In his debut for the blog Frank Doherty writes about the recent student union elections in NUI Galway and how these compare to how things were done in the past

Students’ Union full-time officer elections took place on Thursday, 7^th March. At the same time, a referendum of the SU’s position on women’s reproductive rights and the on-going national struggle to provide access to abortion for all women was balloted.

The voter turnout of 3,646 in the presidential election and 3,596 in the referendum has been hailed as the highest in years; however that is only the case for the recent years for which the SU has kept records. Although the number is very high in the history of the SU, as a percentage of the student population it remains low in comparison with turnouts, and more importantly activism, in the 70s and 80s.

This year’s total poll in the presidential race, a percentage of the 17,318 or so students with a vote, works out at just under 21%. This is a small figure; it means that for 79% of students in NUI Galway, all paying members of the union, the elections (and one could infer the entire union) are perceived as irrelevant to their lives as students.

Some commentators may retort that these are “normal” numbers and that young people aren’t politically engaged or naturally shouldn’t be so. However, against this we have the historical record. In the 1982/83 presidential election, for example, the total poll tallied to 2,451 in a student population of just over 4,000. That means 61% of students eligible to do so, voted.

This contrasts starkly with contemporary turnout figures, and makes clear there are extant deficiencies in how the union interacts with and relates to its membership. That entire topic itself is due more consideration, here it merely illustrates the union’s current, apparent irrelevance to a large majority of its membership, while the SU, internally, lauds the great turnout “achieved”.

The most positive signals from the results, in terms of the development of serious engagement of NUIG students with the social and political issues of contemporary society at large, came from the only poll of substance; the pro-choice referendum. The 3,596 total poll for the referendum broke down as follows: 2,478 yes, 976 no and 142 spoils. The fact that 69% of the total count voted in favour of the referendum is a clear sign of student sentiment around the issue and its significance will be discussed at the end of the article.

Juxtaposed to this sentiment, no candidate in any of the three elections would express a position on the referendum on their campaign material and neither would most express a clear position on the canvass. They were all sure to state that they would accept the democratic decision of students, but would shrug any attempt to extract a personal position – afraid of alienating a few votes. The lone core principle of most candidates was simply to get elected and gain position – pure careerist “hackery”. This reflects the same behaviour of mainstream career politicians whenever an issue of contention comes up in national politics.

Looking back

The adage that students are apolitical in their nature or ‘politics never changes’ or ‘all politicians are the same’ is completely false. If we go back through the history of student politics in NUI Galway, we find periods of both political conservatism and radical activity. We can compare the current clientelist, service providing students’ unions nationally to active, campaigning students’ unions in the past. We can compare tactics of today’s student leaders in taking comfort and companionship from lobbying their future selves, to the taking up of progressive political positions and use of direct action, protests and strikes.

In RAG Week 1976, the €1,000 raised was pledged to establishing Galway’s first family planning clinic, where people could inform themselves about contraception, then still illegal in Ireland. This was an important, progressive, political position to take. The church, much more powerful then than it is now, was in uproar and student houses were picketed by believers saying the rosary. Conservative students, including those in religious vocations and members of young Fine Gael, opposed the position and got the decision rescinded in a tight vote of 417 to 379 in an emergency meeting. Nonetheless, in that narrow defeat the positive example of progressive student politics stands out.

The independent student publication, Unity (which ran every Wednesday on campus throughout the 70s and 80s), provides many insights to fluctuating politics over the twenty or so years of its existence. In a 1973 issue, a young Eamon Gilmore, still in the Workers’ Party, wrote about the establishment of a Movement for Social Progress amongst students in UCG. The previous December, 800 students marched over the poor relations between students and the city, including extortionate rent and inadequate housing for students and the poor alike. This was a massive turnout when one considers the size of the student population which was less than a quarter of its current size.

By 1979, however, things were very different. In a column entitled “Observations” by a scribe under the penname Angus McBuffer, a student’s union discussion document “Is there any reason to protest?” is analysed. In the piece, the author describes the depoliticised union in blunt terms stating, “They are more worried about jobs in the future and haven’t a notion of getting involved in any ‘leftist activities’ or undue agitation which would be detrimental to their middle class horizons. Since when did genuine grievances become a no go area?” How relevant a question for today!

It is clear that the adage ‘politics never changes’ is baseless. Major shifts from periods of activism to periods of inactivity have taken place even within the microcosmic history of NUIGSU. When a small group of similarly-minded people have been allowed to run the unions (and the same is true of national politics), inactivity and conservatism have reigned. It has only been when the popular majority has materially engaged with political or social issues, and makes clear its position and its willingness to struggle for its objectives collectively, that things have ever changed for the common good. This is the challenge we face.

Significance of the pro-choice referendum

All governments since the X-case ruling in 1992 have failed to legislate for abortion when the mother’s life is at risk, including at risk from suicide. Today, national opinion polls show large majorities in favour of legislation for X, with support ranging from 60% to 87%. The inactivity of successive governments to legislate for abortion is analogous to USI’s inaction on fighting student fees. The disconnect between strong student sentiment on the abortion issue and union candidates’ apathy speaks volumes. A serious and progressive political stance was voted for by greater numbers than voted for each winning candidate, yet none of these candidates subscribe to this view. What a division between real political issues and personality politics!

