ORTHODONTIC UPDATE OCTOBER 2009 RICHARD CY SHIN, DDS MS DIPLOMATE, AMERICAN BOARD OF ORTHODONTICS On

ORTHODONTIC UPDATE OCTOBER 2009

RICHARD CY SHIN, DDS MS

DIPLOMATE, AMERICAN BOARD OF ORTHODONTICS

One of the most common questions that I get from the parents is “Doctor, when is the best time to start braces for my child?” I am sure that if either of the parents have had orthodontic treatment in the past, he or she would certainly want to make sure that the child is checked to see if they need orthodontic treatment or not.

Many parents are already confused by the mixed messages they received from many sources regarding the best time to start orthodontic treatment.

Is it best to start treatment when just a few of the permanent teeth have erupted (usually between 7 – 10 years)?

Or, when all of the permanent teeth have erupted (usually around 12-13 years)?

Or, is it best to start in primary “baby” teeth (before any of the permanent teeth erupt, i.e. under 6 years)?

The answer is there is not one best time for every child! Each child presents with a unique problem and therefore each child will need a specialized treatment plan beneficial to him or her.

The American Association of Orthodontists recommends that all children have an orthodontic check-up no later than 7 years. Most children, by 7 years, would have reached key landmarks such as: 1) eruption of the front permanent teeth, 2) eruption of the back permanent teeth “six-year-old molars”.

The orthodontists can identify any key problems with the relationship of these permanent teeth and recommend either to start an Early Phase treatment. This is sometimes called Phase 1 treatment or Interceptive treatment. The goal of the treatment is to improve any dental problems that these permanent teeth might pose to the future growth and development of the developing permanent teeth and the jaws. Usually, the magnitude of the problem will be reduced and in some cases eliminated and greatly help in the Comprehensive orthodontic treatment stage when all of the primary teeth have been lost.

Here are some of the common examples of the malocclusions (“bad bites”) that I see in children in this clinic.

ANTERIOR CROSSBITE

Here you can see that one (often more) front tooth is coming out in front its counterpart on the top. This can be particularly damaging to the health of the gums of the teeth in crossbite, as well as to the bite.

POSTERIOR CROSSBITE

This time, the crossbite is in the back of the mouth, where the top teeth on one side are biting on the inside of the bottom teeth on that side. Left untreated, a posterior crossbite can cause a permanent asymmetry to the bite and to the face, because the crossbite forces the growing lower jaw to position itself to one side. This results in relatively more lower jaw growth on the side away from the crossbite and relatively less lower jaw growth on the side of the crossbite. Some studies have shown that an untreated crossbite can be a contributing factor in TMJ (temperomandibular joint) problems in an adult.

CROWDING

In this picture, we see severe crowding in the lower arch. Not only does such crowding look unsightly, but it is extremely difficult to clean efficiently and effectively around such crooked teeth. Poor oral hygiene will result in gum bleeding, bone loss and ultimately, tooth loss.

OPEN BITE

An open bite is often associated with habits such as thumb or finger sucking, mouth breathing or with disharmonious growth between the upper and lower jaw. Instead of the forces of eating and chewing being distributed amongst all the teeth, those forces in an open bite patient are only on the back teeth. Such uneven distribution of forces may result in gum and bone loss around the back teeth, tooth fracture, early tooth loss and future TMJ dysfunction. Quite often, open bite patients have associated speech problems.

PROTRUSION OF THE TEETH OR “OVERBITE”

Excessively protruding front teeth are obviously more prone to trauma. What a heartache it is to see that patient in our office with their front teeth broken off because of a fall or a sports injury. Children with upper tooth protrusion are commonly very self-conscious of their appearance.

RETRUSION OF THE TEETH OR “UNDERBITE”

An underbite is a condition where the upper jaw is too small and often too narrow to match with the size of the lower jaw, and consequently, when the upper teeth come in, they fall behind the lower teeth. Here again, we see uneven distribution of forces with resulting premature bone and tooth loss, as well as TMJ dysfunction. If not treated in a growing child, an underbite will usually require corrective jaw surgery in the late teens or early adulthood.

SUMMARY

Growth is critical for an ideal treatment result and it is the best helper available to the orthodontist to position of the upper and lower jaws. When the both jaws are in harmony, the erupting teeth will grow into the best possible position.

OHmyGawd. More controversy…

source: delacroix@allkpop

A person named Shin, who writes songs / music for a hobby, had posted a song titled “Cause I want you” on a song writing community on the web and was contacted by Soh, a Cassie who was involved in Love Melody project. Soh told Shin, “I came across this song and I love it. Do you mind if I use this song so I can give a little gift to TVXQ? We’re not doing it to make money.” Shin happily agreed to this, but a month later he found out that the song which copied the melody of his song was being sold online. What enraged him further was the fact that Cassies posted the songwriter as “Eun Hyun,” not giving any credit to Shin.

