<content:encoded><![CDATA[<div class='snap_preview'><p style="text-align:justify;">Despite  (or maybe even because of) the somewhat variable weather fronts of the past few days I&#8217;ve had a superb time.  Friday I bimbled towards the coastal salt flats only to discover that they were almost permanently under water at the present.  At least not even sparingly dry enough to thing about pitching down.  Hmmm! Slight change of plans lateish in the day meant some hard tabbing to get to an alternative camp site.  I was looking for something a bit special to hide away and let the world pass.  The Old Decoy would do the job so I pushed hard and arrived just as the light faded.  I could have used the bothy there but I had reasons to want to be &#8216;outside&#8217;.</p>
<p style="text-align:justify;">First pitch (in earnest at least) for the MLD Duomid (Sinylon version)  was probably best not accomplished using a head torch, but I had practiced and knew how I wanted the shelter configured.  All the lines were attached so that saved some potential grief.  I pitched with the &#8216;foot end&#8217; of the shelter into the wind as this seemed to be the method of choice on some of the reviews I&#8217;ve read.  I  pegged the two end corners well down to the ground and tensioned the centre base and mid-wall guy enough to slightly pull out the end panel. One &#8216;head end&#8217; corner &#8211; the one adjacent to the door side panel I allowed to stay slack, but the other opposite corner I pulled in quite tightly.  This had the effect of allowing me to form a &#8216;beak&#8217; using the door side centre guys &#8211; for ventilation.  The remaining mid panel guys (the &#8216;head end and non-door side) I put under slight tension, and that was it.  Inside I was amazed at the airiness and space.</p>
<p style="text-align:justify;">Getting this right, or at least to my satisfaction took about 25 minutes &#8211; thankfully it was fine and calm.</p>
<p style="text-align:justify;">Inside I used a small groundsheet to give somewhere to spread my Neoair mattress, Alpkit Hunka XL and sleeping bag, with enough space for footwear, dry clothes, pack, food etc.  Time for a brew.</p>
<p style="text-align:justify;">I had decided some time ago to give gas canister stoves another whirl and this involved some fiddly bits and pieces and extra weight &#8211; for the tests I wanted to do, not the stoves themselves.  For example I had two different windshields, albeit lightweight aluminium, but still some unnecessary weight &#8211; I also took with me (weight(!!!) for this&#8230;.you could die laughing &#8211; I took a set of digital kitchen scales.  A very necessary piece of backpacking equipment!!!  The reason was simple.  I wanted to know fairly precisely how much gas I would be using in different stoves and windshield configurations.  For my own satisfaction I also took a digital stopwatch!.</p>
<p style="text-align:justify;">DON&#8217;T WORRY&#8230;&#8230;I&#8217;m not going to start spouting facts and figures.  The stove experiment was purely for my own satisfaction and to supply me with some guidelines as to how much gas to carry in future, and how much gas various tasks consumed.  I&#8217;m sorry to admit that all this needed some very precise boil time measurements too.</p>
<p style="text-align:justify;">The upshot of the experiment is that I now reckon that an allowance of 10 grams of gas per 500 ml of boiled water is ample.</p>
<p style="text-align:justify;">The stoves I used were a Optimus Crux and a SnowPeak  Giga Power GST-100 &#8211; both performed well, were light, economical and just about foolproof.  I used only one pot &#8211; Tibetan Ti &#8211; 700 pot with lid.  Windshielding was 0.2 mm aluminium sheet.  I cut an oversized disc with a centre hole to sit on the top of a Coleman 250 canister (70/30 Butane/Propane mix) I curled up about 1 cm around the circumference to form a lip and then formed an almost complete cylinder (small cutout for the gas control) which sat on the disc and encased about 80% of the pot side.  This meant that the whole of the cylinder was exposed for cooling but the pot had maximum coverage to retain heat.  Note&#8230;..throughout each session with the stoves I constantly checked the canister top temperature and at no time did it exceed mildly warm.  The disc reflector did a grand job.   This windshield system does have one major drawback.  It will only work with pots of the same diameter.  Larger (MSR Kettle, or Tibetan Ti-900)  would require a different length of wrap-around and a larger disc.  That said the system should work for any diameter pot with appropriate preparations beforehand.</p>
<p style="text-align:justify;">The testing, recording and retesting took most of the two full days I was out, but I did manage some quiet time to just contemplate.</p>
<p style="text-align:justify;">To say the weather was changeable would be a major understatement.   During the first night it rained and rained and then rained some more.   Stair-rodding wasn&#8217;t the word, more like drain-rodding.  BUT this was great, I proved my seam sealing was good and also that the MLD Duomid could take a real pounding.  Did I say the rain was accompanied by gales?  I don&#8217;t think I mentioned the wind yet.  Anyone who knows the East coast flatlands also knows about the winds off the sea.  I don&#8217;t think I need to say more &#8211; except that the Duomid took the lot with no trouble.</p>
<p style="text-align:justify;">My Alpkit Hunka XL had its first outing in earnest and I deliberately used a Golite Adrenaline 20 sleeping bag &#8211; primarily because of its colouring on the main body (light blue) and that seems so show every spot of damp/condensation.  There must have been a fairly high dew point for much of the weekend (I did experience some mild condensation on the inside of the Duomid) but at no time did my sleeping bag show any sign of dampness.  I guess it&#8217;s safe to say that the Hunka is breathable.</p>
<p style="text-align:justify;">Finally, my new Gregory Z pack.  What can I say, I read the reviews before buying, everything good and more is all I can add.  It&#8217;s simply great.</p>
<p style="text-align:justify;">Perhaps now it&#8217;s understandable why I feel that I&#8217;ve had a great break.</p>
<p style="text-align:justify;">Hopefully my next post will include some video of the windshielding system I&#8217;ve described.</p>

