If you read the print version of the NZ Herald today you would have seen featured on the front page a miraculous case of a man [Rom Houben] recovering from a persistent vegetative state and communicating with the world through a touch screen with the help of a carer. The topic of persistent vegetative state (PVS) is an interesting one and has received increasing attention in recent years. It would seem that this man was incorrectly diagnosed after an accident as being in a PVS while at the time of the accident it is more likely that he was in a minimally conscious state (MCS). A fine distinction sometimes and an excellent summary of the differences between the two diagnoses and the difficultly of accurately deciding between them can be found at the Science Based Medicine site.

Essentially a PVS is defined as the patient exhibiting no signs of consciousness, as with everything, whether you find something is dependent on how hard you look, simply opening a couple of drawers and glancing in the cupboard may not turn it up. In determining a case of PVS a more thorough search will reveal fewer legitimate cases as you may find extremely subtle signs of intermittent consciousness that will then flip the designation to a MCS. This process is also dependent on the sensitivity of the equipment used to perform the examination, the sophisticated scanning technology we have today simply did not exist 20 years ago. This equipment is the equivalent of rummaging around in the back of the couch and looking behind the fridge.

That this man was unfortunately diagnosed incorrectly is not in dispute, we have made significant advances in brain imaging technology that allows us to determine activity quite well. The issue here is the man’s ability to communicate so coherently and poetically. After so long without mental stimulation it seems bordering on the fantastic that this could be the case. When watching the video of the touch screen being used to bring this man’s thoughts to the world it seems very close to a practice known as Facilitated Communication, (this is actually confirmed in the TimesOnline article) this consists of a facilitator supporting the arm or hand of a subject ostensibly to allow them to then choose letters and words themselves which otherwise they would not have the strength or the focus to do.

The difficulty here is that this technique is very susceptible to the unconscious influence of the facilitator. In this way it can seem as though it is the patient communicating when in reality it is the thoughts of the facilitator that we are hearing. It is difficult to say for sure in this case, the video is ambiguous as to how much control the patient has over his movements so it is possible that we are indeed being exposed the inner world of a man with a very unique perspective but from the evidence shown it is equally plausible that the facilitator is the true originator of these words.

I would be interested in if any simple tests to determine the true origin of this material have been carried out, some of the suggestions I have seen elsewhere include swapping the facilitator for someone who does not speak the patient’s language, asking the patient questions that presumably only he would know, or asking the facilitator to leave the room while the patient is shown an object or told specific information and then seeing if this can be reliably produced after the facilitator returns. Any of these would help determine whether this man is truely communicating.

The print version of the Herald is mostly credulous in it’s coverage of this story but it appears that enough scepticism has filtered through the journalistic world that the online version has incorporated some of it. Better late than never.

I am sure you have heard of this story by now, but it is worth considering the case of the Belgian Rom Houben’s incredible recovery.  Actually, if you know the case, then you know that he has been aware of his surroundings all along.  For 23 years, he has been called a vegetable by the experts; yet, he has been aware of conversations about him, aware that his father passed away.  His body simply would not allow him to respond in any way.  Now, an expert has devised a way for him to respond.  For 23 years, the doctors were wrong about calling him a vegetable (PVS).  How many others were misdiagnosed?  Terri Schiavo?  Could we please decide from this point forward to call (and treat) humans, humans.  Call vegetables, vegetables.

CHIHIRO é uma cantora de R&B nascida em Fukuoka. Iniciou sua carreira musical como compositora. Em 2006, iniciou sua carreira como cantora. Seu primeiro single “Jewels” estreiou no verão de 2007.
No ano seguinte, em fevereiro, com seu single “Last Kiss”, Chihiro se tornou popular. Em outubro de 2009, seu terceiro single “Beautiful feat. Sweep /Bouquet” foi lançado se tornando um dos mais tocados.
Chihiro além de cantar também é modelo, designer e tem sua própria marca de jóias.

A man thought by doctors to be in a vegetative state for 23 years was actually conscious the whole time, it was revealed last night.

Student Rom Houben was misdiagnosed after a car crash left him totally paralysed.

He had no way of letting experts, family or friends know he could hear every word they said.

‘I screamed, but there was nothing to hear,’ said Mr Houben, now 46.

Doctors used a range of coma tests, recognised worldwide, before reluctantly concluding that his consciousness was ‘extinct’.

But three years ago, new hi-tech scans showed his brain was still functioning almost completely normally.

[...]Mr Houben said: ‘I shall never forget the day when they discovered what was truly wrong with me – it was my second birth.

‘I want to read, talk with my friends via the computer and enjoy my life now that people know I am not dead.’

What are we to make of the Terri Schiavo killers now? Is it morally praiseworthy to advocate for getting rid of inconvenient people? Shouldn’t we look on the misfortunes of others as opportunities to act out our love and compassion for our fellow man? Doctors can be wrong in their diagnoses.