As growing numbers of colleges adopt pro-choice positions, and as SU execs fail to advance them, women in Ireland will still have to wait until July for this partial legislation due 21 years ago. The government’s delay, whatever the excuse, is a plain denial of women’s rights. To this government, payment of socialised private banking debt has proved much more important than the rights of 51% of the country’s population. Pressure needs to be kept up!

So the campaigning will go on with renewed confidence derived from the support of NUIGSU membership, but it is up to the membership to keep pressure on the current union executive as well as the replacement units – those elected over the past two weeks. The referendum pledges support not just for life-saving abortions, but also full reproductive rights for women i.e. a woman’s choice solely. Therefore, after the legislation is passed, the next stage will be campaigning to win full access. One of the challenges involves how the media restrictively frames the debate around the arbitrary determination of health risk rather than giving a woman the right to bodily autonomy, thus stunting progressive public discussion. An addition challenge is the highly funded anti-choice lobby organised both here and abroad.

Pro-choice campaigns will greatly benefit the union’s political and material support, even so the denial of this right is ongoing and its resolution is of the utmost urgency and requires every conscientious person to support the campaign, get involved.

Salvar el alma, se dice pronto, pero se entiende con dificultad y a medias. Para entenderlo bien, sería preciso llegar a conocer internamente lo que es poseer a Dios en una felicidad eterna y lo que es el alma ¿Qué vale el alma?: La sangre de Dios hecho hombre derramada, gota a gota, en la Pasión para la salvación de todas las almas. El alma tiene un valor divino. San Ignacio estaba dispuesto a poner en peligro su vida por la salvación de las almas; y Santa Catalina de Siena besaba las huellas de los que trabajaban por la salvación de las almas.

El alma y Dios. ¿Cómo es Dios?, preguntaba el niño  Tomás de Aquino a los ancianos. Sabemos muy poco de cómo es Dios. Sabemos, estamos seguros, que Dios es la suma perfección, la suma bondad que saciará todos nuestros deseos de felicidad en el cielo. Dios es infinito en Sus perfecciones, nosotros imperfectos y miserables. Nuestro único afán: ¡Salvar el alma!.

La hermana de Santo Tomás de Aquino, le escribió pidiéndole que debería hacer para ser santa. El doctor angélico, le contestó: “Hermana mía, para ser santa te basta y es necesario por tu parte una cosa: ‘querer’; Dios no te negará su gracia”. Nos salvaremos eternamente si queremos salvarnos, pero queriendo de verdad; que ese querer se convierta en obras concretas”. Se nos brinda la gracia, a nadie falta:  “el hombre es quien falta a la gracia” (San Buenaventura). Nuestra santa Madre Iglesia pone a nuestra disposición todos los medios que necesitamos para salvarnos. Estamos en buenas manos. San Agustín: “¿Por qué no he de poder lo que otros han logrado?”

“ Se le acercó uno y le preguntó: Maestro ¿Qué obra buena he de realizar para alcanzar la vida eterna?”. Él le dijo: si quieres entrar en la vida, guarda los mandamientos” (Mt. 19,17). Una vez más: si quieres te salvarás. Hay que querer de veras, nada de veleidades: “quiere y no quiere a un tiempo el perezoso.” (Pro. 13,4) Querer de verdad es poner nuestra voluntad en sintonía con la voluntad de Dios. Hacer siempre y en todo la voluntad de Dios. No retroceder ante ningún sacrificio, no detenernos ante ninguna dificultad.

Es nuestro peregrinar hacia la vida eterna, debemos confiar siempre en la infinita misericordia de nuestro Dios. Es verdad que somos débiles y miserables, pero con San Pablo podemos decir: “todo lo puedo en aquel que me conforta” (Fil. 4,13). Porque, aunque no podamos salvarnos con nuestras débiles fuerzas, estamos seguros que no nos faltará la gracia que San Pablo alcanzó de Cristo: “te basta mi gracia” (2Cor. 12,9). Seamos muy generosos en nuestra colaboración con la gracia de Dios.

Un día y otro, siempre fieles a las gracias actuales, perseverantes hasta la muerte: “Seréis aborrecidos de todos por mi nombre, el que persevere hasta el fin, se salvara” (Mt. 10,22). Ante las tentaciones, tribulaciones y sacrificios que aparezcan en vuestras vidas, ensancha el alma la consideración de la felicidad eterna que el Señor nos tiene preparada: “Ni el ojo vio, ni el oído oyó, ni vino a la mente del hombre lo que Dios ha preparado para los que le aman” (1Cor. 2,9). “Dios espera ardientemente que se llene su casa. Es padre y le gusta vivir con todos sus hijos alrededor” (San Agustín).