<p>And what is this new addition?  Aha, now you may well by thinking that a new stove has arrived, but no, you&#8217;d be wrong.</p>
<p>The return address was not that of Tinny and MBDC but Ron Bell, of Mountain Laurel Designs.</p>
<p>Yes&#8230;it&#8217;s my Duomid (<a href="http://www.mountainlaureldesigns.com/shop/product_info.php?cPath=35&#38;products_id=105&#38;osCsid=a2fc4a15b9cc4001b84ab9ddf0695deb" target="_blank">see the model here</a>) silnylon version, resplendent it all its glory.   Maybe I&#8217;ll get it seam sealed and up sometime this weekend, so some photographs may appear soon.</p>

<p>Secondo Claudio Scajola &#8220;è l&#8217;inizio di un percorso che porterà le imprese italiane a investire in quell&#8217;area, nei prossimi anni, 5 miliardi di euro in energia rinnovabile fino a far diventare il Montenegro l&#8217;hub energetico più importante del Mediterraneo&#8221;.</p>

<p>Al cui interno spiccano alcune voci: i rimborsi ai comuni del minor gettito derivante dall&#8217;Ici sui fabbricati rurali, che peseranno per 1,5 miliardi, e l&#8217;adeguamento dei fondi per spese obbligatorie (300 milioni), impreviste (400 milioni) e del cosiddetto fondo per la riassegnazione dei residui perenti (1 miliardo di euro), che servirà a mettere in campo soldi da destinare alle imprese che vantano crediti verso lo stato.</p>
<p>In realtà, all&#8217;interno di questo dato il calo delle entrate tributarie vere e proprie è anche superiore, ovvero di 32,699 miliardi, ma sconta un leggero contenimento grazie al miglioramento delle entrate extra-tributarie (+380 milioni) e della voce «alienazione e ammortamento» (che porta in dote un +228 milioni).</p>
<p>E lo fa nel momento in cui dice che la consistente variazione negativa proposta per il comparto tributario trova motivazione nel fatto che, mentre le previsioni di entrata contenute nel ddl bilancio erano state formulate nel settembre 2008 considerando un tasso di crescita del Pil dello 0,5%, adesso, nel nuovo quadro macroeconomico, si deve parlare di una diminuzione del prodotto interno del 5,2%.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p><img class="alignleft size-full wp-image-91" title="MLD" src="http://brutallyfrank.wordpress.com/files/2009/05/mld.jpg" alt="MLD" width="114" height="81" />Manual Lymph Drainage (MLD) is the secret behind many a healthy body and beautiful face.  It has the ability to boost the immune system, clear sinuses and can stop a cold in its tracks.  It’s an absolute cosmetic treat but also has a very serious use in helping lymphoedema.</p>
<p>MLD was developed in the 1930s in France by physician, Dr Emil Vodder and his wife Estrid. Vodder noticed how people suffering from chronic catarrhal and sinus infections tended to have swollen lymph glands and, much against medical practice at the time, started to work with the lymph nodes.  The massage he developed had a circular, pumping effect which increases the movement of the lymphatic system of the body.</p>
<p><a href="http://www.pms.ac.uk/compmed/ErnstCV-extended.pdf">Professor E Ernst </a>of the Peninsula Medical School at the University of Exeter agrees the treatment is effective: ‘It reduces lymph oedema through enhancing lymph flow,’ he confirms.</p>
