In ground-breaking project, one of America’s poorest communities is turning to the Middle East to try to resolve its crisis</p>

<p>As Marie Pryor shuffles along a Mississippi roadside collecting discarded drink cans to sell for a few cents, her breath comes in short puffs caused by a congenital heart defect. The same condition caused her granddaughter’s death earlier this year.</p>
<p>The last place on earth she would look for help is Iran, a country widely regarded in America as the enemy. The US and Iran have not had diplomatic relations for 30 years and the two governments trade daily insults over Iran’s nuclear programme. Last week Tehran charged three American hikers with espionage after they apparently strayed across the border.</p>
<p>But with Congress acrimoniously debating the reform of health care, it is to Iran that one of America’s poorest communities is turning to try to resolve its own health crisis.</p>
<p>A US doctor and a development consultant visited Iran in May to study a primary health care system that has cut infant mortality by more than two-thirds since the Islamic revolution in 1979.</p>
<p>Then, in October, five top Iranian doctors, including a senior official at the health ministry in Tehran, were quietly brought to Mississippi to advise on how the system could be implemented there.</p>
<p>The Mississippi Delta has some of the worst health statistics in the country, including infant mortality rates for non-whites at Third World levels.</p>
<p>“It’s time to look for a new model,” said Dr Aaron Shirley, one of the state’s leading health campaigners.</p>
<p>“Forty years ago, when I was a resident at Jackson hospital, I was in charge of admitting sick babies and was astonished at all the children coming in from the delta with diarrhea, meningitis, pneumonia.</p>
<p>“After years of health research and expenditure of millions of dollars, nothing much has changed.”</p>
<p>As the House of Representatives and Senate weigh the cost of President Barack Obama’s health reforms, Shirley points out that good primary care prevents people from ending up in hospital in the first place.</p>
<p>Besides, nowhere is the need for reform more acute than in Mississippi. The southern state has the highest levels of child obesity, hypertension and teenage pregnancy in the US. More than 20% of its people have no health insurance.</p>
<p>Baptist Town, where Pryor lives, is typical. A rundown suburb of Greenwood, the collapse of the cotton industry has led to massive unemployment. The local stores are a pawn shop, Juanita’s Beauty Salon and Bail Bonding, and an office offering “payday and title loans”.</p>
<p>Pryor’s son Kenneth and daughter-in-law Lizzie, who live with her, are both out of work and their only daughter died from her heart condition at the age of 26. With no local clinics or transport, they go to the hospital’s accident and emergency department if they need a doctor.</p>
<p>The idea of looking for solutions in Iran emerged when James Miller, a consultant based in Mississippi, was called in to advise a rural hospital in financial difficulty. He was shocked to find that the state had the third highest medical expenditure per capita, but came last in terms of outcome.</p>
<p>Miller, managing director of Oxford International Development Group, remembered a conference in Europe where Iranian officials had explained how their country had revolutionized its health care system.</p>
<p>Facing shortages of money and trained doctors at the start of the Iran-Iraq war in 1980, the new government launched a system based on community “health houses”, each serving about 1,500 people.</p>
<p>Locals were trained as health workers known as <em>behvarz</em>, who would travel their area, dispensing advice about healthy eating, sanitation and contraception as well as monitoring blood pressure and conditions such as diabetes.</p>
<p>It was a stunning success, reducing child mortality rates by 69% and maternal mortality in rural areas from 300 per 100,000 births to 30. There are now 17,000 health houses in Iran, covering more than 90% of its rural population of 23m.</p>
<p>Miller contacted Shirley, who is seen as a community health pioneer in Mississippi and had recently converted a deserted shopping centre in Jackson into a “medical mall” for the poor.</p>
<p>“I thought if the Iranians could do it with a fraction of resources we have, then why shouldn’t we?” said Shirley.</p>
<p>An Iranian doctor helped them make contact with Shiraz University, which manages more than 1,000 health houses and trains health care workers.</p>
<p>Shirley and Miller visited Iran in May and were astonished to be welcomed with open arms. When they went to remote villages to see the health houses, the Iranians were equally amazed.</p>