San Juan Crisóstomo: “Si descuidamos el alma no podremos salvar ni el cuerpo: porque no ha sido hecha el alma para el cuerpo, si no el cuerpo para el alma” San Ignacio le decía al joven Javier en la Universidad  de Paris: “¿qué aprovecha al hombre ganar el mundo entero si pierde su alma?” (Mt. 16,26). ¿Quiero salvarme? Me salvaré. Para salvarme han de cooperar mi voluntad y la de Dios; y dios quiere salvarnos: “Dios quiere que todos los hombres se salven” (Tim. 2,3-4) Repetimos con San Agustín: “Dios que te creó sin ti, no te salvará sin ti”

Que tengo que morir es infalible;
Dejar de ver a Dios y condenarme
Triste cosa será, pero posible.

Denn um die Herzen, kommen dann die Umrandungen mit dem schwarz, das musst dann auch nicht so ordentlich sein.

Nelson Rodrigues dos Santos nos brinda com um excelente texto que apresenta, na sua primeira parte, de forma clara e direta, os determinantes econômicos e políticos que fazem com o SUS que temos esteja longe daquele com que sonhamos em 1988. A descrição que faz da base infraestrutural do capitalismo contemporâneo nos leva até a interrogar como foi possível aprovar, no Congresso Constituinte, o conceito da saúde como um direito e a proposta de um sistema de saúde baseado na universalidade, na integralidade e na igualdade! A impressão é que o Movimento da Reforma Sanitária Brasileira (RSB) soube aproveitar uma rara janela de oportunidade histórica, que se abriu e se fechou muito rapidamente: nem antes, nem depois, os sanitaristas conseguiram tão alto grau de sintonia com a vontade e a mobilização dos setores populares da sociedade brasileira. Com efeito, é incrível que um movimento social contra-hegemônico, desde antes do SUS e até hoje, como a RSB, tenha logrado o êxito que logrou ao inscrever o capítulo da Saúde na Constituição.

Na segunda parte do seu texto, Nelson trata de três pontos absolutamente cruciais para o SUS que, por um lado, são a repercussão daqueles determinantes nas políticas de saúde e, por outro, indicam parte dos desafios a enfrentar para que o Brasil tenha a esperança de avanço do projeto de um sistema de saúde efetivamente único. Não há dúvida sobre a centralidade – não apenas conjuntural, mas estrutural – das questões do sub-financiamento, da iniqüidade da relação público-privado e da ineficiência gerencial dos estabelecimentos de saúde. Nesse sentido, estamos de acordo com a análise que o texto introduz e não desejamos nela nos alongar.

Mencionamos, contudo, que o autor aborda parte dos desafios, pois nos parece que há que se acrescentar à sua relação, ao menos, mais dois importantes reptos: as questões do modelo de atenção à saúde e da participação e base social do SUS.

A questão do modelo de atenção à saúde , como a vemos, é a da pertinência da expansão da oferta de serviços de saúde, em que predominam práticas individualistas, biologicistas, curativistas e hospitalocêntricas. Ou melhor, mais recursos financeiros, supremacia do interesse público sobre o privado e uma gestão mais eficiente são aspectos cruciais, mas, sem a mudança do modelo de atenção, serão insuficientes para viabilizar o SUS universal e integral que queremos.

De fato, o modelo biomédico-reparador é um grande obstáculo à efetivação do SUS da Reforma Sanitária. E o é, em primeiro lugar, por impedir o alcance da integralidade, ao negligenciar as ações de promoção da qualidade de vida e as intervenções sobre os determinantes sociais da saúde. Em segundo lugar, por alimentar um padrão de realização de capital econômico, na área da saúde, sustentado em uma lógica de atendimento sintomático e de consumo de procedimentos que ampliam a dependência dos usuários aos profissionais de saúde. E em terceiro lugar, é um obstáculo pelo fato dessa lógica estar comprometendo seriamente a relação custo-efetividade das ações de saúde: o aumento dos custos não tem sido acompanhado pela efetiva melhoria das condições de saúde das pessoas. Ao contrário, os efeitos iatrogênicos da prática médico-hospitalar têm se acentuado a ponto de estar, hoje, entre as principais causas de morte, ao menos nos Estados Unidos.

A explicação da força persistente desse modelo biologicista reside, em grande parte, nos interesses econômicos dos conglomerados industriais e financeiros das áreas de medicamentos, vacinas e materiais médico-hospitalares. Trata-se, evidentemente, de agentes poderosos não apenas economicamente, mas também politicamente, haja vista, por exemplo, o investimento que fazem no financiamento de campanhas eleitorais em diversos países.

Há uma chance, contudo, de superar esse obstáculo. Em várias nações, começa a se notar a impossibilidade de manutenção da brutal transferência de renda de quase todos os setores da sociedade para os grandes acionistas da Big Pharma. Não deixa de ser sintomático da gravidade da situação que uma professora da Harvard University, ex-editora do New England Journal of Medicina[2], venha a público afirmar que é preciso salvar a indústria farmacêutica dos seus atuais dirigentes!

