<p>(januar 2009) skiver forfatterene,  at i en undersøgelse af 14248 kvinder var den samlede graviditetsrate for de par, der gennemførte op til 6 IVF behandlinger 72 % .  Op til 86 % af de kvinder der var under 35 år fik et barn, mens kvinder over 40 år opnåede op til 42% et barn ved behandlingerne.</p>
<p>Dette sammenholdt med Professor Roy Homburg´s opgørelse over israelske kvinder, der blev behandlet med IVF op til mere end 10 gange, gør at vi i dag må sige at fertilitetsbehandlingen er blevet meget effektiv, og at resultaterne ikke falder før efter 6-8 behandlinger, i modsæting til den opfattelse man havde tidligere, at efter 3 behandlinger faldt muligheden for en graviditet.</p>
<p style="text-align:left;">Disse resultater, støttes af de nyligt fremlagte resultater fra Copenhagen Fertility Center ( de sidste 1000 behand-linger), hvor også op til 20% af kvinder over 40 år blev gravide per behandling.</p>

<title><![CDATA[Royal Mail, Take 2?]]></title>

<content:encoded><![CDATA[<div class='snap_preview'><p><img class="alignleft size-medium wp-image-577" title="Royal Mail" src="http://cleverlytortured.wordpress.com/files/2009/11/royal_mail_sorry_you_were_out_large1.gif?w=300" alt="Royal Mail" width="300" height="212" />They say you should never go back.  Nothing’s ever the same twice.  And while I agree with that to a certain extent, it’s still not stopping me from going to see a man about a job tomorrow morning.  And that’s purely because I need a change of scenery.  Although it was fun working as a PCO driver for a year or so, the novelty soon wore off and you come to realise that work will always be work.  No matter what you do.<!--more--> Fuck the devil’s ‘W’ word!</p>
<p>Plus, to be honest, it’s no longer financially feasible working as a private hire driver.  The two MOTs a year are bad enough, but on top of that TfL still want to inspect your car to make sure it’s roadworthy every, too.  Again, every 12 months.  Now correct me if I’m wrong but isn’t that what the MOTs are for?  Oh yes.  They charge you a nice little fee for the pleasure, too.  Another £100 or so just for a shiny little sticker.  I remember the first time my car had an ‘inspection’ in Croydon shortly after I bought it.   The geezer opened all the doors, the boot and the bonnet, he had a quick peak inside, took a walk around my car and that was it.   It took him 10 minutes, tops.  It bared a resemblance to my little 3-year-old nephew inspecting his Fisher-Price &#8216;My First Car&#8217; for the very first time.  A few walks around the exterior, pop open the doors, job&#8217;s a good&#8217;un.</p>
<p>A nice little earner that, huh?  Just think how many times a day they do it.   300+ days a year.   £100 a pop.   I’ll let you do the maths.   It’s like everything in London nowadays, though.  It’s all about the money.   Safety, my arse.  The world’s been overrun by greed.   And don&#8217;t even get me started on a litre of petrol touching the £1.10 mark again.  When I first started doing this I was spending over £130 a week, on fuel alone.  A mortgage don&#8217;t have nothing on the amount of money I was paying those petrol stations.  BP Connect became my home away from home.</p>
<p>However, it might just sound like I’m trying to convince myself.   Maybe I am.   I guess I know deep down in my heart of hearts that it’s not what I want.  A return to Royal Mail.  When I left back in 2007 I swore to myself that I would never, ever go back.   Unfortunately though, bills down pay themselves.   Desperate times call for desperate measures.   And right about now my financial is critical.   However, this is all very hypothetical.   I doubt they’ll even take me back.   Let’s just say my departure was far from amicable.   If they do though, I’ve chosen to work nights (22:00-06:00), which could in fact be a blessing in disguise.   That way I won’t be spending all of this winter, up all night, waging wars on myself.   The relapses have already begun.  I’ve been suffering from serious insomnia the past few weeks.  And the past few days it&#8217;s olny got worse.  That doesn’t bode well, that I’m starting to feel as much anxiety at the prospect of going back to the sinking ship that is the Royal Mail as I did when I was working there before.   The same symptoms are already starting to resurface and I’ve not even been for the interview yet.</p>
<p>It’s a shit hole.  However, I’m not saying that I’m better than Royal Mail.   I’m not.   That’s through my own doing, mind, no-one else’s.   I should have tried harder at school.  Lesson learnt, I guess.  Hindsight is such a wonderful thing.  However, what I am better than, is the people who work there.   If it’s anything like Richmond, the majority of workers there were fat, balding, middle-aged men who were both extremely racist, and sexist.   Unfortunately it’ll be the Jubilee Mail Centre in Hounslow so I’m guessing it will be.</p>
<p>Do I really need to have any more dealings with these people?   Especially in the workplace.   And even more so at night when it&#8217;s late.  I&#8217;ll be tired, other people will be tired, and not to mention high, via the caffeine from their coffee consumptions.   Crass remarks will be thrown around willy-nilly and I might just fly off the handle.</p>
<p>I’ll just wait and see what they have to say for themselves tomorrow.</p>