<p>“They told us this is a miracle,” said Miller. “Not only were Americans coming here, but also they were learning from us rather than telling us what to do.”</p>
<p>One villager exclaimed: “We always knew rain fell down but never knew it could fall up.”</p>
<p>They signed an agreement with Shiraz University to form the Mississippi/Islamic Republic of Iran rural health project and applied to the US Treasury for a special licence for “Iranian transactions”.</p>
<p>The next step was to win over communities in Mississippi. They started with Greenwood, where Shirley had already been in talks about setting up a local clinic.</p>
<p>Community leaders were shocked when he advised using Iran as a model. “To be honest, I wasn’t overwhelmed with the idea of copying Iran,” said Larry Griggs, the local fire chief. “It’s not exactly one of the most favourable countries to the US.”</p>
<p>They also had to overcome the legacy of distrust between blacks in the American south and public health officials after a series of scandals over medical experiments. The most notorious was the Tuskegee experiment between 1932 and 1972, in which 399 impoverished, black, illiterate farmers were left to suffer from syphilis even though penicillin was available. More than 100 died.</p>
<p>To sell the Iranian idea, Miller promoted it as “a health care model just like the Beetle”, pointing out that the popular Volkswagen Beetle had been conceived by the Nazi regime to show “good things can come out of somewhere not very popular in the world right now”.</p>
<p>The Iranian experts who came to Mississippi included two of the programme’s architects, Dr Hossein Malekafzali, a former minister who is professor of public health at Tehran University, and Dr Kamal Shadpour, the initiative’s co-ordinator in the health ministry.</p>
<p>The Greenwood community was convinced and leased a defunct car showroom for $1 a month for the first Mississippi health house, which is due to open next month. Fifteen Delta communities have expressed interest and Harvard’s School of Public Health will monitor the project.</p>
<p>Paula Gutlove, deputy director of the Institute for Resource and Security Studies, a US think tank, said there was a positive shock value to using an Iranian model. “The exotic nature of working with Iran makes it intriguing to potential funders and sponsors,” she said.</p>
<p>The first candidates from the Mississippi Delta are expected to be trained as health assistants in Iran this spring. If it works, Shirley hopes to extend the programme to the rest of the US. “Just as Mississippi was ground zero in the civil rights movement, so it can be for health,” he said.</p>
<p>Nonetheless, the Iranian connection poses a problem. Knowing that many Americans might be outraged, they have not spoken about the project. Even the governor of Mississippi is unaware of it. “We’ve been deliberately working under the radar,” said Shirley.</p>
<p>The programme chimes with Obama’s policy of engagement and his support of so-called “smart diplomacy”, using links between scientists as a way of breaking down barriers between countries. Following his speech in Cairo last June, aimed at reaching out to the Islamic world, the president has appointed three science envoys who will head to the Middle East next month.</p>
<p>“The Iranians are a proud people with 5,000 years of history and huge contributions to science and medicine,” said a State Department official.</p>
<p>“A project like the Mississippi one is incredibly powerful as it appeals to that Iranian concept of history. It’s a great way to keep the door open between the two countries.”</p>
<p>Gutlove points out that similar meetings between American and Soviet scientists in the 1980s helped pave the way for the end of the cold war. “What we did in the 1980s created lasting relationships which cut across the divide,” she said.</p>
<p>“It’s a win-win project,” said Shirley. “Not only do we finally have a way of addressing disparities in Mississippi, but also building relations between peoples.”</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>First off, my thoughts are not my own, but merely gleaned from the wisdom of my forefathers, at least those forefathers with the means to publish their wisdom in bound book form.  With that caveat, let me refer to the work of the peerless R.V. Price (the handsome fellow pictured), the writer of <a href="http://www.gutenberg.org/etext/18467">The People’s Common Sense Medical Adviser</a>.  He also established The Invalids’ Hotel and Surgical Institute in Buffalo, NY (which, as the advertisement in his book notes, is “one of the healthiest cities in the United States”).  The following information is taken from the ninety-sixth edition (96th!), published in 1918.  Has anything that has gone through 96 incarnations ever been wrong?  Ponder.</p>