No Brasil, o ministro Temporão, em boa hora, pôs na agenda do SUS a questão do Complexo Econômico-Industrial da Saúde, que tem o desafio de contribuir não apenas para a sustentabilidade financeira do SUS, mas também para a mudança do modelo de atenção, ao investir na produção de insumos indispensáveis ao atendimento adequado às necessidades de saúde dos brasileiros, compreendidas na sua integralidade, incluindo desde as tecnologias de promoção da saúde até as de reabilitação, passando pelas abordagens diagnósticas e terapêuticas que fortaleçam a autonomia dos usuários.

Nesse aspecto, a tarefa dos partidários da Reforma Sanitária é a de fortalecer uma política de ciência, tecnologia e inovação em saúde que assegure a primazia da política sobre a economia, ou seja, que ponha o Complexo Econômico-Industrial da Saúde a serviço do SUS e não o inverso. Tal tarefa não é pequena e exige, entre outras coisas, uma estreita articulação com a política desenvolvimento industrial. Ademais, devem-se enfrentar certas iniciativas da grande indústria farmacêutica que, dada a difícil situação econômica dos países mais ricos, volta-se para os países emergentes, como o Brasil, onde tenta reproduzir práticas comerciais que não levam em conta as necessidades de saúde, mas apenas seus interesses econômicos.

Vista a questão do modelo de atenção à saúde, passemos, agora, ao segundo desafio que acrescentamos àqueles tratados por Nelson Rodrigues dos Santos: a participação e a base social do SUS.

Um breve resgate histórico é interessante para nos lembrar que a idéia de SUS cresceu na confluência, nos anos 80, de três movimentos sociais: o de comunidades das periferias das grandes cidades, o da Renovação Médica nos sindicatos de médicos e o de professores universitários de saúde pública, críticos da perspectiva da Medicina Preventiva, como importada dos Estados Unidos. Vale notar a ausência, nesse processo, do movimento sindical dos trabalhadores do setor mais dinâmico da economia brasileira, que, todavia, encontrava-se bastante ativo, nessa época, ao ponto de liderar a criação de um partido político.

Os anos 80 são um período de grandes mobilizações: “Diretas Já”, eleição de Tancredo Neves no Colégio Eleitoral, eleição do Congresso Constituinte, além da intensificação das lutas populares e sindicais que leva, entre outras coisas, à fundação da Central Única dos Trabalhadores. Certamente, foi esse ambiente que propiciou a janela de oportunidade a que nos referimos acima, permitindo que um movimento relativamente restrito ganhasse momentum.
Nesse ambiente, o Movimento da Reforma Sanitária concebe uma inovadora estrutura de controle social para o sistema de saúde, baseada na organização de conferências periódicas

e de conselhos permanentes, responsáveis por definir diretrizes para as políticas de saúde e por fiscalizar a atuação dos órgãos gestores. E mais ainda: propõe-se a participação paritária entre representantes de usuários e representantes de profissionais e prestadores de serviço, nas conferências e nos conselhos. A força do Movimento é suficientemente grande para superar os vetos do presidente Collor aos artigos da Lei 8080/90 que faziam referência ao controle social e conseguir a aprovação da Lei 8142/90, que restabelece os artigos vetados.

Desde então, contudo, as conferências e os conselhos têm perdido sua influência sobre a condução do SUS. A 12ª Conferência Nacional de Saúde, realizada em 2003, que termina inconclusa, com a dispersão dos delegados, e a 13ª, realizada em 2007, que fica marcada pela cisão radical entre representantes de gestores e representantes da sociedade civil, embora muitos membros de ambos os grupos se sintam filiados ao Movimento da RSB, evidenciam essa perda de influência. Mais ainda: o fato dos delegados da 12ª Conferência entenderem como imperativas suas decisões, apesar da ausência de base legal para isso, e proporem a punição aos gestores que não as cumprirem demonstra, paradoxalmente, a sua impotência.

De modo semelhante, o Conselho Nacional de Saúde participa, cada vez menos, de decisões importantes sobre o SUS. A pauta de discussões no Conselho Nacional de Saúde tem sido ocupada por questões corporativas, de defesa dos interesses do funcionalismo público, em especial: a centralidade que ganhou um tema mais gerencial que político como o das fundações estatais de direito privado é um exemplo disso. Por trás de um discurso que procura identificar a proposta de fundação estatal como uma estratégia de privatização do SUS, o que se verifica é a defesa intransigente de um statu quo que, crêem seus defensores, favorece os interesses dos servidores públicos, notadamente, a estabilidade do vínculo empregatício.

Acrescente-se que as exigências e urgências da gestão estão a distanciar os gestores dos conselhos de saúde, que têm assim agravado seu esvaziamento político. Ao contrário, têm-se fortalecido as Comissões Intergestores Tripartite (CIT) e Bipartites (CIBs), que não contam com a participação social, mas passaram a ser a arena principal de negociação e decisão sobre os rumos do SUS. O Pacto pela Saúde de 2006, a mais importante iniciativa recente de reorientação da condução do SUS, por exemplo, foi debatido e aprovado na CIT e não no Conselho.

