In the latest issue of the Institute for Safe Medication Practices (ISMP) Medication Safety Alert! n

In the latest issue of the Institute for Safe Medication Practices (ISMP) Medication Safety Alert! newsletter, the organization recognized third-year student pharmacist Sal Rivas for identifying and alerting them about a drug manufacturer’s website dosing error that could have resulted in parents inadvertently administering 10 times more than the recommended dosage of phenylephrine HCI 0.125% for children 2- to 6-years-old. The drug in question was a decongestant nose drop. His vigilance allowed the ISMP to contact the manufacturer who promptly corrected the error on the same day. Congratulations to Sal for using his knowledge and problem-solving skills to help the public avoid a potentially dangerous mistake.
