care-the-right-way/">Reforming Health Care the Right Way</a>.</p>
<p>This is a man who previously claimed that there are no uninsured (from a health insurance point of view) in America; afterall he posits that everyone has access to the ER, so what are you complaining about? (paraphrased).</p>
<p>In his blog post today on the day after the <a title="Senate Health-Care Measure Clears Crucial Test Vote " href="http://www.bloomberg.com/apps/news?pid=20601070&#38;sid=aWXyoFrldzDc" target="_blank">historic Senate vote</a> to advance the health care bill, Goodman opines on the right and wrong way to reform US healthcare.</p>
<p>What I find most insightful is the rebuttal comment proferred by Uwe Reinhardt as to the fantasy world this man, and unfortunately many other ideological predisposed converts, apparently inhabit. This industry defies over simplification, and disingenuous over simplification is the prima facie basis on which the opposition has stimulated emotive mis-understanding of the nature of the malady as well as it&#8217;s appropriate and quite comprehensive remedy.</p>

<p><em>I hear Richard Branson of Virgin Air is seriously exploring space travel as a commercially feasible project.</em></p>
<p><em>Once he has that done, I shall be able to book a flight to the distant planet on which John Goodman lives.</em></p>
<p><em>It is the planet on which all physicians always are purely the agent of their patients and do not have any economic conficts of interest — such as making money on tests they prescribe to anxious patients or from referring patients to imaging centers in which they have a state or to collegues with whom they play gold, and so on. Such conflicts of interest do not crop up on John’s planet, not because government forbinds them (there is no government on that planet), but because such conflicts of interest just don’t exist there somehow. Remember: it’s another planet!</em></p>
<p><em>On John’s planet it is also easy to have price competition among physicians, because all ill health on that planet can be cured with just one standard, well defined “unit of health care.” What that is I do not know, but John does, because he lives there. He’s probable consumed some, rationally, I wouold assume.</em></p>
<p><em>Contrast that with an earthbound hospital charge master with 20,000 itsems in it or the physician fee schedule with 7,000 items in it. How would one make diffenetials in the elements of those huge vectors understandable to patients?</em></p>
<p><em>John took on a new religion on that planet to where he actually immigrated — he once lived on earth. On earth he always boldly talked for decades about “Consumer Directed Health Care,” but neither he nor his entire think tanks (the NCPA) every did a stitch of work to help develop the user-friendly price information that patients as “consumers” would need to make rational choices in health care ex ante.</em></p>
<p><em>Once on the new planet, John realized that he sinned on earth and swore to do better there. Of course, on his new planet it’s easy: there is only one type of health care and one price. The redemption was a piece of cake.</em></p>
<p><em>All people on John’s planet have the same income — in fact, they all have John’s high income and all also have Ph.D.’s. or M.D.s So the problem with poor people not being able to afford high deductibles and therefore stiffing doctors and hospitals for it does not exist on John’s planet. Nor is there a problem with health illiteracry, because everyone on John’s planet has a Ph.D. or M.D. Every patient on John’s planet knows exactly what he or she needs before going to a doctor and simple shops around for a low price.</em></p>
<p><em>For the most part, an individual’s need for health care in a coming year on John’s planet is certain and predictable and thus not really insurable. Only the need for about 20% of all health care is stochastic and hence insurable.</em></p>
<p><em>On John’s planet, 50% of any large group of people account for 50% of all health spending — 80% of the people account for 80% of all health spending. Here on earth, 20% of the sickest account for 80% of all health spending, and much of that 80% will also be insured. It will be managed by some insurance clerk coming between doctor and patient.</em></p>
<p><em>Oh how I long to go to the planet where John Goodman lives, where life is so easy and so simple. I am so tired of the mess here on our planet. Aren’t we all?</em></p>
<p><em>Small wonder that John just up and left Mother Earth for a planet where all his theories actually work.</em></p></blockquote>

I saw myself how an operator could help deliver a baby by instructing the guy who made the call. I cannot imagine a situation where you cannot call professional help but can access WikiHow.</p>

<title><![CDATA[Medicine and Web 2.0 University Course: End of 3rd Semester]]></title>