<title><![CDATA[Few cataract complications and their treatments]]></title>

<description><![CDATA[A posterior capsule opacity (PCO) is the most common among those few cataract surgery complications,]]></description>
<content:encoded><![CDATA[<div class='snap_preview'><p>A posterior capsule opacity (PCO) is the most common among those few cataract surgery complications, including eye inflammation, infections and a dislocated IOL. The reason for a PCO comes from the lens capsule, which will be maintained during a surgery. About 20% of patients have haziness on their intact posterior capsules, so that even if the lens has been replaced with a successful IOL, the vision is still blurry. A PCO is not a “secondary cataract”. Cataracts will never recur once removed.</p>
<p>A PCO can be treated with a YAG laser capsulotomy, which takes a few minutes and is safe, effective and painless. During the procedure, your doctor will remove the hazy posterior capsule with a laser, eliminating any incision. Patients can resume normal activities immediately after the surgery, with the help of anti-inflammation eye drops. Only 1% of patients may suffer from a detached retina from a YAG laser capsulotomy.</p>
<p>Another complication for a cataract surgery can be mal-positioned IOL, which may lead to double vision or severe visual acuity decrease. There are two underlying reasons that may cause dislocated IOLs: broken capsule bag and dislocated capsule bag. Since the IOL is placed within the extremely thin capsule bag, a broken bag due to accidents may lead to dislocated IOL. The other reason comes from a dislocated capsule bag, which will surely lead to dislocated IOL.</p>
<p>Dislocated IOLs can also result from operational mistakes during the IOL positioning process. Once an IOL dislocation is found, a second procedure to reposition the lens will be taken soon. With proper treatment, dislocated IOLs won’t bring permanent vision problems.</p>
<p>Although the most severe complication may lead to vision loss, complication-free cataract surgeries account for 98% of the total. <p style="line-height:14.25pt;"><span style="font-size:10pt;font-family:Georgia,serif;">I min praksis ser jeg ofte PCO piger, der som et af hoved problemerne kommer og fortæller de har en stor træthed, specielt om eftermiddagen.</span></p>
<p style="line-height:14.25pt;"><span style="font-size:10pt;font-family:Georgia,serif;"> Dette kan man forklare ud fra biologien, idet hjerne ikke kan tåle den forhøjede insulin, som netop er problemet for PCO piger. Når de spiser sukker, eller noget som kan laves om til sukker i kroppen ( dette kan være kartofler, hvidt brød, ris etc.) så stiger insulinet i blodet hos PCO piger meget højt.</span></p>
<p style="line-height:14.25pt;"><span style="font-size:10pt;font-family:Georgia,serif;"> Ofte kan man forbedre denne træthed ved motion ( der nedsætter insulinet i blodet) og ved diæt. Denne diæt skal ikke indeholde de hurtige kulhydrater, der netop får insulinet til at stige. I en del tilfælde er dette ikke nok og så må man supplere med medicin behandling som f.eks. metformin.</span></p>
<p style="line-height:14.25pt;"><span style="font-size:10pt;font-family:Georgia,serif;">Det er derfor vigtigt at går man rundt som PCO patient med træthed at få undersøgt om der er noget man kan gøre for dette.</span></p>