<p>Patsy, 54, was treated for cervical cancer six years ago and developed lymphoedema.  ‘About five weeks after my operation I realised something was wrong,’ she says. ‘I couldn’t pull my jeans over my thighs.  The sight of my legs ballooning was quite horrible.’  Patsy turned to MLD.  Her insurance paid for one course and her GP funded another. ‘I notice a huge difference,’ she says, ‘It definitely eases the lymphoedema, it keeps it down.  I just wish more doctors knew about it and referred their patients.’</p>
<p>MLD is nothing like normal massage.  There is no pounding of muscles, no probing and pulling; it feels like having your skin softly stroked by a child&#8217;s gentle fingers.  It&#8217;s a light, repetitive movement with an almost hypnotic effect.  After an hour I felt as if I were floating in a deep blue lagoon &#8211; I have never been so relaxed.  MLD practitioner and teacher Dee Jones explains that the relaxation comes about because the massage affects the nervous system, instigating a change from the normal stressed ‘day time’ state of the nervous system to the ‘night time’ state we use when we&#8217;re asleep. </p>
<p>It also boosts your immune system by boosting the lymphatic system (the body’s waste disposal service).  People who have regular MLD simply don’t get so many colds and infections.  Any colds they do get disappear in hours or days rather than dragging on for weeks and sinuses drain as if by magic. </p>
<p>However many people use MLD purely as a beauty treatment.  ‘It really can work miracles,’ says Joy Salem who combines MLD with her ‘fingertip facelift’ beauty treatment. ‘It punches in the jawline, tones the features and reduces lines.’</p>
<p>‘Its effects on the skin are pretty spectacular,’ agrees Dee Jones. ‘When you get the garbage out of the body it gives the skin a chance to regenerate.  It won&#8217;t make you thinner as such but it will certainly make your face <span style="text-decoration:underline;"><em>look</em></span> thinner.  It tightens up all the little saggy baggy bits, all the puffiness.  To a degree it&#8217;s like a face-lift without surgery.’</p>
<p>‘It takes time and commitment,’ warns Dee. ‘You have to free the fat and the water that is trapped by proteins but it will clear.’ </p>
<p>A one-off treatment is pleasant and is great for stress; it will make a difference to puffy eyes and if you have a blocked nose you&#8217;ll notice a swift change as well.  But if you want to build up your immune system you would ideally have treatment three times a week for the first two weeks and then tail off after that, perhaps having a session every two to three months or if a particular problem arises.</p>
<p>GPs can and do refer patients to MLD practitioners but generally only for lymphoedema.</p>
<p>Regrettably there are some people who are not advised to attempt MLD.  Anyone who has suffered from TB needs to avoid it as there is a possibility that TB molecules stored in the lymph system could be reawakened.  And anyone with heart problems (particularly cardiac odema) is also advised against the treatment. </p>
<p>Also do make sure you experience genuine ‘Vodder technique’ MLD.  ‘Many beauticians purport to do MLD,’ warns Joy Salem. ‘But genuine MLD is a very gentle process while some of the machines that claim to help the lymph can actually do more harm than good.’</p>
<p><strong>My verdict:</strong>  a superlative treatment.  Absolutely essential for anyone suffering from lymphoedema and lipodema.  Plus a cosmetic treat for everyone.  Do go for Vodder technique though – I haven’t found the mechanical forms of lymph drainage anywhere near as effective.</p>