<p>Well then, how does one know one has the hypertension?  Does one feel “dizz[y]” and “oppress[ed]”?  Does one have “pain in the heart and down arms”?  Do one’s arteries feel “firm, cord like, full, often beaded”?  Then one might have the hypertension.</p>
<p>As the estimable Dr. Pierce teaches, “high blood pressure, like high temperature in fever, is a condition due to a poison in the blood”.  How do you rid yourself of said poisons?  You could “increase the volume and thinness of the blood by the free use of water”.  Or, if you don’t feel like kowtowing to Big Water, you could try Dr. Pierce’s Pleasant Pellets (which rid the system of “fermenting poisons”).  Or you could use Dr. Pierce’s Golden Medical Discovery, which leads to the “expulsion of poisons in the blood”.  Or perhaps Dr. Pierce’s Anuric Tablets, miraculous medications which “lessen fermentation and poisoning in the intestines by their increase of the flow of bile from the liver which is the natural antiseptic and laxative”.  Or, better yet, by taking all three.  Of course, by taking all three, you might run the risk of your blood pressure becoming super-terrific.  Is that a risk you’re willing to take?</p>

If the particles were to aggregate, or if larger particles were present, then they could theoretically reduce blood flow in capillaries, cause mechanical damage to some tissues, or initiate acute or chronic inflammatory reactions. Reduced blood supply to tissues may lead to ischemia or organ insufficiency in the brain, kidney, liver, heart or lungs.</p>

Scientists have raised the possibility that cardiovascular disease is linked to disturbances in the body&#8217;s 24-hour clock.<br />
Working on mice, the Japanese team found a genetic risk factor for a form of high blood pressure is influenced by 24-hour or circadian rhythms.<br />
The study appears online in the journal Nature Medicine.<br />
Malfunctions in the body clock &#8211; which influences much of the body&#8217;s chemistry &#8211; have been linked to many diseases.</p>

And lead researcher Professor Hitoshi Okamura said the latest study was in line with data which suggested shift workers, long-distance flight crews and people with sleep disorders have a heightened risk of heart problems.<br />
High blood pressure &#8211; known as hypertension &#8211; can lead to heart attack, stroke, kidney damage, and many other medical problems.<br />
Many genes have been identified as being essential elements making up the circadian clock.<br />
For example, mice lacking a pair of molecules known as cryptochromes have an abnormal circadian rhythm.<br />
The latest study, by Kyoto University, found these mice were vulnerable to high blood pressure because of abnormally high levels of a hormone called aldosterone that prompts water retention in the kidneys.<br />
Strong correlation<br />
The researchers showed that the circadian clock directly controls a gene that plays a key role in production of the hormone.<br />
The researchers say a similar gene is found in humans.<br />
They stress more work is needed to determine whether a misfiring circadian clock can lead to high blood pressure in humans.<br />
But Professor Okamura said the research raised the prospect of new ways to treat hypertension.<br />
Professor Bryan Williams, an expert in hypertension at the University of Leicester, described the study as &#8220;fascinating&#8221;.<br />
He said: &#8220;We know that there is a strong correlation between time of day and cardiovascular events, which often coincide with the early morning surge in blood pressure.<br />
&#8220;So this does provide some insights into the mechanism that might underpin blood pressure deregulation in some people.&#8221;<br />
Professor Williams said some people with high blood pressure were known to have high levels of aldosterone.<br />
But he added: &#8220;What we don&#8217;t know is how common this mutation might be in human hypertension.&#8221;<br />
Professor Jeremy Pearson, associate medical director at the British Heart Foundation, said: &#8220;Hypertension is common, but the genes controlling blood pressure are not well understood.<br />
&#8220;Their identification will help design better treatments for high blood pressure.&#8221;<br />
But he also stressed more research was needed before it became clear whether the study had identified a potential target for new treatments.<br />

