<content:encoded><![CDATA[<div class='snap_preview'><p><strong>DIFFERENCE BETWEEN HIP RESURFACING AND HIP REPLACEMENT</strong></p>
<ol>
<li>Hip Resurfacing or Birmingham Hip Resurfacing (BHR) is a bone-conserving implant. BHR procedure removes only the unhealthy bone and replaces it with a metal on metal articulation while in Total Hip Replacement (THR), a long metal component is fixed into the femur (thigh bone) which articulates with a polyethylene cup cemented into the pelvis. THR also sacrifices a large quantity of normal bone.</li>
<li>Hip resurfacing is a technique invented specifically for younger patients with hip problems. Conventional Total hip replacement, while being a good option for an elderly person (above 70 yrs) is a poor choice for young patients as it will fail rapidly.</li>
<li>BHR uses metal on metal hip joints which have been shown to be significantly more wear resistant than the traditional metal on plastic hip joints used in total hip replacement surgery.</li>
<li>The femoral head in BHR closely matches the size of natural femoral head and is substantially larger than the femoral head of a total hip replacement. This leads to greater stability in the new BHR joint.<br />
BHR patients can indulge in full sporting activities without the risk of dislocation. However, in THR the range of movement is restricted as the risk of dislocation is higher.</li>
<li>In BHR, preservation of bone stock at the time of operation and less stress shielding makes it easy to revise the surgery, if needed. Whereas, by its very nature, revision surgeries in THR are difficult.</li>
<li>The long metal component used in THR may cause alteration in the leg length which is not the case in BHR.</li>
<li>Rehabilitation is much faster in BHR than in THR.</li>
<li>BHR can be done by a minimally invasive approach and the invasion can be kept to less than 10cms. The operation also involves minimal damage to bone.</li>
</ol>
<p><strong>ADVANTAGES OF HIP RESURFACING</strong></p>
<ol>
<li>No plastic (polyethylene) is used like in conventional hip replacement. Since an anatomical sized &#8216;metal on metal&#8217; bearing is used it lasts for a very long time, manifold that of conventional hip replacement. The anatomy and bio-mechanics after resurfacing mimic a normal hip very closely.</li>
<li>Post operatively, patients are encouraged to be very active and must play some sport, swim, etc. No activity is restricted including sitting on the floor, crossing legs etc.</li>
<li>The polyethylene &#8216;wear particles&#8217; do not damage the surrounding bone like in conventional hip replacement and the quality of bone actually improves with time after hip resurfacing. This makes a revision solution (if at all needed) very straight forward in a hip resurfacing surgery.</li>
<li>The patient is usually made to walk full weight bearing the day after the operation and is usually discharged from the hospital at about 5- 6 days from the operation. They can resume any work at 3 weeks from operation and sport is started 6 weeks from operation.</li>

<p>The normal hip joint consists of a ball rotating inside a socket. The ball is called the head of the femur and the socket in the pelvis is called the acetabulum. The surface of the femoral head and the acetabulum (i.e. the ball and the socket) is covered by a protective layer of cartilage. This makes the joint smooth and reduces friction during movements. It covers and protects the bone like the rubber of a car tyre.</p>
<p>Any condition that damages the cartilage will lead to pain, dysfunction and eventually arthritis. Cartilage can get damaged due to a number of reasons including:</p>
<ul>
<li>Injury</li>
<li>Infection</li>
<li>Ankylosing Spondylitis</li>
<li>Avascular Necrosis (loss of blood supply to bone)</li>
<li>Rheumatoid Arthritis</li>
<li>Osteoarthritis (OA)</li>
<li>Developmental problem like bone Dysplasias (Developmental Dysplasia of the Hip or DDH)</li>
<li>Slipped upper femoral epiphyses</li>
<li>Avascular Necrosis (AVN)</li>
<li>Inflammatory Arthritis, etc.</li>
</ul>
<p>Hip resurfacing (also known as hip resurfacing surgery, Birmingham hip resurfacing, Birmingham hip resurfacing surgery, BHR, Articular Surface Replacement, ASR and surface replacement arthroplasty), an orthopedic procedure, is an approach to the treatment of hip pathology. It&#8217;s a technology that replaces the worn surfaces of the hip joint with caps of high carbide cobalt chrome.</p>
<p>Hip Resurfacing Surgery is a bone-conserving alternative to conventional Total Hip Replacement (THR). In a hip resurfacing, the head, neck and femur bone are preserved in an effort to facilitate future surgery should it be necessary and to enable the patient to take advantage of newer technology or treatments in the future.</p>
<p><strong>BENEFITS OF HIP RESURFACING SURGERY</strong></p>

