MicroRNAs as Clinical Biomarkers and Therapeutic Tools in Perioperative Medicine

Over the past decade, evolutionarily conserved, noncoding small RNAs—so-called microRNAs (miRNAs)—have emerged as important regulators of virtually all cellular processes. miRNAs influence gene expression by binding to the 3′-untranslated region of protein-coding RNA, leading to its degradation and translational repression. 108 more words

Advances in the Anesthetic Management of Transcatheter Aortic Valve Implantation

AORTIC STENOSIS (AS) is a common heart valve disease in elderly individuals. In the presence of comorbidities such as angina, syncope, heart failure, and other cardiac symptoms, its fatality rate can be as high as 50%.1 Since Cribier et al2 performed the first human transcatheter aortic valve implantation (TAVI) in 2002, the procedure has developed rapidly in European countries and the US, and an expert consensus statement and guidelines on the technique have been released.3–5 For patients with severe AS with or without symptoms, surgical aortic valve replacement (SAVR) remains the primary recommended treatment, but for high-risk patients with AS and surgical contraindications or with a life expectancy greater than 12 months, TAVI is recommended as an alternative to open surgery. 10 more words
