<title><![CDATA[Retail Gimmicks More Fine-Tuned for Black Friday (AAPL, AMZN, WMT, BBY, COST, TGT, KSS, M,  JWN)]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p><img class="alignleft size-full wp-image-54433" title="money-stack-image" src="http://247wallst.wordpress.com/files/2009/11/money-stack-image2.jpg" alt="" width="99" height="99" />Black Friday is under 48 hours away.  We have seen much data and much analysis out there on the topic, but there are two fairly easy conclusions here&#8230;. well, make it three.  First is that consumers are going to get deals galore.  The add-in third notion, or the second, is that inventories at stores will be very low and many items may have to be bought online (with free or low-priced shipping to boot).  But the big conclusion here is that it seems a foregone conclusion that the great deals and (quasi-) price matching and free shipping offered by retailers are still likely to create margin pressure for the retailers even if they have strong sales.</p>
<p>This is a sampling review of some of the promotions from major retailers.  No particular order has been given.  Apple, Inc. (NASDAQ: AAPL) and Amazon.com Inc. (NASDAQ: AMZN) are worth noting, but the major retailers featured are Wal-Mart Stores Inc. (NYSE: WMT), Best Buy Co. (NYSE: BBY), Costco Wholesale Corporation (NASDAQ: COST), Target Corp. (NYSE: TGT), Kohl’s Corp. (NYSE: KSS), Macy’s, Inc. (NYSE: M), and Nordstrom Inc. (NYSE: JWN).<br />

Yesterday my inbox had a very un-Apple Apple Inc. (NASDAQ: AAPL) advertisement.  While most companies and retailers are getting more fine-tuned, Apple was very vague and the exception. It was nothing short of people drinking on the job and trying to come up with a tease.  They talked about the big sale for Apple&#8230; Yet there was nothing in it as a holiday shopping event.  Apple isn&#8217;t going to lose customers for its trick, but it was a lame attempt to lure you into the online store and its retail stores.  I would almost have rather them called me  a loser for not buying anything from them yet.</p>
<p>Wal-Mart Stores Inc. (NYSE: WMT) is showcasing &#8220;5 AM to 11 AM in-store specials&#8221; for Friday only that will vary from around the country.  Mine  is offering a 32&#8243; Emerson LCD HDTV for $248, a 42&#8243; Emerson Plasma HDTV for $448, and a Sony Bravia 46&#8243; LCD HDTV for $798 in TVs.  Wal-Mart already started the toy blitz to keep parents from going to Toys R Us.  Online it has Nintendo Wii 2-for $30 game bundles or a Wii value bundle for $269.  The cheap decorative Christmas tree is the Holiday Time Unlit 6.5&#8242; Jackson Spruce Artificial Christmas Tree for $40.00 and $0.97 shipping. Amazon.com Inc. (NASDAQ: AMZN) is of course a competitor to EVERYONE, but the real war now looks to be Amazon vs. Wal-Mart for Joe Public&#8230; and that means you as a consumer will win.</p>
<p>Best Buy Co. (NYSE: BBY) must still be flooded with TV’s. It  knows cheap TV&#8217;s are a way to  give you a TV and sell you game systems, recorders, accessories, and of course a wall-mount and installation.  Its &#8216;4 Days Only&#8217; special has a cheap laptop of Gateway NV5332u Laptop Package with Wireless Mouse &#38; Laptop Case for $429.97 and the cheap netbook in the store is the Gateway LT2005u netbook with 10.1&#8243; LED-backlit widescreen for only $229.99. Best Buy&#8217;s unadvertised Black Friday Panasonic HDTV price is $797.99 for a TC-P50X1 50&#8243; Class 720p plasma HDTV with free shipping.  Its 32&#8243; Dynex LCD HDTV is $299.99.</p>
<p>Costco Wholesale Corporation (NASDAQ: COST) is showing more specials now compared to last week on our first look.  It has a Proscan 40&#8243; 1080p LCD HDTV $499.99 After $100 off.  Its hot buy netbook is the Acer Aspire One 10.1&#8243; Netbook with an Intel Atom processor for $229.97.  They are advertising a 40&#8243; LCD Samsung TV for $649.99 after $200 off and a hot buy Proscan 26&#8243; LCD HDTV for $279.97.  Can you use the food stamps for these specials?</p>
<p>Target Corp. (NYSE: TGT) is still offering free shipping for online purchases of $50.00 or more on 100,000 items.  It was offering $3 appliance deals for toasters, coffee makers, and slow roasters.  Its 5 AM 2-DAY SALE shows some features as Nikon digital cameras for $88, TomTom GPS Nav for $97 (save over $150) and $246 for a 32&#8243; Westinghouse LCD HDTV.</p>
<p>Kohl’s Corp. (NYSE: KSS) has a 1-Day Pre-Thanksgiving sale for 10% to 25% off of selected kitchen electronics.  More importantly, it is offering $10 KOHL&#8217;S Cash for every $50 you spend now through November 29.  That Kohls&#8217; Cash earned online will arrive by email by December 1.40% to 60% off selected diamond &#38; Gemstone jewelry. Its Friday shopping is actually at 4 AM local time this Friday, so you might wonder how happy those sales people ar at 4:15 when you are asking them 20 questions.</p>
<p>Macy’s, Inc. (NYSE: M) is offering Thanksgiving Sale free shipping on $99 purchases sitewide (online).  Macy’s is also said to be offering in-store savings pass for the Thanksgiving Sale of 25% to 50% on many select items, and an extra 20% or 15% off if you use your Macy;s card or a WOW! pass (again, select items).  Nordstrom Inc. (NYSE: JWN) has effectively matched Macy’s with free shipping on orders of $100 or more and has its myriad of sale items.</p>
<p>What is interesting is that free shipping is still available but seems less advertised than it was last week.  There is also less focus on price matching, although this is still the policy of Wal-Mart, and Target has a policy for this that unfortunately seems to be changing or reacting to what has been seen elsewhere.</p>
<p>The deals are still plentiful for consumers this year.  It still seems like there will be margin pressure on many of the top retailers.  But for whatever it is worth on an opinion, price matching and free shipping seem less advertised or less featured online than just a week ago.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p style="text-align:left;">Trots att det gått lite mer än 15 år sedan min morfar gick bort och 7 ½ år sedan min mormor gick bort känner jag än idag en otroligt stor saknad. När jag var liten tillbringade jag den mesta tiden med att vara hemma hos mina morföräldrar och på somrarna bodde jag hos dem. Det blev även många resor över till Finland som var deras hemland. Min morfar och jag hade så underbart roligt tillsammans och varje lördag var det bastu som gällde och tipsextra på tv. Min mormor bjöd mig alltid på glass och jag kommer ihåg en gång då hon fyllde deras frys med glass så att jag inte skulle vara utan. På vintrarna åkte vi iväg antingen till Vänern eller Vättern och åkte skridskor. Det var min mormor som lärde mig att simma när jag var runt 5-6 år och plaskade runt bland klipporna vid Askerviks camping utmed Vänern. Det är väldigt många glada och trevliga minnen jag har från mina år med mina morföräldrar.</p>
<p style="text-align:left;">Men den 1 september 1994 slogs min värld i spillror då min morfar gick bort i cancer. Jag glömmer aldrig den dagen. Jag jobbade då på Discos skivor och när telefonen ringde och chefen svarade kände jag direkt att det var om min underbara morfar. Jag tog luren och det var min mamma som sa att jag skulle ta mig till KSS för att morfar hade gått bort. Just då hann jag inte bryta ihop för jag blev så stressad att ta mig till KSS så fort som möjligt och när jag kom fram och upp till avdelningen och såg min familj orkade inte mina ben bära mig och jag bröt ihop fullständigt. De kommande veckorna efter morfars bortgång var riktigt jobbiga och att orka jobba det gick inte, men min chef var riktigt snäll och gav mig extra ledigt, för han visste om hur nära relation min morfar och jag hade.</p>
<p style="text-align:left;">Söndagen den 12 maj 2002 var dagen då min kära mormor gick bort till följd av en hjärtattack. Allting gick väldigt fort när det gäller mormor. Dagen innan hon gick bort hade hon hållit på och planterat blommor och på natten fick hon hjärtattacken och iltransport med ambulans till KSS. Själv hade jag varit ute och festat under lördagen och kom hem rätt sent på natten. Jag hann aldrig gå och lägga mig förrän jag fick ett samtal att mormor gått bort. Mamma och jag tog en taxi till KSS och jag har aldrig nyktrat till så fort. När vi kom upp till avdelningen där vi mötte upp de familjemedlemmar som kommit in tidigare leddes vi in till ett mindre rum av en sjuksköterska. Där talade hon om att allt hade gått väldigt fort när mormor gick bort och när jag hörde de orden så vek sig benen på mig och sjuksköterskan hann få tag i mig innan jag slog i golvet. Jag kunde inte fatta att min underbara lilla mormor var borta. Allt var så sjukt. Ena stunden var hon där och nästa borta. Min mamma tog med mig ut där vi väntade in min ena moster från Stockholm och när hon kom till KSS brast allt ännu en gång. Fy fan vad jobbigt allt kändes. När vi kom upp till avdelningen igen hade personalen gjort i ordning så att vi fick se mormor en sista gång och ta farväl. Det kändes på ett sätt skönt, men det var jobbigt att veta att man aldrig mer skulle få träffa henne. Chefen på mitt dåvarande jobb fixade så jag fick ledigt två veckor förutom de dagar man får när en närstående går bort.</p>
<p style="text-align:left;">Tiden efter mormor gått bort gick efterhand bra, men så fort jag såg något på tv med sjukhusmiljö kom allt tillbaka och det tog ett bra tag innan jag kunde se nyheter eller program om sjukhus. Även än idag kan jag ibland känna en stor saknad och tårar börjar rinna när jag ser sjukhusmiljöer på tv och blir påmind om när mina morföräldrar gick bort.</p>
<p style="text-align:left;">De senaste dagarna har jag tänkt väldigt mycket på mina morföräldrar. Jag har ingen aning om varför jag tänker så mycket på dem nu, men en anledning kan ju vara att jag fyller 40 på söndag och att jag vill ha dem hos mig på min födelsedag.</p>
<p style="text-align:left;">Det har varit jobbigt att skriva om detta, men jag kände att jag behövde skriva av mig lite. Jag vet att min mormor och morfar har det bra där de är nu. I alla fall intalar jag mig det. Det måste vara så. Jag saknar er så otroligt mycket, men en vacker dag kommer vi ses igen.</p>

