<p>For many men, stress urinary incontinence (SUI) is a common side effect of prostate cancer treatments<sup>1a</sup>. Unfortunately, amidst the flurry of questions and concerns surrounding the cancer diagnosis, many men don’t even recall hearing this part of the conversation when they discuss treatments with their doctor. Let’s face it, most everyone’s ability to process new information shuts down as soon as they hear the word “cancer,” and understandably so.</p>
<p>Fortunately, prostate cancer is entirely treatable in the vast majority of cases. However, the treatment is not without potential side effects, and stress urinary incontinence is one of the most common<sup>1a</sup>. For most men, it’s a temporary problem. But for some, the leakage can become a permanent, life-altering issue.</p>
<p>Many men resolve themselves to thinking they must just learn to live with incontinence. After all, the prostate surgery likely saved their life, so how could they complain about leaking urine once in a while?</p>
<p>The fact is that SUI isn’t just a nuisance. It can actually cause other health-related issues and can completely destroy a man’s self-confidence and social life<sup>1</sup>. For example, the constant worry about odor and leakage can cause a man to withdraw from activities he once loved,including exercise and intimacy with his partner<sup>1</sup>. Another problem can result with long-term use of absorbent pads, which can cause skin breakdown that can lead to troublesome and painful skin infections<sup>2</sup>. The problem can negatively impact not only a man’s relationships but also his health—lack of exercise can lead to weight gain and other health problems.</p>
<p>Fortunately, there are permanent solutions to post-prostatectomy SUI that can put an end to these troublesome symptoms and help prostate cancer survivors reclaim the active, vibrant life they once knew.</p>
<p>A male urinary sling can permanently restore continence for most men with mild to moderate</p>

<p>stress urinary incontinence (SUI). The sling is made of sterile surgical mesh (the same type that is used to repair hernias), which works like a hammock to lift and support the urethra, restoring their natural position and their ability to maintain continence. The sling is completely concealed within the body and seems to be well-tolerated by most men<sup>3</sup>. Sling placement takes less than an hour and, in most cases, can even be performed as an out-patient procedure<sup>1b</sup>.</p>
<p>For men with more severe SUI, an artificial urinary sphincter offers a greater level of control in a permanent solution<sup>1c</sup>. Also concealed entirely within the body, the artificial sphincter consists of a small cuff placed around the urethra that, when inflated, applies pressure to prevent urine from escaping. The device is discreet and simple to use, restoring continence with very little impact on your normal restroom routine.</p>
<p>The artificial urinary sphincter implantation procedure also takes about an hour and can be performed as an outpatient procedure<sup>1d</sup>. The system is an effective long-term treatment for incontinence, having been used for almost 40 years to permanently restore continence for thousands of men worldwide <sup>4,1</sup></p>
<p>If you have been dealing with post-op SUI for more than 12 months, it’s time to talk to your doctor about the minimally-invasive treatment options available to you. You may even want to involve your significant other in the conversation, as SUI no doubt has an effect on your close relationships. The truth is that SUI is often quite problematic for wives in traditional households—in many cases, she’s the one buying the absorbent products and doing the laundry. Partners can often provide valuable insight for the doctor into the severity of the problem.</p>
<p>I recommend making an appointment with your urologist,primary care doctor or family physician to talk specifically about your UI problem. Perhaps out of embarrassment, or not wanting to “disappoint” their doctor with a complaint about an unwelcome side effect of a life-saving procedure, many patients fail to mention this problem on follow-up visits. You, as the patient, need to be your own advocate. Physicians who’ve never experienced the problem of SUI often don’t realize just how life altering it can be.</p>
<p>Once you’ve evaluated the options and chosen the one that’s best for your unique situation, you will be amazed with the difference it makes in your everyday life. I’ve seen an extraordinary number of patients for whom these procedures have made a tremendous impact in their quality of life. With the wide availability of treatments, there’s no reason to suffer in silence any longer.</p>

<li>“Aggressive” IV fluids to flush her bladder and urethra. This has caused a lot of swelling in her feet and ankles.</li>

<li>Morphine for the pain when she pees.</li>
</ul>
<p>Yesterday, she seemed to have passed a lot of water, and her urine was yellow instead of orange. I take this as a sign that she’s improving.</p>