O enfraquecimento da participação social no SUS, em particular, e na atividade política, em geral, tem várias explicações possíveis. A mais abrangente é aquela que remete ao contexto mundial, nos anos 90, de derrocada do “socialismo real” e de vitória político-ideológica do neoliberalismo com seus impactos sobre a capacidade de intervenção dos partidos políticos de esquerda, tradicionalmente ligados aos movimentos sociais.

Menciona-se também a diversificação do movimento social, com o surgimento de formas não-classistas de organização, mas articuladas a questões de gênero, ambientais e étnicas, o que representaria uma diminuição da capacidade de intervenção sobre políticas sociais gerais.

Outra explicação, agora mais pontual, destaca o deslocamento de lideranças sindicais e populares, incluindo militantes do movimento sanitário, para posições no aparelho de Estado, com as vitórias eleitorais de coalizões políticas progressistas, o que teria fragilizado as organizações dos trabalhadores.
Mais recentemente, já no período dos governos Lula, é mencionado um processo de cooptação de lideranças, transformadas em uma “aristocracia operária” dócil aos imperativos da realpolitik, como definida pelos companheiros que estão nas posições de governantes.

Se lembrarmos que o SUS é uma proposta democratizante, que visa a melhorar as condições de saúde de todos, cuja base de sustentação social seriam os setores populares da sociedade – aqueles que, estruturalmente, teriam interesse direto na implantação de um sistema universal de saúde –, percebemos a dificuldade em que nos encontramos. Quem hoje defende o SUS?
É notório que os segmentos do movimento sindical com maior capacidade de vocalização política, desde a extinção do Inamps, preferiram pôr nas suas pautas de reivindicação – e efetivamente a conquistaram das associações patronais ou dos governos – sua inclusão como beneficiários de planos e seguros privados de saúde.

O fenômeno recente de crescimento da chamada classe C vem atraindo as atenções de empresas de todos os ramos, inclusive das de planos e seguros de saúde. Essa “nova classe média”, por sua vez, parece ávida por consumir esses planos e seguros.

Como os setores populares mais organizados e os mais abastados da sociedade não se interessam pelo SUS, contamos apenas com as pessoas mais pobres e com menor capacidade de mobilização para, potencialmente, defendê-lo. Nesse sentido, a perspectiva principal é a de consolidação de um apartheid na saúde: serviços privados para os ricos e os remediados, SUS para os pobres.
Essa perspectiva, que representaria a derrota do projeto da RSB, não é, todavia, inexorável. Em parte, os mesmos motivos que vêm tornando o modelo biomédico-reparador insustentável comprometem a possibilidade de o seguro privado vir a satisfazer a demanda por serviços de saúde da classe média, incluindo os trabalhadores mais bem organizados. Dito de outro modo, a elevação dos custos dos serviços de saúde, determinada fundamentalmente pelos interesses econômicos do conglomerado industrial-financeiro da saúde, está inviabilizando a possibilidade de planos e seguros se consolidarem como alternativa para os setores médios da sociedade. O número de reclamações contra as operadoras de planos e seguros nos órgãos de defesa do consumidor é apenas um indício dessa dificuldade.

Dada essa dificuldade, abre-se a possibilidade de construção de uma aliança política entre a população dependente do SUS, a “nova classe média” e mesmo a classe média tradicional. Essa aliança pode, então, vir a ser o ator político a defender o SUS, como proposto pelo Movimento da Reforma Sanitária, renovando o sentido da participação social.

Transformar uma aliança possível em uma mobilização efetiva é o desafio maior dos reformistas sanitários – estejam na academia, na gestão de sistemas e serviços de saúde ou no movimento social. Não há, certamente, uma via óbvia a indicar. No entanto, há tarefas que se parecem impor.

No âmbito da academia, urge a retomada de reflexões sobre a relação entre saúde e sociedade. Se o mote do Movimento da Reforma Sanitária Brasileira, nos anos 70 e 80, foi saúde e democracia, cremos que, na segunda década do século XXI, o mote será saúde e desenvolvimento. Hoje, o Brasil cresce economicamente e melhora relativamente a distribuição de sua renda. Mas que tipo de desenvolvimento o país está promovendo? E onde se situa a saúde nesse processo? A contribuição do setor da saúde para um desenvolvimento sustentável e redutor de desigualdades pode vir a ser percebida amplamente. O amadurecimento dessa discussão, ao evidenciar a atual configuração das relações entre saúde e sociedade, pode contribuir para que as propostas relativas ao financiamento adequado do SUS e à superação da relação predatória do setor privado com o setor público, por exemplo, ganhem apoio político e mobilizem a aliança a que nos referimos.