<description><![CDATA[We are right at a week away from the highly awaited Black Friday for 2009&#8217;s holiday and Christ]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p><img class="alignleft size-medium wp-image-54018" title="Bull market xmas tree" src="http://247wallst.wordpress.com/files/2009/11/bull-market-xmas-tree.jpg?w=200" alt="" width="126" height="130" />We are right at a week away from the highly awaited Black Friday for 2009&#8217;s holiday and Christmas season.  As you likely know, this is <em>THE</em> day that retailers look forward to all year and critically depend upon as an anchor to how each retailer&#8217;s full year earnings results turn out.  You may already be tired of Christmas ads and the holidays haven&#8217;t even arrived yet.  With over 10% unemployment, a recession-end that doesn&#8217;t feel like a recession-end, a very tight discretionary spending budget, and a general lack of consumer confidence, it is no surprise at all that the focus for the Holiday Season in 2009 is one of deals and thrift.</p>
<p>These are not in any particular order, but the promotions have been reviewed at Wal-Mart Stores Inc. (NYSE: WMT), Best Buy Co. (NYSE: BBY), Costco Wholesale Corporation (NASDAQ: COST), Target Corp. (NYSE: TGT), Kohl&#8217;s Corp. (NYSE: KSS), Gap Inc. (NYSE: GPS), Macy&#8217;s, Inc. (NYSE: M), and Nordstrom Inc. (NYSE: JWN).  There is also already promotion between MasterCard Incorporated (NYSE: MA) and Amazon.com Inc. (NASDAQ: AMZN).  Admittedly, this is just a sampling of major outlets.</p>
<p>What is amazing is just how much of the deal-making is already out before the holiday season starts as retailers key off of each other.  It is almost impossible to avoid thinking how such a promotional Christmas and Holiday Season in 2009 is going to add pressure to margins at almost all the first-line retailers.<br />

(NYSE: WMT) is offering many shipments of electronics for an add-on price of a mere $0.97 for many electronics items.  It is also hard to not notice the Holiday Time Pre-Lit 6.5&#8242; White Colorado Pine Artificial Christmas Tree with multi-color lights for a whopping $35.00&#8230;. or if you are really pinched and already have a bunch of lights you can get the Holiday Time Unlit 6&#8242; Welsley Pine Artificial Christmas Tree for $20.00.  As always, you can expect that Wal-Mart will be using those toys as a loss-leader to get people in to keep Toys-R-Us at bay.  And a $248 Emerson 32&#8243; LCD HDTV.</p>

(NYSE: BBY) must be just absolutely flooded with TV&#8217;s&#8230; that or it knows this is a hell of a way to give you a TV and sell you game systems, recorders, accessories, and of course a wall-mount and installation&#8230;. 32&#8243; class flat panel HDTV&#8217;s starting at $299 and 40&#8243;&#8221; class  flat panel HDTV&#8217;s starting at $499&#8230;. And if you just have to have a netbook, you can buy an entry-level Asus -Eee PC netbook with an Intel Celeron processor for $249.99.</p>

<p>Macy&#8217;s, Inc. <p>As this is still a week out, it is possible that even more promotional ads and promotions will come on that have not even yet to be decided.  Throw in regular price-matching wars with Wal-Mart and Target (and others) and this is going to be one of the biggest cut-throat holiday season for retailers we have seen in years and years.</p>
<p>What is obvious is that you never know how the top-line revenue figures will come in for the holidays until after Black Friday and until we get into the weekend figures after Thanksgiving.  But what is also obvious is that retail giants have just boxed themselves into a very low margin holiday season.  It is no wonder that almost all retail executives have been very shy and conservative about predicting the outcome of the holiday quarter ahead. Considering that this is the quarter retailers are supposed to thrive and post significant earnings, that only adds caution regardless of how well the public turns out to buy things this year.</p>
<p>The good news is that the retail environment, both brick &#38; mortar and online, are going to be heavily promotional.  That is great for you and me as consumers who likely have to buy gifts this holiday season.  The flip side here is that the heavy promotion and price-sensitivity of both retailers and consumers is going to put a real squeeze on margins this Holiday Season for just about all of the first line retailers.  Barring a pre-Christmas stimulus or barring a sudden change in consumer sentiment that doesn&#8217;t appear to be coming, it seems that the run-up seen in many key retailer stock prices is going to be exponentially more difficult to see a repeat near-term until many share prices are more reasonable.</p>

<title><![CDATA[Macy's Highlights Value-Performance Conundrum in Dept. Stores (M, JWN, KSS)]]></title>

(NYSE: M) is showing just how much of a conundrum there is between today&#8217;s retail stock valuations and the reality of what to expect from earnings ahead.  Macy&#8217;s had a narrower loss in the last quarter of $35 million, or -$0.08 EPS; but outside of items its earnings were -$0.03 EPS.  Revenues were down over 3% to $5.28 billion. Thomson Reuters had estimates at -$0.07 EPS and $5.25 billion in revenues.    The company did manage solid inventory management controls and its ongoing localized merchandise by region was part of why it raised its sales and earnings guidance for the year.</p>
<p>Still, there are issues between today&#8217;s share price versus valuations and where it came up from.  This also highlights issues that lie ahead for Nordstrom Inc. (NYSE: JWN) and Kohl&#8217;s Corp. (NYSE: KSS) ahead of their earnings this week.<br />

Macy&#8217;s online sales were up 21.1% in Q3 and up 15.6% so far this year.  Same-Store sales were off by about 7.5%.  Macy;s is now targeting a drop of 1% to 2% in same store sales during the crucial fourth quarter, its holiday quarter that makes or breaks the year.  Previously, we had been told to expect a drop of 5%.  That full-year same store sales figure was also narrowed to a range of -5.4% to -5.7% from a prior drop expected at 6% to 8%.  Macy&#8217;s expects full-year sales at stores open at least a year to fall between 5.4 percent and 5.7 percent, better than the original forecast of a 6 to 8 percent decline.</p>
<p>There is a problem in this same store sales data.  This shows that the falling off the proverbial cliff seen in the fourth quarter in 2008 is being more heavily influenced this year by a higher unemployment rate.  It also shows that the consumer is going to be very tight-fisted over Christmas.</p>
<p>The full-year guidance was raised to a range of $1.01 to $1.06 EPS.  This is much better than the guidance offered before of $0.70 to $0.80 EPS, but the consensus estimate from Thomson Reuters was already way ahead of the company at a level of $1.11 EPS.</p>
<p>So what about valuations?  Retail stocks were crushed harder in the Q4 period of 2008 than they were in the February and March death spiral seen in financial stocks.  Much of whether you like the valuations of retail stocks depends ultimately on what you feel is a fair normalized earnings capacity for a 2009 to 2010 period.  If Macy&#8217;s makes its bottom-line figure of $1.06, then even after a 6% drop to $18.20 today it trades at about 17.1-times this year&#8217;s expected earnings.  If it can meet the January-2011 fiscal expectations of $1.44 EPS then it trades at 12.6-times forward earnings.  So a 2009 to 2010 (calendar) blended earnings multiple is close to 15-times normalized earnings.  That is not expensive but it is also far from cheap.</p>
<p>Macy&#8217;s has more than tripled off of its 52-week lows from last November.  But shares are also still down by more than half of what they were in 2006 and 2007.  The problem with Macy&#8217;s is not its own metrics.  The issue comes down to one where traders and investors are facing a fork in the road.  The market and shares have recovered handily.  At the workforce and consumer level there are under 90 in 100 workers with a job and under 83 in 100 who are fully employed with little to no prospects for real job opportunities in the future.  And many economists and consumers feel that the economic recovery is only temporary and that there will be a double dip recession.</p>
<p>Nordstrom Inc. (NYSE: JWN) is also on deck this week with earnings due tomorrow.  It faces much of the same issues, except that it has recovered even more from its lows as a stock.  It also trades at higher earnings multiples. Kohl&#8217;s Corp. (NYSE: KSS) is also on deck for Thursday.  While it has not recovered off of lows as much as Macy&#8217;s and Nordstrom, it faces the same valuation issues.</p>
<p>Investors may still buy Macy&#8217;s back up after this morning&#8217;s drop.  But it is going to take lower prices to drag significant new money off the sidelines at these valuations and after the solid run-up we have witnessed since March.  This also may have set the tone for what to expect from the other mall-based department store players.</p>