Shin contacted the Cassies who led this project, and said that he doesn’t care whether they sell the song for money or not, he just wanted himself listed as the songwriter. He contacted Soh and Soh responded by telling him not to falsely accuse Cassies, and even said “We revised the melody and it’s just sampling, why are you making such a big deal out of this” and “Go ahead and sue us, lets see who gets hurt.”

Love Melody has already been advertised as a song written and sung by Cassies and Shin has no doubt suffered damage from this situation. Will he be able to hear any word of apologies from the biggest and most powerful fan club in Korea? We’ll see but the song writer seems to want justice. With TVXQ going through so much drama, it seems their fan club can’t escape it as well.
————————————————–

wahhh~ “Go ahead and sue us, lets see who gets hurt.” I think that’s the kind of mentality SM would have and not DBSK. Isn’t this ironic. And the piano part of the song sounds like…cut and paste to me who’s not really familiar with wth ’sampling’ is like if it’s not plagiarizing. Check it out at Allkpop. I think they should just write his name up in credits and apologize. Hinting that they might tap on Cassie strength to silence him is also just shameful and as a DBSK fan myself, I cannot say I’m with them on this one.

Hello all!!! My affections for Devil Survivor have recently been revived! I had stopped playing befo

Hello all!!! My affections for Devil Survivor have recently been revived! I had stopped playing before because of the whole Lucifer battle thing. I honestly couldn’t get past it. After clearing Gin & Haru’s route and Yuzu’s route twice (worst time of my life), I finally got to follow Atsuro’s and that’s where I encountered Lucy for the first time. I spent weeks—two month’s worth of them—leveling up my party to 99. After succeeding (with the exception of four demons… Haha) I still couldn’t beat that fool! So… I stopped playing for awhile. I just started again yesterday and decided to skip the damn battle. (The game soon regained its amazing, super-duper appeal shortly afterward) I’m psyched to try again, though, because I just learned that Lucy only has three forms and every time I lost, he was always on the last one. This was a major relief, seeing as I originally thought he would regenerate forever, making me lose all hope. Hah. Anywho, I’m aiming for Naoya’s route now. Yuzu’s route was just DUMB—which was not surprising, but still. I mean, running away? That girl is a moron—plus she never SHUTS UP. Gah! Not to mention Honda was a little creepy, but I mean, you can’t really blame him seeing as he lost someone so important to him, intensifying the fact that everyone within that circle was slowly going insane. I enjoyed Atsuro’s, though. He’s my super duper favorite for a non-bad guy, so it was fun kissing up and getting on his good side and stuff. Hehe. I feel bad for him sometimes, though. Naoya’s always bossing him around, telling him to shut up, calling him an idiot, etc. and he just stands there and takes it. Although, I must admit, it’s pretty funny; Naoya’s a jerk and Atsuro’s his eager little bitch. Anyway, the only thing about starting cycle after cycle is reliving those first few days over and over again. They’re so… BORING! Especially the first day and the day before. They are the WORST. How many times must I be forced to watch the prologue before they supply me with the “skip” option? I mean, seriously. And you’re letting me keep my lv. 99 demons, so what the hell do I have to catch these lv. 1 bastards again for…? Then later on, you have to start over with leveling up the humans. Oh, and I like to think of a new name for the M.C. for each cycle and eventually, one runs out of names… I wouldn’t really call that a fault, though, since that was my own choice. Mmkay, so I think I’ve ranted enough. I’m going to go play now and hopefully I’ll be done with this cycle in a couple days. Man, having to balance video gaming with school and stuff is a pain. Really makes me miss summer: Devil Survivor from Noon ’til 5 AM, every day… Beautiful moments… *sniffles*

Within an hour of delivering her baby she reported a lump in her breast. She was told that it was a blocked milk duct. For months and months when she would continue to question this lump with health professionals she would be told it was a blocked milk duct and given flyers on massage technique. Finally almost 6 months later they referred her to have the “blocked duct” drained and she was told that she had advanced stage breast cancer. She now, 4 years later, has had a masectomy but has cancer in her brain, liver, lungs and bones. Despite this she says now is the best time of her life as with this illness has come her appreciation for every minute that she lives and loves with her husband and two children (3 and 5).

She is living the purest form of joy and happiness- to be alive.