Na esfera da gestão, a tarefa necessária é melhorar a qualidade dos serviços. A experiência concreta, com serviços que atendam razoavelmente às expectativas, é o melhor aliado na conquista da adesão dos usuários a qualquer mobilização em defesa do SUS. Só assim os setores médios da sociedade terão a confiança necessária para abandonar os planos e seguros privados e adotar o SUS. Há diversas experiências que atestam a possibilidade de melhoria da qualidade e de ampla adesão da população ao SUS, não apenas em municípios, mas também em estados, cujos governos decidiram priorizar os investimentos na saúde. No plano nacional, um bom exemplo é o Serviço de Atendimento Móvel de Urgência (SAMU), de uso irrestrito por todos os segmentos da população.

No movimento social, os militantes da Reforma Sanitária têm a responsabilidade de superar o corporativismo e politizar as discussões sobre a saúde. Ou seja, precisam envolver a sociedade como um todo no debate sobre os sistemas de saúde, demonstrando que se trata de decisões relacionadas ao tipo de sociedade que se quer construir. Com efeito, a atenção à saúde que podemos ter depende do modelo de desenvolvimento – equitativo ou iníquo – que adotamos. Neste sentido, as estratégias de mobilização a se seguir em torno da saúde estão mais próximas das estratégias da luta ecológica do que as da luta sindical. Se o desenvolvimento sustentável exige a preservação ambiental, o desenvolvimento social requer um sistema de saúde, em que o acesso seja definido pela necessidade de saúde e não pela capacidade de pagamento das famílias. À sustentabilidade e à equidade, como temos assistido em todo o mundo, se opõe o capitalismo. Retomar a luta anticapitalista é, portanto, urgente. Nos países centrais, a grave crise econômica atual pode constituir a oportunidade para a renovação dos movimentos sociais. No Brasil, a redução da miséria pode estar criando as bases para a superação das políticas sociais da carência em prol das políticas dos direitos.

Enfim, com esses dois adendos à agenda sugerida por Nelson Rodrigues dos Santos, esperamos estar contribuindo para o debate muito oportunamente proposto pela Revista Saúde em Debate. Esse conjunto de desafios, do sub-financiamento às fragilidades da participação social, é de enorme magnitude. Contudo, os êxitos que logrou o Movimento da Reforma Sanitária até aqui demonstram que tem a capacidade de sintonizar-se com o momento histórico e, assim, superar os maiores obstáculos. Oxalá, consigamos!

As we said in the previous chapters, the 9^th note is the same note as the 2^nd, the 11^th is the same as the 4^th and the 13^th is the same as the 6^th. But sometimes you’ll see chordnames like ‘Dsus2′, ‘Csus4′ or ‘E6′. Why do we write there ’2′, ’4′ and ’6′ instead of ’9′, ’11′ and ’13′?

Well, that’s because of the following rule:

When the chord has a 7 or b7 in it, we write 9, 11 and 13. When a chord has no 7 or b7 we write 2, 4 and 6.

Examples

Csus2

This chord consists of the notes 1 (C), 2 (D) and 5 (G). ‘Sus’ means ‘suspended’, and that means that the 3 is suspended by the 2. There’s no 7 or b7 in this chord, so we write ’2′. A C9 would consist of the notes 1 (C), 3 (E), 5 (G), b7 (Bb) and 9 (D). A Cmaj9 would consist of the notes 1 (C), 3 (E), 5 (G), 7 (B) and 9 (D).

Csus4

This chord consists of the notes 1 (C), 4 (F) and 5 (G). This time the 3 is suspended by the 4. It’s not the same as a C11 or a Cmaj11.

C6

No suspending here. This chord consists of the notes 1 (C), 3 (E), 5 (G) and 6 (A). It’s not the same as a C13 or a Cmaj13.

The C7sus4-chord

Here’s a very common chord that’s used a lot, and that we’ve got to mention:

This chord consists of the 1 (C), 4 (F), 5 (G) and b7 (Bb). It’s not a C11 because in C11 there’s a 3.

Because in suspended chords the 3 is suspended by another note like the 2 or the 4, we cannot tell if it is a minor or a major chord anymore. So you’ll see that in rockmusic, which has derived from bluesmusic that has the 3-b3-tension as a characteristic, the ‘X’7sus4-chord is commonly used.

Slash-chords

Slash-chords are chords written with a slash: /

They look like this: C/E

It means a regular C-chord, but with an E as a bassnote (or ‘rootnote’).

You’ll see them in all kinds of shapes. Any note can be the note in the bass (or ‘rootnote’). So a C/C# is possible. It just sounds terrible…

There are minor slash-chords too, like Cm/D.

- Alán Camilo Cienfuegos weighs in on the USI Disaffiliation debate, arguing that left-wing students should remain committed to working with and within the USI.

The decision of University College Dublin to disaffiliate from the national Union of Students in Ireland is utterly foolish. In a time when the efficacy of the various organisations of working people and the disaffected in Irish society are being blunted by the ever-useful government tactic of divide-and-conquer (public vs. private sector, etc.), to have one of the largest universities in the country break with their student’s national union and essentially go it alone is nothing but a victory for those whose interests lie in seeing unions in general broken up and emaciated, namely the government and wealthy they serve. That the campaign in favour of disaffiliation was spearheaded by the likes of Young Fine Gael, the lapdogs of their parents in government, should be evidence enough of the motivations for such a move, but to see leftists standing in the same camp as such vermin is, to say the least, surprising.