<p>24/7 Wall St. asked <a href="http://www.corebrand.com/component/option,com_frontpage/Itemid,1/" target="_blank">Corebrand</a>, the brand research and consulting firm, to look at the top 100 brands based their contribution to market capitalizaton. Using this method, the hardest working brand was Hershey (NYSE:HSY), followed  by Coca-Cola (NYSE:KO) and Harley-Davidson (NYSE:HOG)</p>
<p>Corebrand described the process briefly to 24/7 Wall. St.</p>
<p>24/7 Wall St.: Corebard often refers to the brands on this list as the&#8221;hardest working brands&#8221;. How did you come to that description?</p>
<p>Corebrand: There are a lot of people measuring and examining the “strongest brands” or the “most valuable brands”.  Our opinion is that examining one without the other is somewhat meaningless.  How “strong” a brand is nice to know but not very relevant unless you understand how that strength benefits business.  Similarly, “value” is little more than a measure of corporate size unless you understand the drivers of that value and how to influence it. By examining the strength of the brand and it’s contribution to total market value, we can help companies and their leadership manage that strength and value over time.</p>
<p>24/7 Wall St.: Is there any advantage or disadvantage to having a brand value be a very large percentage of market cap in the present and as an indication of a company&#8217;s future performance?</p>
<p>Corebrand: The brand will need to be in balance with the rest of the company’s assets.  A company should strive to have it’s brand strong enough to fend off competitors or changing market conditions but not so strong that it becomes overly dependent on the brand as a single driver of value.  If a company can achieve and maintain its appropriate maximum strength without becoming over-dependent, it will see greater returns in bull markets and retain greater value in bear markets.</p>

<p>Second-quarter profits for the retail<br />
ers have been coming in better than expected, mirroring a trend I&#8217;ve seen with other industries. Nearly 75% of the retailers have topped expectations.*</p>
<p>Though the numbers are better than forecast, they need to be taken in context with the economic backdrop. Most retailers failed to achieve growth, either in terms of same-store sales or profits. The third-quarter could remain challenging, given the consumer confidence numbers. The Conference Board&#8217;s index of consumer confidence stands at just 54.1. Though an improvement from July, it is still a weak reading.</p>
<p>Some Retailers Delivering Real Growth<br />
Not all retailers are struggling, however. In fact, just last week, 2 retailers reported both sales and earnings growth.</p>
<p>Aeropostale (ARO &#8211; Snapshot Report) achieved an impressive 12% increase in same-store sales, which helped lead to a 20% increase in total sales. Earnings of 57 cents per share were a penny above expectations, and compare to 31 cents per share last year. Tops sold particularly well. President Mindy Meads noted on the conference call that &#8220;frankly our customer cannot get enough of our names.&#8221; She observed that just about anything bearing her company&#8217;s name, whether it be &#8220;a T-shirt, a polo or candy,&#8221; is selling well. The teen apparel retailer guided for third-quarter earnings of 76 to 78 cents per share, slightly above the average forecast. Though full-year projections were not provided, nearly all of the covering analysts raised their forecasts. The revisions pushed the Zacks Consensus Estimate 7 cents higher to $3.01 per share.</p>
<p>Ross Stores (ROSS) said total sales rose 8%, driven by a 3% increase in same-store sales. Profits totaled 82 cents per share, versus 54 cents last year. The off-price retailer saw good demand for dresses and shoes. The company raised its full-year earnings guidance to between $3.00 and $3.12 per share; previously, the company had forecast profits of $2.62 to $2.72 per share. Revisions by nearly all of the covering analysts pushed the Zacks Consensus Estimate 28 cents higher to $3.10 per share.</p>
<p>In both cases, gross margins were aided by effective inventory management. Though sales were good, it is notable that neither company had to sacrifice pricing to bring customers in.</p>
<p>I would be remiss if I did not mention Dollar Tree, Inc. (DLTR &#8211; Snapshot Report). This morning, the company reported second-quarter sales of $1.22 billion, an 11.9% increase. Same-store sales rose 6.8%. The company&#8217;s profit of 63 cents of per share was well above both the Zacks Consensus Estimate and year-prior results. Not only did the higher sales help, but DLTR also expanded its gross margin by 130 basis points. Dollar Tree raised its full-year earnings guidance to $3.10 and $3.25 per share. The Zacks Consensus currently calls for $3.02 per share, and is likely to be revised higher over the next several days.</p>
<p>Others Achieving Growth By Expansion<br />
Though ARO and ROST delivered same-store sales growth, the majority of retailers were unable to do so. There were some, however, that were able to offset the decline in same-stores sales with growth elsewhere. Two examples are Dick&#8217;s Sporting Goods (DKS &#8211; Snapshot Report) and Kohl&#8217;s (KSS &#8211; Snapshot Report).</p>
<p>DKS grew net sales by 3.7% thanks to new stores and the addition of e-commerce sales. Same-store sale lagged, however, falling 4.1%. Golf Galaxy sales were particularly weak. Non-GAAP profits were down slightly at 36 cents per share. The drop was a result of lower margins, reflecting clearance activity at Golf Galaxy and pricing promotions at Dick&#8217;s Sporting Goods stores. The company&#8217;s guidance was better than analysts had projected, however. DKS anticipates non-GAAP profits to total between $1.02 and $1.07 per share. Revisions by the majority of the covering analysts have resulted in a new Zacks Consensus Estimate of $1.06 per share.</p>
<p>Kohl&#8217;s operated 65 more stores at the end of the second-quarter than it did a year prior. These new stores helped the company to generate a 1.3% increase in net sales, despite a 3.2% drop in same-store sales. Second-quarter profits were down slightly, however, at 75 cents per share, because of higher operating expenses. Nonetheless, brokerage analysts are more optimistic about the remainder of the year. Their revisions have resulted in a full-year Zacks Consensus Estimate of $2.76 per share, 15 cents higher than the average profit forecast of a few weeks ago.</p>

<p>Retailers will start to release second-quarter results this week, accounting for nearly 10% of the 246 scheduled reports. Dow component Wal-Mart (WMT &#8211; Snapshot Report) will be joined by 15 other members of the S&#38;P 500, including J.C. Penney (JCP &#8211; Analyst Report), Kohl&#8217;s (KSS &#8211; Snapshot Report) and Macy&#8217;s (M &#8211; Snapshot Report).</p>
<p>The Fed will hold a 2-day meeting on Tuesday and Wednesday. No change in interest rates will be made, though the statement should acknowledge that the economy is nearing stabilization.</p>
<p>The bond markets could be volatile, not only because of the Fed meeting, but also because of quarterly refunding. The Treasury Department will auction 3-year, 10-year and 30-year bonds. The offerings should total $75 billion.</p>
<p>Most of the week&#8217;s economic data will be released after the Fed&#8217;s meeting, with the exception of the preliminary estimate of Q2 productivity. We&#8217;ll see the latest industrial production and consumer sentiment numbers on Friday.<br />
    * Tuesday: Q2 productivity<br />
    * Wednesday: June wholesale inventories, June trade balance, Fed statement (about 2:10 p.m.), July treasury budget, weekly crude inventories<br />
    * Thursday: July retail sales, June business inventories, July import and export prices, weekly initial jobless claims<br />
    * Friday: July Consumer Price Index (CPI), July industrial production and capacity utilization, preliminary August University of Michigan consumer confidence</p>
<p>No Fed officials are scheduled to speak.</p>
<p>As I write this on Friday morning, the S&#38;P 500 has broken above resistance. Though the trend still favors the bulls, the major indexes are overbought. Furthermore, the tailwind from second-quarter earnings is disappearing. I think a pullback will occur soon, though I do feel a bit like a guy who is trying to sell umbrellas in the midst of a drought.</p>
<p>Companies That Could Issue Positive Earnings Surprises<br />
One of the 10 covering brokerage analysts on Cree, Inc. (CREE &#8211; Analyst Report) recently raised his fiscal fourth-quarter forecast. Though the change was not enough to move the Zacks Consensus Estimate from its current level of 11 cents per share, it did result in a more bullish most accurate estimate of 14 cents per share. Furthermore, projections for fiscal 2010 have gradually been rising, suggesting that guidance could be good. CREE has topped expectations twice and matched forecasts twice during the last 4 quarters. Cree is scheduled to report on Tuesday, Aug 11, after the close of trading.</p>
<p>Though J.C. Penney Company (JCP &#8211; Analyst Report) experienced a 12.3% drop in July same-store sales, the decline was less than the company had feared. As a result, JCP raised its adjusted second-quarter profit guidance to a loss of 1 cent per share. The Zacks Consensus Estimate also calls for an adjusted loss of 1 cent per share. Though JCP matched expectations last quarter, it did top forecasts the previous 6 quarters. J.C. Penney is scheduled to report on Friday, Aug 14, before the start of trading.</p>
<p>Kohl&#8217;s (KSS &#8211; Snapshot Report) just raised its second-quarter guidance. Crediting growth in July sales (accessories, home goods and footwear sold well), the company now projects profits of 73 to 74 cents per share. Previously, the department store chain had guided for profits of 56 to 64 cents per share. The Zacks Consensus Estimate has been revised up to 74 cents per share, though there could still be more upside given that KSS has topped expectations for 5 consecutive quarters. Kohl&#8217;s is scheduled to report on Thursday, Aug 13, before the start of trading.</p>
<p>Companies That Could Issue Negative Earnings Surprises<br />
Ethan Allen (ETH &#8211; Snapshot Report) has missed expectations for 3 consecutive quarters. The majority of the covering brokerage analysts are bracing for another disappointing quarter, judging by the 9-cent drop in the average forecast. The Zacks Consensus Estimate is calling for a loss of 25 cents per share. Ethan Allen is scheduled to report on Wednesday, Aug 12, before the start of trading.</p>

