My blood pressure was down today – 138/87 – low for me when I arrive at HACU. Not as low as it used to be or as low as I’d like it but lower. So that was a good start.

Cannula Queen got my blood first time. Excellent.

Then my platelet count came back = 205.

No one believed it could be true so they tested it again (why do they not ever test it when it’s low!?!?!). So I sat and waited and it was true.

My platelets today are 205. They have not been that high through this whole ITP thing. And they have never gone up at this point post infusion. (Day 7).

There could be a number of reasons for this:

1. I’ve given up aspartame (particularly Pepsi Max)
2. I have started drinking Mona Vie – a juice / tonic which has Acai berry and load of other fruits in it – I think I might become a distributor if it is the reason, so if you want to buy it send me an email.
3. Dapsone may be working
4. Mabthera may be working
5. My Haematolgist sent a letter to the surgeon (and saw his secretary) who is supposed to be removing my spleen – this caused me to stop attention seeking and get those platelets up up up!

It’s exciting to be that high, I go back again on friday for another test. Hopefully I will still be up.

There has never been a single documented case of fatal cannabis overdose. Also, the government’s own figures don’t tally. While drug figures from the Office Of National Statistics register 19 cannabis related deaths, the mortality stats from the same office log only 1 death.

Clearly the media disproportionally focuses on some drugs rather than others, thus distorting public opinion. The graphic below illustrates the death-rate per user of each drug:

So it appears that Marijuana is less toxic than aspirin.

Los casos de gripe A casi se triplicarán esta semana

El virus está teniendo una incidencia leve en los pacientes, pero Sanidad activa el nivel de epidemia al estimar que esta semana superaremos los 700 casos de gripe por cada 100.000 habitantes

Hace dos semanas, el número de casos de gripe A por cada 100.000 habitantes en la Comunidad de Madrid era de 93. La semana pasada, ese mismo baremo se elevó a más de 250, siempre según datos de la Red Nacional de Vigilancia Epidemiológica. La cifra, según fuentes de la Consejería de Sanidad, rondará los 700 durante la semana en la que entramos, triplicándose prácticamente respecto a la anterior. Por eso, el Gobierno regional ha decidido activar un plan de respuesta ante la pandemia con diferentes medidas que contrarresten la situación, según Sanidad.

Las medidas principales consisten, siempre en función de las necesidades de cada una de las zonas, en la contratación de personal sanitario para evitar colapsos. Además, todos los centros de salud tendrán activado un sistema para separar los casos sospechosos de los pacientes comunes. Ayer, en los hospitales se vivía normalidad.

La Consejería de Sanidad recomiendan a la población que, ante los primeros síntomas de fiebre u otros, el enfermo no acuda en primer lugar al Hospital. El motivo es que no pierda tiempo de manera inncesaria ni haga perderlo a los facultativos del centro médico. Antes recomiendan seguir dos pasos.

Llame al 112 y comente los síntomas. En la mayoría de los casos, primero recomendarán tomar paracetamol y guardar reposo.
En caso de que persistan los síntomas, Sanidad recomienda acudir al centro de atención primaria más cercano a su casa.

Por otra parte, esta semana estará marcada, desde hoy, por el descenso de las temperaturas. Según las previsiones de la Agencia Estatal de Meteorología, hoy los termómetros alcanzarán los 14 grados de máxima (ocho menos que ayer) y las mínimas llegarán a los 9 grados. Mañana las temperaturas bajarán aún más: con mínimas previstas en siete grados. El día más frío de la semana será el jueves, con mínimas de seis grados y máximas de 14, según el mencionado órgano.

Giving paracetamol (Panadol) to babies after vaccinations as a precaution against fever may lower the effectiveness of the immunisation, say researchers.

A trial of 450 infants having vaccines found that paracetamol doses over the next 24 hours did indeed reduce fever. However, the Czech researchers also found a significantly lower vaccine response with the painkiller. Study leader Professor Roman Prymula said paracetamol was sometimes given to allay parents fears of high fever in children after a vaccination. But the trial, which included children having routine immunisations and booster vaccines, found that the practice may actually do more harm than good.

But when the researchers looked at vaccine response they found lower levels of antibodies in those who were given the paracetamol.

“My advice would be if the child has a fever, don’t always assume it’s down to the vaccine – are you happy there isn’t another explanation.