<p><strong>Enuresis and Micturition Problems</strong><br />
Coming of age, most men need to visit the men’s room more frequently. It is a fact of life that usually doesn’t attract any particular medical attention, even if the consequences can be serious enough in terms of social and practical inconvenience.<br />
The extent of the problem is massive:</p>
<p>20% of men aged 40-50 experience voiding problems<br />
33% of men aged 50-60 experience problems<br />
More than 50% of men aged 60 or more find their micturition problematic</p>
<p><strong>Prostate and the Ageing Bladder</strong><br />
Most micturition problems are caused by a benign enlargement of the prostate gland, but problems can have other causes as well, like cancer in the prostate gland or in the bladder, constipation, or cystitis.<br />
Urination difficulties are usually caused by a combination of aging of the bladder and enlargement of the prostate gland.</p>
<p>The bladder is a muscle. As one gets older, the muscle controlling the bladder gets weaker and the compliance of the bladder is reduced. The consequence is on one hand that the capacity of the bladder is reduced, while on the other hand the muscle has problems contracting sufficiently to empty the bladder entirely. As a result of this, the volume at disposal is significantly reduced &#8211; forcing the patient to go more often to the bathroom.<br />
The prostate gland keeps growing all life. Situated around the urethra under the bladder, it will press on the urethra as it grows and make it increasingly difficult to urinate.<br />
Nerves get less sensitive with age. Therefore the elderly gets the signal of the full bladder later, and the time from registration of the urge until one must, is shortened. Sometimes down to 1-2 minutes. You will then define it as “urge-incontinence”</p>
<p>Micturition problems are caused by a combination of these effects.</p>
<p><strong>Living with Enuresis – The four Strategies</strong></p>
<p><strong>    Watchful Waiting</strong></p>
<p>Common enlargement of the prostate gland is harmless as such. It should be accepted as a sign of age. But the waiting has to be &#8220;watchful&#8221; in order not to overlook a prostate cancer or other disease.<br />
In rare cases, where the squeeze on the urethra prevents you from emptying the bladder entirely, or causes increased pressure in the kidneys even a benign enlargement of the prostate may require treatment.<br />
A watchful waiting could be combined with the use of incontinence products like UROX.</p>
<p><strong>    Surgical Treatment</strong></p>
<p>Surgical treatment usually means pealing the prostate gland from within by means of a cystoscope. In this operation, the cystoscope is often directed through the urethra up to the prostate gland, where the gland is sliced of, or burned away with a laser. The surgical treatment is very effective but since there is a significant risk of side effects it is only used in cases of severe micturition problems.<br />
Adverse effects can be impotence (&#60;10%), stricture* in the urethra (&#60;10%), and 1-5% becomes chronically incontinent.</p>
<p><strong>    Medical Treatment</strong></p>
<p>Medicine will either relax the muscle cells in the prostate gland, to reduce the tension it is exerting on the urethra, or control the growth of the prostate gland.<br />
In both cases the medicine has a number of known unpleasant side effects, like dizziness and nausea &#8211; which may be out of proportion to the problem the medicine was intended to cure.</p>
<p><strong>    Alternative Treatment</strong></p>
<p>Except for an extract of the plant “saw palmetto”, which has been shown to have a moderate but measurable effect on enuresis, no alternative medication has been clinically or experimentally proven to work.<br />
Never the less, many men tend to find some relief in alternative medicine, the most popular being an extract of pumpkin seeds. Since alternative treatments are felt to relieve, but not cure enuresis, it can be an excellent companion to a UROX system.</p>

<td align="left" valign="top">The most obvious feature on an adult woman is the pubic hair. It grows from the soft tissue above the pubic bone and is called the <strong>mons veneris</strong> (<strong>1</strong>) (<a class="zem_slink" title="Latin" href="http://en.wikipedia.org/wiki/Latin" rel="wikipedia">Latin</a> for “mountain of venus”). In mature unshaven women, the pubic hair continues down and around the vulva to the <strong><a class="zem_slink" title="Anus" href="http://en.wikipedia.org/wiki/Anus" rel="wikipedia">anus</a> (6)</strong>. The hair covered area between the mons and the anus is also made of soft fatty tissue (like the mons). This is the outer lips of the vagina, or <strong><a class="zem_slink" title="Labia majora" href="http://en.wikipedia.org/wiki/Labia_majora" rel="wikipedia">labia majora</a> (5)</strong> (Latin for major lips). The labia majora are prominent in some women and minimal in others. For some, the skin of these outer lips is darker.</td>