I still don&#8217;t, but thanks to an opportunity to write for another blog owned by a friend of mine (<a title="My Latest Distraction" href="http://www.mylatestdistraction.com">My Latest Distraction</a>), I decided to test out WordPress, so I could get a feel of how the experience differs. So far, I like the interface better, the design themes are pretty clean and attractive right from the jump (as opposed to having to do alot of tinkering or needing a higher level account at LJ), and somehow, I feel better about this.</p>
<p>Of course, this is initial impressions. Maybe once I&#8217;ve kicked the tires more, and the vacation effect wears off, I&#8217;ll be more critical. So far, though, I&#8217;m diggin&#8217; the scene.</p>
<p>Once I&#8217;m done writing this paper for Tuesday, I&#8217;ll be takin&#8217; pictures and writing for my first post over at <a title="MLD" href="http://www.mylatestdistraction.com">My Latest Distraction</a>. It&#8217;ll be about&#8211;</p>

<title><![CDATA[Living With Lymphedema]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p><a href="http://www.mayoclinic.com/health/lymphedema/DS00609"><span><span>Lymphedema</span></span></a> control  is my daily challenge.   I have secondary <span><span>lymphedema</span></span> as a result of cancer surgery and radiation.   Manual lymphatic drainage followed by compression wrapping was discovered about 50 years ago in Germany and it is still the gold standard of treatment today.  One would think that, similarly to advances in cancer treatment, advances would have been made in the treatment of  <span><span>lymphedema</span></span>.  But that is not he case.</p>
<p>I believe the reason for this is two-fold:</p>
<ol>
<li>There is not the demand or focus on <span><span>lymphedema</span></span> (no celebrity calling for its attention and fund raising for research efforts) and</li>
<li>Manual lymphatic drainage (<span><span>MLD</span></span>) and compression works.  However, <span><span>MLD</span></span> is time-consuming, takes a certified lymphatic massage therapist to be administered properly and is only truly effective when followed by compression wrapping.</li>
</ol>
<p>Compression wrapping, in turn, is time consuming to do.  Although some people may have mastered the art of wrapping one limb, to self-wrap two or three limbs is nearly impossible especially if fingers or toes need to be wrapped as well.  Then there is the issue of rolling the wraps after each wearing, washing them often, replacing the batting used under the wraps frequently, and taking them with you as a necessity whenever you travel.</p>
<p>What else can be done especially when all limbs and the trunk is involved.  Pumps don&#8217;t work and neither do diuretics.  It is necessary to wear compression sleeves when traveling, the cost of  which are not covered by medicare.<span> Living with <span>lymphedema</span> is not easy.</span></p>