<li>More of the natural bone is preserved</li>
<li>A resurfaced is similar to the natural hip joint resulting in increased stability, flexibility and range of motion</li>

<p>Birmingham Hip Resurfacing (BHR) is an alternative to Total Hip Replacement (THR) in younger and more active patients. Typically, physically active patients, under 65 years of age who suffer from hip arthritis, hip dysplasia, avascular necrosis of the hip or inflammatory arthritis are candidates for BHR. Patients over the age of 65 can also benefit from BHR if their bone quality is strong enough to support the implant. <p><strong>HIP RESURFACING PROCEDURE DETAILS</strong><br />
After making incisions into the skin, the surgeon approaches the hip joint and prepares the bone for resurfacing using specialized instruments. He prepares the head of the femur to receive the resurfacing component. The socket is then shaped to accept the new resurfacing cup. Then the socket is inserted in a position to give stability to the hip, and allow the bone to grow around it. This encourages long-term security in the newly refinished hip joint. Finally, using bone cement, the resurfacing head component is fixed into position. The incisions are then closed with sutures. <content:encoded><![CDATA[<div class='snap_preview'><p>A novel form of treatment is now available in India for patients with early stage avascular necrosis of the hip.  This treatment is based on regenerative medicine.</p>
<p>Avascular necrosis of the hip is a condition of unknown origin affecting many young and middle aged patients. Steroid intake, alcoholism, Sickle cell diseas, hypertension, decompression sickness are some known causes. Moderate to severe disease is treated by hip resurfacing whereas core decompression is offered to the first stage of the disease. Grade II A disease is an intermediary stage between the two extremes and  has no definite treatment options. There is controversy on the appropriate treatment. Hip replacement is perhaps too drastic an operation for this early stage of the disease. Core decompression has limited success rate.  Left to progress it leads to deterioration and hip arthritis.</p>
<p>For this stage, a biolgoical option is now available. This is based on the potential of stem cells to heal the avascular bone. Stem cells are drawn from the patient and cultured in the lab. A few weeks later, they are injected back into the affected portion of the femoral head along with a core decompression. In a core decompression the surgeon will drill a tunnel in the hip bone leading into the affected area.  The regenerative bone cells are injected into the affected area. The bone forming cells called Osteoblasts synthesize new bone in the dead area. This leads to healing and hence a biological cure. The technique is called Ossron.<br />
For advanced stages of the disease resulting in hip arthritis, Hip resurfacing is offered at the same center by experienced Orthopedic surgeon India</p>

<p><!--more-->I just completed the new exercises that Karen, the physical therapist as the hospital, gave me. The physical stuff is relatively straight-forward. I just need to regain my strength.</p>

<p>Many&#8211;probably most&#8211;of the patients were not resurfacing candidates, given their age and physical condition. But there were several younger patients who seemed eligible.</p>
<p>Resurfacing, I guess,  is still a niche.</p>

<p>The hospital has its act together,  moving patients swiftly through a series of visits to labs, class rooms, and doctor&#8217;s offices.</p>
<p>Any idea of how much a private room goes for? Something like $800, $900 for a river view.  For an extra $800, you get a private nurse, too. Only in New York.</p>