He peeps through the window and wonders who in the neighborhood could have suddenly become sick. To his astonishment, he sees the paramedics rush towards him. He opens the door with utter confusion. The doctor informs him that he has to go with them immediately and with much persuasion he agrees. Once reaching the hospital the person suffers a massive heart attack, but he survives it, as the specialists tend to him prepared.</p>
<p>Nanotechnology can do wonders in medicine by saving lives. A small device implanted in a human body can anticipate the physiological turbulences and alert the hospital even before the person becomes a victim of such turbulence in his body. A patient of hypertension or diabetes has to take a pill for each, every day for the rest of life, irrespective of sugar levels in the blood on a given day or the reading of blood pressure. A far better option in such a scenario is an implanted chip that can release the required quantities of medicine to adjust the sugar level in the body and modulate the pressure of blood as needed.</p>
<p>This is just one of the very many facets of what nanotechnology can bring to the table. Space technology, primary education, hospitality, manufacturing, medicine, banking services and many other paradigms of society can be led to the next level by this technology. The ability to manipulate the matter at the atomic level is revolutionary for the advancement of technology. DNA sequences have been a mystery for long and the recent researches have largely been able to decipher these secrets. A concerted and a well-coordinated research of nanotechnology can decode the DNA sequences to a phenomenal extent. Once done, this can pave the way for curing and eliminating most of the life-threatening diseases.</p>
<p>Playing around with the atomic arrangement of molecules can help innovate new types of food grains and exponentially increase their production and this will greatly help reduce the mortality rate across the globe. According to <a title="Jean Ziegler, Former Professor of Sociology at the University of Geneva and the Sorbonne, Paris" href="http://en.wikipedia.org/wiki/Jean_Ziegler" target="_blank">Jean Ziegler</a> more than 36 million people died due to hunger or malnutrition across the world in 2006 alone. Such tragic events can largely be minimized by effective culmination of scientific pursuits.</p>
<p>The epochal uses of nanotechnology are many and varied. With my limited knowledge on the subject I have touched upon a few. This short write-up is a humble tribute to the fiftieth anniversary of the lecture delivered by Richard Feynman at Caltech on nanotechnology on the 29th of December 1959.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>When the Quality and Outcomes Framework was first introduced, I was involved in a lot of work (much of it for pharma companies) that attempted to help GPs meet their targets. Much of this was in hypertension, which made sense in a peverse way as there&#8217;s oodles of money to be made in hypertension work &#8211; for both doctors and pharma companies. </p>
<p>Salutary reading, then, to see this: </p>
<blockquote><p>I seethe with indignation when careless commentators announce blithely that hypertension is the commonest condition seen in general practice, as they often do. It is not. It may be the commonest condition included in the QOF, but it is knocked into a cocked hat by childhood eczema and kicked clean overboard by skin disease as a whole.</p>
<p>A recently published and well received dermatology Health Care Needs Assessment found that skin conditions are the most frequent reason for people to consult their general practitioner with a new problem. Around 24% of the population in England and Wales (12.9 million people) visited their general practitioner with a skin problem in 2006, with the most common reasons being skin infection and eczema. It found also that of the nearly 13 million people presenting to GPs with a skin problem each year in England and Wales, around 6.1% (0.8 million) are referred for specialist advice.</p></blockquote>
<p>The author argues that target setting and financial incentives &#8216;disadvantage huge and clearly definable groups of patients, distort clinical priorities and drain primary care of flexibility&#8217;. Very timely, given that his article is in response to the British Medical Association&#8217;s request that targets be relaxed in order to free up time for swine flu vcaccination. </p>

<p>Obesity is a national epidemic which has created a national crisis. This has nothing to do with looks. This is about the health of our citizens, our communities, our economic state, our children and our future. During the past 20 years there has been a dramatic increase in obesity in the United States. Obesity comes with a myriad of health and lifestyle consequences, and the cost of obesity has risen along with the obesity rates.</p>
<p>Obesity for an adult is defined as a BMI of 30 or more. Overweight for an adult is a BMI of 25 or greater. BMI is calculated by dividing weight in kg by height in meters.</p>
<p><strong><span style="text-decoration:underline;">THE BAD NEWS</span></strong><strong><span style="text-decoration:underline;"> </span></strong></p>
<p><strong>The Rise in Obesity in the US<a href="http://cqfitness.wordpress.com/wp-includes/js/tinymce/plugins/paste/pasteword.htm?ver=327-1235-syntaxhighlighter2.3.5#_edn1"><strong>[i]</strong></a>:</strong></p>
<p>About 66% of US adults are currently overweight or obese! About a third of US adults are obese. In 1998 there were no recorded states with obesity rates greater than 14%.</p>

<p>This includes $61 billion in estimated medical spending and $56 billion in indirect costs. Recent estimates put current health related costs at well over 9% of US health expenditures.</p>

<p><strong>The lifestyle costs of Obesity:</strong></p>

<li>It can be hard to find clothes that fit, and seats are often not designed for larger body sizes.</li>
<li>Everyday activities such as driving, walking, climbing stairs or carrying shopping bags may be uncomfortable or even painful.</li>
<li>Many people with obesity say they feel bad about themselves and suffer from low self-esteem.</li>
<li>Low self esteem can affect social life and work. For example, a person may feel uncomfortable applying for a job because they don&#8217;t think they’ll be good enough to get one or worry they will be judged because of their weight.</li>
</ul>

<p><strong>Research over the last 40 years tells us that overweight kids are at greater risk of becoming obese adults, predisposing them to life-long obesity-related health issues. </strong></p>