Maaya Sakamoto, nasceu em Itabashi, 31 de Março de 1980, é uma seiyuu e cantora japonesa que iniciou a sua carreira em 1996 fazendo a voz da personagem Hitomi Kanzaki na série de anime Escaflowne. Aos 15 anos ela fez um concurso onde iriam escolher a voz de Kanzaki Hitomi, protagonista de Tenkuu no Escaflowne, e venceu. Além de fornecer a voz de Hitomi, ela também deu voz à música de abertura do anime, Yakusoku Wa Iranai. Ela também dublou diversos animes, como El Hazard, The Record of Lodoss War, RahXephon e Tsubasa Chronicle.

Maaya Sakamoto é dona de uma voz que encanta pessoas por todo o mundo, e já trabalhou em conjunto com a renomada compositora Yoko Kanno. Seu CD de maior sucesso foi o Single Collection – Hotchpotch lançado em 1999, em seus 19 anos. Outro trabalho digno de nota é o álbum Lucy, onde Maaya adota um visual realmente diferente do adotado em seus CDs anteriores. Mathematician Marcus du Sautoy is becoming an increasingly regular front-man for the BBC science documentary Horizon and, to date, his contributions have always been satisfyingly informative. The recent episode The Secret You is no exception.

In his quest to discover the underlying biochemistry and physiology of consciousness, du Sautoy visits a number of laboratories around the world where self-awareness and the notion of “the inner me” are being investigated. In doing so, he frequently participates in experiments; at one point he quips “another day, another scanner”(50:49).

There are a raft of ethical questions which arise from functional Magnetic Resonance Imaging (fMRI) and other neuroimaging methods, some of which I have written about elsewhere (see Disorders of consciousness: do state-of-the-art neuroimaging techniques shed new light on the brain-injured patient?).

For me, the most interesting ethical questions in the programme arise from the work of Professor Adrian Owen. du Sautoy and Owen discuss experiments conducted with patients in PVS, a Persistent Vegetative State (16:44 to 19:22, though the discussion makes most sense if you start at 15:30).

Previously our abililty to tell whether or not a PVS patient was genuinely conscious was constrained by the fact that they had no physical ability to demonstrate their awareness. In ground-breaking experiments, however, Owen and his colleagues have communicated with patients by asking them to imagine performing certain tasks, for example playing tennis, and using fMRI to show that the appropriate areas of their brains are activate. By developing this further, it is possible to get the patient to imagine two different activities which are clearly distinguishable from one another in terms of brain activity. These can then serve as proxy signals as “yes” and “no” answers to questions posed.

A patient could be trained to make certain areas of her brain active as a proxy answer to questions posed by researchers (e.g. by imaging she was playing tennis or walking around her house)

These experiments have revolutionised our understanding of brain-injured patients. In particular it brings into question the practice of withdrawing food and water from patients in PVS on the assumption that the are not aware.

Try to imagine the torture my sister endured in the last two weeks of her life. Being unable to defend herself, very likely aware of precisely what was happening and having absolutely no ability to escape it. For nearly two weeks, Terri was denied food and fluids. Not so much as an ice chip was given to her. It was pitiful, it was barbaric and it was clear that she was made to suffer. When my sister expired, she was a shell of her former self.

It is because of the suffering my sister and others endured that I believe the PVS diagnosis must be either reevaluated or completely abolished. With so many medical and neurological professionals admitting that there are inaccuracies and with the diagnosis being a death sentence, it is time to rethink disability and the way we regard it and our fellow human beings.

It is not too much of a burden to provide sick people with food and water.

Em 12 de dezembro de 2008, Supercell lançou “Hajimete no Koi ga Owaru Toki” na Nico Nico Douga e permaneceu em #1 na lista de ranking diário por 3 dias seguidos. Essa foi a última música pela Nico Nico Douga. Fizeram o major debut com a Sony Music Entertainment Japan com um lançamento profissional de seu álbum Supercell em março de 2009, agora atualizado, com mais músicas e um DVD. Supercell lançou seu primeiro single major “Kimi no Shiranai Monogatari” em 12 de agosto de 2009 com Nagi nos vocais. Este foi o primeiro lançamento do grupo sem Hatsune Miku no vocal. Um projeto em anime intitulado Black Rock Shooter será lançado em 2010 baseado na música e vídeo “Black Rock Shooter” destacando as ilustrações de Huke. Uma edição piloto do anime será lançada em DVD e Blu-ray em 30 de setembro de 2009. Ryo e Huke são colaboradores do projeto com a supervisão de Yutaka Yamamoto e seu estúdio de animação Ordet. Shinobu Yoshioka é o diretor do projeto. Black Rock Shooter é a primeira produção solo de Ordet com estúdio de animação.