<p>I went out to NC this past Saturday to see Tyler. it was so hard to see him the way he was. The chemo has made his skin look like it’s peeling and it’s dark in some spots, he told me he is turning into an Indian <img class="wp-smiley" src="http://tyler.mldfamily.org/wp-includes/images/smilies/icon_smile.gif" alt=")" /> I told him that I was jealous because whenever I try to get a tan I look like Sebastian from the Little Mermaid, he thought that was funny.</p>
<p>His day starts out at about 7:30-8:00, whenever the 12 hour IV that was hooked up to him the night before starts beeping. One of us then picks him up out of his bed ( he has lost most of his muscle control therefore he can not do anything by himself) and we change him. The the meds start to be administered. He takes a total of 8 orally, then there is a 2 hour IV that pumps into him in the morning and then again at night. Then blood is drawn and driven to the clinic so the results are ready for us at his 1 pm clinic appointment at <span class="yshortcuts">Duke University</span>. He was having trouble keeping his oral meds down so I came up with a sticker chart for him. Every time he keeps a med down, he gets a sticker, it’s working because he has kept his meds down now for 2 days! Every week he has to earn more stickers ( he takes a total of 56 meds a week) and <span class="yshortcuts">on Sunday</span> I find out if he met his goal for the week <img src='http://s.wordpress.com/wp-includes/images/smilies/icon_smile.gif' alt=':)' class='wp-smiley' /> . He really doesn’t want to eat, however I was able to get him to eat more than he has since Septamber, even if it ws Papa johns Cheese Bread, none of us cared, we were just happy that he ate and since I have gone he has kept up his appetite.</p>
<p>I suggested taking him to feed the geese behind the apartment on Sunday and he was excited to do that, Robert had to carry him down part of the way because his legs hurt to walk most of the time.</p>
<p>As always, trips go by way faster than you want them to, and it was so hard for us to say bye to each other. He made me promise to bring his little brother next time I fly out there. Tyler is a trooper and is in the top 5% of all the kids who have gone through the same process.<br />
He is considered a star patient <img class="wp-smiley" src="http://tyler.mldfamily.org/wp-includes/images/smilies/icon_smile.gif" alt=")" /> I could write more and more about our trip but it would probably take up an hour of everyones time to read it <img class="wp-smiley" src="http://tyler.mldfamily.org/wp-includes/images/smilies/icon_smile.gif" alt=")" /></p>
<p>In closing, I am doing everything I can to bring awareness to our communities about this disease. Based on statistics, there are at least 5 more kids running around Albuquerque that have this disease and don’t know about it.</p>

Nos amis kinésithérapeutes, orthophonistes et infirmiers passent les premiers. Ils seront les premeirs à adapter leur programme éducationnel. Il faut savoir que la modification du cursus de formation, accompagne le programme de délégation de tâches. Ainsi, les infirmiers ont déjà des formations au grade de master : cadre de santé, master dédié aux nouveaux actes techniques. Les kinésithérapeutes et les orthophonistes ont également des projets de Master.</p>
<p>La formation d&#8217;audioprothésiste, elle, s&#8217;articule autour d&#8217;un DE, puis peut être complété par des DU ou des Masters. Mais quid de l&#8217;articulation entre la délégation de tâches et de la LMD. Nous pouvons développé de nouvelles compétences, et les faire enseigner, toujours au bénéfice du patient !</p>

This technique stimulates the lymph nodes to open, and helps encourage drainage of the fluid and waste through the appropriate channels. The treatment is very comfortable and nonaggressive.</p>

It also is successful in breaking fibrosis and fibrotic areas of a lymphodemous limb.</p>
<p>This treatment was created and developed Danish therapists Dr. Emil Vodder and his wife, Estrid, in the 1930&#8217;s and was introduced in Paris in 1936. They are also credit with creating a specialty of medicine called Lymphology.</p>
<p>First brought to North America in 1982, the school is located in<br />
Victoria, British Columbia, Canada. Before it was introduced the standard treatment course in North American was either a surgery called debulking or the use of compression machines wherein the limb was literally squeezed by pneumatic air pressure. </span></span></p>