<p><strong>Asthma, Diabetes</strong><strong> (</strong>the Centers for Disease Control and Prevention (CDC) estimated that one in three American children born in 2000 will develop diabetes in their lifetime), G<strong>allstones, Heart Disease, High Blood Pressure, Liver Problems, Menstrual Problems</strong><strong>, </strong><strong>Trouble Sleeping</strong>, Metabolic Syndrome (Metabolic syndrome includes: Abnormal lipids, High blood pressure Insulin resistance), Depression and Low Self-Esteem.</p>
<p><strong><span style="text-decoration:underline;">THE GOOD NEWS!</span></strong></p>
<p>The good news is that the health problems (and hence the economic cost) associated with obesity respond well to diet and exercise. Obesity is almost always TOTALLY preventable and curable without drugs or drastic medical intervention. It’s as simple as that (or is it?).</p>
<p><span style="text-decoration:underline;">THE CHALLENGE</span></p>
<p>Obesity is most often the result of lifestyle and dietary choices. Habits are hard to change, especially when our whole social structure is built around them. Food addictions are as real and can be more challenging to deal with than drug addictions. <p>&#8216;The researcher found that the circadian clock controls a gene that determines levels of a hormone that, when produced in excess, causes <strong>water retention in the kidneys, and thus high blood pressure. </strong>&#8220;Hypertension is common,&#8221; an expert says, &#8220;but the genes controlling blood pressure are not well understood. As people age, this situation can get worse. Nearly half of all older Americans have hypertension. This disease makes people five times more prone to strokes, three times more likely to have a heart attack, and two to three times more likely to experience heart failure.</p>
<p>The problem with this disease is that nearly one third of the folks who have hypertension do not know it because they never feel any direct pain. But overtime the force of that pressure damages the inside surface of your blood vessels.</p>
<p>However, according to experts, hypertension is not predestined. Reducing salt intake, adopting a desirable dietary pattern losing weight and exercising can all help prevent hypertension.</p>
<p>Obviously, quitting bad habits and eating a low fat diet will help, but the most significant part that you can do is to exercise. And just as exercise strengthens and improves limb muscles, it also enhances the health of the heart muscles.</p>
<p><strong><em>Heart and Exercise</em></strong></p>
<p>The exercise stimulates the development of new connections between the impaired and the nearly normal blood vessels, so people who exercise had a better blood supply to all the muscle tissue of the heart.</p>
<p>The human heart basically, supply blood to an area of the heart damaged in a “myocardial infarction.” A heart attack is a condition, in which, the myocardium or the heart muscle does not get enough oxygen and other nutrients and so it begins to die.</p>
<p>For this reason and after a series of careful considerations, some researchers have observed that exercise can stimulate the development of these life saving detours in the heart. One study further showed that moderate exercise several times a week is more effective in building up these auxiliary pathways than extremely vigorous exercise done twice as often.</p>
<p>Such information has led some people to think of exercise as a panacea for heart disorders, a fail-safe protection against hypertension or death. That is not so. Even marathon runners that have suffered hypertension, and exercise cannot overcome combination of other risk factor.</p>
<p><em><strong>What Causes Hypertension?</strong></em></p>
<p>Sometimes abnormalities of the kidney are responsible. There is also a study wherein the researchers identified more common contributing factors such as heredity, obesity, and lack of physical activity. And so, what can be done to lower blood pressure and avoid the risk of developing hypertension? Again, exercise seems to be just what the doctor might order.</p>
<p>If you think that is what he will do, then, try to contemplate on this list and find some ways how you can incorporate these things into your lifestyle and start to live a life free from the possibilities of developing hypertension. But before you start following the systematic instructions, it would be better to review them first before getting into action.</p>
<p>1. See your doctor<br />
Check with your doctor before beginning an exercise program. If you make any significant changes in your level of physical activity — particularly if those changes could make large and sudden demands on your circulatory system — check with your doctors again.</p>
<p>2. Take it slow</p>
<p>Start at a low, comfortable level of exertion and progress gradually. The program is designed in two stages to allow for a progressive increase in activity.</p>
<p>3. Know your limit</p>
<p>Determine your safety limit for exertion. Use some clues such as sleep problems or fatigue the day after a workout to check on whether you are overdoing it. Once identified, stay within it. Over-exercising is both dangerous and unnecessary.</p>
<p>4. Exercise regularly</p>
<p>You need to work out a minimum of three times a week and a maximum of five times a week to get the most benefit. Once you are in peak condition, a single workout a week can maintain the muscular benefits. However, cardiovascular fitness requires more frequent activity.</p>
<p>5. Exercise at a rate within your capacity</p>
<p>The optimum benefits for older exercisers are produced by exercise at 40% to 60% of capacity.</p>
<p>Indeed, weight loss through exercise is an excellent starting point if you wan tot prevent hypertension. Experts say that being overweight is linked to an increased risk of developing hypertension, and losing weight decreases the risk.</p>