<p><strong>From “Foreword”:</strong>  Dr Donald and Eloise Bosch have often been asked why they came to work in Oman.  Their answer is simple.  They are members of the Reformed Church in America, a mainline ecumenical Protestant denomination, which has a long history of providing services throughout the world.  It has a department designated “World Ministries” which seeks to place professionals where their skills are most needed.  Hence when they were ready to leave America, Oman needed them.  Except for the time spent relieving medical colleagues in Iraq, Kuwait, and Bahrain and occasional furloughs to America, they have lived and worked in Muscat.</p>
<p>“As a result of their seafaring skills, the country as a whole developed an open, friendly attitude toward foreigners.  Unlike some other Arab communities which remained desert-bound, the Omanis sailed to East Africa, India and even as far as China, bringing back tales of different societies and people.  Hence the wonderful stories, some perhaps slightly exaggerated, like that of Sinbad the Sailor.” (Chap. 1, pg 5)</p>
<p> <strong><span style="text-decoration:underline;">Interesting facts:</span></strong></p>
<p>-The first medical person to start working permanently in Oman was a woman, an American nurse, Mrs James Cantine.</p>
<p>-In the 19<sup>th</sup> century, slaves could receive freedom by clasping the flagpole in the courtyard of the British Consulate!</p>
<p>-All the employees of the Women’s Hospital in Muscat were women except for the guard and a maintenance man.</p>
<p>-Sometimes patients to the mission hospital paid with a goat, eggs, baskets of dates, chickens or even gazelles.</p>
<p>-Being the only school to admit girls, the Mission school was the first co-educational school in Oman.</p>
<p>-The road between Muscat and Muttrah was the only 3 miles of paved roads in all of Oman in the 1960s.</p>
<p>-Dr Bosch and Eloise arrived in Oman in 1955.  At that time, the population of Muscat was estimated to be 5,000-7,000 and that of Muttrah, about 10,000-12,000.<img class="alignnone size-full wp-image-1973" title="donald and omani ladies" src="http://andyinoman.wordpress.com/files/2009/10/dscn9845.jpg" alt="donald and omani ladies" width="500" height="375" />  Don with some local Omani ladies showing off their silver.  (Their are some incredible photos and stories in this book!)</p>
<p> I imagine how their faith must have been tested and tried as they sent all their stuff to Oman.  Eloise reports, “Unfortunately, some things were pilfered from our boxes.  Some of the clothes we had purchased for our children and some of Don’s medical certificates and diplomas were stolen.  Other personal effects which couldn’t be replaced were also missing.  But we were grateful that most of our necessities survived.” Attitude changes everything.</p>
<p>“As we arrived at the Mission compound, we were thrilled to see palm branches laced over the entrance and a big sign “Ahlan Wasahlan” (welcome), put up just for the Bosch family.  Many people rushed out to shake our hands and hug us.  We immediately felt that this was where we belonged.”</p>
<p>Dr Bosch – “Here I was, a complete stranger to him and yet he was willing to put his life in my hands.  The answer to such complete confidence lies in the reputation of our colleagues and predecessors, the members of the American Mission, who laid a foundation of goodwill and concern for others.  We soon discovered that Omanis don’t forget.  If you do them a good turn, they will remember it and one day return good for good. (pg 17)<img class="alignnone size-full wp-image-1975" title="Mutrah souq scribe" src="http://andyinoman.wordpress.com/files/2009/10/dscn9847.jpg" alt="Mutrah souq scribe" width="500" height="375" />  I loved the story of the illiterate man who brought a letter to a scribe at the Mutrah Souq.  Because it was a personal letter, he didn&#8217;t want the scribe to hear the message, so he asked him to put ear plugs in his ears before reading so that he wouldn&#8217;t hear the contents of the letter!  <img src='http://s.wordpress.com/wp-includes/images/smilies/icon_smile.gif' alt=':-)' class='wp-smiley' />   Classic!</p>