<p>So far, the glide path is smooth for surgery on Monday. On Friday afternoon, I find out what time to show up. On Sunday, I alter my diet slightly and have a you-know-what. On Monday, it&#8217;s show time.</p>
<p>One tremendous advantage of revision surgeries, which is not talked about all that much, is the absence of the range of motion restrictions for six to eight weeks. People mention it, but I had not realized how limiting it can be to avoid internal rotations and 90-degree bends. It must be a real pain to put your socks and shoes on with those restrictions in place.</p>
<p>It is hard to get excited by surgery, but I do feel relieved that I can count almost count down to the big day with just one hand.</p>

It provides a hefty counterweight to some of the links to posts by surgeons who are not fans of the procedure.</p>

<p>I do, however, glance at the research, and this <a href="http://">piece</a> caught my eye because it was written by surgeons who have been resurfacing for a long time.</p>

<p>Simply stated, their conclusion is encouraging for patients of experienced surgeons: &#8220;Hip resurfacing using a metal-on-metal articulation with a stemmed femoral component is a durable and successful procedure. It is technically demanding.&#8221;</p>

All good. The only downside is I found out that I won&#8217;t know until the day before surgery, when I am supposed to receive a call between 1 and 7 pm, what time to show up at the hospital. Don&#8217;t you think a little more than 12-16 hours notice is called for? I guess not.</p>

<p>My limp seems to get worse each week.  I  managed to practice Aikido a few times this week, but don&#8217;t think that I have many days left. I can&#8217;t do forward rolls and have trouble getting off the mat.</p>
<p>We have two cars that collectively have 320,000 miles. I don&#8217;t think my hip is far behind. Time to replace the worn-out parts.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>Patients have impressions of hip resurfacing surgery that are not supported by peer-reviewed research, according to a recent <a href="http://www.orthosupersite.com/view.asp?rID=43768" target="_self">study</a> by researchers at the Mayo Clinic.</p>
<p><!--more--></p>
<p>In the study, 82% felt hip resurfacing arthroplasty was a safer procedure than total hip replacement, 79% percent felt there was less soft tissue damage associated with hip resurfacing, and 80% felt they would return to their activities more quickly.</p>
<p>But, the study&#8217;s authors say, &#8220;there are no reasonable (prospective blinded) data to support patients’ beliefs that hip resurfacing arthroplasty has lower complication rates or will provide better functional outcomes than conventional THA.&#8221;</p>
<p>&#8220;It seems as though surgeons, the industry, orthopedic societies, and the media have done a relatively poor job educating the public concerning the realities of hip resurfacing arthroplasty,&#8221; the authors conclude.</p>
<p>I am two weeks away and do not anticipate a quicker recovery or less soft-tissue damage from hip resurfacing. I am happy, however, that Dr. Su will not be sawing off the top of my femur. I want to keep as much bone as possible. That is my lay reason for getting a resurfacing,  rather than a replacement.</p>

My colleague Gerry was back at work, from the comfort of his living room, in a few days.  Others spend many weeks away from work. The <a href="http://health.groups.yahoo.com/group/surfacehippy/message/152273">topic </a>is active on the surfachippy message board. Many patients caution against doing too much too soon.</p>

<p>I too can easily work from home, so I hope that I can at least start working a few hours a day relatively quickly and gradually pick up my pace. I am not sure whether that means days or weeks. I am just fortunate that my job is not physically taxing and that I have a flexible working arrangement.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>I hopped on the scale a week ago&#8211;four weeks before my surgery&#8211;and decided it was time to lose weight prior to the big day. Over the past several months, as the level of hip pain increased, so has my weight. My experience has been that losing weight is the best &#8220;cure&#8221; for arthritis. Less pain=less pain=greater desire to stay active.</p>
<p><!--more-->Last Tuesday, I weighed in at 188, below my heaviest weight of 196 eight or nine years ago, but far above the 170-172 range in which I feel most comfortable. Over the past week, I have lost almost seven pounds&#8211;not bad. I am hoping that I can lose ten pounds or so in the next three weeks so that I will not be carrying around extra weight during recovery.</p>