<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">In general, after 50 years of hormone contraception since we were students,  with more than a hundred million women  now on low-dose hormone contraception, such synthetics – a progestin with or without ethinylestradiol EE2 &#8211; are  <a href="http://en.wikipedia.org/wiki/Combined_oral_contraceptive_pill#Side-effects" target="_blank">considered pretty safe</a> in current low dose </span></span><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>compared to unplanned pregnancy or physical contraceptive methods</em></span></span><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">, provided contra-indications are respected; with almost 50% reduction in future endometrial  and ovarian cancer; and no  change in mortality from breast cancer. </span></span></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">But in <a href="http://www.societyendo.org/endocrinologist/080/080.pdf#page=15" target="_blank">A Short History of Oral Contraception</a> (2006), Prof Wolfgang Oelkers notes that Ethinylestradiol EE2 was only discovered in 1938 and the first highly active progestin in 1954, leading to the registration of the first oral contraceptive OC pill in USA in 1961-  and within 10years the occurrence of malignant hypertension on such OC pill, with acute and often irreversible renal failure due to the EE2 activating both the renin-angiotensin syndrome and thrombosis. (I had the misfortune to have to put on chronic dialysis a beautiful young university student who presented on such OC pill in 1974 with thrombotic thrombocytopenic purpura with irreversible renal failure, while I was working at the Leeds General Infirmary under the dialysis pioneer Dr Frank M Parsons).<br />
</span></span></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><a href="http://www.endocrinology.org/sfe/endocrinologist/end08015.pdf" target="_blank">Oelkers </a> notes that even ultralow dose combinations of EE2 and 3rd-generation progestins can still cause thrombosis; and <em>&#8220;</em></span></span><em>The thromboembolic risk associated with the recently introduced transdermal combined  contraceptive Evra®does not seem to be any better than modern oral contraceptives, since it also uses EE. This, unlike natural oestradiol, seems to affect hepatic protein synthesis  independent of its route of application.    In postmenopausal hormone replacement therapy, the transdermal route of oestradiol application seems to be devoid of a prothrombotic risk.  Development of a transdermal combined hormonal contraceptive with oestradiol instead of EE would provide a great next  chapter in this endocrine story. </em><a href="http://en.wikipedia.org/wiki/Contraceptive_patch" target="_blank"></a></p>
<p><a href="http://en.wikipedia.org/wiki/Contraceptive_patch" target="_blank"> Wiki reports that <em> </em></a><em> The Ortho Evra contraceptive patch and the Evra contraceptive patch are both intended to gradually release into the systemic circulation approximately 20 µg/day of ethinyl estradiol and 150 µg/day of norelgestromin. </em>Since such hormones bypass hepatic metabolism, no wonder the doses delivered still have thrombotic risks. <em></em></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">Does that make such  massively profitable contraceptive hormones that were designed in fierce competition for fertility (and ovarian) suppression in healthy young women both safe and effective for the ageing no-longer-healthy fattening (post)menopausal women? Parenteral physiological balanced HRT does not cause the fattening and muscle loss that OHT does. </span></span></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">Now young Janey asks an important question: </span></span><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>“I am 56 yrs old and very happy with Yaz (ethinyl estradiol EE2 plus drospirenone). I do menstruate while on the   placebo. I would like to find a doctor who will let me stay on Yaz, which I  like a lot, or if absolutely necessary try the Testosterone, Bi-estrogen,  Progesterone combo. I worry about weight gain that occurs in almost all  cases of oral HRT. I need the bone protection and hair, skin and vagina  health benefits that have been wonderful on Yaz. “</em></span></span></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">CANCER RISK OF ORAL XENOHORMONE THERAPY:</span></span></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">In July this column last visited <a href="http://healthspanlife.wordpress.com/2009/07/17/appropriate-postmenopausal-hormone-therapy-hrt-is-essential-but-why-use-oral-ht-when-especially-long-term-progestins-greatly-increase-risk-of-ovarian-as-well-as-breast-cancer/" target="_blank">ovarian cancer </a> as a relatively rare disease but with high (70%) mortality due to its late presentation, hence </span></span><span style="font-family:Arial,sans-serif;"></span><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">feared more  than other womens&#8217; cancers eg breast, endometrial or cervix.</span></span></p>