<p>The outer lips (labia majora) surround some soft flaps of skin which are hairless. These inner lips are called the <strong>labia minora (4)</strong> (Latin for minor lips). With sexual stimulation, they swell and turn darker as they get filled with blood. The space between the inner lips and the anus is called the<strong>perineum (8)</strong>.</p>
<p>If the inner lips are spread apart (as seen in the picture), one can see that they protect a delicate area between them. This area is called the vestibule. At the top of the vestibule, right below the mons area, the inner lips are joined to form a soft fold of skin, or <strong>hood (2)</strong>, that covers the <strong>clitoris (3)</strong>.</p>
<p>The clitoris (pronounced KLIT-or-iss [<em>no, it doesn't rhyme with Dolores</em>]) is the most sensitive spot in the entire genital area. It is made up of <a class="zem_slink" title="Erectile tissue" href="http://en.wikipedia.org/wiki/Erectile_tissue" rel="wikipedia">erectile tissue</a> that swells during sexual arousal.</p>
<p>Below the clitoris is a small slit called the <strong>urethra (11) </strong>opening. The urethra is a thin tube about an inch and a half long that connects to the bladder. This is where urine comes out.</p>
<p>Below the urethra opening is the larger opening of the <strong>vagina (10)</strong> (also called the introitus). The vagina is the birth canal, and connects the outside world of the vulva to the womb (or uterus).</p>

The Vagina is tucked between the <a class="zem_slink" title="Urinary bladder" href="http://en.wikipedia.org/wiki/Urinary_bladder" rel="wikipedia">urinary bladder</a> in the front, and the rectum in the back. If nothing is inside, the vaginal walls touch each other. When something is inside the vagina, (e.g., tampon, finger, penis, baby), the walls spread and “hug” the object .</p>
<p>The walls of the vagina are actually made up of 3 types of tissue. The inside wall is called <em>mucosa</em>, and is similar to the inside of your mouth. Just below the mucosa is a layer of tissue that can fill with blood. This is the<em>erectile tissue</em>, and swells when a woman is <a class="zem_slink" title="Sexual arousal" href="http://en.wikipedia.org/wiki/Sexual_arousal" rel="wikipedia">sexually aroused</a>. The deepest layer is a coat of muscle. This <em>muscular coat</em> is a wrap of tissue that can relax or constrict.</p>
<p>The vagina varies in length from woman to woman, but is generally 2 1/2 to 4 inches long (from vulva to cervix). In addition, the vagina has the ability to stretch quite a bit, thus allowing an erect penis in, and a baby out.</p>

Some time ago I wrote about having <a href="http://schwoit.com/2011/09/01/a-new-hope-not-a-star-wars-blog/" title="A New Hope … Not a Star Wars Blog">a new hope</a> for the cure to a medical dilemma I have suffered for many years. The issue is that my I suffer from urinary strictures. A stricture is basically a build up of scar tissue and can be caused by injury or disease. The result is a narrowing of the urinary tract,and in my case, blockage.</p>
<p>Now let me assure having the ability to urinate is a pleasure when you have simply not being able to pee at all. The worst experiences I have ever had was in the Shellharbour Public Hospital Emergency Department 2 years ago. I spent hours hopping from leg to leg until eventually the ED doctor had researched and sourced a temporary remedy and put in a sub-pubic catheter. This is basically stabbed in directly to the bladder. This did mean carrying a bag around with me as a companion for 2-3 months whilst arranging dilatation surgery with my urologist. The worst part of carrying a water bag was I had tickets to the inaugural Sydney Telstra 500 which had Cold Chisel tickets, I was too embarrassed to attend and so stayed at the hotel and missed one of the biggest concerts of the year.</p>
<p>A few days prior to surgery the catheter itself became blocked. After another visit to the ED and a few hours of hopping to and fro it was replaced.  Thankfully the dilatation surgery soon followed. In this surgery the urethra is basically stretched thus allowing water flow.  I did have to carry a bag for a day or two post surgery whilst the area healed, though it was a standard catheter which runs through your own piping so to speak. It is one of the weirdest feelings when those a removed, it doesn&#8217;t hurt, but feels wrong! Afterwards I spent a year with regular visits to a urologist to have the stricture dilated under a local anaesthetic.</p>
<p>Unfortunately another blockage occurred. This time we went to Wollongong Public Hospital Emergency Department. Again a wait involving hops from foot to foot occurred. However a nurse was able to get a standard catheter in. I had this with me for a month or two and was undergoing what is called a &#8220;trial by void&#8221;. Basically seeing if not having water flow directly pass the stricture would create a new channel non-surgically. In effect it did, however another dilatation was on the cards. That happened early in 2011 (March). Post op my urologist referred my to another urologist.</p>
<p>That urologist decided after reviewing my notes that the best thing would be to do an exploratory surgery to take measurements and get a first hand look at the impacted area. In particular ensuring that the area is not near the control centres that if disturbed could leave me incontinent. He also threw in a special catheter-less dilatation to get me through. The outcome is that I have a 3cm stricture that is not near a control centre. That means I am a good candidate for buccal mucosal urethroplasty.</p>
<p>So basically what this means is that skin from the inside of my mouth will be grafted over the top of the stricture. This will then be followed by 10 days of doing as little as possible in the hospital whilst the graft takes. After that is another 2 weeks of doing very little whilst the healing completes. Beyond that I will not be able to do anything too adventurous for 3-6 months to ensure that the good work is not undone.</p>
<p>The success rate is very high, there is though a threat that if enough tissue is not taken the stricture may reoccur, in which case the process is performed once more.</p>
<p>The challenge though is the out of pocket expense of around $3,600. This will require both Bobbie and I significant dedication to achieve our goal date of March 2012 for the surgery. Hopefully by the middle of the year my long dreaded fight with this annoyance will be a thing of the past! Then we can move on to IVF &#8230; the subject of another post me thinks.</p>