“But if the child is otherwise well they probably don’t have anything else wrong with them and you need to question whether they really need their fever brought down.

“Giving paracetamol before or after vaccines is not to be encouraged because firstly it has little benefit and secondly this preliminary data suggests it may do harm.”

Programme du jour : Malade !

 

J’ai été malade une bonne partie de la nuit, je fais un peu de fièvre. Je me réveille vers 9h, prends mon petit déjeuner, puis joue un bon moment avec Maman et Damien.

Vers 11h30, je retourne me coucher pour me réveiller vers 16h. Je suis encore un peu malade, Maman m’emmène chez un pharmacien-docteur qui diagnostique une bronchite. Un peu de paracétamol (que Maman a dans sa valise), du sirop pour la toux et si j’ai toujours de la fièvre dans 48h, il faut consulter un “vrai” médecin. Nous profitons de l’arrêt à la pharmacie pour prendre un petit encas et acheter quelques cartes postales.

18h30, nous sommes de retour au Riad, je joue avec un petit garçon qui s’appelle Théophile, qui vient d’arriver lui aussi. Nous mangeons ensemble, je prends une petite douche puis vais me coucher vers 21h. Ma nuit sera à nouveau agitée. Rédacteur : Aurélia, ma Maman.

Unele tratamente le-am început deja (pe cele cu humex si tantum verde), iar despre celelalte … Să zicem că nu am toate ingredientele ))))

Promit că lucrez la partea asta şi pînă la răceala următoare o să fiu mai pregătită.

I bet almost everybody had experienced headaches and take usually those over the counter drugs. Well that includes me. I usually take paracetamol because it does not give me stomach upset. I try not to take Ibuprofen and mefenamic acid for my OTC. It is just too strong for my tummy. But paracetamol does not work for my mother, she needs to take ibuprofen for her headache. I do not know if it is just psychological for her that paracetamol does not work.

However, the ob-gyne of my sister told us, that even they say that paracetamol is safe, taking too much of it is not good. I think we need to take care of our health more. But with all the stress around us, for sure we can not avoid having headaches all the time, speaking on my experience.

If you got a consistent and severe headache, it is best safe to visit the doctor.

Die vorbeugende Gabe von Paracetamol kann vor diesem Hintergrund daher nicht mehr empfohlen werden. Dies sind die Folgerungen eines aktuellen Artikels, verfasst von Professor Roman Prymula von der University of Defence in Hradec Kralove (Königsgrätz) sowie Kollegen.

Treating vaccinated children with paracetamol as a precautionary measure against fever may reduce the effectiveness of the vaccine itself, according to research by Prymula et al. published in The Lancet on the 17^th October.

Fever is classed as a temperature of over 38^oC and is a normal part of the inflammatory response to infection, and after vaccination. Fever is worrying in children because of the risk of febrile convulsions (seizures that occur in children with a high temperature). Some countries recommend antipyretic drugs (drugs that reduce the body temperature), like paracetamol, after vaccination in children with a history of febrile convulsions. However, evidence to support their use is scare.

The Czech and Swiss researchers performed a randomised, controlled trial of 459 children receiving primary and booster vaccinations against a combination of common childhood diseases (including vaccines for whooping cough, diphtheria, rotavirus, tetanus) in ten centres across the Czech Republic to test the effects paracetamol during vaccination. Approximately half the children received three doses of paracetamol every 6-8 hours over the 24 hours post-vaccination, whilst the rest did not receive any paracetamol. Treatment with paracetamol after vaccination significantly reduced fever in children, but also significantly lowered antibody responses (including those against tetanus and diphtheria) to several of the vaccine antigens (meaning the vaccine does not produce as effective a protective immune response). The study also showed that high fever (> 39.5^oC) is actually a rare event after vaccination, both treatment groups had 2% or less children with a high fever after vaccination.

This research suggests that antipyretic drugs, like paracetamol, should not be routinely given to children after vaccination because it may interfere with how well the vaccine works. A linked article in the same issue of The Lancet discusses some of the important implications following the results of this study. More is needed to understand just how paracetamol reduces the protective host immune response that occurs during vaccination, and whether other antipyretic drugs (such as ibruprofen) also have the same effects as paracetamol.