<p><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">So it is worth revisiting the <a href="//" target="_blank">major Danish </a>observational study spanning 10 years published last July ;   which documented almost a million women for   8 years. The crude primary ovarian cancer OvCa incidence rate in never-users (5 million women-years) was only 0.04%,  vs  0.052% in current  HT users  (1.4million women-years) ie overall, </span></span><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>hormone therapy </em></span></span><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">HT increased the risk by 1.3, or 30%. That study concluded: “</span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">Combined therapy with norethisterone was associated with an increased risk of epithelial ovarian cancer (RR, 1.55; 95% CI, 1.36-1.76), which was not significantly  different from the RRs associated  with medroxyprogesterone, levo- norgestrel, or cyproterone acetate CPA”. </span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">The only  regime in that series which was not statistically significantly  associated with increase in OvCa was in the  23 women who developed OvCa  on solo transdermal E2TD  ie </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>hormone replacement HRT </em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">(out of a total of 64000 women-years on E2TD), where OvCa relative  risk  increased by 13% but the  95% confidence interval spanned unity (0.74 – 1.71) ie not significant .  By contrast, <em>oral </em>estrogen HT  for some 287 000women years  increased  OvCa risk significantly by  34%;  and </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>any progestin </em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">added to estrogen  ie in some 847 000 women years  increased OvCa risk above no HT by 47% to 68%.</span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">This neutral effect on OvCa  only of unopposed E2TD is most reassuring for women. </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>All the synthetic progestins they compared &#8211; including the antiandrogen CPA- were associated with significantly more OvCa (let alone BRCA).</em></span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">The nub of the matter is that  gynecological cancers generally do not apparently proliferate without   the influence of female cyclical  hormone levels- FSH, LH, estrogen &#8211;  and especially <em>oral estrogens and progestins.</em> In <a href="http://www.ncbi.nlm.nih.gov/pubmed/18282803" target="_blank">the main study </a>of cancer with Turner syndrome,  in  3425 women </span></span></span><a href="http://www.ncbi.nlm.nih.gov/pubmed/18282803"></a><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"> in UK followed for some 17years ie 58000 patient-years, </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>breast cancer was 70% less common than average women,</em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"> while the only gynecolological cancers that appeared to increase were endometrial cancer 8fold at age 15-44years, and gonadoblastoma of the ovary by 8% by age 25years- and <a href="http://en.wikipedia.org/wiki/Gonadoblastoma" target="_blank">gonadoblastoma</a> is over 90% associated with the &#8216;male&#8217; chromosome Y </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"> .  These statistics are reassuring considering that most such women were treated with oral estrogen therapy, and that uterine cancer is avoidable if estrogen therapy is appropriately opposed with some progestin, with periodic withdrawal bleeding allowed. </span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">Unbalanced anabolics eg vitamins or sex hormones merely promote dormant malignant cells already present, they do not cause cancer de novo; and adult cancers take an average of 20 years to present  clinically. </span></span></span></p>