<p>“Hardly any of the Omani staff had been formally educated beyond the sixth standard, yet with careful guidance and training they were able to assume responsibility for most of the work in the hospital.”</p>
<p>“We would encounter amusing situations when we spoke a grammatically correct sentence which the Omani national apparently would not understand, but when the interpreter spoke the the identical sentence, he understood immediately.  The most likely reason for this was that the Omanis did not expect an expatriate to speak Arabic.”<img class="alignnone size-full wp-image-1976" title="bosch family" src="http://andyinoman.wordpress.com/files/2009/10/dscn9846.jpg" alt="bosch family" width="500" height="375" />  The Bosch family in November of 1962</p>
<p>“We had very serious and sometimes fatal problems with a local custom in which the women, or perhaps their mothers, would pack the birth canal with rock salt immediately after the delivery of a baby.  The ostensible purpose of doing this was to prevent infection and tighten the stretched birth canal.  But the end result was sometimes catastrophic because the salt was an irritant which scarred the cervix (lower end of the uterus), so much so that with a subsequent pregnancy the cervix would not dilate (open) and the baby couldn’t be delivered.” (pg. 20)</p>
<p>“Yet another interesting case was that of a young lad who was brought in with a huge gash in his abdomen, carrying all of his intestine in his loin cloth.  He had been fishing in his little boat when a sharp-nosed fish jumped from the water, stabbed him in the abdomen, and fell back into the water leaving the boy with an enormous abdominal gash through which his intestine naturally extruded.  Alone on the boat, he somehow managed to paddle himself to the shore and was immediately brought to the hospital.  In the operating room it was determined that the intestine itself had not been damaged, hence all that was required was to clean the abdomen, put the intestine back where it belonged and sew him up.  Ten days later, he walked out of the hospital and promptly returned to his little boat, figuring, I suppose, that no self-respecting fish would repeat such a dastardly act.”</p>
<p>“Interestingly enough, many of the Bedouin patients rejected an enclosed room in favour of a spot beneath a tree on the compound.  These folks had lived all their lives in fresh air and could not tolerate a closed room.  So we had to number the trees to mark the patient’s locality.  The patient’s chart would read: Under tree 1, Under tree 2, Under tree 3 and so on.”</p>
<p>There is an fascinating account in Chapter 4 of how slave traffic was a serious problem at the turn of the century between East Africa and the Gulf countries and how a series of events led to the Freed Slaves School in Muscat!<img class="alignnone size-full wp-image-1972" title="family book store" src="http://andyinoman.wordpress.com/files/2009/10/dscn6832.jpg" alt="family book store" width="500" height="375" />  That should be enough to wet your appetite to go out and buy this excellent book!    Available at the &#8220;Family Bookshop&#8221; in Noor Plaza, Madinat Al Sultan Qaboos (MSQ),  Al Khuwair.  If you attend church in Ruwi on Sunday evenings, you can even get this lovely couple to sign the book!  <img src='http://s.wordpress.com/wp-includes/images/smilies/icon_smile.gif' alt=':-)' class='wp-smiley' /> </p>