Eu já tinha lido no GINA 2009 (Iniciativa Global para o Controle da Asma) sobre o uso de Paracetamol na gestação e em crianças menores de um ano como fator de risco para o desenvolvimento da asma.

Ao procurar as referências citadas no GINA 2009,  descobri que uma delas oferece acesso pleno e a outra, apenas o resumo do estudo. 

Várias outras referências científicas podem ser localizadas no PubMed ao se usar asthma como palavra-chave (Mesh Term) e paracetamol  como palavra do texto. Estas escolhas podem ser feitas clicando-se em “Preview/Index”.

Mas também achei várias referências na literatura leiga sobre isto, uma delas publicada na Folha Online em 2008.

Diante de informações novas o ideal é que façamos uma análise cuidadosa da literatura.

Yes that’s right ONE HUNDRED AND NINETY NINE.

I am feeling a little stunned.

Normal is between 150 and 400!!

 They might drop back again but right now they are the highest they have been since this whole ITP thing started!

I go back to hacu at the hospital again Monday and Thursday and Friday next week.

But right now I am dancing a jig!

Also I started an antibiotic called Bactrim today which is used to prevent some kind of lung infection that you can get when on high dose longterm prednisone.

So now I am taking the following antibiotics:

• Bactrim – to prevent an infection
• Amoxycillin / Clavulanic Duo Forte to fight the infection from last week
• Dapsone for my platelets

Also taking

• Slow K as my potassium is low
• 30mgs prednisone (weaning from 90mgs)
• Paracetamol when I need it
• Folic acid to stop the dapsone from lowering my red cells which is a common side effect
• Nexium 40mgs to stop all the above wreaking too much havoc with my gut
• Chocolate just because

Programme du jour : Ecole, médecin, et Papa.

 

J’ai passé une nuit un peu difficile. A 3h du matin, Papa me trouve brulante, j’ai 39.8°C de fièvre. Il tente la posologie préconisée actuellement pour le paracétamol, à savoir commencer par une dose de charge (2 fois la dose normale d’un coup). Une 1/2h plus tard, je n’ai plus de fièvre et je dors paisiblement.

Papa me réveille vers 6h45… je semble en forme, et je n’ai toujours pas de fièvre. Je m’habille, déjeune et je suis à l’école pour 7h30. La maitresse est prévenue, si je fais la fièvre, elle appellera Papa.

Entre temps, un rdv avec mon pédiatre est tout de même fixé pour 16h.

Papa vient me chercher vers 15h30, je suis en pleine forme, nous nous rendons tout de même chez le médecin. Il ne me trouve rien de particulier, à part mon rhume que je me traine depuis 10 jours. Il en profite pour me mesurer (97 cm) et me peser (15.3 Kg), j’aurai 4 ans dans 4 semaines.

Nous passons ensuite chez le comptable de Papa récupérer des documents. J’en profite pour lui montrer comment je sais écrire “PAPA”, sans exemple. Il est très impressionné. Nous passons ensuite à la pharmacie, à la superette, et arrivons à la maison vers 18h. Maman arrive à ce moment. Elle restera une petite heure avec nous.

La fatigue commence à se faire sentir. Mon repas commence très moyennement par des pleurs… mais se termine très bien. Papa me donne ensuite ma douche, et j’ai le temps de faire un peu de coloriage avant d’aller me coucher pour 19h30… toujours sans signe de fièvre. Rédacteur : Patrick-Robin, mon Papa.

One for the conspiracy theorists out there: Basshunter is not a man, he is a robot created by drugs companies to increase sales of headache remedies. I’ll have you convinced by the end of this rant.

Euro-Dance is hardly a genre that inspires much respect from people with ears and brains but Swedish dance producer Basshunter sets new standards for awfulness in an already awful genre, plumbing the depths of acceptability in recorded music.

Yet somehow, this man from the same country as Abba has tricked some unwitting record company into sanctioning the release of a second album. That can only be rationally explained by wild accusations of corporate bribery.

Everything about the album stinks. Imaginatively titled ‘Bass Generation’, the cover features Jonas Altberg, the ‘brains’ of the operation, sitting smugly atop a sports car, with his arms folded. The message couldn’t be clearer: Buy this album if you are a twat.

Basshunter’s music falls along the same patterns, ‘the bass’ which he is apparently hunting, yet features in all of his songs, suggesting it’s not a full-time job, and some terrible lyrics involving a girl either staying or going. As the first single from Bass Generation demonstrates.