<p><strong> </strong>Penyakit kanker kulit dewasa ini cenderung mengalami peningkatan jumlahnya terutama di kawasan Amerika, Australia dan Inggris. Berdasarkan beberapa penelitian, mereka orang-orang kulit putih yang lebih banyak menderita jenis kanker kulit ini. Hal tersebut diprediksikan sebagai akibat seringnya mereka terkena (banyak terpajan) cahaya matahari. Di Indonesia penderita kanker kulit terbilang sangat sedikit dibandingkan ke-3 negara tersebut, namun demikian kanker kulit perlu dipahami karena selain menyebabkan kecacatan (merusak penampilan) juga pada stadium lanjut dapat berakibat fatal bagi penderita<sup>(1).</sup></p>
<p>Kanker ini dapat dilihat secara visual langsung dan dengan mengadakan pemeriksaan biopsy, diagnosis dapat ditegakkan dengan cepat. Oleh karena itu sebelumnya kanker kulit dapat dideteksi secara dini. Tetapi kenyataannya masih banyak pasien datang berobat untuk kanker kulit berada dalam stadium lanjut, disertai kerusakan-kerusakan setempat yang sulit diobati atau dengan anak sebar. Hal ini sangat disayangkan oleh karena kalau dideteksi sedini mungkin dapat segera dilakukan tindakan pengobatan, maka hasilnya akan sangat memuaskan. Oleh karena itu pengetahuan mengenai tanda-tanda dini dari kanker kulit sangat penting, baik untuk pasien, maupun untuk para praktisi dokter dan petugas kesehatan<sup>(2).</sup></p>
<p>Jenis tumor ganas kulit yang banyak ditemukan diseluruh dunia ialah karsinoma sel basal (basalioma), karsinoma sel squamosa, yang tergolong non melanoma dan melanoma maligna. Karsinoma sel basal adalah paling umum. Di Amerika, sekitar 800.000 orang menghidapi kanker ini setiap tahun. 75% kanker kulit adalah kanser sel basal. Karsinoma sel skuamos pula didapati apa 200.000 orang Amerika setiap tahun. Melanoma adalah yang paling jarang dijumpai tetapi menyebabkan paling banyak kematian. Menurut WHO, sebanyak 160.000 orang menghidapi melanoma setiap tahun dan sebanyak 48.000 kematian dilaporkan setiap tahun<sup>(3)</sup>.</p>
<p>Kanker merupakan penyebab kematian yang ke enam di Indonesia, sedangkan pada negara-negara maju merupakan penyebab kematian yang kedua setelah penyakit-penyakit kardiovaskular. Kanker diderita oleh semua golongan masyarakat. Golongan<em> </em>sosial yang ekonominya kurang umumnya berobat pada stadium lanjut, sehingga sangat sukar untuk menyembuhkannya walaupun dengan cara-cara pengobatan yang mutakhir seperti sekarang ini.</p>
<p>Khusus keganasan kulit memang sedikit disinggung di seminar Kanker nasional pertama maupun yang kedua. Akan tetapi semua pihak mengakui bahwa keganasan kulit merupakan 3 besar di antara keganasan gayudara dan leher rahim (serviks). Pada beberapa daerah seperti di Medan malah menduduki tempat teratas<sup>(4)</sup>.</p>

<p><strong> </strong>Tujuan dari penulisan referat ini adalah untuk mengetahui tentang definisi, etiologi, patofisiologi, manifestasi klinis, diagnosis serta penatalaksanaan dari tumor ganas kulit.</p>