<p>Enuresis and Micturition Problems<br />
</strong>Coming of age, most men need to visit the men’s room more frequently. It is a fact of life that usually doesn’t attract any particular medical attention, even if the consequences can be serious enough in terms of social and practical inconvenience.<br />
The extent of the problem is massive:</p>
<p>20% of men aged 40-50 experience voiding problems<br />
33% of men aged 50-60 experience problems<br />
More than 50% of men aged 60 or more find their micturition problematic</p>
<p><strong>Prostate and the Ageing Bladder</strong><br />
Most micturition problems are caused by a benign enlargement of the prostate gland, but problems can have other causes as well, like cancer in the prostate gland or in the bladder, constipation, or cystitis.<br />
Urination difficulties are usually caused by a combination of aging of the bladder and enlargement of the prostate gland.</p>
<p>The bladder is a muscle. As one gets older, the muscle controlling the bladder gets weaker and the compliance of the bladder is reduced. The consequence is on one hand that the capacity of the bladder is reduced, while on the other hand the muscle has problems contracting sufficiently to empty the bladder entirely. As a result of this, the volume at disposal is significantly reduced &#8211; forcing the patient to go more often to the bathroom.<br />
The prostate gland keeps growing all life. Situated around the urethra under the bladder, it will press on the urethra as it grows and make it increasingly difficult to urinate.<br />
Nerves get less sensitive with age. Therefore the elderly gets the signal of the full bladder later, and the time from registration of the urge until one must, is shortened. Sometimes down to 1-2 minutes. You will then define it as “urge-incontinence”</p>
<p><strong>Micturition problems are caused by a combination of these effects.</strong></p>
<p>Living with Enuresis – The four Strategies</p>
<p>Watchful Waiting</p>
<p>Common enlargement of the prostate gland is harmless as such. It should be accepted as a sign of age. But the waiting has to be &#8220;watchful&#8221; in order not to overlook a prostate cancer or other disease.<br />
In rare cases, where the squeeze on the urethra prevents you from emptying the bladder entirely, or causes increased pressure in the kidneys even a benign enlargement of the prostate may require treatment.<br />
A watchful waiting could be combined with the use of incontinence products like UROX.</p>
<p><strong> Surgical Treatment</strong></p>
<p>Surgical treatment usually means pealing the prostate gland from within by means of a cystoscope. In this operation, the cystoscope is often directed through the urethra up to the prostate gland, where the gland is sliced of, or burned away with a laser. The surgical treatment is very effective but since there is a significant risk of side effects it is only used in cases of severe micturition problems.<br />
Adverse effects can be impotence (&#60;10%), stricture* in the urethra (&#60;10%), and 1-5% becomes chronically incontinent.</p>
<p><strong> Medical Treatment</strong></p>
<p>Medicine will either relax the muscle cells in the prostate gland, to reduce the tension it is exerting on the urethra, or control the growth of the prostate gland.<br />
In both cases the medicine has a number of known unpleasant side effects, like dizziness and nausea &#8211; which may be out of proportion to the problem the medicine was intended to cure.</p>
<p><strong>Alternative Treatment</strong></p>
<p>Except for an extract of the plant “saw palmetto”, which has been shown to have a moderate but measurable effect on enuresis, no alternative medication has been clinically or experimentally proven to work.<br />
Never the less, many men tend to find some relief in alternative medicine, the most popular being an extract of pumpkin seeds. Since alternative treatments are felt to relieve, but not cure enuresis, it can be an excellent companion to a UROX system.</p>