Yet that identical bassline features on the next track, with a variant popping up on the two that follow that and so on. It’s repetitive soulless garbage for people who don’t like music, but need something to play loudly as they drive purple Hondas down the streets of towns and villages all over the UK.

For the rest of us it’s what classical music must sound like in hell, with the sole piece of joy coming from the Tetris sample in album closer ‘Plane to Spain’, but that is a fleeting moment of respite in this festival of crap. Listen to some of it, I guarantee* you will be reaching for the paracetamol within the hour.

*Guarantee void everywhere.

El tiempo pasa y en estos momentos lo que queda son sólo impresiones. Pero bueno, ahora que estamos más tranquilos disfrutando de los fondeos de estas islas
venezolanas, voy a estrujar un poquito el cerebro para recuperar ese tiempo pasado.
Ya empiezo a recordar y creo que me quedé a la espera del lanzamiento de un cohete en Kourou, lanzamiento que finalmente se retrasó pero no por ello nuestra
partida de aquel país extraño, difícil de entender y terriblemente caro.

De Kourou partimos otra vez hacia Illes du Salut para limpiar el casco y librarlo de toda clase de molusco que lo entorpeciera para su navegación.

 Una vez el casco libre de caracolillo, abandonamos las islas para dirigirnos a Paramaribo (capital de Surinam), país que con tal nombre no se podía dejar de visitar.
La idea inicial era pasar una semana para comprar comida y así seguir rumbo al Caribe pero la semana pasó a ser un mes. Mes fondeado en un pequeño pueblo
(Domburg) provisto de todo lo necesario con 8 barcos más (la mayoría holandeses) con los cuales pasábamos veladas interminables alrededor de una mesa bebiendo cervezas y saboreando platos hindús.

 Todo ello acompañado de pescadores también holandeses que estaban encantados con tanto europeo no vistos hasta la fecha en el río Surinam.
Fue un mes tranquilo en donde la urgencia de sacar el barco del agua para pintarlo, desapareció ya que al estar en agua dulce, todo aquello que llevábamos del mar, murió.

Además al estar en agua dulce, era fantástico poder bañarse a cada momento, limpiar la cubierta, ropa y demás con el agua del río sin preocuparte en el consumo.
Pero volvamos a esas impresiones de lo que ha sido Surinam. Tal vez lo que en su momento me llamó más la atención fue ir remontando el río que entre el silencio del día tranquilo y apacible solo roto por algunos pájaros y por el Talula abriendo el agua en su proa, fue el ir percibiendo a lo lejos música china que iba inundando toda la ribera. Al llegar al fondeo y bajar por primera vez a tierra, mirabas rostros a cada cual más exótico. Rostros asiáticos que compartían el lugar con otros de raza negra. Además si paseabas por la carretera, veías que ésta iba seguida
a sus lados por canales llenos de nenúfares y flores de loto que armonizaban con esas gentes que habían decidido vivir en sus países de origen pero en América. Total, que Surinam es un país donde el contraste racial es sorprendente y su paisaje es básicamente selva amazónica llena de toda clase de aves, flores, árboles y reptiles, surcada por ríos y afluentes siendo esta la vía principal de comunicación.
Pero la llegada a este país no resultó tan armoniosa y tranquila como mis palabras hasta ahora. Desde el mar hasta la desembocadura del río, hay un canal de señalización que te previene de poder quedar embarrancado en medio del mar. El seguir exactamente la ruta marcada era básico ya que fuera de él, la profundidad no superaba a veces el medio metro. Así que empezamos a navegar hacia la entrada sin tener demasiado claro de poder alcanzar nuestro objetivo.
Eso si, de momento nos atravesamos delante de un mercante que le fue de poco para envestirnos.
Al día siguiente intentando fondear fuera del canal (para llegar a nuestro objetivo pasaron más de dos días), y complicándose la maniobra por estropicio del
motor que sube y baja el ancla, repasamos una boya de hierro que nos dejó a Joan Antoni y a mí, con el corazón “partío” y “partía” quedó también un poco la
fibra del Talula. Fibra que se volvió a repasar intentando sacar el barco en un varadero donde habían sacado sólo dos veleros en 10 años y como ya podéis
imaginar, del asunto no controlaban. Una vez “repasada” la fibra y volviendo a fondear el barco fuera del canal y decidiendo que ahí no lo intentábamos dos veces, unos pescadores atravesaron todo el río con su red sin divisar que a lo lejos tenían dos barcos fondeados. Total, que el otro se salvó por centímetros pero a nosotros nos cogió de pleno la red que envolvió todo el barco en pocos
segundos. Los pescadores la fueron recuperando rápidamente y cuando llegaron hasta nosotros, el más joven empezó a amenazar a Joan Antoni con un cuchillo
culpándonos de estar en medio de su red. Joan Antoni más enfadado que impresionado, saltó a su canoa e intentó romperla para podernos liberar de ella
mientras el joven seguía protestando y pidiéndonos dinero por las roturas. Finalmente pudimos librarnos de la red y los pescadores se fueron corriendo detrás de ella ya que al romperla, salió disparada río abajo.
En fin, aventuras que te dejan un mal sabor de boca pero que supongo, forman parte de este viaje.
Y así con el Talula más rallado que antes, partimos de Surinam hacia Tobago con nuevo tripulante que recogimos de la calle con solo un mes y medio, llena
de pulgas y que si no la hubiésemos cogido, hubiera muerto en pocos días por desnutrición o en la cazuela de cualquier familia china.