<p><strong>II.1 Definisi</strong></p>
<p>Kanker kulit adalah proses keganasan yang timbul dipermukaan kulit dan berasal dari sel epitel, sel pluripotensial atau dari sel melanin di dalam kulit<sup>(2).</sup></p>
<p>Tumor ganas kulit merupakan suatu penyakit yang ditandai dengan pertumbuhan sel-sel kulit yang tidak terkendali, dapat merusak jaringan di sekitarnya dan mampu menyebar ke bagian tubuh yang lain. Karena kulit terdiri atas beberapa jenis sel, maka kanker kulit juga bermacam-macam sesuai dengan jenis sel yang terkena<sup>(5).</sup></p>
<p><strong>II.2 Anatomi dan Fungsi Kulit</strong><strong> </strong></p>
<p>Kulit adalah suatu organ pembungkus seluruh permukaan luar tubuh, merupakan organ terberat dan terbesar dari tubuh. Seluruh kulit beratnya sekitar 16 % berat tubuh, pada orang dewasa sekitar 2,7 – 3,6 kg dan luasnya sekitar 1,5 – 1,9 meter persegi. Tebalnya kulit bervariasi mulai 0,5 mm sampai 6 mm tergantung dari letak, umur dan jenis kelamin. Kulit tipis terletak pada kelopak mata, penis, labium minus dan kulit bagian medial lengan atas. Sedangkan kulit tebal terdapat pada telapak tangan, telapak kaki, punggung, bahu dan bokong<sup>(6)</sup>.</p>
<p>Kulit melindungi tubuh dari trauma dan merupakan benteng pertahanan terhadap bakteri, virus dan jamur. Kehilangan panas dan penyimpanan panas diatur melalui vasodilatasi pembuluh darah kulit atau sekresi kelenjar keringat. Setelah kehilangan seluruh kulit,maka ciran tubuh yang penting akan menguap dan elektrolit-elektrolit yang penting akan menghilang dari tubuh, akan menguap dan lektrolit-elektrolit akan hilang dalam beberapa jam saja. Contoh dari keadaan ini adalah penderita luka bakar. Bau yang sedap atau tidak sedap dari kulit berfungsi sebagai pertanda penerimaan atau penolakan sosial dan seksual. Kulit juga merupakan tempat sensasi raba, tekan, suhu, nyeri dan nikmat berkat jalinan ujung-ujung saraf yang bertautan<sup>(7)</sup>.</p>
<p>Secara embriologis kulit berasal dari dua lapis yang berbeda, lapisan luar adalah epidermis yang merupakan lapisan epitel berasal dari ectoderm sedangkan lapisan dalam yang berasal dari mesoderm adalah dermis atau korium yang merupakan suatu lapisan jaringan ikat<sup>(6)</sup>.</p>
<p>Secara anatomis kulit tersusun atas 3 lapisan pokok terdiri dari : a. lapisan epidermis, b. lapisan dermis, c. subkutis, sedangkan alat-alat tambahan juga terdapat pada kulit antara lain kuku, rambut, kelenjar sebacea, kelenjar apokrin, kelenjar ekrin. Keseluruhan tambahan yang terdapat pada kulit dinamakan appendices atau adnexa kulit<sup>(2)</sup>.</p>
<p><strong>A. EPIDERMIS</strong></p>
<p>Epidermis adalah lapisan luar kulit yang tipis dan avaskuler. Terdiri dari epitel berlapis gepeng bertanduk (keratinosit), mengandung sel melanosit, Langerhans dan merkel. Tebal epidermis berbeda-beda pada berbagai tempat di tubuh, paling tebal pada telapak tangan dan kaki. Ketebalan epidermis hanya sekitar 5 % dari seluruh ketebalan kulit. Terjadi regenerasi setiap 4-6 minggu<sup>(6)</sup>.</p>
<p>Epidermis terdiri atas lima lapisan (dari lapisan yang paling atas sampai yang terdalam) :</p>
<ol>
<li>Stratum      Korneum. Terdiri dari sel keratinosit yang bisa mengelupas dan berganti.</li>
<li>Stratum      Lusidum. Berupa      garis translusen, biasanya terdapat pada kulit tebal telapak kaki dan      telapak tangan. Tidak tampak pada kulit tipis.</li>
<li>Stratum      Granulosum. Ditandai      oleh 3-5 lapis sel polygonal gepeng yang intinya ditengah dan sitoplasma      terisi oleh granula basofilik kasar yang dinamakan granula keratohialin      yang mengandung protein kaya akan histidin. Terdapat sel Langerhans.</li>
<li>Stratum      Spinosum. Terdapat berkas-berkas filament yang dinamakan tonofibril,      dianggap filamen-filamen tersebut memegang peranan penting untuk      mempertahankan kohesi sel dan melindungi terhadap efek abrasi. Epidermis      pada tempat yang terus mengalami gesekan dan tekanan mempunyai stratum      spinosum dengan lebih banyak tonofibril. Stratum basale dan stratum      spinosum disebut sebagai lapisan Malfigi. Terdapat sel Langerhans.</li>
<li>Stratum      Basale (Stratum Germinativum). Terdapat aktifitas mitosis yang hebat dan      bertanggung jawab dalam pembaharuan sel epidermis secara konstan.      Epidermis diperbaharui setiap 28 hari untuk migrasi ke permukaan, hal ini      tergantung letak, usia dan faktor lain. Merupakan satu lapis sel yang      mengandung melanosit.</li>
</ol>
<p>Fungsi Epidermis : Proteksi barier, organisasi sel, sintesis vitamin D dan sitokin, pembelahan dan mobilisasi sel, pigmentasi (melanosit) dan pengenalan alergen (sel Langerhans)<sup>(6)</sup>.</p>
<p><strong>B. DERMIS</strong></p>
<p>Merupakan bagian yang paling penting di kulit yang sering dianggap sebagai “True Skin”. Lapisan dermis ini paling tebal dapat dijumpai di punggung dan paling tipis pada palpebrae. Hubungan antara dermis dan epidermis ini tidaklah sebagai bidang yang rata, tetapi berbentuk gelombang. Bagian dermis yang menonjol ke dalam epidermis dinamakan papilla, sedangkan bagian epidermis yang menonjol ke dermis disebut <em>rete ridge. </em>Papila ini pada telapak tangan dan jari-jari terutama tersusun linier yang member gambaran kulit yang berbeda-beda sebagai dermatoglyphic (sidik jari). Bagian dermis papiler ini tebalnya sekitar seperlima dari tebal dermis total. Bagian bawah dari dermis papiler ini dinamakan dermis retikuler yang mengandung vasa darah dan lymphe, serabut syaraf, adnexa dan lainnya<sup>(7)</sup>.</p>
<p>Dermis ini tersusun dari beberapa unsure atau organ yang meliputi: unsure seluler, unsure fibrous, substansi dasar, pembuluh darah dan limphe, system saraf. Kelima unsure atau organ yang menyusun dermis akan kita bahas satu demi satu.</p>
<ol>
<li>Unsur seluler lebih banyak didapatkan pada stratum papillaris yang terdiri dari:
<ol>
<li>fibroblast: merupakan sel pembentuk unsur untuk fibrous dan substansi dasarnya</li>
<li>Sel mast : merupakan sel pembentuk dan penyimpanan histamine dan histamine like substance yang berperan dalam anafilaksis.</li>
<li>Makrofag : merupakan sel fagosit yang berfungsi memfagosit bahan-bahan asing fan mikroorganisme.</li>
<li>Leukosit : Banyak dijumpai pada proses-proses peradangan yang dapat berupa mononuclear ataupun granulosit.</li>
</ol>
</li>
<li>Unsur fibrous lebih padat pada stratum retikularis dibandingkan pada stratum papilaris. Unsur fibrous terdiri dari :
<ol>
<li>Kolagen : merupakan 70% dari berat kering seluruh jaringan ikat, serabut ini terbentuk oleh fibroblast, tersusun atas fibrin dari rantai polypeptide. Serabut ini bertanggung jawab pada ketegangan kulit merupakan unsure pembentuk garis langer (cleavage line)</li>
<li>Elastin : Hanya 2 % dari berat kering jaringan ikat. Serabut elastin, ini juga dibentuk oleh fibroblast tetapi susunannya lebih halus disbandingkan dengan kolagen. Serabut elastin ini bertanggung jawab atas elastisitas kulit.</li>
<li>Retikulin : Merupakan serabut kolagen yang masih muda dan hanyalah dapat dilihat dengan pewarna khusus.</li>
</ol>
</li>
<li>Substansi dasar, tersusun dari bahan mukopolisakaris (asam hialuronat dan dermatan sulfat), yang juga dibentuk oleh fibroblast. Substansi dasar hanya merupakan 0,1% dari berat kering jaringan ikat, tetapi substansi dasar ini mampu menahan sejumlah air, sehingga akan menempati ruang terbesar dari dermis.</li>
<li> Pembuluh darah dan limfe :</li>
</ol>
<p>Pada kulit yang masih normal, darah yang sampai pada kulit merupakan 10% dari seluruh peredaran darah dalam tubuh. Pembuluh darah di dalam kulit terdiri dari 2 plexus yaitu :</p>
<ol>