Shin splints affect most runners at one time or another. Shin splins are caused by the shin separating from the bone. This happends the majority of the time for 1 of 2 reasons. The first is that people do too much to fast. Muscles and tendons are unable to heel and become stong fast enough. The other reason is tight Soleus muscles. When the Soleus (one of the 2 calf muscles) is tight the shin must work much harder to pull the foot up causing overuse.
In order to prevent shin splints make sure not only to stretch but also to do PNF Stretching also known as foam rolling. Foam rolling as well as deep tissue message will work out knots in your muscles which static stretching is unable to help. The other thing to do is to make sure that you do not increase training volume more than 10% a week.

A top biker’s guide to picking the right motorcycle boots Motorbike boots are one of the most expens

A top biker’s guide to picking the right motorcycle boots

Motorbike boots are one of the most expensive investments a rider has to make, commonly costing between £50 and £400. So why bother?

Normal shoes, builders’ boots and walking boots can be dangerous when riding on powerful motor vehicles. They wear quickly, especially if you’re a rider who likes to ‘feel the road’ a bit, and they can become flimsy. Boot laces can get caught in pedals and normal shoes can easily be pulled off leaving your feet exposed.

In stark contrast – a pair of good motorcycle boots will be comfortable even at the 300 mile mark. They will protect your toes, ankle, calf and shin and save your feet from turning to hamburgers in a crash. And sometimes they look pretty good too. Good biking boots offer superb protection and with a wide range to choose from there is no excuse for riding in sandals. Here are the three main types:

Motorcycle touring boot
These are usually the least expensive biking boots and are perfect for the everyday commuter who goes out in all weather and wants his boots to be comfortable and safe. You can expect to pay £50-£100 for a good pair and they should last you a long time unless you have a crash.

The Ghostbikes Stealth touring motorcycle boot

Check they are waterproof – there’s nothing worse than wet feet. A professionally made pair should have a HIPORA lining which guarantees they will keep water out.

Just because touring boots are lower in price doesn’t mean they all skimp on protection. Look for boots that have a solid shin, ankle guards, with toe caps and heel caps to keep your toes on your feet. Otherwise you might as well ride bare-foot.

Motorcycle sports boot
This footwear is ideal for bikers who feel the need for speed and also want to look the part while they’re cruising. Like designer trainers, this style of footwear often comes in a wide variety of colours or styles and is intended to look great and fit like a glove. The price ranges from between £70-£170.

The Ghostbikes Neo motorcycle sports boot

They should still have all the protective features of a touring boot.

They should be comfortable. A good boot doesn’t put too much pressure on your feet but should grip you well enough to stay on even if you tug them hard.

Motorcycle racing boot
Racing boots are for serious riders who push their kit to the limit and demand all the latest technology and protection to keep them safe and improve their track performance. Unsurprisingly they are often the most expensive and can cost over £300, although good quality ones can be found for under £100.

The Ghostbikes Cougar racing boot

Don’t believe the hype surrounding a designer brand boot. Check out what protection it offers and, if you can, try the boot for size and fit.

Shop around. Motorcycle clothing retailers have a whole range of prices – make sure you get the best deal.

Fuji.

Having completed our trek up and down Mt. Fuji, with my leg in a degree of controlled agony, my eyes almost cemented together through lack of sleep and my underarms more malodorous than twelve-hour roadkill, getting naked in a Japanese hot-spring (onsen, in the native tongue) was not my idea of relaxation. I couldn’t have been more mistaken.

The onsen was basically a public bath-house. With my prudish English streak in overload, I felt bloody uncomfortable about climbing into some massive communal bath with my Crown Jewels exposed for the entire orient to see. This, of course, highlights the typical male ego; no one really wanted to look at my member, for they were of course washing themselves and not rampantly drunk homosexuals bent on assailing my genitalia (though I’m positive an old fellow kept eyeing me from the jacuzzi…). After a somewhat timid first soak, it was relatively easy to feel, well, at ease, though I’m sure it would have been a lot worse if the bath-houses hadn’t been gender divided. Oh, that would have been dreamily awkward…

Me big European man…

I do apologise, I was…fantasising…

Ahem.

There were several hot baths (some incredibly hot) and one extremely icy cold bath that I suppose had some sort of cleansing, homeopathic healing thing that my increasingly hippie mum would probably buy in for. I lasted about twenty seconds in the sauna before I felt like my body would implode. How can anyone breathe in those damn things? I thought I was experiencing some sort of mild heart-attack. The outdoor bath was certainly the best, despite the view of several Japanese men sunning themselves on the loungers. With such fine Japanese weather, the cooling breeze and the warm water would’ve easily put me to sleep. Regardless, it was an experience I wouldn’t forget, and one that become twice more familiar in the coming week.