<description><![CDATA[I&#8217;m really proud that I can organize and run the world&#8217;s first university credit course ]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p>I&#8217;m really proud that I can organize and run the world&#8217;s first university credit course focusing on <a href="http://med20course.wordpress.com/" target="_blank">Web 2.0 and medicine</a> at a medical school. This was the third, and so far most successful, semester with 115 students who filled a survey before and after the course. I hope I can publish the results in a peer-reviewed paper soon with the help of real experts in this field.</p>
<p><a href="http://med20course.wordpress.com/2009/11/30/semester-3-week-10-web-3-0-web-4-0/" target="_blank">In the final lecture</a>, I used <a href="http://www.Prezi.com" target="_blank">Prezi.com</a> again and talked about the future of medicine as well as the results of the surveys. I&#8217;ve already got some invitations to do at least a part of the course at other international universities. What is sure is that the next semester will launch in February in Debrecen with assignments and tests because I would like to engage students even more. See you there!</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>There is an interesting article <a href="http://www.ehealtheurope.net/news/5438/doctors_warned_of_facebook_flirts" target="_blank">in E-Health Europe</a> about how patients try to contact doctors on Facebook, the popular social networking site, and how doctors shouldn&#8217;t respond to them. In my &#8220;<a href="http://med20course.wordpress.com" target="_blank">Medicine and Web 2.0</a>&#8221; university credit course, we cover this important issue several times and I try to provide students with useful pieces of advice about how to avoid such problems.</p>
<blockquote><p>The Medical Defence Union said it was aware of a number of cases where patients have attempted to proposition doctors by sending them an unsolicited message on Facebook or similar sites.</p>
<p>The medical defence body said it would be “wholly inappropriate” to respond to a patient making an advance in such a way.</p>
<p>Dr Emma Cuzner, MDU medico-legal adviser, said the pitfalls posed to doctors using social networking sites and inadvertently breaching patient confidentiality had already been well documented but the dangers of patients using the sites to approach doctors were less well publicised.</p>
<p>In an anonymised case highlighted by the MDU a female GP was asked out for a drink by a patient as she left the surgery. When she declined the patient contacted the doctor via Facebook and sent her a bunch of her favourite flowers which he had found out about from her freely available Facebook page.</p></blockquote>

<content:encoded><![CDATA[<div class='snap_preview'><p>A few months ago, I <a href="http://scienceroll.com/2009/06/18/pew-internet-research-e-patients/" target="_blank">wrote </a>about the <a href="http://pewinternet.org/Reports/2009/8-The-Social-Life-of-Health-Information.aspx?r=1" target="_blank">latest Pew Internet Research report</a> that focused on online health information and the world of e-patients. I also shared excerpts:</p>
<blockquote><p>61% of American adults look online for health information.</p>
<p>In 2000, 46% of American adults had access to the internet, 5% of U.S. households had broadband connections, and 25% of American adults looked online for health information. Now, 74% of American adults go online, 57% of American households have broadband connections, and 61% of adults look online for health information. We use the term “e-patient” to describe this group.</p>
<p>American adults continue to turn to traditional sources of health information, even as many of them deepen their engagement with the online world.</p>
<p>A majority of e-patients access user-generated health information.</p></blockquote>
<p>Now Susannah Fox who is an Associate Director of the Pew Internet &#38; American Life Project and principal author of the Project’s survey reports on e-patients and online health, published <strong><a href="http://e-patients.net/archives/2009/11/the-pew-internethealth-faq.html" target="_blank">a fantastic and very useful guide</a></strong> for interpreting Pew Internet data dedicated to healthcare.</p>

<title><![CDATA[The 'Through-put' Economy of Money Driven Medicine]]></title>

<p>Well the same holds true with respect to the &#8216;resistance is futile&#8217; health care borg aka &#8216;the tapeworm medical industrial complex&#8217; economy. Our health care system is at risk of collapse; with 46 million uninsured, 25 million under-insured, primary care physicians bailing on the system daily, health care premiums sky rocketing, while benefits are being reduced and cost shifted from the plan to the employee. No one is happy with this status quo, quite to the contrary of some public opinion polls that tout we have the &#8216;best health care system&#8217; in the world!</p>
<p>Surely we have the best high tech or &#8216;rescue care&#8217; medicine that can be found. But when it comes to the &#8216;value proposition&#8217; the story gets a little more complicated, and requires a bit more than sound-bytes or political grandstanding to get at the truth.</p>

If we thought the <a title="Senate Finance Committee Hearings" href="http://finance.senate.gov/sitepages/hearings.htm" target="_blank">Senate Finance Committee hearings</a> followed by the debate of Chairman&#8217;s mark was exhausting, just wait for what&#8217;s in store from the special interests, and their &#8216;Quack-er&#8217; proxies in the Senate during the impending floor debate of the merged bills.</p>