<li>Plexus superficialis : terdapat pada bagian atas dermis dan tersusun sejajar dengan epidermis. Plexus superficialis ini terdiri dari atas kepiler-kapiler, <em>endarteriole</em> dan venulae yang member makan ke papilla.</li>
<li>Plexus profunda : Terdapat pada bagian bawah dermis atau dekat subcutis dan terutama terdiri atas pembuluh-pembuluh darah yang lebih besar dari pada plexus superficialis.</li>
</ol>
<p>Pada jari-jari di antara arteriole dan venulae terdapat kelompokan otot polos yang mempunyai fungsi khusus yaitu mengatur shunt <em>arterio-venosa </em>dan sering dinamakan glomus. Sedangkan pembuluh limfe biasanya mengikuti pembuluh darah<sup>(2)</sup>.</p>
<ol>
<li>Sistem saraf</li>
</ol>
<p>Kulit diinervasi oleh kira-kira 1.000.000 serabut saraf aferen. Sebagian besar terdapat pada wajah dan ekstremitas, sedangkan pada punggung relative sedikit. Serabut saraf ini mempunyai akson dengan badan sel yang berada pada <em>dorsal root ganglia </em>. Serabut saraf ini masuk kulit melalui lapisan lemak subkutan, kemudian masing-masing terbagi dua yaitu serabut saraf bermyelin dan serabut saraf tidak bermyelin. Serabut saraf bermyelin berjalan horizontal membentuk anyaman dengan serabut yang sama, kemudian naik ascenden bersama pembuluh darah dan menginervasi dermis bagian superficial. Dalam perjalanan selanjutnya serabut ini dibungkus oleh sel Schwann dan sebagian tidak bermyelin. Sebagian berakhir di dermis, beberapa melakukan penetrasi membrane basalis tetapi tidak jauh melanjut ke epidermis.</p>
<p>Ada 3 macam serabut saraf yag terdapat pada kulit, yaitu :</p>
<ol>
<li>Serabut adrenergic : berfungsi untuk menginervasi pembuluh darah (untuk vasokonstriksi pembuluh darah, m erector papilare (untuk kontraksi otot tersebut), dan kelenjar apokrin (untuk pengatur sekresi kelenjar apokrin.</li>
<li>Serabut kolinergik : berfungsi menginervasi kelenjar ekrin.</li>
<li>Serabut sensorik : berfungsi untuk menerima rangsangan dari luar tubuh. Ada beberapa akhiran serabut saraf sensorik, yaitu : 1. Korpuskulum Meisnerri, 2. Korpuskulum Paccini, 3. Akhiran serabut saraf bebas.</li>
</ol>
<p>Ketiga akhiran serabut sensorik tersebut lebih jauh adalah sebagai berikut :</p>
<ol>
<li>Korpuskulum Meisnerri berfungsi menerima rangsangan sentuhan dan tekanan ringan. Terdapat pada papilla dermis dan paling banyak dapat dijumpai pada telapak tangan dan kaki.</li>
<li>Korpuskulum Paccini berfungsi untuk menerima rangsangan tekanan dalam dan terdapat pada dermis bagian dalam terutama pada bagian-bagian badan yang sering menahan beban berat.</li>
<li>Akhiran saraf rambut bebas berfungsi untuk menerima rangsangan panas, dingin, nyeri, gatal. Akhiran saraf bebas ini terdapat terutama pada papilla dermis dan sekitar folikel rambut.</li>
</ol>
<p>Batas antara epidermis dan dermis dibentuk oleh zone membrane basalis. Dengan menggunakan mikroskop electron, membrane ini dapat dilihat terdiri dari 4 komponen yaitu : membrane sel dari sel basal dengan hemidesmosom, celah intermembranous, lamina basalis, komponen fibrous dermis yang dapat dilihat dengan mikroskop biasa dengan pewarna khusus menggunakan PAS. Zone membrane basalis ini merupakan filter semipermeable yang memungkinkan pertukaran sel dn cairan antara dermis dan epidermis<sup>(2)</sup>.</p>
<p>Fungsi Dermis : struktur penunjang, mechanical strength, suplai nutrisi, menahan shearing forces dan respon inflamasi<sup>(6)</sup>.</p>
<p><strong>C. SUBKUTIS</strong></p>
<p>Merupakan lapisan di bawah dermis atau hipodermis yang terdiri dari lapisan lemak. Lapisan ini terdapat jaringan ikat yang menghubungkan kulit secara longgar dengan jaringan di bawahnya. Jumlah dan ukurannya berbeda-beda menurut daerah di tubuh dan keadaan nutrisi individu. Berfungsi menunjang suplai darah ke dermis untuk regenerasi.</p>
<p>Fungsi Subkutis / hipodermis : melekat ke struktur dasar, isolasi panas, cadangan kalori, kontrol bentuk tubuh dan mechanical shock absorber<sup>(6)</sup>.</p>
<p><strong>VASKULARISASI KULIT</strong></p>
<p>Arteri yang memberi nutrisi pada kulit membentuk pleksus terletak antara lapisan papiler dan retikuler dermis dan selain itu antara dermis dan jaringan subkutis. Cabang kecil meninggalkan pleksus ini memperdarahi papilla dermis, tiap papilla dermis punya satu arteri asenden dan satu cabang vena. Pada epidermis tidak terdapat pembuluh darah tapi mendapat nutrient dari dermis melalui membran epidermis<sup>(6)</sup>.</p>
<p><strong>II.3 Fisiologi Kulit</strong></p>
<p>Kulit merupakan organ yang berfungsi sangat penting bagi tubuh diantaranya adalah memungkinkan bertahan dalam berbagai kondisi lingkungan, sebagai barier infeksi, mengontrol suhu tubuh (termoregulasi), sensasi, eskresi dan metabolisme.</p>
<p>Fungsi proteksi kulit adalah melindungi dari kehilangan cairan dari elektrolit, trauma mekanik, ultraviolet dan sebagai barier dari invasi mikroorganisme patogen. Sensasi telah diketahui merupakan salah satu fungsi kulit dalam merespon rangsang raba karena banyaknya akhiran saraf seperti pada daerah bibir, puting dan ujung jari. Kulit berperan pada pengaturan suhu dan keseimbangan cairan elektrolit. Termoregulasi dikontrol oleh hipothalamus. Temperatur perifer mengalami proses keseimbangan melalui keringat, insessible loss dari kulit, paru-paru dan mukosa bukal. Temperatur kulit dikontrol dengan dilatasi atau kontriksi pembuluh darah kulit. Bila temperatur meningkat terjadi vasodilatasi pembuluh darah, kemudian tubuh akan mengurangi temperatur dengan melepas panas dari kulit dengan cara mengirim sinyal kimia yang dapat meningkatkan aliran darah di kulit. Pada temperatur yang menurun, pembuluh darah kulit akan vasokontriksi yang kemudian akan mempertahankan panas<sup>(6)</sup>.</p>
<p><strong> </strong></p>
<p><strong>II.4 Klasifikasi Tumor Ganas Kulit</strong><strong> </strong></p>
<p>Kulit terdiri atas beberapa jenis sel, maka kanker kulit juga bermacam-macam sesuai dengan jenis sel yang terkena. Akan tetapi yang paling sering terdapat adalah karsinoma sel basal (KSB), karsinoma sel skuamosa (KSS) dan melanoma maligna (MM)<sup>(5)</sup>.</p>
<ol>
<li><strong>A. </strong><strong> Karsinoma Sel Basal</strong></li>
</ol>
<p>Karsinoma sel basal (KSB) merupakan suatu tumor ganas kulit yang paling sering pada manusia. Biasanya mengenai pada daerah yang sering terpajan dengan sinar matahari. KSB ini pertumbuhannya lambat dan jarang metastasis, tapi dapat menyebabkan kerusakan lokal dan kecacatan apabila tidak diobati<sup>(10)</sup>.</p>
<p>KSB adalah suatu tumor ganas kulit (kanker) yang berasal dari pertumbuhan neoplastik sel basal epidermis dan apendiks kulit. Pertumbuhan tumor ini lambat, dengan beberapa macam pola pertumbuhan sehingga memberi gambaran klinis yang berveriasi, bersifat invasive, serta jarang mengadakan metastasis<sup>(8)</sup>.</p>
<ol>
<li><strong>B. </strong><strong>Karsinoma Sel Skuamosa</strong></li>
</ol>
<p>Karsinoma Sel Skuamosa adalah kanker yang berasal dari lapisan tengah <em>epidermis</em>. Penyakit Bowen adalah suatu bentuk karsinoma sel skuamosa yang terbatas pada epidermis dan belum menyusup ke jaringan di bawahnya (<em>dermis</em>).<br />
Kulit yang terkena tampak coklat-merah dan bersisik atau berkeropeng dan mendatar, kadang menyerupai bercak pada <em>psoriasis</em>, <em>dermatitis</em> atau infeksi jamur<sup>(8)</sup>.</p>
<ol>
<li><strong>C. </strong><strong>Melanoma Maligna</strong></li>
</ol>
<p>Melanoma maligna merupakan tumor ganas kulit yang sangat ganas dan berasal dari sistem melanositik kulit. Biasanya menyebabkan metastasis yang sangat luas dalam waktu singkat, tidak saja melalui aliran limfe ke kelenjar regional, tetapi juga menyebar melalui aliran darah ke alat-alat dalam, serta dapat menyebabkan kematian<sup>(8)</sup>.</p>