<title><![CDATA[Social Networking and an Impartial Public Service]]></title>

<p>Last week there were a couple of stories in the Ottawa Citizen (on <a href="http://www.ottawacitizen.com/business/Private+lives+public+servants+becoming+issue+Public+Service+Commission/2088376/story.html">Thursday</a> and <a href="http://www.ottawacitizen.com/business/Internet+challenge+impartial+public+service/2089508/story.html">Friday</a>) that touched on impartiality in the federal public service.</p>
<p>From the Friday story, titled <a href="http://www.ottawacitizen.com/business/Internet+challenge+impartial+public+service/2089508/story.html">Internet age a challenge to impartial public service</a>:</p>
<blockquote><p>The commission [ie, the PSC] concluded the young PCO analyst who posted his support for the Liberal party had crossed the line with &#8220;improper political activity,&#8221; but found no evidence that this affected his ability to do his job.</p>
<p>The employee wasn&#8217;t disciplined for his Facebook entry other than receiving a letter from the commission warning against &#8220;political activities&#8221; that could undermine his ability to do his job impartially &#8212; or leave that impression.</p>
<p>The unnamed bureaucrat wasn&#8217;t a senior executive but he did work for PCO, which has a more politically sensitive mandate than other departments.</p></blockquote>
<p>Some thoughts:</p>
<p>1. Tempting to write this off as a PCO thing, since life is different at &#8220;the centre.&#8221; But that would be a mistake &#8212; especially for people working in communications and marketing, as we routinely bump up against the political/administrative divide in government. Impartiality is important throughout the bureaucracy.</p>
<p>2. Nor is it an age thing &#8212; leaving aside the fact that &#8220;young PCO analyst&#8221; is pretty ambigous, it is clear that social networking is increasingly popular among all workplace demographics.</p>
<p>3. In one sense, nothing has changed. Listing your political affiliation on a social networking site is analogous to wearing a party badge or political button on your coat. If you want to be seen as impartial, don&#8217;t do it.</p>
<p>4. Social networks like make it all too easy for this kind of thing to happen. When I signed up for Facebook, I remember that the &#8220;political affiliation&#8221; question was part of the registration process. Incredibly easy for an inattentive user to pick something that they will later regret since their focus is on getting signed up.</p>
<p>5. Yes social networking is redrawing the line between public/private. My rule of thumb &#8211; err on the side of caution and treat online participation as more public than private. I assume that how I present yourself online always has a an impact on my personal reputation and the reputation of my employer. (Writing this, it occurs to me that it&#8217;s probably no different in the private sector.) By the by, the Office of the Privacy Commissioner has been doing great work on <a href="http://blog.privcom.gc.ca/index.php/2009/09/25/there-are-other-social-networks-with-privacy-concerns/">privacy issues around social networking</a>.</p>
<p>6. As cases like these start to pile up, it will be tempting to simply block off the social web in government workplaces. This would be a mistake, as not only would business value be lost, but it would just drive the activity &#8220;underground.&#8221; People will find a way regardless &#8212; so better to encourage responsible use than to make people do it behind your back. The recently reanimated <a href="http://www.stopblocking.org/?p=40">StopBlocking.org</a> has great info on why it&#8217;s better to allow your people access.</p>
<p>7. On policy: I don&#8217;t feel that new rules need to be invented. There&#8217;s enough policy out there already. I am in favour of education and guidance though &#8212; to help us navigate through this <a href="http://spaghettitesting.ca/2009/10/08/we-still-havent-learned-how-to-use-social-media">new and unfamiliar terrain</a>. Whether in terms of reminding employees of their responsibilities as public servants or by having early adopters and leaders showing us how it&#8217;s done. Or creating a touchstone of some sort that interprets existing rules in the light of new realities presented by the social web (for this last one, <a href="http://twitter.com/spaghetti_p/statuses/4684213832">I&#8217;m looking at you TBS</a>).</p>

Das Wetter gestaltete sich, trotz unsicherer Vorhersage, zunächst recht gut und erst am Nachmittag setzte etwas Nieselregen ein. Wir haben uns gerade auch deswegen über die sehr hohe Resonanz gefreut. Die meisten unserer Yachten in Heiligenhafen lagen am Steg für Ihren Besuch bereit und wir hatten eine Vielzahl guter und informativer Beratungsgespräche.</p>

<content:encoded><![CDATA[<div class='snap_preview'><p>LOW GI (Glycaemic Index) is super important for someone with PCOS.  It&#8217;s important for controlling insulin resistance and will help you lose weight and decrease (or get rid of!) your PCOS symptoms.</p>
<p>To give you a little bit more idea of what a low GI diet is, keep reading!</p>
<p>The way in which carbohydrate foods are rated according to the effect they have on glucose levels in the blood is called glycemic index. This is more generally known as GI.</p>
<p>High GI foods are rapidly absorbed and bring on a swift rise in blood pressure and blood glucose levels. Low GI foods, work in the opposite way by breaking down in a more gradual manner and stabilizing blood glucose levels. This is important as it helps to avoid the seesaw effect, which may lead to a constant hunger, as well as mood and energy swings. Because foods low on the GI take longer to be digested they release energy into the bloodstream at a slower and steadier rate and this supply of energy helps to prolong that &#8220;full&#8221; feeling. We all know that if we feel satisfied for longer amounts of time, we are less likely to snack in between meals.</p>
<p>On the other hand, however, foods high on the GI can promote wildly variable blood sugar levels, which can cause the pancreas to over-produce insulin and this, in turn can lead to illnesses like Syndrome X, hypoglycemia, and type II diabetes. Research has also shown that there is evidence to suggest foods lower on the GI can help to improve the good cholesterol levels and this is know to lessen the risk of heart disease. </p>
<p>The majority of low GI diets don&#8217;t insist you only eat low GI foods, but they do encourage you to add more foods with a low GI as part of your healthy eating regime. By simply including a low GI food in your meal, it will have the effect of lowering the glycemic index for the complete meal. People who have an active lifestyle should use an equal balance of low and high GI foods to ensure they have the necessary energy to carry out their exercise</p>
<p>High glycemic index foods include many carbohydrates such as baked goods, bread, cereals, pasta, and rice whereas low glycemic index foods are more about fruits, legumes vegetables, and whole grains. Nonetheless there are differences within these broad categories. For instance, both bananas and potatoes would be considered to have a mid ranging GI. How food is processed and cooked also has an influence on the GI levels. Instant oatmeal has a higher GI than traditional rolled oats because the processing lets the starch be more effortlessly exposed to digestive enzymes.</p>
<p>While heightening the amount of low GI foods in your diet can bring on weight loss and improve your general health and well-being this is not the only thing that should be taken into consideration. An example of this is chocolate which is a low GI food but full of energy with little nutrients so it is not going to help anyone lose weight. However, on the whole, GI is a useful tool for identifying foods which have low levels of saturated fat and are rich in nutrients.</p>