This brief piece is primarily a response to an article by Aidan Rowe entitled “The long march through the USI: UCD disaffiliation and beyond”, in which he essentially puts forward the point that the USI is an irrelevancy to students in general and to the student ‘left’ in particular, and that the decision by UCD to disaffiliate is somehow progressive, although he does not detail exactly how or why. He states that leftists who engage in pro-USI advocacy in disaffiliation campaigns such as this one “deliver crucial votes for USI which keep colleges affiliated to a structure which offers nothing to students beyond an empty, apolitical and ineffectual “representation”. Indeed, the gist of the article is provided by the final line “…why should we care about USI?”

Such a misguided view is entirely understandable in times when the level of people’s struggle in the country is low, and the tendency of many leftists to look for scapegoats to rationalise their own impatience is more common. However, it would be wrong not to argue against such views whenever and wherever they arise. The thrust of Rowe’s article smacks of crying “the USI isn’t what leftists want it to be, so why should we bother with it?” The fact is, although the USI is indeed a reactionary-dominated organisation, it still represents the vast majority of the students in the country, and has consistently, when it took the effort, brought tens of thousands of students onto the streets of Dublin in protest at fee increases and grants cuts. The fact that it took no proper action with the aid of those numbers is not so much an argument against the USI as against the ineffectiveness of the Irish left in engaging with and seriously working within the USI to influence its policies and actions (or lack thereof).

Comrade Rowe makes the distinction between the role of ‘traditional’ unions with which leftists engaged, and the specific example of the USI today, wherein the ‘traditional’ unions were at the cutting edge of struggles for worker’s rights and the primary breeding ground for radical consciousness. He states that “The same can hardly be said of USI, which doesn’t even rise to the level of the toothless post-Celtic Tiger social partnership unions, which at least retain a functioning branch infrastructure and the potential (albeit remote) of engaging in industrial action”. This utopian view ignores the fact that throughout history trade unions have also on many an occasion been nests of the most reformist and reactionary of views, until advanced workers and outside political agitators of the most developed class and political consciousness have succeeded, through arduous struggle and perseverance, in turning them around, seizing them from the bourgeois opportunists and turning them into militant organisations of the working class.

The same principle can basically be applied to the unions of Ireland today, including the USI. If the USI is to be a site of struggle and militancy, then it is the duty of student leftists and militants to make it such, not decry its shortcomings and yearn for something better, something more in line not with reality but with their own ideals. Such dreaming is nonsense. If we are to condemn the USI as utterly reactionary and useless, then if we are to be consistent, we must also condemn the other trade unions in the country as the same, and refuse to work in them or have anything to do with them. No serious revolutionary worth their salt would suggest such a thing, and it cannot simply be waved away with vacuous statements like “[the unions] at least retain a functioning branch infrastructure and the potential (albeit remote) of engaging in industrial action”. They do indeed, but so does the USI – it’s potential is clearly hinted at by its capacity to mobilise the thousands of students we saw each year at the national demonstration in Dublin before it was discontinued, regardless of the fact that they did nothing meaningful with those numbers. If such massive demonstrations are possible with reactionary leaders, imagine what could be achieved with principled leftists in positions of power?

The domination of the USI by careerists and government hacks has been echoed by the same such domination in almost every union organisation in every country throughout the history of organised labour. This does not mean we abandon the largest and most organised sections of workers (or in this case, students) to the reactionaries, to have their way with them, to lead them on the path of reformism and lobbying without any opposition. To do so is and always has been, from a revolutionary point of view, utterly criminal.

However, let us humour the dreams of ultra-leftism for a moment, and assume that in the near future other universities and colleges also disaffiliated from the USI, and that that organisation was utterly decimated – what would then replace it? We would have a situation whereby each individual Student’s Union, doubtless dominated by reactionaries due to their dismissal as ‘irrelevant’ by the elitist student left, would essentially be acting on its own, with no core demands or common positions, no uniformity of tactics, strategy or even purpose. The fracturing of the national union would result in the final destruction of an already-weak unity of third level institutions in the face of continuing government attacks on student welfare.

It has been put forward by certain comrades on the student left that there should be an alternative organisation set up, to replace or compete with the USI. Ideally, yes, of course we should have a radical union of students with a mass membership and support base, and the organisational and bureaucratic structures to support it and make it effective. But we do not, and the idea that the Irish student ‘left’, as it stands, is capable of creating such a body is frankly laughable. Discounting the likes of SF/Republican Youth (formerly Ógra Shinn Féin), Socialist Youth, and the Socialist Worker Student Society (SWSS), who are concentrating on building their own organisations, we are essentially left with the remains of Free Education for Everyone (FEE) as the only ones who have any desire to build such an alternative ‘union’, supposedly on a more or less broad basis. The fact that, outside of the Galway branch (which was the exception, yet has still come to naught), no branch of FEE has succeeded in any way in the last few years of even increasing its own membership beyond 4 or 5 people, or increasing its influence among the student masses to even a small fraction of that of the established SU’s, or even maintaining any consistent level of agitation as basic as leafleting and postering, should be evidence enough of the sorry state of the miniscule student left and its complete inability to build anything other than irrelevant cliques on the fringes of student life. There are of course arguments to be made about the de-politicisation of society and students in general in explaining the failure of the left to grow itself on our campuses,. The pathetic turnout for the UCD disaffiliation referendum is indeed testament to this. But it cannot count for all of our failures, many of which are down to basic errors in organisation and political line (which are deserving of an analysis of their own).