<p><strong>II.5 Etiologi</strong></p>

<li><strong>A. </strong><strong>Karsinoma Sel Basal</strong><strong> (KSB)</strong><strong> </strong></li>
</ol>
<p>Lebih dari 90% penyebab KSB  terpapar sinar matahari atau penyinaran ultraviolet lainnya. KSB juga bisa ditemukan di kulit kepala. Paling sering muncul pada usia diatas 40 tahun. Faktor resiko lainnya adalah:</p>
<ul>
<li>Faktor genetik (sering terjadi pada kulit terang, mata biru atau hijau dan rambut pirang atau merah).</li>
<li>Pemaparan sinar X yang berlebihan atau penyinaran lainnya<sup>(8)</sup>.<strong> </strong></li>
</ul>
<ol>
<li><strong>B. </strong><strong>Karsinoma Sel Skuamosa</strong></li>
</ol>
<p>Lebih dari 90% kanker kulit tumbuh di daerah yang terpapar oleh sinar matahari atau sinar ultra violet lainnya. Hal ini diduga merupakan penyebab utama dari semua jenis kanker kulit. Faktor resiko lainnya adalah:</p>
<ul>
<li>Faktor genetik (kanker kulit lebih sering ditemukan pada orang berkulit terang, mata biru atau hijau dan rambut pirang atau merah)</li>
<li>Pencemaran oleh bahan kimia</li>
<li>Pemaparan berlebihan oleh sinar X atau radiasi lainnya<sup>(9)</sup>.</li>
</ul>
<ol>
<li><strong>C. </strong><strong>Melanoma Maligna</strong></li>
</ol>
<p>Faktor resiko terjadinya melanoma adalah:</p>
<ul>
<li>Riwayat keluarga yang menderita melanoma</li>
<li>Rambut merah atau pirang</li>
<li>Adanya tahi lalat atipik multipel (tanda lahir)</li>
<li>Terdapat keratosis aktinik pre-kanker</li>
<li><em>Frekels</em> (bintik-bintik coklat) yang sangat jelas di punggung bagian atas</li>
<li>Mengalami serangan lepuhan akibat luka bakar sinar matahari sebanyak 3 kali atau lebih sebelum berusia 20 tahun<sup>(8)</sup>.</li>
</ul>
<p><strong> </strong></p>
<p><strong>II.6 Gejala Klinis</strong></p>
<ol>
<li><strong>A. </strong><strong>Karsinoma Sel Basal</strong></li>
</ol>
<p>Predileksinya terutama pada wajah (pipi, dahi, hidung, lipat nasolabial, daerah periorbital), leher. Meskipun jarang dapat pula di jumpai pada lengan, tangan, badan , tungkai, kaki, dan kulit kepala.</p>
<p>Gambaran klinik KSB bervariasi. KSB terbagi menjadi 5 bentuk :</p>
<ul>
<li>Nodulo-ulseratif, termasuk ulkus rodens.</li>
<li>Berpigmen.</li>
<li>Morfea atau fibrosing atau sklerosing.</li>
<li>superficial.</li>
<li>Fibroepitelioma.</li>
</ul>
<p>Disamping itu terdapat pula 3 sindroma klinis, dimana epitelioma sel basal berperan penting, yaitu :</p>
<ul>
<li>sindroma epitelioma sel basal nevoid.</li>
<li>Nevus sel basal unilateral linier.</li>
<li>sindroma bazex<sup>(8)</sup>.</li>
</ul>
<ol>
<li><strong>B. </strong><strong>Karsinoma Sel Skuamosa</strong></li>
</ol>
<p>Predileksi terjadi pada daerah kulit yang terpapar sinar matahari dan membrane mukosa, namun dapat pula terjadi pada setiap bagian tubuh.</p>
<p>Pada orang kulit putih lebih sering dijumpai pada daerah muka dan ekstremitas, sedangkan pada orang kulit berwarna gelap di daerah tropic lebih banyak pada ekstremitas bawah, badan, dan dapat pula dijumpai bibir bawah serta punggung tangan<sup>(5)</sup>.</p>
<p>Gambaran klinis KSS bervariasi, dapat berupa :</p>
<ul>
<li>Nodul berwarna seperti kulit normal, permukaannya halus tanpa krusta atau ulkus dengan tepi yang berbatasan kurang jelas.</li>
<li>Nodul kemerahan dengan permukaan yang papilomatosa atau verukosa yang menyerupai bunga kol.</li>
<li>Ulkus dengan krusta pada permukaannya, tepi meninggi, berwarna kuning kemerahan. Dalam perjalanan penyakitnya, lesi akan meluas dan mengadakan metastasis ke kelenjar limfe regional atau ke organ-organ dalam.</li>
<li>KSS yang timbul dari kulit normal (de novo) lebih sering mengadakan invasi yang cepat dan terjadi metastasis, dibandingkan lesi yang timbul dari keratosis aktinik<sup>(5)</sup>.</li>
</ul>
<p><strong> </strong></p>
<ol>
<li><strong>C. </strong><strong>Melanoma Maligna</strong></li>
</ol>
<p>Melanoma bisa berawal sebagai pertumbuhan kulit baru yang kecil dan berpigmen pada kulit yang normal. Paling sering tumbuh pada kulit yang terpapar sinar matahari, tetapi hampir separuh kasus tumbuh dari tahi lalat yang berpigmen. Melanoma mudah menyebar ke bagian tubuh yang jauh (<em>metastase</em>), dimana akan terus tumbuh dan menghancurkan jaringan. Semakin sedikit pertumbuhan melanoma ke dalam kulit, maka semakin besar peluang untuk menyembuhkannya. Jika melanoma telah tumbuh jauh ke dalam kulit, akan lebih mungkin menyebar melalui pembuluh getah bening dan pembuluh darah dan bisa menyebabkan kematian dalam beberapa bulan atau tahun<sup>(8).</sup></p>
<p>Perjalanan penyakit melanoma bervariasi dan tampaknya dipengaruhi oleh kekuatan pertahanan oleh sistem kekebalan tubuh<sup>(5)</sup>.</p>
<p>Beberapa penderita yang keadaan kesehatannya baik, bisa bertahan hidup selama bertahun-tahun meskipun melanomanya telah menyebar. Tanda-tanda peringatan akan terbentuknya melanoma:</p>
<ul>
<li>Bintik atau tahi lalat berpigmen (terutama yang berwarna hitam atau biru tua) yang semakin membesar</li>
<li>Perubahan warna pada tahi lalat, terutama pigmentasi merah, putih dan biru di kulit sekelilingnya</li>
<li>Perubahan pada kulit diatas bintik yang berpigmen, misalnya perubahan konsistensi atau bentuk</li>
<li>Tanda-tanda peradangan pada kulit di sekitar tahi lalat<sup>(8)</sup>.</li>
</ul>
<p>Clark dan Mihm (1965) atas dasar tingkat penyebaran secara histologik mengklasifikasikan melanoma maligna menjadi stadium, yaitu :<strong> </strong></p>
<ol>
<li>Sel melanoma maligna berada di dalam epidermis tetapi tidak menembus membrane basal (karsinoma insitu)</li>
<li>Melanoma maligna sampai ke stratum papillare</li>
<li>Melanoma maligna masuk di antara dermis papillare dan dermis retikulare</li>
<li>Melanoma maligna masuk dalam dermis retikulare</li>
<li>Melanoma maligna masuk ke dalam jaringan subkutis</li>
</ol>
<p>Breslow (1970) mengukur ketebalan invasi tumor dengan micrometer. Kedalam invasi diukur dari lapisan granuler epidermis sampai bagian terdalam dari invasi, dan apabila ada ulcerasi pengukuran dari dasar ulserasinya. Pembagian menurut Breslow adalah sebagai berikut :</p>
<ul>
<li>0,75 mm</li>
<li>&#62; 0,75-1,50 mm</li>
<li>&#62; 1,50-3,99 mm</li>
<li>&#62; 4 mm</li>
</ul>
<p><strong>II.7 </strong><strong>Diagnosis dan Pemeriksaan Penunjang</strong></p>
<ol>
<li><strong>A. </strong><strong>Karsinoma Sel Basal</strong></li>
</ol>
<p>Diagnosis ditegakkan melalui gejala klinisnya. Untuk memperkuat diagnosis dapat dilakukan pemeriksaan penunjang berupa biopsy.</p>
<ol>
<li><strong>B. </strong><strong>Karsinoma Sel Skuamosa</strong></li>
</ol>
<p>Diagnosis ditegakkan melalui gejala klinisnya. Untuk memperkuat diagnosis dapat dilakukan pemeriksaan penunjang berupa biopsy.</p>
<ol>
<li><strong>C. </strong><strong>Melanoma Maligna</strong></li>
</ol>
<p>Diagnosis ditegakkan melalui gejala klinisnya. Tanda yang dapat dilihat adalah berdasarkan empat ciri berikut:</p>
<ol>
<li><strong>Bentuk</strong> : Ketumbuhan mempunyai bentuk yang tidak seragam , misalnya tidak      simetri. (A-asymmetry)</li>
<li><strong>Linkungan</strong>:      Linkungan adalah tidak jelas, terutamanya untuk melanoma. (B-border)</li>
<li><strong>Warna</strong> : Pelbagai warna boleh dilihat dan distribusi warna tidak seragam.      (C-colour)</li>
<li><strong>Diameter</strong> :      Diameter lebih besar daripada 5-6 milimeter. (D-diameter)</li>
</ol>
<p>Untuk memperkuat diagnosis dapat dilakukan pemeriksaan penunjang berupa biopsy. Pemeriksaan rontgen thorax, USG dan atau CT-Scan untuk mengetahui daerah metastase.</p>
<p><strong> </strong></p>
<p><strong>II.8 </strong><strong>Penatalaksanaan</strong></p>
<ol>
<li><strong>A. </strong><strong>Karsinoma Sel Basal</strong></li>
</ol>
<p>Biasanya kanker diangkat melalui pengorekan lalu dibakar dengan jarum listrik (<em>kuretase</em> dan <em>elektrodesikasi</em>) atau dipotong dengan pisau bedah.<br />
Sebelumnya diberikan suntikan <em>anestesi</em>. Jarang dilakukan terapi penyinaran.<br />
Untuk tumor kambuhan dan karsinoma sel basal yang menyerupai jaringan parut, bisa dilakukan pembedahan mikroskopik atau bedah <em>Mohs</em><sup>(8)</sup>.</p>
<ol>
<li><strong>B. </strong><strong>Karsinoma Sel Skuamosa</strong></li>
</ol>
<p>Karsinoma sel skuamosa dan penyakit Bowen diatasi dengan mengangkat tumor, baik dengan cara <em>kuretasi</em> dan <em>elektrodesikasi</em> maupun memotongnya dengan pisau bedah. Keratosis aktinik bisa berubah menjadi karsinoma sel skuamosa. Keratosis aktinik dihancurkan dengan larutan nitrogen atau krim fluorourasil<sup>(5)</sup>.</p>
<ol>
<li><strong>C. </strong><strong>Melanoma Maligna</strong></li>
</ol>
<p>Untuk bisa memahami melanoma maligna dan pengobatannya, penting untuk disadari bahwa prognosis tergantung pada kedalaman invasi tumor yang diketahui pada waktu eksisi pertama tanpa memperhatikan tipe tumor semula. Sebagian besar klinik mengukur invasi dengan menggunakan tehnik yang disebut sebagai “ketebalan Breslow” (Breslow thickness).</p>
<p>Semua tipe melanoma sebaiknya di eksisi pada kesempatan sedini mungkin. Radioterapi dan krioterapi saat ini belum dapat membantu banyak dalam penyembuhan penyakit ini. Masih menjadi perdebatan tentang seberapa luas eksisi harus dilakukan, yang ada hanya kesepakatan bahwa kalau bisa sesempit mungkin. Sama sekali tidak ada bahaya dalam eksisi awal yang sempit. Yang harus segera dilakukan adalah mengangkat melanoma<sup>(5)</sup>.</p>

<p><strong>II.8 Prognosa</strong></p>

<li><strong>A. </strong><strong>Karsinoma Sel Basal</strong></li>
</ol>
<p>Pengobatan pada KSB primer biasanya memberikan angka kesembuhan sekitar 95%; sedangkan pada KSB rekuren sekitar 92%. Dijumpai angka kekambuhan 5 tahun pada metode kuretase dan elektrodesikasi sebesar 7,7%; bedah mosh 1%<sup>(8)</sup>.</p>
<ol>
<li><strong>B. </strong><strong>Karsinoma Sel Skuamosa</strong></li>
</ol>
<p>Prognosisnya sangat bervariasi, tergantung pada banyak faktor diantaranya lokasi, ukuran tumor, dan tingkat diferensiasi sel-sel, serta kedalaman perluasannya. Lesi-lesi kecil yang timbul dari kulit yang rusak secara klinik mudah disembuhkan, sedangkan lesi pada bibir mudah metastasis dan mempunyai prognosis yang jelek<sup>(9)</sup>.</p>
<ol>
<li><strong>C. </strong><strong>Melanoma Maligna</strong></li>
</ol>
<p>Prognosis melanoma maligna sangat bervariasi. Ditentukan oleh beberapa factor, yaitu :</p>
<ol>
<li>Sifat Tumor</li>
<li>Stadium klinis</li>
<li>Lokasi metastasis</li>
<li>Faktor penderita</li>
</ol>
<p>Bila tumor kurang dari 1,5 mm pada waktu dilakukan eksisi pertama, maka kemungkinan bertahan selama 5 tahun sekitar 90%; bila kedalaman lebih dari 3,5 mm, maka angka tersebut akan turun sampai 40% atau kurang<sup>(8)</sup>.</p>