<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">It is common cause from clinical menopause  practice and trials that pharmacological ie unphysiological  oral estrogen – progestin  therapy – while improving bone density- increases body fat and if anything decreases  lean ie muscle mass and collagen -hence the increasing postmenopausal fatness frailty and urinary incontinence of elderly women on oral HT. </span></span></span></p>

<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">The new combinations with  drospirenone for contraception ( Yaz/Yasmin- drospirenone DSP -ethinylestradiol  EE2 in fertile women), and post menopause (Angeliq &#8211; DSP -estradiol E2)  certainly seems to reduce fluid retention and thus weight and hypertension problems, and  to have anti-androgenic benefits when required. There is apparently no published longterm data on DSP to judge it&#8217;s influence on cancer and mortality.</span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">This column has regularly detailed reasons for postmenopausal women PMW to avoid oral  transhepatic  sexhormone therapy with the high doses of oral estrogen needed to control menopause symptoms, and the multiple adverse effects of  transhepatic xenohormones like EE2, premarin and progestins. But 50 years of experience including the under 60&#8217;s cohort of the WHI, and the Oulu trial (<a href="http://www.ncbi.nlm.nih.gov/pubmed/16953985?" target="_blank">Heikkinen ea</a> </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">)  certainly showed overwhelming benefit of oral estradiol/conjugated estrogen-progestin combination when started appropriately in well young PMW for up to 10 years. It has been well known for  three decades that continuing such OHT well beyond 10 years gradually increases the incidence of BRCA above non-users.</span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">However, as we have repeatedly discussed, why should Kitty subject  herself  to the longterm risks of eg breast cancer from such oral use of any designer hormones like orohepatic  estrogens and progestins? when evidence is that physiological human HRT with non-oral, or oral micronized (see Dr Lee Vliet&#8217;s books)   sexhormones, has no risks, only benefits &#8211; especially when human E2+- estriol E3 are balanced by progesterone  P4 and testosterone TT as by creams, or implants, or tiny subcutaneous self-injection, all easily available by prescription in US. </span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">And when oral EE2 ethinylestradiol for contraception  is associated with low but real thrombosis and biliary risks; and when it is enormously potent compared to human estradiol; and when it&#8217;s successor competitor diethylstilbestrol  DES is still causing horrendous problems in women and their children and grandchildren  after it was recklessly prescribed  from the 1940s to the 1970 without there ever being evidence of benefit let alone safety.</span></span></span></p>
<p><span style="color:#292526;"> <span style="font-family:Arial,sans-serif;"><span style="font-size:small;">Lowdose EE2 has certainly proved it&#8217;s relative safety when used as birth control in young healthy non-overweight women. But just as oral prempro has proved that it  causes problems and little benefit when </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>started </em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">after the age of 60years in overweight women ie those already with atheromatous disease, why take potent synthetic oral EE2 post menopause? Using a potent synthetic  is neither prudent, physiological nor </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>replacement. </em></span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">Recently a <a href="http://cat.inist.fr/?aModele=afficheN&#38;cpsidt=21824518" target="_blank">Brazilian trial </a>confirms equal benefit of</span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em> “nonoral HRT (nasal spray- estradiol -micronized vaginal progesterone) ;  or oral HT (low-dose estradiol-drospirenone )  for 2 years on metabolic, vascular and body fat risks in early postmenopause;</em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em><strong>” </strong></em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"> but once again that </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em>“</em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><em><strong>Triglycerides and von Willebrand factor levels decreased significantly  only with nonoral treatmen</strong></em></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"><strong>t”</strong></span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">- ie the nonoral- parenteral- route is better protection against atheroma and thrombogenesis.</span></span></span></p>

But since no longterm trials or studies of DSP use are available yet, it is too early to judge if DSP+E2  is as safe as physiological HRT with appropriate combination of E2/ estriol E3/ P4/ TT. </span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">But as we have repeatedly pointed out, the evidence from both evolution, and 60 years of experience, and trials, is that physiological parenteral human hormones ie not by the orohepatic route have distinct safety and physiological benefits, as comprehensively detailed by <a href="http://linkinghub.elsevier.com/retrieve/pii/S0378512208002041" target="_blank">l&#8217;Hermite, Genazzani ea </a>on behalf of the International Menopause Society  recently</span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"> , as well as all the data this column has previously reviewed on the importance of balance non-oral testosterone as part of the  HRT regime. </span></span></span></p>
<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">So the answer for Janey is: be a volunteer guinea-pig if you like, but dont use Yaz (ie EE2)  but rather use  the natural  E2 -containing Angeliq &#8211; in low dose. Since all other synthetic progestins increase ovarian cancer in women, rather use natural progesterone parenterally until Schering publishes a controlled trial </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">showing </span></span></span><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;"> that drospirenone does not increase ovarian cancer when fed to reproductive age female rodents (let alone women) for the human equivalent of a decade or more. There are no human studies reporting drospirenone use for a decade or more.<br />

<p><span style="color:#292526;"><span style="font-family:Arial,sans-serif;"><span style="font-size:small;">But   preferably enroll yourself in a longterm randomized comparative trial of the new (eg Angeliq)  versus the old. &#8211; balanced appropriate estradiol-estriol-progesterone-testosterone, whether as a cream or depot injection-  titrated to what suits you.  Why risk the new but long-term unproven when the evidence for the old  (as long as human evolution with balanced non-oral human estrogen +P4+TT) is so strong. </span></span></span></p>