When you have stronger pelvic floor muscles/kegels you can get a tighter &#8220;grip&#8221; on a penis or even just a regular vibrator. Exercise increases the blood flow and awakens nerve endings to feel more sensation. The easiest way and also the best way to start working those muscles is by squeezing your pelvis. Everyone has done this squeeze to stop peeing because the lid was left down and it was dark or the next rest stop was still 10 miles away. It feels like you are squeezing your butt muscles but these are part of the same muscle. Not only are you clenching your butt but also your vagina. Usually it is suggested you do these kegel exeercises(squeezing your butt muscles) in increasing duration and reps just like working at the gym for other muscles. Once you have done these for awhile then you want to move onto Kegel Balls also called Ben Wa Balls. The Ben Wa Balls originally came from the Orient and they have been written about in history books and scrolls. It has been said that geishas learn this trick young so they can attract the American GI&#8217;s during war by their &#8220;special skill&#8221; that American women didn&#8217;t normally do.</p>
<p>There are different sizes and shapes of kegel or Ben Wa balls on the market and the size you order will depend on your skills of squeezing. We suggest that most women start with a Kegel ball that is larger so you can get used to the feeling and learning how to squeeze an item in your vagina. You can work your way up or in this case down size until you can hold the little gold or silver balls called Ben Wa Balls. The Ben Wa balls are the smallest of the kegel balls available and if you do not have a strong grip they will fall out as soon as you stand up. Most of the balls have a string attached to make for easy withdrawal but can also be used for some resistance training of the va jay-jay. As you are squeezing you can gently tug on the string and try to pull them out.</p>
<p>Another  method of exercising kegels is the use of a wand. The wands can do the same as pulling on the string but they are often available in many different shapes, lengths, and widths. Most wands have a bulbous head that is similar to the size of the larger Ben Wa Balls. The wands are more for use at home since it could be embarrasing to walk around with a metal or plastic object hanging out. I have heard of a lot of women who wear their kegel balls to work and even out to run errands so they can really give their vagina a workout.</p>
<p>As far as the men are concerned Kegels can also have a positive affect on your love life as well. For men they can find their kegel or pelvic floor muscle by squeezing your butt muscles. These muscles also contract your muscles in your penis and are the same one you use to stop peeing. These muscles can be used to stop the flow of semen and stop you from coming too soon with some practice because you are closing that door and not allowing it to exit. Women can also feel you clenching and releasing these muscles during intercourse. With practice you can even do a hands free orgasm which could be an event your partner might like to watch. <p>What can you do to help prevent urinary tract infections?</p>
<p>Urinary tract infections (UTI&#8217;s) are the second most common reason for patients to visit the doctor, second only to respiratory tract infections such as the common cold. Unfortunately, the majority of cases of UTI occur in women.</p>
<p>What can you do to help prevent urinary tract infections?</p>
<p>The National Institutes of Health recommends the following tips to reduce your risk of developing a UTI.</p>
<p>1. Drink plenty of water daily to help flush out the system. Nothing is better than water.<br />
2. Don&#8217;t hold it when you need to urinate! Women often try to finish a task before they go to the bathroom. Holding it may allow the bacteria to progress into a full-fledged bladder infection. Try to urinate every 2-3 hours during the day.<br />
3. You&#8217;ve probably heard that you should wipe from front to back after a bowel movement. This is especially important to help prevent bacteria from getting into the urethra.<br />
4. Taking showers instead of baths helps prevent bacteria from entering the urethra and causing a UTI.<br />
5. Always wash your genital area both before and after sexual intercourse to help prevent transferring bacteria into the urethra. Urinate after sexual activity.<br />
6. Feminine hygiene sprays and douches, particularly scented douches, can irritate the urethra and possibly lead to a UTI.<br />
7. Drinking cranberry juice is a fairly well-known and natural way to both help prevent urinary tract infections, as well as help speed the recovery process when a UTI develops.<br />
8. Another nutritional way to prevent UTI is to take vitamin C supplements. Vitamin C, or ascorbic acid, increases the acidity level of urine and discourages the growth of some bacteria.<br />
9. Women who suffer from extremely frequent urinary tract infections may be prescribed an antibiotic to take immediately after sex to help prevent the likelihood of urinary tract infection occurrence.</p>
<p>New research suggests that a woman&#8217;s blood type may play a role in her risk of recurrent UTIs. Bacteria may be able to attach to cells in the urinary tract more easily in those with certain blood factors. Other studies show women and children who tend to get UTI&#8217;s may lack proteins called immunoglobulins, which fight infection. A vaccine may help patients build up their own natural infection-fighting powers. Researchers are testing injected and oral vaccines to see which works best.</p>
<p>Ask your doctor if you should see a Urologist to help prevent and treat urinary tract infections.</p>