We left the onsen at around two o’clock in the afternoon and got back to Tokyo at about four, arriving at H.’s an hour and a half or so later. As I lay down in my room, I was aware that my leg ache was still very much there; trying to ignore it, I closed my eyes in a vain effort to sleep, but to no avail. The pain was too overpowering. Considering my options, I knew I would have to act. Realising a trip to the doctor would be in order, I told H. and was whisked off to the local hospital.

Early comments from the hospital receptionist said that any sort of x-ray or scan would cost upwards in the region of €500. With only about €350 left, ‘concerned’ would have been putting it lightly. A young chap saw to me; he was extremely polite (with truly awesome hair) and H. did all the translating for me. The doctor was left rather puzzled – he gave no ultimate diagnosis, as I seemed to have no pain in the areas he expected me to have any, and pain in the areas he didn’t. After climbing Mt. Fuji, it was also pretty difficult to pinpoint exactly where the ache was coming from. He said I had a 1% chance of having developed DVT (in my hypochondria, I had linked my dead-leg incident on the plane to my shin) but considered it very unlikely at my age; eventually, he said it may have been the side-effect of some sort of flu as I appeared to have a mild fever. Bizarrely, he overlooked the glaring yellow bruise complete with matching lump on my lower leg, instead prescribing aspirin and heat-wraps. Being a startling delightful fellow (or more likely thinking, “Haha, you dumb fuck, get travel insurance next time!”), he charged nothing for the consultation.

Considering my mind had been put at ease, I immediately felt better. A little internet research in the evening revealed that my ache had probably been caused by a bone bruise. The bruise itself would disappear in days, but the lump could remain for months or more. Indeed, it’s only been this month – the start of November – that I have noted its more-or-less complete absence. Funnily enough, I still get the odd pain there, depending on the weather. Frightfully unspiffing of it. Still, I rather preferred that over an aneurysm or tissue-eating necrosis.

The weekend passed lazily. D., disappeared off for a few hours to meet some of his work-camp friends in Tokyo (he had been in Japan a month prior to my arrival), and H. and I traipsed about the local vicinity, checking out the large, sprawling malls and arcades. My gods, do the Japanese know how to shop. And play arcade games. And drift RC cars.

Tự do, bay bổng, truyền cảm.

Shin Lam trong khoảnh khắc đoạt giải - ảnh do Bình Nguyên (VnExpress) chụp

Tree Pose is a standing balancing pose that enhances your focus, self-awareness, and concentration.

Tree Pose is a standing balancing pose that enhances your focus, self-awareness, and concentration. It simultaneously harnesses your mental ability to concentrate while calming your mind. When we write/work, we are sometimes easily distracted or our mind starts to wander, which impedes focus. Just stand up and do Tree Pose, and bring your thoughts back into proper focus and order.

Tree Pose helps your balance, flat feet, and it strengthens your calves and ankles, among other things, which do not get as much work as the other parts of your body.

Preventing Injuries

Do not put your foot on your knee. Do not stick your tailbone out. Do not lean forward. Do not be impatient with yourself. Do not tense up. If you have high blood pressure, you do not have to raise your hands above your head.

Forming Your Tree

Make sure that your tailbone is tucked in, your shoulders are down & back, your abdominal muscles are engaged, and your gaze is focused on a non-moving object.

Stand up straight in proper posture, which means that all parts of your body are engaged. You are the trunk of the tree, strong and powerful. Shift your weight to your left foot.

Tuck your tailbone in, tighten your buttocks, and press your shoulders down and back.

Using your hand to assist, if necessary, put the sole of your foot at your ankle, below your knee on your shin, or above your knee on your thigh. Your toes are pointing down. Do not place your foot on your knee.

After you get your foot positioned, turn your knee outward, which means that your inner thigh is turned straight ahead and your knee is pointing to the side. Lift your neck and head.

Concentrate, inhale and exhale through your nose, and bring your palms together in prayer position at heart level. Balance.

When you feel balanced, inhale and raise your palms above your head. You can stop here or you can keep going to the next step.

Widen your arms above your head. Now, you are the tree branches and leaves, soaring, steady, and strong. Inhale and Exhale through your nose.

When you are ready to exit Tree Pose, bring your hands into prayer position, exhale, and bring your hands down to prayer position at heart level. Place your foot back on the floor, and raise the other leg.

What You Will Feel

You will feel strong, powerful, focused, and undeterred. Your trunk is doing most of the work. You will feel your ankles, calves, and thighs working to keep you steady.