<ol>
<li>Tumor ganas kulit adalah proses keganasan yang timbul dipermukaan kulit dan berasal dari sel epitel, sel pluripotensial atau dari sel melanin di dalam kulit.</li>
<li>Menurut jenis sel yang berdiferensiasi, tumor ganas kulit diklasifikasikan sebagai berikut: karsinoma sel basal (KSB), karsinoma sel skuamosa (KSS) dan melanoma maligna (MM).</li>
<li>Menurut etiologinya, tumor ganas kulit dapat disebabkan oleh (1) faktor ekstrinsik berupa paparan sinar ultraviolet, paparan sinar-X, pemakaian bahan kimia dan adanya jaringan parut yang luas dan lama; (2) faktor intrinsik berupa genetik, sistem imun yang rendah dan ras.</li>
<li>Karsinoma sel basal biasanya terdapat pada wajah dan leher dengan gejala klinis berupa nodul ulseratif, berpigmen, morfea, superfisial dan fibroepitelioma. Biasanya ditandai dengan tepi ulkus yang meninggi tanpa adanya metastasis jauh.</li>
<li>Predileksi karsinoma sel skuamosa pada daerah kulit yang terpapar sinar matahari dan pada membran mukosa dengan gambaran klinis berupa (1) nodul berwarna seperti kulit normal dengan permukaan halus tanpa krusta atau (2) nodul kemerahan dengan permukaan papilomatosa/verukosa yang menyerupai bungan kol atau (3) ulkus dengan krusta pada permukaannya dengan tepi meninggi berwarna kuning kemerahan.</li>
<li>Melanoma maligna paling sering tumbuh pada kulit yang terpapar sinar matahari tapi hampir separuhnya tumbuh dari tahi lalat yang berpigmen, terdapat tanda-tanda peradangan pada kulit sekitar tahi lalat, terjadi perubahan warna, ukuran, bentuk/konsistensi serta mudah mengadakan metastase jauh.</li>
<li>Diagnosis tumor ganas kulit ditegakkan berdasarkan gejala klinis dan pemeriksaan penunjang.</li>
<li>Penanganan KSB dan KSS biasanya dengan mengangkat tumor, baik dengan cara kuretase dan elektrodesikasi maupun memotongnya dengan pisau bedah. Sedangkan penanganan MM prinsipnya adalah melakukan eksisi yang pada awalnya dilakukan pengukuran ketebalan invasi terlebih dahulu dengan teknik Breslow thickness.</li>
<li>Prognosa dari KSB adalah baik dengan angka kesembuhan skitar 95% sedangkan pada KSS tergantung dari lokasi, ukuran, tingkat diferensiasi sel-sel dan kedalaman perluasannya, dan pada MM prognosa ditentukan oleh sifat tumor, stadium klinis, lokasi metastase dan faktor penderita.</li>

<p>Har varit hos min son på KSS i Skövde de senaste tre dygnen; blev ikväll avbytt av min kvinna, trots att jag hade räknat med att detta skulle ske under morgondagen (torsdag).</p>
<p>Vad är då nytt?</p>
<p>Min son blir allt sämre. Grinigare, oroligare. Dessutom har han nu fått utslag över närmast hela kroppen, vilket ställt flertalet läkare &#8211; till den grad att jag och Vincent idag fick tid hos en hudläkare.</p>
<p>Personligen tänkte jag att nu har han säkert även fått hudcancer &#8211; för ingenting negativt förvånar mig längre.</p>
<p>Och oron började när hudläkaren tittade, kände, rörde och analyserade min sons utslag. När hon började skriva ut ett recept på hudsalva, och förslag till åtgärder, men när hon sedan tvingades hämta ytterligare en specialist tänkte jag det värsta.</p>
<p>Det blev heller inte bättre av att de båda läkarna var oense, att den äldre kvinnan som kallades in talade ett latinläkarspråk jag tidigare inte hört; att de argumenterade sinsemellan och förklarade för mig att Vincents utslag på kroppen beror på två saker: 1) värme; då hans hjärta arbetar så hårt som om han joggade dygnet runt svettas han kopiöst och får därmed lätt utslag; och att 2) han på vissa platser på kroppen fått barneksem; vilket tydligen beror på om antingen jag eller min kvinna har någon (NÅGON) allergi i släkten. Inte visste jag att eksem beror på allergi i generna, men så är det tydligen&#8230;</p>
<p>Hur som helst undrar jag när eländigheterna ska ta slut, och när min son ska få komma hem igen. Han tycks bara bli sämre och sämre och sämre. Under de dagar jag nu varit i Skövde har han knappt sovit, varit jobbigare än tidigare under året och dessutom gråtit så mycket fortast han vaknat att han blivit i det närmaste blå i ansiktet. Hans mage fungerar inte. Hjärtat fungerar inte. Huden fungerar inte. Man upptäckte dessutom igår via röntgenbilder att hans lungor får för mycket blod &#8211; är därmed alltför ansträngda. Hans blodtryck är inte bra; särskilt övertrycket, så nu har man även höjt hans hjärtmedicin. Och hans vikt varierar dag för dag, verkar närmast öka så lite under veckorna att man börjar undra.</p>
<p>Vad ska bli nästa sak man upptäcker? Jag slutar snart förvånas. Hur ska min son orka med en hjärtoperation? Och när blir nu denna hjärtoperation? och vilken typ av hjärtoperation blir det? Hur ska han orka? Och när ska vi egentligen få ta med honom hem till Sverige, till Mariestad?</p>

<title><![CDATA[Marmeladprinsen fortfarande inlagd på sjukhus]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p>Närmast hela detta år har Marmeladprinsen, Vincent Frost, varit inlagd på KSS i Skövde. Det ser heller inte ut att bli någon förändring av detta faktum. Hans mor och undertecknad byter av varandra i vad som närmast kan liknas vid skift, sover vid hans sida, tar hand om honom, två eller tre dygn i taget &#8211; varför jag kanske inte hör av mig eller svarar på mejl.</p>
<p>Min sons tillstånd är enligt följande:</p>
<p>Han lider av någon för läkarna obekant virusinfektion som drabbat hans luftrör, inhalerar därför Ventolin och Adrenalin. Samtigt har hans hjärtklaffsläckage ökat, från 1 till 2 på en skala till fem. Varför man satt in såväl medicin emot detta (med noggrann kontroll och uppsikt mot biverkningarna) samt vätskedrivande medicin. Vincent kräks även upp mycket slem, tja, kräks ofta, varför man också har noggrann kontroll över hans vikt och näringstillförsel. Dessutom, såsom detta inte vore nog, lider han av en slags kolik: han måste ha medicin för att förenkla sin avföring, vilket i detta nu inte kommer dagligen eller ens regelbundet. Tanken är att det ska komma dagligen och regelbundet. Och återigen i detta nu, måste han dagligen få lavemang samt noggrann uppsikt över tarmarnas arbete (osv.)</p>

<description><![CDATA[I måndags började Marmeladprinsen Vincent Frost att kräkas stora mängder slem, mat och andas häftigt]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p>I måndags började Marmeladprinsen Vincent Frost att kräkas stora mängder slem, mat och andas häftigt, gny, gråta, bli blek med glansiga ögon och må allmänt dåligt. Jag kallades från mitt palats på Mariagatan till residenset vid Lindebäcksvägen för att se över honom, ta hand om honom.</p>
<p>Den här gången kände vi genast igen symptomen. Ringde akuten vid Kärnsjukhuset i Skövde, och fick genast komma in. Hade vi väntat längre kanske det även denna gång blivit ambulans. Väntetiden var sedan lång, men vi blev inlagda och sedan ett i förrgår natt var jag ensam med Marmeladprinsen på sjukhuset. Det blev ingen sömn den natten. Inhalationer av adrenalin, alvedon oralt och i stolpiller, tröst på alla de vis, försök till vila och sondmatning etc. Det tar på krafterna.</p>
<p>Men jag är ändå så otroligt glad att han så tidigt hamnade under professionell tillsyn, samt att vi blev inlagda med honom. Det kändes tryggt.  Det känns tryggt.</p>
<p>Nu är jag bara hemma över natten, pga ett viktigt möte på vuxenpsykiatrin imorgon. På torsdag har jag bokat möte med min vän Josho som ska hjälpa mig med renoveringen av köket. Och på fredag har jag ännu ett viktigt, kanske det viktigaste!, psykiatrimötet, under vilket jag förmodar att min närmaste framtid avgörs.</p>
<p>Men jag kommer på dagarna tills dess att Vincent blir utskriven att vara med min familj på sjukhuset i Skövde.</p>

<p>So hieß nicht nur das <a href="http://www.reit-s.de/" target="_blank">Café</a> in dem ich heute mit Steffen und <a href="http://marthandi.blogspot.com/" target="_blank">Andreas</a> war, sondern das war dieses Treffen für mich heute auch. Wir sind im Begriff eine sogenannte KSS (Klein, Stark Schwarz (Anspielung auf Espresso)) zu gründen. Das ganze ist ein Programm ausgelegt für 2-3 Personen, was quasi ein organisierter Freundeskreis ist, in dem man miteinander, aneinander wachsen und voneinander lernen kann. Das besondere hierbei ist der Schwerpunkt auf dem geistlichen Aspekt. Also wir wollen gemeinsam unsere Beziehung zu Gott vertiefen und uns gegenseitig helfen geistlich voran zu kommen. Da wir alle nicht so massig Zeit haben, ist eine solch organisierte Sache in meinen Augen sehr sinnvoll und ich freue mich schon sehr auf die kommende Zeit und bin gespannt was Gott mit uns noch so vor hat.<br />
Heute haben wir das ganze ertmals zu dirtt besprochen und haben uns dazu im Café getroffen und uns einen netten Nachmittag gemacht. Nächste Woche wollen wir uns wieder treffen, dann aber schon konkreter werden. Ich hoffe wir schaffen es!</p>