Comrade Rowe states “The path to success for any group begins with the admission of reality and the abandonment of unhelpful illusions.” Indeed. The chief illusion at the moment is that the section of the Irish student left to which Rowe belongs is capable of building any kind of alternative to the USI. The USI, when stripped of its infestation with careerist rats, is nothing less than a mass-based organisation of hundreds of thousands of Irish students, capable, we have seen, of mobilising tens of thousands of them in protest when the effort is made. The student left must seize the machinery of such an organisation, machinery which it clearly cannot construct on its own. To do this, the student left must first grow itself, into a disciplined, cohesive and committed core of activists, tactically and strategically united, and must then seriously engage itself in patiently and methodically raising the level of student political consciousness on and off campuses, seizing the local Student’s Unions on each campus, with the ultimate goal of clawing back the USI from the reactionary filth that has wormed its way into positions of control. Alongside this it is vital that the student left continue to build its own organs of agitation, conduct constant propaganda, protest, and direct action when and where circumstances allow. It must also be noted that the rise in student militancy will to a large extent come with a rise in the level of struggle in the country generally, and that great patience will be required to see the fruition of our goals.

Comrade Rowe claims that the structures of the USI themselves make it impossible to reclaim the union. He states: “Neither can anti-democratic structures be utilised to bring about democratisation in the absence of a pro-democracy groundswell capable of forcing such a change.” What is meant by ‘democratisation’ is left undefined. Each SU’s officer board and Class Representative Council is elected directly by the student body of each university; in turn, these send delegates to the national council of the USI to democratically vote on union policy. One wonders how much more ‘democracy’ is needed, without the decision making processes of the organisation becoming so convoluted and inefficient as to be unworkable.

With principled leftists in positions of power in the USI it will be capable of great things, and more than capable of giving whatever government is in office at the time some serious headaches if it continues to attack the welfare of the most vulnerable in society, such as students, in order to pay the for the extortions of the rich. Without the USI, indeed without a national union of any kind, Irish students will be atomised even further, and the divide-and-conquer games of the government will be even easier to play. And, as usual, the student left will be standing on the sidelines, decrying the state of student politics, having abandoned the only real mass-based student organisation in the country to the depredations of the politicians-in-training.

Em 20 de março o RS será sede do 3º encontro de Frentes Parlamentares de Hospitais Filantrópicos e Santas Casas da região Sul

RS, SC e PR irão debater financiamentos da saúde e soluções para o endividamento de hospitais.

No RS As Santas Casas e entidades sem fins lucrativos estão presentes em 245 unidades hospitalares, elas representam mais de 15,5 mil leitos do Sistema ùnico de Saúde (SUS) e empregam mais de 58 mil profissionais.

Em busca de melhores condições para os hospitais filantrópicos estive representando a Frente Parlamentar de Apoio às Santas Casas de Misericórdia, Hospitais e Entidades Filantrópicas na Área da Saúde do RS em uma reunião em São Paulo, nesta segunda feira 25.

Na ocasião foi elaborado um pedido a ser encaminhado ao Ministro da Saúde solicitando que sejam aumentados em 100% os 100 procedimentos de média complexidade mais executados em hospitais Filantrópicos e Santas Casas.

No dia seguinte, terça feira 26, seguimos à Brasília para entregar este documento, em mãos ao Ministro da Saúde Alexandre Padilha, ao presidente do Senado Renan Calheiros e ao presidente da Câmara dos Deputados Luis Eduardo Alves.

A Frente parlamentar de apoio às Santas Casas de Misericórdia, Hospitais e Entidades Filantrópicas na Area da Saúde quer garantir o funcionamento de entidades no RS e encontrar soluções para o endividamento adquirido por elas ao longo dos anos. neste momento é preciso promover um refinanciamento para estas casas.

Na parte da tarde, participei de encontro das Frentes Parlamentares dos Estado e  das Federações das Santas Casas de Misericórdis de todo Brasil promovido pela Frente Parlamentar Federal de Apoio às Santas Casas, Hospitais e Entidades Filantrópicas na área de saúde.

Sempre quando discuto situações que envolva a saúde pública no país chego a conclusão de que, em parte, a culpa total não é do governo. Talvez omisso, por não exigir a qualidade do serviço, mas total não. Sei muito bem que dentro do serviço publico existem cargos com salários defasados, mas o bom  atendimento é dever desses funcionários. Coisa que a gente não vê dentro e fora do SUS.