Tobago no sé si será la isla más bonita del Caribe pero la magia que tiene esta isla, hasta ahora no la he encontrado en ninguna otra. Playas paradisíacas,
aguas que parece aire, peces de mil colores en medio de corales exuberantes, palmerales y arenas blancas en medio de valles llenos de bambú y riachuelos que
afloran por todas partes y que desembocan en las playas haciendo del baño una delicia finalizada con agua dulce. Delicia que sólo duró 10 días ya que
tuvimos que abandonarla para irnos a Trinidad y sacar finalmente ahí el barco del agua.

Trinidad fueron días de 12 horas lijando tapada de arriba abajo siendo las jornadas interminables y tremendamente agotadoras. Pero todo lo que se empieza se acaba y la foto que os enviamos fue justo en el momento que acabábamos de lijar el barco.

 Pero aquí no se acaba todo ya que luego vinieron días de pintura que siempre veíamos amenazados por los constantes aguaceros que iban entrando y que nos hacía temer que cayera justo en el momento menos apropiado, estropeando todo lo pintado.

De allí pasamos a Los Testigos, unas islitas venezolanas paraíso de las aves en donde la tranquilidad fue la nota predominante. Fueron días de baños en aguas más bien fresquitas, paseos por la playa y horas de lectura que fueron muchas ya que  al final, tuvimos días de mucho viento desapacible por lo que la Nochebuena y la Navidad las pasamos prácticamente encerrados en el barco. Al final decidimos cambiar de isla y nos fuimos a Margarita, isla que no destaca ni por sus fondeos ni por su belleza pero si por ser puerto libre en donde la botella de ron cuesta dos euros y la cerveza es más barata que la Coca Cola.
Y ahora estamos en una isla bastante pequeña llamada Cubagua, en la que destacaría su vegetación agreste (llena de cactus por todas partes), pero con playas encantadoras en donde puedes encontrar toda clase de almejas (guacucos y chipi chipis) y sobre todo ostras.
Aunque éstas últimas se precien por aparecer en la guía náutica de Venezuela y no en los fondos marinos de esta isla que es donde corresponde.
La única nota triste de esta isla, es que aquí viven unas cuantas familias cuyos recursos económicos son mínimos por lo que se te hace difícil estar disfrutando de un roncito al atardecer en tú barco y ver a esas pobres gentes pasando hambre. Pero bueno, mañana iré a tierra con comida para los niños y con paracetamol para la abuela ya que hoy se me quejaba del dolor de brazos cuando lava la ropa y
lo bien que le va tomarse luego esa “pastillita” (un paracetamol ) que otro barco le había dado anteriormente.
En fin, realidades e impresiones que vas viviendo a lo largo de este camino y que te enseñan a ver la otra cara de la moneda aunque para nosotros esto sea, como
decía Andrei (un holandés simpatiquísimo que se encontraba también en Domburg) a última hora de la noche y cuando ya nos íbamos todos a descansar a
nuestros barcos y en castellano: “mañana será otro día en el paraíso”.