<p><span style="font-size:medium;color:#ff0000;font-family:Arial;"><strong>DECONGESTIVE THERAPY</strong></span></p>
<p><span style="font-size:medium;color:#000080;font-family:Arial;">I received an email request to describe what decongestive therapy is since I have mentioned it so many times. It is the treatment of choice presently used for lymphedema. </p>
<p>To understand how and why decongestive therapy works, first you need to undersand exactly how the lymph system is made up. We generally think of the lymph system as these &#8220;channels&#8221; the go throughout our body and are connected by lymph nodes. However, it is also comprised of an unblievable network of lymph capillaries that network under and through the skin layers our of body. This almost microscopic network collects fluids and channels it into the lymph veins to be filtered through the lymph nodes and eventually carried out of the body. In lymphedema, because of an impaired lymph system, fluid collects in the interstitial areas (tissues) between cells. This fluid is composed of proteins and electrolytes and even includes the liquid of blood plasma. You normally have about 15 quarts of this fluid in your system. With our condition, this liquid doesn&#8217;t move as it should and eventually the affected areas swells from the excess fluid, the whole system becomes &#8220;clogged.&#8221;</p>
<p>Decongestive therapy is a massage technique that helps unclog the system. It gently moves this fluid, in the direction of normal body flow so that it can be eliminated. In unclogging the system, the tiny network of lymph capillaries are able to help move the fluid.</p>
<p>There are two basic types of decongestive therapy.</p>

</span><span style="font-size:medium;color:#ff0000;font-family:Arial;"><strong>WHAT HAPPENS WHEN LYMPHEDEMA IS NOT TREATED?</strong></span><span style="color:#008000;"></p>
<p></span><span style="font-size:medium;color:#000080;font-family:Arial;">I sound like a broken record on urging everyone to get decongestive therapy, because for me, it came too late. By the time I had heard of this treatment my left leg (the worse one) was already beyond treatment. I have had three lymphedema therapists tell me that there was nothing that even this treatment could do.</p>
<p>Like most hereditary/primary lymphedema patients (Milroy&#8217;s Syndrome) my legs are not as large as lot I have seen. They are about twice the normal size. But, the left one is totally fibrotic, literally as hard as a rock. It is the hardest and most grotesque leg you would ever NOT want to have or see. As such, the infections, which have been plentiful and furious, especially during the past two years can only be controlled through months of IV antibiotics. Sometimes, I have even had to use a combination of Unasyn and Gentimycin.</p>
<p>It is also where the first of my two lymphomas appeared. I am now also loosing venous flow to my left foot because of the fibrosis. On my left ankle, I have an ominous and rapidly growing purplish nodular growth (soon to be biopsied). The change in this leg has been more dramatic and intense during the past year than all the other 50 years I have had lymphedema. What ever you do&#8230;get that lymphedema diagnosed early and get treatment immediately. I wish with all my heart that none of you have to experience the complications I now face.</span></p>

Di questa cifra Eurostat ha già accertato l’inesistenza di quasi 15 miliardi e, secondo quanto risulta al Giornale, i conti italiani sarebbero «sotto revisione» per quanto riguarda la somma rimanente. </p>
<p>Facciamo un passo indietro: l’indicatore più popolare per misurare la «performance» della politica economica di un governo è l’evoluzione del rapporto fra il deficit e il Prodotto interno lordo. Questa misura è importante sia in senso assoluto (perché se eccede stabilmente il 3% l’Europa apre una procedura di infrazione) sia in senso relativo perché si possono confrontare i comportamenti dei diversi Stati fra di loro e rispetto alla media europea. Ogni governo è «responsabile» degli anni per i quali firma la legge finanziaria che, come è noto, stabilisce spese e entrate per l’intero anno. Pertanto economicamente vengono per convenzione attribuiti al centrosinistra i risultati degli anni dal 1997 al 2001, al centrodestra quelli degli anni dal 2002 al 2006, di nuovo a Prodi gli anni 2007 e 2008. </p>