Mai oder Sommerwochen gibt es jetzt schon reichlich Nachfrage, es lohnt sich also schon einmal zu planen, wir helfen Euch wie gewohnt bei der Suche nach der richtigen Yacht und dem richtigen Revier.<img class="alignleft size-medium wp-image-219" title="Pauline" src="http://pcostsee.wordpress.com/files/2009/08/pauline.jpg?w=300" alt="Pauline" width="300" height="199" /></p>

<p>Ab 2010 wird nach heutigem Stand für Yachten mit einem Funkgerät an Bord für den Skipper dieser Yacht ein Funkzeugnis gefordert. Bei Yachten über 12m Länge ist der Besitz z.B. des SRC nun aufgrund der Ausrüstungspflicht mit einem Funkgerät dringend erforderlich.</p>
<p>Aber keine Sorge, bei  Yachten bis 11,90 m Länge werden wir für alle unsere Kunden ohne entsprechendes Funkzeugnis eine Lösung anbieten weiterhin eine Yacht chartern zu können. Es gibt viele Segelyachten bis 11,90 m ohne Funkgerät an Bord. Fragen Sie uns!</p>
<p>Dennoch empfehlen wir den Erwerb des SRC-Funkzeugnisses im Winter 2009 oder Frühjahr 2010 zu prüfen,  hier eine <a href="http://www.pc-ostsee.de/yachtcharter/infos/funkzeugnis.html" target="_blank">Auswahl an Segelschulen.</a> </p>

Det kan dröja innan man får någon storlek på magen. Så jag får vara nöjd med putet så länge. Det finns många som googlar och lägger ut texten om sina livmödrar. Jag ser en hel del humor i det.<br />
Tex. kan man få lära sig att vi med bakåtlutat livmoder blir mer bajsnödiga när vi har mens. Och då drar jag så klart snabba slutsatser till att jag är mer bajsnördig nu. Mycket praktiskt att veta.</p>
<p style="text-align:left;">Jag har en teori om att man borde kunna dra paralleller mellan sexuell läggning och kroppliga symptom.<br />
En hobbyteori jag har är att det finns ett samband mellan min (lindringa) <a title="PCO" href="http://www.pco-s.com/omPCO/omPCO.html" target="_blank"><strong>PCO</strong> </a>och min <strong>sexualitet</strong>. När sen min kära vän flatan S visade sig också ha PCO i veckan fick jag vatten på min kvarn.<br />
Dock kan jag inte hitta samband med lutningen på livmodern. Trots att en annan flat-vän jag har är bakåtlutad.<br />
Att googla och leka doktor är ett av mina nöjen vars resultat jag inte lägger så stor vikt vid.</p>
<p style="text-align:left;">Det där blodprovet var det ingen fara med. Det var mest en hel massa ångest en stund. Nu är harmonin åter.</p>

(No doubt to escape the heat of Oman!!!
